This is due to the hypersensitivity of these patients to unavoidable dental stimuli and communication disorders. Previously, we developed a new method of mitigating dental stimuli for neurodiverse patients, realized in a mobile application. The aim of the study was to evaluate parental satisfaction with dental care against the back-ground of using a mobile application for sensory deprivation of children with autism. The study included 168 families with neurodiverse children who worked with the mobile application for 1 year. As a subjective criterion for evaluating the effectiveness of the mobile application, the opinion of the direct user (families - study participants) was analyzed, represented by a wide range of parametrics in terms of the multidimensional concept of \"satisfaction\" of expectations from the use of technology. The Customer Satisfaction Index was used to measure satisfaction with the interaction with the application during one year. The main purpose of this review is to analyze the scientific literature on the topic and to study the possibility of early diagnosis of retinal dissection in pregnant women. A pregnant woman with eye diseases needs careful dynamic monitoring, in particular, by an obstetrician-gynecologist and an ophthalmologist, as well as an individual approach to decide on the choice of a method of delivery. When planning pregnancy at the stage of pre-gravidar preparation, patients with visual impairments are recommended to undergo an extended ophthalmological examination. Modern pharmacological and instrumental capabilities of medicine make it possible to effectively and without harm to the mother and fetus treat pathological changes in vision when they are perfectly detected. The most dangerous complication associated with the eyes is rheumatogenic retinal detachment, which can lead to disability. With careful dynamic monitoring by an ophthalmologist, pregnant women with severe gestosis and concomitant diabetes mellitus can avoid complications. Modern medical capabilities make it possible to effectively and safely treat ocular pathologies in pregnant women. In the process of working on the article, the following methodology was used: selection of literary sources using modern information databases, analysis of the information received, systematization of materials and presentation of conclusions. To study the effects of complex therapy with the addition of enhanced external counterpulsation (EECP) on exercise tolerance, echocardiographic parameters, markers of glycemic control and clinical outcomes in patients with ischemic chronic heart failure (CHF) with concomitant type 2 diabetes mellitus (DM).\u0000Material and methods. Patients with ischemic CHF II-III functional class with type 2 DM were randomized into group 1 (n=20) – optimal medical therapy (OMT) and EECP (35 hours, 2 courses per year), group 2 (n=21) – OMT and EECP (35 hours, 1 course per year), control group (n=20) – OMT and placebo-counterpulsation (35 hours, 1 course per year). The primary endpoint was achieving an HbA1c level of ≤7%. Secondary endpoints included the development of a combined endpoint (CEP: cases of adverse outcomes, hospitalizations for CHF, new onsets of atrial fibrillation, decreased renal function), as well as an increase after 12 months in the distance covered in 6-minue walk test (6MWT) by 20% or more from the baseline.\u0000Results. After 12 months, the proportion of patients achieving an HbA1c level of ≤7% in group 1 increased by 30%, in group 2 – by 14.3%, and remained the same in the control group (p=0.005). The proportion of patients with an increase in distance traveled according to 6MWT data >20% after 12 months in the 1st, 2nd and control groups was 100%, 76.2% and 15.0%, respectively (p<0.001). The development of CEP was observed in 3 (15.0%) patients of the 1st group, 7 (33.3%) – of the 2nd group and 12 (60.0%) – of the control group (including 1 case of death), p =0.003.\u0000Conclusion. The proportion of patients suffering from carbohydrate metabolism disorders (CMD) is also steadily increasing. The close connection between carbohydrate metabolism disorders and the progression of CHF allowed such patients to be classified into a separate phenotype cluster - the “metabolic phenotype of CHF”. The pathophysiology of the “metabolic phenotype of CHF” is based on significant metabolic disorders, which determines the characteristic features of the metabolome of this cohort of patients. Therefore, the purpose of this work is to compare and evaluate the relationship between metabolites that occupy one of the most important places in the metabolic processes of the body - the levels of branched chain amino acids (BCAA) and the phenotypic characteristics of patients with CHF. The study included 218 patients: 133 patients