Mutihir","doi":"10.4314/ABS.V5I1.40678","DOIUrl":"https://doi.org/10.4314/ABS.V5I1.40678","url":null,"abstract":"The study retrospectively evaluated the haematocrit values of clients accepting female sterilization over a 15year period in Jos University Teaching Hospital, Jos, Plateau State, Nigeria. Between January 1985 and December 1999, 2986 female sterilizations, through minilaparotomy, were performed. The mean (± SD) age and parity of the patients were 36.4 ± 4.2 years and 8 ± 2.0 respectively. 93.4% were between the ages 30-44years and 89.5% of them had delivered more than 5children. Interval sterilization was performed in 93.7% while 85% of the cases were done under local anesthesia. General anesthesia was used in 1.8% of the clients. The mean range of packed cell volume was 35.4%-39.6% with a mean (± SD) of 38.1 ± 2.9%. There was no anaemia related morbidity or mortality associated with the clients. The authors concluded that quality guidelines in voluntary surgical contraception ensured safety and continuity of services. Keywords : Haematocrit, Minilaparotomy, female sterilization. Airauhi","doi":"10.4314/ABS.V1I2.40630","DOIUrl":"https://doi.org/10.4314/ABS.V1I2.40630","url":null,"abstract":"Sleeping sickness (SS) in the Abraka belt: a preliminary geographical description of the disease focus.Annals Biomedical Sciences 2002;1:111-117.This retrospective study provides an analysis of the epidemiological data on Sleeping Sickness (SS) cases between January 1999 and December 2001 at the Baptist Medical Centre (BMC) Eku, Nigeria, as well as a preliminary geographical documentation of the disease focus in the Niger Delta area of Nigeria. Twenty six parasitologically confirmed SS cases by microscopic examination of blood, lymph node aspirate or cerebrospinal fluid (CSF)) made up of 14 (53.8%) males and 12 (46.2%) females aged between 0 – 70years (mean 29.88 +18.89), were seen during the period of study. Peak incidence occurred at age 0 – 20 years with 11 (42.3%) cases. There were more males (53.8%) than females (46.2%). Fifteen (57.7%) of the patients had stage I infection with positive results for blood smear and lymph node aspirate microscopy, while 11 (42.3%) had stage II infection with trypanosomes in their CSF. This study reveals that the Abraka Sleeping Sickness focus (ASSF) is a stretch of forested wetland along the margin of the river Ethiope located at the boundary between Edo and Delta States, South South of Nigeria between latitudes 5o 30 and 6o 30 North of Equator. It is an area of rainforest vegetation with an average temperature of 27oC, made up of 9 endemic villages namely Eku, Orhuoka, Abraka, Ugono, Kwale Obiaraku, Umuebu, Urhonigbe and Sanubi with an estimated population at risk of 56,851. This update of the epidemiological status of SS in the Abraka belt represents an effort towards the development of control measure against the disease in the area. They are therefore essential for the sustainance of its endemicity and reemergence of epidemics in many disease foci within sub- Saharan Africa. To investigate the epidemiological and clinical significance of reservoir hosts in the Abraka Sleeping Sickness Focus (ASSF), this survey was conducted between 8th April and May 11th 2002 in seven endemic villages with an overall population of 13,683. Pretested structured questionnaires were administered following informed consent on 2437 participants (1061 males and 1376 females) to assess knowledge and beliefs about the disease. Card Agglutination Test for Trypanosomes (CATT) screening was performed using whole blood on 1568 (11.6%) subjects of the overall population. Study participants were aged between less than 1 year and over 61 years. Confirmation of sleeping sickness (ss) was by the detection of trypanosomes in blood, body fluids and biopsy tissues. Thirteen (0.8%) seropositive subjects were parasitologically confirmed and treated with melasoprol at the Baptist Medical Centre (BMC) Eku. One (0.06%) patient died during the course of treatment. Forty-two (0.3%) subjects dropped out from the study while 128 (0.9%) seropositive and aparasitaemic subjects are currently being followed up. Five hundred and twenty (21.3%) of the 2,437 subjects interviewed reported having heard about the disease, while only 316 (12.9) had correct knowledge about the vector for the disease. Three hundred and sixty six (15.0%) believe those with the disease should remain in hiding, 422 (17.3%) believe the disease is a taboo while only 592 (25.3%) believe the disease is treatable by orthodox means. Our result provides data on active case detection and suggests the need for the formulation of health policies aimed at promoting compatibility of beliefs and knowledge about sleeping sickness with appropriate treatment seeking behaviour in the area. This approach will highlight acceptable levels of effective disease suppression with the involvement and cooperation of the affected communities. Of the 54,26 (48.1%) were males while 28 (51.9%) were females. The mean age (+SEM) of the children was 6.9+0.50 years (Range 0.90-15 years). Forty-nine (90.1%) of these children with ANS were drawn from families of low socio-economic status. Prevalence of UTI was 24.1%. Although UTI prevalence in families (32.1%) was higher than in males (15.4%), The difference was not statistically significant ( xy2=2.14: p>0.05). The prevalence of UTI amongst these patients was remarkably higher than what is observed both in healthy childhood population and those with other association morbidities.\u0000Escherichia coli (61.5%) Klebsiella pneumoniae (15.4%) and Staphylococcus aureus (23.1%) were the bacterial isolates that demonstrated appreciable in-vitro sensitivity to ofloxacin, gentamycin, ceftriazone, ceftazidime and azithromycin only. Some interaction may exist between UTI and ANS but further studies are advocated in other centers to evaluate the observation. N. T. Clinic of University of Benin Teaching Hospital (UBTH), Benin City following a complete physical examination. A CT scan of the cranium, helped to reveal communication between the accessory nose and the true nose. The abnormality was thereafter successfully managed surgically.The importance of good diagnostic workup is stressed in such congenital abnormalities to reveal any syndromic presentation and /or definitive diagnosis for proper management. Some of the disorders pose management difficulties to the dentist because he is not trained to recognise them. For instance disorders that have psychological or multifactorial aetiology, which tend to characterise mental illness are not well treated.\u0000Pain is the commonest reason for the patient to visit the dentist. If the pain is non-organic, the diagnosis and management pose a problem to the dentist. Most pains that arise from psychological illness exist in association with personality and neurotic disorders. These disorders include dentophobia, stress, bruxism and more severe conditions associated with behavioural problems or dysfunctions. The dentist ought to be trained in rudimentary Psychiatry to be able to recognise and determine when psychiatric referral is necessary. There was a preponderance of females (72.7%) over males (27.3%) and most (59.1%) patients were in the 11-31-year age group, with a median age of 25.0 years. Facial swelling was the commonest associated clinical features (81.8%) and in 91% of cases, the lesions readily yielded blood on aspiration. Soft tissue and bony lesions were encountered with equal frequencies. Sclerotherapy was the commonest mode of treatment (63.7%). Non-availability of advanced imaging techniques and limited treatment options were the major constraints to the management of these lesions. The mean birth-weight was higher in males 3276 ± 484g than in females 3112 ± 562g. The mean birth-weight (both sexes combined) showed an upward trend from 3170 ± 582g in 1998 to 3243g in 2001; giving a difference of 73g. The increase may be due to improvement in socio-economic well being of the populace. There was also a trend towards heavier babies in the wet season. Male to female (M:F) ratio of 1.4:1 was observed with the commonest location of lesion being at the lumbo-sacral region. Majority of the patients came from rural subsistent communities. The first order birth was noticed to be the most susceptible to neural tube defect. Commonly associated abnormalities included hydrocephalus, sphincteric dysfunction, limb paralysis and dysfunction.
