Odai","doi":"10.4314/ABS.V1I1.40620","DOIUrl":"https://doi.org/10.4314/ABS.V1I1.40620","url":null,"abstract":"A retrospective study, in which 685 referrals to the Oral and Maxillofacial Surgeon (OMFS) were analysed for sources, disease prevalence, concordance in diagnosis and time lapse before presentation. 363(53.0%) patients were males and 322(47.0%) were females. Patients ranged in age from one day to 80 years with a mean age of 27.5 ± SD years. General Medical Practitioners (GMPS) sent the most referrals (507 or 74.0%), specialist medical practitioners (SMPs) sent 86(12.6%) and general dental practitioners (GDPs) sent 59(8.6%) referrals. The medical specialty with most referrals was paediatrics (29.0 % of the SMPs referrals). 44.5% of the referrals had no provisional diagnosis, a 34.5% concordance in diagnosis with the OMFS was observed and 86.7% of referrals were sent without preliminary management. In 72.1% of the cases, the medical information in the referral letter was considered inadequate. Time lapse between referral and presentation to the OMFS was between one day and 10 years. Enlightenment of the public and the medical profession on the scope of the specialty is suggested. {"title":"Activated Thromboplastin Time In Women With Recurrent Spontaneous Abortions","authors":"O. Awodu, M. Information regarding haemostasis and recurrent abortion is scanty in Nigerian women. In this study, we aimed at assessing haemostasis in pregnant women with recurrent spontaneous abortions using the Activated Partial Thromboplastin Time (APTT). The APTT tests were performed on 23 pregnant women with prior history of recurrent abortions and, on two control groups comprising 50 healthy pregnant women and 50 non-pregnant women. The pregnant groups were all in second or third trimester of pregnancy. Six (24%) of the women with recurrent spontaneous abortions had shortened APTT ( 40 seconds) while 16 (64%) had normal values (30 – 40 seconds). Nine (18%) and 7 (14%) of the pregnant control had shortened and prolonged values respectively. One (2%) of the non- pregnant control had shortened APTT while 2 (4%) had prolonged APTT. There was a statistically significant difference between the mean APTT values of patients with recurrent spontaneous abortions and non- pregnant control (p = 0.0187). We conclude that the APTT in recurrent spontaneous abortions is not significantly different from that of normal pregnant women. However, it is abnormal when compared with non-pregnant women. Pregnant women with history of recurrent spontaneous abortions should therefore be routinely screened for haemostatic abnormalities. Enaibe","doi":"10.4314/ABS.V2I2.40653","DOIUrl":"https://doi.org/10.4314/ABS.V2I2.40653","url":null,"abstract":"Histological studies of the effect of oral administration of Damiana extract on the liver of mature wistar rats was carried out at the Department of Anatomy, University of Ilorin, Kwara State between December 2002 and July 2003. This study involved the oral administration of 0.52mg Damiana extract daily on various days. The experimental animals were randomly divided into four groups (A, B, C, D) of five each with the D as control. The rats in group A received 0.52mg in 1ml of Damiana extract for a day, those in group B 0.52mg Damiana extract in 1ml daily for a period of five days, while those in group C got 0.52mg/ml Damiana for ten days. The animals were sacrificed at the end of each experiment and the liver sections were obtained for routine histological investigation. Histological changes observed in the liver sections included dilatation of the central veins, cyto-architectural distortions of the hepatocytes, vacuolations, sparse and scattered sinusoids in the treated liver sections. Our results suggest that the liver functions could also have been adversely affected due to the distortion of the cyto-architecture and sparse and scattered sinusoid of the liver. A biochemical evaluation of liver function was carried out on all patients using Alanine and Aspartate transaninases, alkaline phosphate and serum bilirubin.\u0000The patients positive for HBV markers had their liver function tests compared with those of patients negative for the HBV markers. No statistically significant difference attributable to the presence of HBV was observed between the mean values of the serum enzymes amongst the groups. Isolated elevations in enzymes levels were, however, noted and randomly distributed in the various groups of patients with and without HBV markers. This is a prospective study of 213 patients with sickle cell anaemia (SCA) (112 males and 101 females) aged 6 months to 18 years for determination of their Hepatitis B status using 2 markers: Hepatitis B surface Antigen (HBsAg) and antibody to the core antigen (Anti HBc). A prevalence rate of 37% was documented with high affectation of children in their first 2 years of life (26.8% of the age group). Hepatitis B surface antigenaemia was found in 7% of the study group. Examination of the transfusion pattern of the study group showed no significant association between the frequency of transfusion and the acquisition of Hepatitis virus (HBV) markers (correlation co-efficient of 0.07). Documented, however, was an increasing frequency of transfusion with age.This study shows a high prevalence of HBV in SCA patients with early acquisition of the infection in the study group. The need for a HBV control programme in the vulnerable group of children is highlighted. The HB surface antigenaemia of 7% in the present study shows the shortcoming of using HBsAg as a lone marker in screening for HBV. Of the 34 cases, 30 (88.2%) were males and 4 (11.8%) females, giving a male/female ratio of 8.5:1. The age range was 1 to 51 years (mean 25.23±5.6years). The 21 to 30 age group was the most vulnerable while those above 60 were least affected. Gun was the weapon used in most of the cases (64.7%) and most of them were due to armed robbery attack (50.0%) and accidental discharge from police rifles (36.4%).\u0000The incidence of penetrating abdominal injuries especially through gunshot injuries is on the increase and is a major cause of morbidity and mortality in our community. Out of these, seven eyes had successful intraocular pressure reduction with average reduction ranging from 6mmHg to 17mmHg while four eyes had unsatisfactory intraocular pressure reduction with average being 2mmHg. The complications encountered were very minimal. It is recommended that Nigerian Ophthalmologists should consider micro-trephination glaucoma surgery as an effective, cheap and simple alternative surgery with minimal risk in intra ocular pressure reduction in developing countries. In consequence, rapid adjustment of the circulation to changes in posture may be impaired so that postural hypotension occurs. One such condition where this is well demonstrated is the Shy-Drager Syndrome, multiple system atrophy with autonomic failure characterized by postural hypotension. Though a rare disease, a lot of interest has recently been focused on it by the National Institute of Neurological Disease and Stroke (NINDS) because of difficulties in diagnosis and treatment. Typical cases of Shy-Drager Syndrome have not been reported in the Nigerian literature although related cases abound. The following are case reports of two adult Nigerian males. The first is that of a 70-year-old Bini man, a known hypertensive who was being managed for benign prostatic hypertrophy (BPH), parapareisis and occasional disorientation. He was later diagnosed as a case of Shy-Drager Syndrome. The second is that of a 52-year-old Bini man who became incapacitated by recurrent syncopal attacks. Both responded well to treatment with fludrocortisone acetate. A very rare condition in our environment.
