Fedenko","doi":"10.36691/rja4136","DOIUrl":"https://doi.org/10.36691/rja4136","url":null,"abstract":"BACKGROUND: Skin epidermal barrier impairment in atopic dermatitis (AD) patients facilitates percutaneous penetration of allergens and leads to maintenance of skin inflammation and primary sensitization to the allergens. We hypothesized that skin care products may have a positive effect on the course of the disease in patients with AD and pollen sensitization during the pollen season. AIMS: The aim of our study was to investigate the clinical efficacy and safety of La Roche Posay formulas: Lipikar Syndet and Lipikar Baume AP+M in the prevention of AD exacerbations in children and adults, sensitized to different pollen allergens during the pollination season. MATERIALS AND METHODS: Sixty patients aged 5 to 60 years with clinical history of mild to moderate AD sensitized to different pollen allergens were enrolled in a single-center, simple, open, prospective comparative study, and formed 2 groups. The main group used Lipikar Syndet and Baume AP+M daily with standard AD treatment for 28 days during the pollen season. The control group received only standard therapy. The study included 3 visits: at each visit, the severity of AD (SCORAD), quality of life (DLQI/CDLQI), pruritus intensity (NRS), and corneometry were assessed. The changes in the SCORAD were considered to be the primary endpoint of the study. RESULTS: A decrease of SCORAD in the main group was registered by day 14 of the study (p0.0001), while no changes of SCORAD in the control group. SCORAD in the main group was lower than in the control group (p= 0.005 ) by day 28 of the study. At the secondary endpoints was shown that in the main group the quality of life improved by the day 28 of the study (p=0.0005), the intensity of pruritus decreased (p0.0001) and the level of skin hydration increased (p0.0001) by the day 14 of the study compared to the control group. No adverse events were reported. Lysenko","doi":"10.36691/rja5379","DOIUrl":"https://doi.org/10.36691/rja5379","url":null,"abstract":"Aim: The relevance of the study of cholinergic urticaria is conditioned by the prevalence of the disease, predominantly young age of patients, long-term persistence of symptoms, proven decrease in the quality of life, high probability of life-threatening reactions. This study aims to review the clinical, laboratory, and pathogenetic features of patients with cholinergic urticaria in order to identify correlations between various characteristics and the prognosis of the disease. \u0000Materials and Methods: A retrospective-prospective analysis of Moscow EMIAS electronic medical records from 2017 to 2021 was performed. 17715 patients were diagnosed with chronic induced urticaria, of which 613 patients were presumed to have cholinergic urticaria. At the time of the interview, 86 people met the criteria and agreed to be examined. The diagnosis in 38 patients with a relevant trigger-associated history was verified by provocative testing. \u0000Results: indicate that the demographic data obtained are consistent with international data. Women prevailed in the study (68.4%). The average age of the patients was 26.5 years; age of disease onset was 18 years; duration of disease was 57.5 months. The presence of anaphylaxis was recorded in 26% of cases, and angioedema was significantly more frequently observed in patients with systemic reactions. The group of patients with positive results of the test with autologous serum was characterized by older age, longer duration of the disease, and lower effectiveness of standard doses of antihistamines. As in world practice, a significant effect of cholinergic urticaria symptoms on quality of life was also noted. Analysis of the values of the validated Dermatology Life Quality Index questionnaire found almost 80% of patients reported a moderate, severe, or extremely severe impact on quality of life, physical and social activity, and only 21% reported an insignificant impact. \u0000Conclusions: The data indicate a significant impact of cholinergic urticaria on the quality of life. The microbial barrier of the skin in patients with atopic dermatitis has its own characteristics due to changes in the species composition of the microflora towards contamination by conditionally pathogenic microorganisms, which, in turn, have a significant effect on the course of the disease, leading to secondary infection of the skin and the development of exacerbations. Through the disrupted epidermal barrier, percutaneous penetration of microbes and allergens occurs, sensitization to various proteins, including proteins of bacteria and fungi, characterizing the t2 immune response. Clinical cases of patients with hereditary angioedema identified in the Republic of Kazakhstan are presented. The data of