of the main group suffering from CHF and having CMD; comparison group - 85 patients with CHF without CMD. The groups were comparable by gender (p=0,324) and age (p=0,936). The levels of BCAAs - leucine, isoleucine, valine - in the blood plasma of patients were assessed by high-performance liquid chromatography combined with mass spectrometry (HPLC-MS). Based on the results of the analysis, the following data were obtained: plasma levels of valine and leucine in patients with CHF and CMD were higher (p=0,022 and p<0,001, respectively) than in patients with CHF and normal carbohydrate metabolism. A direct relationship between the level of leucine + isoleucine and the concentration of glucose (r=0,3, p<0,001) and an inverse relationship between the plasma concentration of valine and the level of N-terminal propeptide of natriuretic hormone (NTproBNP) (r=-0,388, p<0,001) were found. It has been established that a high level of tissue insulin resistance in patients is associated with increased concentrations of BCAA. The occurrence of obesity depends on a combination of various and dangerous etiological factors. Obesity and overweight have many pathogenetic links that cause adverse and dangerous health effects, significantly impair the quality of life and reducing its duration. High prevalence, life-threatening consequences, unsuccessful therapy, large financial costs and social discrimination put obesity and overweight in a special place among modern problems of medicine and healthcare. This study is devoted to the study of etiology, pathogenesis and influence of overweight on human health, evaluating the outcomes in clinical studies of obesity in children and adolescents. The study of the problem of obesity in children and adolescents should be aimed at identifying the relationship between various components of pathogenesis and the development of adverse consequences of obesity, taking into account the role of etiology – genetic and environmental factors. It is necessary to systematically approach the treatment of obesity and its complications, taking into account the multifactorial etiology and pathogenesis of this condition. It is observed in 10–15% of men and 20–35% of women and is often accompanied by long-term incapacity and subsequent disability of the patient. That’s why it’s not surprising that endovenosus laser ablation therapy (EVLT) is replacing surgery as the main treatment for varicose veins. Currently, laser radiation with wavelengths in the λ range from 0.45 to 2 is used for EVLT. A positive treatment result was achieved at all wavelengths that were used. The resulting differences in clinical applications in terms of pain levels, complications, and postoperative healing were rates largely subjective, leading to controversy regarding the choice of the most effective method. This was facilitated by the evolution of interpretations of the processes occurring during EVLT, which, from the authors’ point of view, is useful for their understanding. Endovasal methods have been proven to be cost-effective and safe and can be performed under local anesthesia on an outpatient basis. To date, this technique has become quite widespread in vascular surgery. This paper provides a review of the literature on the prevalence and pathophysiology of varicose veins of the lower extremities, the achievements of endovasal methods of treating varicose veins of the lower extremities, and studied modern methods of surgical correction of varicose veins. We analyzed and described the main issues related to endovenous techniques for obliterating varicose veins. Conducted on 20 case histories of patients with venous insufficiency of the lower extremities, operated on at the CHU “KB “RZD-Medicine” named after. N.A. Semashko” comparative analysis of modern surgical methods for the treatment of varicose veins of the lower extremities. The operation was performed on one lower limb in 18 patients, and on two lower limbs in 2 people. All patients underwent endovasal laser coagulation (EVLC) in combination with miniphlebectomy. Currently, there is a low staffing of medical organizations with local doctors and medical specialists, including dermatovenerologists. This determines the low availability of primary medical care for the rural population for diseases of the skin and subcutaneous tissue. Purpose of the study. To analyze the activities of dermatovenerologists in providing primary specialized medical care to the rural population for diseases of the skin and subcutaneous tissue in the Republic of Bashkortostan. Materials and methods. The calculation of indicators characterizing the activities of dermatovenerologists in the Republic of Bashkortostan for 2017-2021 was carried out: the provision of the population with dermatovenereologists, staffing of regular