Cardiovascular magnetic resonance (CMR) has become the first choice of non-invasive assessment of myocardial inflammation in suspected pts. Aims: The aim of this study was to report clinical, paraclinical and follow up data observed in pts with acute myocarditis confirmed by CMR in a single center. Methods: We retrospectively studied27 pts admitted for acute myocarditis between November 2010 and November 2012. All pts had ECG, echocardiography and CMR. Ultrasensitive cardiac troponin and CRP were measured. Coronary angiogram was performed in case of acute myocardial infarction-like syndrome or in the presence of CV risk factors. We reviewed the files of the hospital out-patient clinic and contacted the pts or their cardiologists by phone for those followed outside the hospital. Results: There were 23 males (85.2%) and 4 females, aged 36 ± 19 yrs. ST elevation was found in17 pts (62.9%). All had elevated cardiac troponin. Echocardiography showed abnormalities of wall motion in16 pts (59.2%). Mean LVEF on CMR was 53.96 ± 9.9% and late gadolinium enhancement was in lateral in 80%, in inferior in 10% and anterior or apical wall in 10%. Coronary angiogram was normal, performed in14 pts (51.8%). Complications included VT in4 pts (14.8 %), AF in 2, and cardiac tamponade in 1. Follow-up was obtained for23 pts (85%).One died for pulmonary embolism on lung cancer. All others had a favorable evolution. Conclusion: Our study showed that myocarditis affects in majority young and male patients. CMR appears as the main modality of diagnosis. Coronary angiogram is mandatory in case of CV risk factors and/or myocardial infarction-like presentation. Evolution is often favorable. Optimal medical therapy is still to be defined. Pts can be considered as cured in the absence of chest pain and in case of normalization of echocardiography and/or CMR at follow-up.","PeriodicalId":280405,"journal":{"name":"Brazilian Journal of Medicine and Human Health","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-09-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124884594","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: This retrospective case-control study was performed in the neonatal intensive care unit (NICU) of a tertiary center: Santo Amaro Hospital, Salvador, Brazil. All infants with birth weights ≤ 1500 g who were treated in the NICU during the study period ( n= 114) were included. The sample was divided in two groups: early (≤ 48 h after birth) and late (> 48 h after birth) onset of PN. We also analyzed a subgroup of neonates with birth weights ≤ 750g, as well as a group for whom PN was initiated very early (≤ 24 h after birth). Results: A total of 114 neonates ( n= 48 / 39.8% in early PN group),were included in the study. Birth weight recovery time (9.9 ± 4.0 vs 12.8 ± 6.1, p = 0.007), lenght of hospitalization (43.2 ± 4.0 vs 71.8 ± 41.7, p = 0.007), and time of EN onset (2.2 ± 1.2 vs 7.9 ± 5.8, p <0.001) , were significantly lower in the early PN onset group than in the late onset group. Lenght of hospitalization was positively correlated with the timing of EN onset (r= 0,37, p = 0,001). No significant difference in adverse outcomes: death (18.6% vs 13.6%, p = 0.6), intracranial hemorrhage (ICH) (8.3% vs 12.1%, p = 0.9 ) and patent ductus arteriosus (PDA) (8.3% vs 31.8%, p = 0.1) was obbserved between groups. Conclusion: In this study, the early initiation of nutritional support in very low birth weight neonates can reduce the length of hospitalization thereby reducing costs, without increasing the incidence of adverse effects.","PeriodicalId":280405,"journal":{"name":"Brazilian Journal of Medicine and Human Health","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127295709","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective: To describe and correlate postural deviations and the type of predominant weight load. Methods: This is a cross-sectional descriptive study, conducted with patients at the Center for HTLV, Salvador – Bahia. Data collection was performed in two steps, first evaluating postural orthostatic through digital photos of the individual, following the protocol SAPO ® to mark anatomical points, the second step was the Baropodometric evaluation. For correlation of weight load and postural changes we used Pearson or Spearman correlation tests (a<0.05). Results: We studied 31 individuals, predominantly female (54.8%), african descent (58.1%), with a mean age of 52.8 (+ 9.89) years. Analysis showed a predominance by an anteriorized position of the head (80.6%), trunk displaced posteriorly (64.5%), anteriorized body (96,8%), hip extension (77.4%), pelvic retroversion (61.3%), flexed knee (51.6%) with genu valgus (right 64.5%, left 74.2%) right hindfoot valgus (64.5%) and left varus (71.0%), and a reduction in the ankle angle (100%), discharge weight of the right foot and bilateral forefoot. It was observed a significant correlation between changes in anteriorized body, flexed knee and reduction in the ankle angle to the weight load of bilateral forefoot, and pelvic retroversion with the weight load of his right foot. Tumor size itself does not determine whether the lesion is in situ or invasive, so it is essential to emphasize that DCIS can present with variable extension. We report a case of a 44-year-old female patient that was diagnosed a great (12 