It is characterized by a deficit in communication and social interaction, and is associated with restricted and repetitive behavior patterns. Stereotypes include inflexible adhesion to specific non-functional routines and rituals and a persistent concern with parts of objects rather than the object as a whole. Up to the present time, there are no specific tests that permit a laboratory diagnosis of the disorder to be carried out, and the syndrome is confirmed by clinical observation in the first 36 months of the patient’s life. Clinical manifestations such as epilepsy, mental retardation, sleep disorders, hyperactivity, irritability and auto- and heteroaggressiveness may alter the patient’s prognosis. Around 50% of children with ASD fulfill the criteria for attention deficit hyperactivity disorder (ADHD). Symptoms of oppositional defiant disorder (ODD) associated with autism appear to indicate a distinct phenotype requiring specific therapeutic measures. ASD is not a discrete nosological entity but, rather, a multifactorial syndrome associated with different phenotypic and biological presentations. Various disorders such as pathologies of the gastrointestinal tract have been linked to ASD, not only insofar as causality is concerned but also with respect to their role in aggravating the disease. Other associated disorders include lesions in physiological processes such as the redox metabolism, mitochondrial dysfunction and enzymatic regulation of essential metabolites. Castro","doi":"10.17267/2317-3386BJMHH.V2I2.420","DOIUrl":"https://doi.org/10.17267/2317-3386BJMHH.V2I2.420","url":null,"abstract":"Osteoporosis has been reported among Human T-cell Lymphotropic Virus type 1 (HTLV-1) infected aged patients with HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) diagnosis. However, the association between osteoporosis and HTLV-1 infection remains unclear. This study aimed to evaluate the presence of bone disorders in young HTLV-1 asymptomatic individuals. A cross sectional study was carried out at the HTLV Reference Center in Salvador, Brazil. Forty-seven HTLV-1 infected asymptomatic and 108 healthy subjects aged between 20 to 45 years were included. Biochemical markers of bone metabolism were measured and bone mineral density (BMD) was determined at the femoral neck and at the lumbar spine (L1 -L4). Significant low BMD (Z-score <-1 ) was found in HTLV-1 infected individuals (1.177 ± 0.103) compared to control subjects (1.225 ± 0.146). In logistics regression analysis HTLV-1 infected subjects were more likely to have low BMD (OR = 3.48; 95%CI 1.29- 9.43) adjusted for low education and body mass index (BMI). Osteoporosis (Z-score <-2) was not found among HTLV-1-infected group. In conclusion, our results found a low BMD in patients infected with HTLV-1 compared to uninfected controls. However, osteoporosis was not observed. Method: Application of questionnaire to cardiologists and cardiology residents March/2012 to December/2014. Results: The questionnaire was answered by 52 cardiologists 37 (71.8 %) specialists and 15 (28.9 %) cardiology residents. The average time for the formation of the group was 19.28 (±10.16) years, 39 (75 %) work in outpatient SUS, 34 (65%) in private practice. Of these 26 (50%) reported not having followed any cases of acute RF in the last year and 9 (17.3%) said they had not accompanied chronic RF cases last year. Among the 35 respondents (67.3%) report that they attended to most cases in the past and 37 (71.2 %) believe that there has been a decline in the disease. Although the majority, 47 (90.4 %) have knowledge about the last guideline of treatment and diagnosis of RF, the accuracy rate of all therapeutic options used in primary prophylaxis of these patients was 5 (9.6%) and secondary 7 (13.4 %), however 49 (94.2 %) made reference to benzathine penicillin as an option. Conclusions: The current study identified that, although RF is still an important clinical condition in our environment, the physicians interviewed did not show adequate knowledge on the subject, which could be related to poor patient adherence to RF prophylaxis. Although its pathogenesis is not clearly defined, the cardiac form is characterized by chronic inflammation and myocardial fibrosis (MF). The Cardiac Magnetic Resonance (CMR) is the gold standard noninvasive method for analysis of MF through the myocardial delayed enhancement technique (MDE). Objective: To evaluate the use of CMR in patients with CD, with emphasis on evaluation of myocardial fibrosis and inflammation. Method: Study of literature review, developed from the electronic PUBMED database using the descriptors: “cardiac magnetic resonance”, “chagas disease” from January 1993 to June 2014. Results: Of the 62 