anamnesis, clinical and laboratory-instrumental indicators in patients were evaluated; the polymorphism of clinical and anamnestic data is emphasized, which does not allow to establish specific syndromes in a timely manner and determine the clinical diagnosis, which, due to delayed diagnosis, leads to the late appointment of specific basic treatment, preventive measures and preventive intervention in connection with traumatic life and medical situations, leading to life-threatening states. The main clinical manifestations of hereditary angioedema are recurrent dense edema that persists for 25 days. The most involved human organs are the skin, upper respiratory tract and gastrointestinal tract. Kadyshev","doi":"10.36691/rja1589","DOIUrl":"https://doi.org/10.36691/rja1589","url":null,"abstract":"BACKGROUND: Anaphylaxis is a situation of risk of death, so any doctor should be ready to identify its symptoms and provide emergency assistance. \u0000AIMS: This study is devoted to a comparative analysis of the level of knowledge on anaphylaxis in children among doctors of various specialties. \u0000MATERIALS AND METHODS: The study was conducted anonymously using an online questionnaire (Google platform) among 5 groups of doctors: allergologists-immunologists, resuscitators-anesthesiologists, emergency physicians, pediatricians and students of accredited university graduates. The questionnaire included 6 tasks on anaphylaxis in children and 4 tasks on other diseases similar to anaphylaxis, each had 2 questions (diagnosis and treatment) with three answer options, one was correct and gave 1 point. \u0000RESULTS: 1,871 respondents from 9 countries answered the tasks correctly. The average score of allergists on tasks of anaphylaxis was the highest (9 points out of 12) in comparison with other groups (p0.001). The average score on non-anaphylaxis tasks was high in all groups (7 points out of 8). The level of knowledge of respondents did not correlate with age and work experience. \u0000In the tasks of anaphylaxis, allergists made the appropriate diagnosis in 83.7% of cases, resuscitators, pediatricians and students - in 2/3, emergency physicians in 50%. For the treatment of anaphylaxis allergists prescribed epinephrine in 66.7% of cases, and other groups - in 50% or less. \u0000Percentage of correct answers in the tasks of food anaphylaxis (diagnosis) was the highest among allergists (80-96%), compared with other groups (36%-82%) (p0.001). In the treatment of food anaphylaxis, allergists prescribed epinephrine more often (up to 85%), compared with other groups (up to 77%). Percentage of correct responses of drug anaphylaxis (diagnosis and treatment) among all groups was high, but resuscitators responded better (94% and 64%, respectively). The level of knowledge on idiopathic anaphylaxis was the lowest in all groups. \u0000CONCLUSIONS: Underdiagnosis of anaphylaxis in children can reach 50%, but even with the correct diagnosis, epinephrine is not prescribed in half of the cases. In particular, clinically significant families of allergens have been identified, including tropomyosin, which can cause a wide range of cross-IgE-mediated reactions. One of the risk factors for the sensitization development are helminthiases, which are common in different regions, there forein the population of endemic areas. The global prevalence of helminthiasis remains high: it is estimated that 1.5 billion people worldwide are chronically infected with at least one helminth. \u0000The purpose of this review is to analyze current studies aimed at determining the relationship between helminth infections and the development of sensitization to helminth tropomyosin and the clinical course of allergic diseases. \u0000Methods: Analysis of scientific publications described the relationship between helminths tropomyosin and the development of allergic diseases and sensitization in patients with parasitic infection. We used the PubMed database for the review. This group of diseases is characterized by a long duration, a significant impact to the patients quality of life, as well as the risk of severe life-threatening reactions. Diagnosis of induced urticaria - patient-safe reproduction of symptoms by exposure to a trigger (temperature, mechanical, etc.). Currently, there are no registered medical devices in the Russian Federation for the threshold diagnosis of most physical forms of urticaria, which makes it difficult to diagnose and dynamically monitor the state of these groups of patients and the response to treatment. \u0000The question of therapy with CIndU also remains open, due to the insufficient effectiveness of the recommended and available methods of therapy. Volkova","doi":"10.36691/rja1575","DOIUrl":"https://doi.org/10.36691/rja1575","url":null,"abstract":"BACKGROUND: There are few works devoted to