cm) DCIS of the breast without microinvasion through pathologic examination. The patient presented with an increase of the right breast, acyclic mastalgia and right bloody nipple discharge. Mammography was inconclusive, the ultrasound suggested inflammation and the incisional biopsy revealed DCIS nuclear grade 3 with compromised lateral margins. It was performed simple mastectomy and axillary node sampling. The clinical presentations of benign and malignant diseases of the breast are variable. The most common symptoms reported by women are pain, palpable mass or nipple discharge. The literature reveals that the extent of this type of cancer is variable, being found in a review intraductal lesions up to 5.4 cm, size smaller than that one that was found in this patient. This finding makes the clinical apresentation interesting to be discussed. Although mammography is the most efficient method to diagnose this injury, clinical breast exam should be performed in all gynecological independent of patient age because it is useful for diagnosing early lesion, considering that it is a palpable lesion.","PeriodicalId":280405,"journal":{"name":"Brazilian Journal of Medicine and Human Health","volume":"69 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129322027","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a case report of a 62-year-old caucasian man without known cardiovascular risk factors presented at the emergency room (ER) for palpitations. The initial EKG showed rapid atrial fibrillation around 100 bpm and the patient complained of a recent dyspnea at exertion. Cardiac ultrasound was performed to rule out valvular or ischemic disease and measure the size of the left atrium. It showed a voluminous mass of 6 cm x 5 cm in the left atrium. The surgical removal of the tumor was indicated and a CT scan of the coronary arteries was performed in this 62-year-old patient that showed mild stenosis on the LAD. The patient was sent to cardiac surgery. This case suggests that prompt surgical treatment was safe with a low operative mortality and good long term prognosis. Cardiac ultrasound was the method of choice for diagnosis and follow-up. Contrast echocardiography allowed differentiating between cardiac tumors, myxomas and thrombi in most cases due to the difference in vascularisation.","PeriodicalId":280405,"journal":{"name":"Brazilian Journal of Medicine and Human Health","volume":"33 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116978730","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: A cross-sectional study was conducted in a sample of 2,297 individuals of > 20 years old, in Salvador-Brazil. A standardized questionnaire was applied to collect data about socio-demographic characteristics. Workers were grouped according to predominantly physical, psychic and mixed workloads, and associations were verified against the characteristics of those affected in comparison to individuals without reports of pain in the neck and upper limbs. The prevalence of pain was estimated by the OR adjusted (95% CI). Univariate analyses and logistic regression were performed. Results: NULP was found in 222 individuals (9.47%), and predominated in women and widowers exposed to physical workloads; moderate alcohol consumption was the protector in this occupational condition; for psychic workloads, being a woman and belonging to high social class appeared to be associated with the presence of pain in the region; for mixed workloads, being single appeared as a factor with less chance of association with NULP. Conclusion: NULP presents high populational prevalence, and workloads can influence such conditions. Being a woman or widow exposed to physical loads increase the risk of having NULP; When the worloads are psychic, the same occurs to women and people at higher social levels. Thus, the short form of the International Physical Activity Questionnaire (IPAQ) has been used as a good tools for evaluating and monitoring physical activity in adults. Moreover, the determination of maximum oxygen uptake (VO 2max ) has been described as one of the best methods for evaluation of physical capacity. Objective: To investigate the correlation between level of physical activity measured by IPAQ short form and VO 2max in women with overweight or obesity, and the association between body mass index (BMI) and waist circumference (WC) with the two methods. Methods: In this cross-sectional study, 48 women, aged 45.27 ± 11.56 years, 35 (72.9%) obese, answered to the IPAQ short version and underwent exercise testing to determine the maximal oxygen uptake. Results: The IPAQ continuous score was not correlated with VO 2max , nor with BMI or WC. On the other hand, it was demonstrated a significant inverse correlation between VO 2max and BMI (r = -0.50, p <0.01) and WC (r = -0.65, p <0.01). Conclusions: The IPAQ proved to be an inadequate instrument for assessing the physical performance of overweight or obese women. In contrast, the VO 2max showed a significant inverse correlation with BMI and waist circumference, reinforcing the central role of obesity as a limiting factor for aerobic performance.","PeriodicalId":280405,"journal":{"name":"Brazilian Journal of Medicine and Human Health","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125365245","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