articles found, 10 were selected. Three of these studies compared CMR with endomyocardial biopsy and failed to demonstrate a high sensitivity of CMR for detecting MF. Recently, some authors have been investigating MF and cardiac inflammation with CMR in Chagas’ disease, and were able to establish the CMR as an important tool for prognostic stratification of patients with chronic chagasic cardiomyopathy (CCC). Methods: A cross-sectional, observational descriptive study about the procedures of valve surgeries performed in Brazil, from January 2008 to December 2013, by region and federative unit. The study was conducted using the electronic database of Health System Information (Datasus). Results: In the period of study, the total authorizations for hospital admissions for heart valve surgery in Brazil was 65,138 with an average of 10,586 per year. The Southeast region had the highest number of admission and the North the least. The Southeast region concentrated about 45% of valve surgeries in the last six years, while the North was responsible for only 3.80%. The number of heart valve surgeries in Brazil was 5.8/100,000 inhabitants. The national mortality rate during this procedure was 8.8%. The average cost in dollars applied for each valve surgery in 2012 was U$5,947 per 100,000 inhabitants and about $65,757,845 were spent in hospitalization in 2013. Conclusions: The study showed that Brazil spends a high investment in the surgical treatment of valvular heart disease. Methods: Population based study, descriptive and comparative, based on secondary data available in the database of the Information System on Live Births (SINASC), which was generated by the Department of the Health System of Brazil (DATASUS). The group analyzed comprises all the newborn children of parturients aged between 10 and 34 years. The variables were categorized as: maternal age (10-14; 15-19 and 20-34 years-old); delivery route (vaginal or cesarean); birthweigh (< 2,500g; between 2,500g and 3,999g; ≥ 4000g); number of prenatal visits (None, 1 to 6, ≥ 7). Results: There was a downward trend in vaginal delivery, in Salvador and in Brazil, among the three groups analyzed. Prematurity rates (<37 weeks) as well as low birth weight rates (< 2,500g) in Salvador were higher than those in Brazil in both age groups studied. In Brazil, the rates of individuals who attended the ideal minimum number of prenatal care program were 42.1% among adolescents (10-14 years old) and 65.4% among young adults (20-34 years old) women. In Salvador, those rates were 15% and 19%, respectively. Methods: Searches on MEDLINE, LILACS/SciELO and Cochrane Library databases were performed using the following keywords: HTLV-1 Infection, Human T-lymphotropic virus type 1; Paraparesis Tropical Spastic; Tuberculosis. The following data were evaluated: Study design, sample size, number of controls, frequency of HTLV-1 infection in patients with TB and uninfected controls, mortality in HTLV-1/TB coinfected individuals compared with controls group, response in vivo and in vitro to PPD, frequency of individuals with tuberculin skin test (TST) positive or negative. Results: Nineteen articles were selected: twelve investigated prevalence, four mortality, three evaluated both prevalence and mortality and six described immunological findings. The majority of the studies was conducted in South America (Brazil and Peru), and Japan. Seven out of 12 studies found an increased risk of HTLV-1 in patients with TB diagnosis. The prevalence of HTLV-1/TB co-infection ranged from 1.49 % in Brazil to 11.4 % in patients in Peru. Two out of five studies found a higher mortality of patients with HTLV-1/TB co-infection compared to patients with TB alone. Three studies conducted in Africa (Guinea Bissau and Senegal) found no increase in the mortality of patients co-infected with TB and HTLV-1. A decreased response to PPD in vitro or in vivo was observed in co-infected individuals compared with patients with TB alone. Conclusion: Patients with TB diagnosis have a higher prevalence of HTLV-1, compared with uninfected controls. Epidemiological data on morbidity and mortality of accidental and neonatal tetanus in Brazil from 2008 to 2011 were collected from the System of Information of Notifiable Diseases available on line. There were 1299 cases of accidental tetanus cases with 419 (32%) deaths. Men aged 20 to 59 years were most affected. High numbers of accidental tetanus are probably related to more exposure to risk factors. The lack of special campaigns for tetanus revaccinations every ten years is another important factor. The fewer number of cases of neonatal tetanus (23) is related to the success of governmental vaccination program targeted to this age group.
