Dr. Goyal is assistant professor at the Johns Hopkins University School of Medicine in the Division of General Internal Medicine. One of the objectives was to evaluate if the benefits of meditation could be similar to what we observe with placebo effect. Despite of limitations the trials showed better effect with meditation, what is most relevant in a time where we’ve seen many trials applying the concept of non inferiority. It’s a very important research center publishing relevant information in a very important Journal. This demonstrates the increase interest for this issue. The importance was better understood by observing the phenotype in patients harboring mutations of the monocarboxylate transporter 8 (MCT8) gene immediately linked to Allan Herndon-Dudley Syndrome (AHDS), in which severe neurological findings are associated with abnormal Thyroid hormone levels. The disease is X-chromossome linked, with males presenting a homogeneous neurological psychomotor phenotype and mental retardation associated with low serum T4 and elevated T3 levels. The presence of genetic polymorphisms in enzymes that metabolize folic acid and vitamin supplements used to prevent these anomalies have not yet been well established in the literature. The aim of this study was to conduct a review of the literature about the role of folate and gene polymorphisms in the metabolic pathway of folic acid as regards the occurrence of NSCL/P. We reviewed theoretical material that addressed data on these topics. According to the studies analyzed in the literature, there was no consensus with regard to the protective effect of supplemental folic acid taken in the periconceptional period in the prevention of NSCL/P. Description of cases: Case 1: Child, female, 23 months of age, diagnosed with CF after repeated respiratory infections during the first month of life, with airway colonization with Pseudomonas aeruginosa and important nutritional impairment. During hospitalization, it was identified hepatomegaly with increased hepatic canaliculary enzymes and aminotransferases, and imaging studies suggesting chronic liver disease. It was conducted genetic study and it was detected a homozygous DF508 mutation. Case 2: Child, female, 7 years of age, presented hepatointestinal form of CF, with histological diagnosis of cirrhosis at 3 years of age and esophageal varices at 5 years without respiratory manifestations of CF, but with nutritional deficits. It has been conducted genetic study which detected mutations DF508 and G542X. Comments: These reported cases are distinguished by early manifestations of liver and also the severity, progression and chronicity of the disease in one of them, revealing the importance of including FC in the differential diagnosis of liver disease, regardless of age, even in the absence of pulmonary disease. Objective: To describe the off-label prescriptions in type 2 diabetes (DM2) prevention, reported international scientific literature, through an integrative bibliographical review. Method: An integrative review was made by searching the Medline international database for review of manuscripts. Selection of these databases was based on the wide range of journals covered by each of them and our goal was to provide an overview of the scientific production devoted to the topic over the timeframe under analysis. The following inclusion criteria were considered during the review: articles published between January 1985 and June 2013; use of the keywords “off-label use” OR “off-label” OR “off-label prescribing” OR “diabetes prevention” OR “prevention” MeSH “diabetes mellitus” entered into the search form, and availability of an abstract in English. Results: A total 852 scientific productions were identified, and 30 studies were selected by contain information about the off-label prescriptions in DM2. Conclusion: The practice of off-label prescribing itself has notable benefits. In some situations, an off-label prescription is the only treatment available to a patient, either because a more targeted drug is does not exist, or because other methods of treatment are ineffective or unavailable due to patient intolerance. Tunes","doi":"10.17267/2317-3386BJMHH.V2I1.356","DOIUrl":"https://doi.org/10.17267/2317-3386BJMHH.V2I1.356","url":null,"abstract":"This paper aims to evaluate the effects of smoking on periodontal microsurgery technique using the subepithelial connective tissue graft (SCTG) for treatment of gingival recession on smokers. 14 patients nonsmokers and 12 smokers were selected, presenting Miller’s Class I and II gingival recession ≥ 2.0 mm. For both groups, the subepithelial connective tissue graft was used with the aid of a surgical microscope. The clinical parameters of width and height of the gingival recession, height and thickness of keratinized tissue, probing depth and clinical attachment level were evaluated before and six months after surgery. At the end of the study, there were obtained an average percentage of 96.66% of root coverage on nonsmokers and 82.49% on smokers (p=0.03). Complete root coverage was observed in 78,57% and 50% of patients, respectively. Different levels of disease activity (LDA) have been established, however, its impact on pain and quality of life have yet to be been evidenced. The aim of this study was to evaluate the relationship of different levels of disease activity on the painful profile and quality of life (QOL) of patients diagnosed with RA. This was a cross-sectional study, conducted in RA patients attending an Educational Outpatient Care Service in Salvador, Bahia, Brazil. The LDA was defined according to values of Erythrocyte Sedimentation Rate (ESR), Visual Analog Scale (VAS), and number of swollen and sore joints, according to the Disease Activity Score in 28 joints (DAS28). Types of pain were assessed using the Douleur Neuropathique en 4 questions (DN4). To evaluate QOL, the Short Form (36) Health Survey (SF-36) and Health Assessment Questionnaire (HAQ) were applied. The association between LDA, QOL and painful profile was verified using One Way-ANOVA and Bonferroni correction post-test. A high LAD was observed in 67.7% of the 96 patients  evaluated in this study. Pain sensation was reported by 94.8 % of participants with 40.6 % reporting it as nociceptive and 80.2% as intense. It was also observed that the higher LAD found the higher was the pain intensity reported (p=0.001) and lower QOL scores (p<0.001). Although the LDA did not correlate with the type of pain (p=0.611), it was correlated with the total score obtained in the QOL from the HAQ (p=0.001). The greatest impact on QOL evaluated through the SF-36 were physical (p<0.001) and functional capacity (p<0.001). Medical records were revised and patients categorized in high and low proviral load groups: >5% and <5% -infected cells, respectively. Frequency of symptoms was quantified. A total of 64 patients were evaluated. Twenty three (36%) patients had high proviral load. All patients had clinical abnormalities reported in the medical records. The most frequently comorbidities were hypertension (37.5%) and depression (25%) and the most frequent symptoms were neurological (82.8%) and ophthalmologic (65.6%). There were no differences in the frequencies of clinical manifestations in patients with low and high proviral load, except for urinary retention, that was more prevalent in the group with high proviral (34.8%). Data were analyzed in simple and relative frequency. Underreportings for each county were estimated by the positive difference between the records of CIAVE and SINAN. We found 5.988 reports from SINAN, 676 from SINITOX and 652 from CIAVE. The study sample consisted of mothers and teachers of five children / adolescents with autism. All the mothers reported some improvement in the children due to the diet, while teachers attributed any improvement to their own work or family support. Only one of the mothers related the search for a diet as treatment to a specific explanatory model for the disease. For mothers, the difficulties of their children leads to a sense of frustration and impotence facing autism. It remains, however, hope, revealed by the search for therapeutic options, which is expressed by the demand of the diet. Teachers highlighted the potential of these children and their positive development with the pedagogical work. Both mothers and teachers highlighted their lack of knowledge regarding the cause of autism.