the study of the role of cytokines in children with allergic rhinitis (AR) with comorbid obesity. \u0000AIMS: To study the level of cytokines IL1, IL33, IL6, TNF- and total IgE in the blood serum of overweight/obese children with intermittent and persistent AR. \u0000MATERIALS AND METHODS: The prospective cohort study of 69 children aged 7-10 years with AR was conducted from November 2020 to February 2021. All children carried out the definition of serum concentrations IL1, IL33, IL6, TNF- and total IgE. Bioimpedansometry was used to assess body weight with the calculation of %FM (percentage of body fat mass). \u0000RESULTS: The study included 44 children with AR and overweight/obesity (group I), 25 children with AR and normal body weight (group II). Analysis of AR in the groups showed that in children of group I, intermittent AR was detected in 11 (25.0%) children (subgroup IA), while persistent course in 33 children (75.0%) (subgroup IB). In children of group II, intermittent AR was detected in 13 (52.0%) children (subgroup IIA), while persistent AR in 12 children (48.0%) (subgroup IIB). Revealed that the level of IL1 in blood serum was significantly higher in the subgroup IB compared with children of subgroup IA (p=0.009). The concentration of IL-33 in children of subgroup IB was significantly lower compared to subgroup IA (p=0.039). The level of IL33 in serum negatively correlated with % FM in children of group IA (r=-0.6673, p= 0.035). The level of IL6, TNF and total IgE in obese children did not depend on the severity of AR. \u0000CONCLUSIONS: Overweight/obesity contributes to the severity AR in children aged 7-10 years. First installed that the predictors of persistent AR in children with obesity are decrease in the level of IL33 and increase in IL1 in the blood serum. Food allergy (FA) is an urgent problem for public health around the world: this disease reduces the quality of life of patients, increases the risk of developing unpredictable anaphylactic reactions. \u0000Materials and methods. An analysis was made of the results of genome-wide association studies aimed at studying the influence of genetic factors in the development of FA. The review includes original articles published for the period from January 1, 2012 to December 31, 2021 \u0000Target. Conduct an analysis of genetic studies in cohorts of patients with FA aimed at assessing the role of genetic factors in the development of this pathology. \u0000Results. This review systematize data on the relationship of genetic variations associated with FA. Eight studies were analyzed, the maximum effect with the development of IgE-mediated FA on peanuts was found for the rs10018666 variant of the SLC2A9 gene in Europeans. Some allergens, associations with specific loci have been found, for example, variants rs9273440 (HLA-DQB1), rs115218289 (ITGA6), rs10018666 (SLC2A9) and others are unique to peanut. Associated variants are predominantly associated with disorders of the innate/adaptive immune response and the functioning of the epithelial barrier, confirming their leading role in the development of FA. However, much less is known about the role of non-specific factors (suspended particles in the atmospheric air-PM). \u0000AIMS: To define eosinophilic inflammation on the basis of several biomarkers in T2 endotype of asthma exposed to PM. \u0000MATERIALS AND METHODS: We studied 61 patients with T2 endotype of asthma (ages 18-65). Group 1 included 34 patients with allergic asthma and group 2 included 27 patients with non-allergic asthma. Moreover, 30 healthy matched controls without asthma and other allergic diseases were enrolled the study. We performed clinical examination and allergy testing. Additionally, serum levels of IL-33, IL-25, IL-4, IL-5, IL-13, DPP4 (multiplex assay) and periostin (ELISA) were evaluated. The analyses of annual concentration average (Avr) and maximal annual concentration (MaxAvr) average of PM2.5 and PM10 in Kazan were carried out using the database of the Center for Hygiene and Epidemiology in the Republic of Tatarstan for 2014-2020. Statistical analyseswere performedusing the R statistical software (version 4.0.5). The study was funded by RFBR (project number 19-05-50094). \u0000RESULTS: We detected increased blood eosinophil count and IL-5 levels in asthma patients. High levels of total IgE (p=0.0001), that correlated with IL-4 levels were observed only in patients with allergic asthma (rS=0.38; р=0.045). Moreover, elevated IL-25 levels were found in patients with allergic asthma (p=0.009). No significant differences in IL-13 levels in patient with asthma were found. Regression analysis revealed that the PM2.5Avr increase by 1 mcg/m3 results in the growth of IL-33 and IL-25 levels, but the PM10Avr increase raises the IL-25 levels only in patients with non-allergic asthma.