positions of dermatovenereologists, staffing with individuals, part-time ratio, share of visits per one rural resident and one patient with skin diseases, number of visits in connection with the disease, in connection with the preventive purpose. Results. Level of supply of dermatovenerologists in the Republic of Bashkortostan in 2021. amounted to 0.54 per 10 thousand population. In recent years, the provision of dermatovenerologists in the rural population remains low: in 11.1% of rural areas there are no dermatovenerologists, and in 31.5% the level of provision is insufficient. For the period 2017-2021 a decrease in the number of visits to a dermatovenerologist for skin diseases was established by 16.3% (per rural resident from 0.29 to 0.24 visits per resident), as well as a decrease in the number of visits to a dermatovenerologist by 22.0% . (from 4.63 to 3.61 visits per patient). An extremely low volume of telemedicine consultations was revealed, which is a significant factor in reducing the availability of medical care for the rural population. Conclusions. In the Republic of Bashkortostan in recent years, the provision of dermatovenerologists for the rural population remains insufficient. Defects in the preparation of medical documents lead to serious errors, such as incorrect diagnosis, unreasonable choice of diagnostic methods and treatment. To clarify the causes of their occurrence and adverse consequences, the study and analysis of medical records in dental organizations is of no small importance. Properly completed medical documentation serves as the basis for conducting forensic investigations within the framework of a criminal case in case of defects in the provision of medical dental care. The outpatient card of a dental patient is the object of quality control of medical care in dental organizations in the event of conflict situations. Purpose of the study: To analyze the quality control and safety of medical activities and the examination of the quality of medical dental care carried out in the dental clinic of the Military Medical Academy named after. CM. Kirov. Material and methods. During the study, analytical and statistical methods were used. The collection of primary information was carried out on the basis of data from primary medical documentation - 1067 outpatient records of dental patients. Results and condemnation. A statistically significant influence of a doctor’s work experience on the completeness of providing information about patients’ complaints was revealed. Doctors with less experience provide more complete descriptions of patient complaints. Among all complaints, aesthetic ones were most often described; complaints of a morphological and functional nature were noted quite rarely. In the cards of form No. 043/u, in 46.67% there was no information about the bite, the hygiene index was not determined, treatment plans and visit diaries were not filled out correctly. Conclusions. Internal quality assessment helps to detect defects in filling out dental patient outpatient records. Their distinctive feature is that they are capable of spontaneously gradually degradation in the body due to natural biochemical processes. There is an increasing spread pharmaceutical form, which use similar excipients in their technology – medicinal ocular insert. Modified cellulose, gums and rare-cross-linked acrylic polymers are used to create a biodegradable matrix base.\u0000In terms of development approach, ocular inserts can be divided into two groups according to the principle of matrix biodegradation. In non-biodegradable composites, the effect of prolongation of the therapeutic effect is observed due to the release of the active substance from the non-decomposing matrix. This fact can be confirmed by both pharmacopoeial and non-pharmacopoeial methods. The classic tests regulated by the Pharmacopoeia of the XV edition include the \"Dissolution\" test (GPM.1.4.2.0014 \"Dissolution of solid pharmaceutical dose forms\"), and for non–pharmacopoeial ones – dialysis through a semipermeable membrane according to Kruvchinsky or using Franz's dialysis cells. The analysis of the kinetics of dissolution helps to determine and select the most suitable matrix-forming agent to be used in the composition with other substances in the composition of ocular insert.\u0000Ocular inserts with a biodegradable matrix have a second factor that affects the release of active substances – the formation of complexes or associates between parts of the matrix and the active ingredient molecules. These intermolecular compounds can indirectly prolong the effect of the drug. All features must be taken into account in the development of ocular inserts, since they, among other things, help to model fundamentally new ways of targeted delivery of substances.
