It creates larger and less efficient lung sacs that can’t properly manage the normal gas exchange during breathing. Systemic Lupus Erythematosus (SLE) is a chronic inflammatory disease with pulmonary manifestations is giant bullae due to excessive surface tension secondary to surfactant failure. Female patient with history of SLE presenting with shortness of breath during moderate-intensity activities. CT Scan showed giant emphysematous bullae in her right hemithorax. Patient had double curve scoliosis which may affect her chest expansion. Evaluation of respiratory functions showed lung restrictive disease and its predictive post-operative value of forced expiratory volume in one second (ppoFEV1) below 30%. During 8 weeks pulmonary rehabilitation program patient’s clinical state is improved and associated with increase in forced expiratory volume in one second (FEV1). Home based aerobic exercise consisted of walk 15 minutes/day, 5 days a week. Due to her limited chest expansion, active breathing exercise and scoliosis program exercise administered in the rehabilitation program. In such patients, weak quadriceps muscle and decreased walking speed are commonly found. The study aimed to prove whether taping application on quadriceps muscle in obese patients with knee OA affects quadriceps muscle strength and walking speed. This study is a single-blind randomized controlled trial. Subjects are overweight or grade-1 obese, knee OA (Kellgren-Lawrence 1-3) patients with Visual Analog Scale (VAS) of 1-4 who were admitted to our obesity clinic on August-September 2019. One group received aerobic exercise with static cycle and taping on quadriceps muscle, while the other received the same exercise, but with sham taping. Taping was changed regularly 2 times a week for 4 weeks. The pain scale was assessed before, regularly according to tape changed, and at the end of the study. While, quadriceps muscle strength and walking speed were assessed before, at 2 weeks, and the end of the study. The taping group showed significant improvement in pain, muscle strength, and walking speed (p<0.05). Improvement in pain also found in the sham taping group. Surface EMG (sEMG) signals are recorded by non-invasive electrodes and are preferably used to obtain information about the time or intensity of superficial muscle activation. This study aims to find the pattern of muscle activation when STS in feet forward. This is a retrospective study using sEMG for the pattern of muscle activation of 14 males and 14 females in Hasan Sadikin General Hospital Bandung Indonesia, in March 2019. The average height, weight, body fat, and fat-free mass in men were significantly different from women participants. The muscle activation of muscle group stabilizer (Rectus Femoris (RF) and Biceps femoris lateral (BFL)) is bigger than muscle group sequence (Tibialis anterior (TA) and Gastrocnemius medialis (GM) during STS in this research. Rectus femoris was the muscle that had the highest mean maximum force in every STS phase. The mean maximum force of RF, BFL, and GM in Phase II was the highest compared to another phase. On the other hand, the mean maximum force of TA was the highest in Phase V. Methods: Three stroke patients, onset ≥ 6 months, with paresis of wrist and fingers extensors (MRC 2/5), flexor spasticity >3 (MAS), were given BONT-A injections before training. The training consisted of 40 minutes electrical stimulation on the affected side, proportional to the EMG signal picked-up from the unaffected side, simultaneously doing hand function training, for 3-5 days/ week. Outcome measures were Box and Block Test (BBT) and Nine Hole Peg Test (NHPT) scored before, after training, and at follow-up of 4 and 9 months. After 18 sessions, grasp function (BBT) improved in 3 patients, pinch grip (NHPT) improved in 2 patients. One patient was lost to follow-up. At 4 and 9 months, BBT improved further in 1 patient, deteriorated in 1 patient, while NHPT deteriorated in both patients. Conclusion: Grasp function improved at long term follow-up in 1 chronic stroke patient who consistently used the affected dominanthand. No improvement was seen in 1 patient affected at the non-dominant hand. Measurement of functional is an essential components, and the patient’s ability to function can be viewed as a summary measure of the overall impact of elderly health conditions. This geriatric models of assessment used in patient first visit to assess geriatric patients who have complicated condition and need special approach. CGA has several essential aspects of evaluating elderly patients, so it is known for it’s costly and time consuming. This caused by difficulties in taking histories of geriatric patients, and systematically reviewing the patients. CGA aimed to integrates the functional and medical goals of care, allowing physicians to improve clinical outcomes and patients satisfaction. This tools can be used in primary care (e.g puskesmas), clinic, hospital, long term care or home, and as a strategy to make evaluation process more efficient. Prognosis is focused on medical and function information. Quadriceps Elastic Taping (ET) application might immediately improve gait performance while other modalities may need time to show improvement. Case Description: Seven patients with KOA (9 knees) presented with pain during weight bearing. ET was prescribed as a supplementary treatment to improve gait performance. Increased gait performance such as walking speed and reduced excessive knee flexion during loading response was seen in 77% and 66% of subjects respectively. In order to evaluate changes in muscle contraction immediately after ET application, surface EMG examination was done and we found only 55% of subjects have increased percentage of maximum voluntary contraction (MVC). Discussion: Increased cutaneous input during ET application in the afferent nerves may activate a loop via afferent and efferent nerve fibres which eventually increasing quadriceps muscle activation that may reduce joint load during gait. Unexpectedly, increased quadriceps contraction after ET application was inconsistent, therefore placebo effect may also play an important role in developing the findings in this study. Conclusion: ET application have potential to improve gait performance in patients with KOA. Methods: A retrospective study of stroke patient's medical records. Functional outcome improvement was measured by evaluating the Barthel Index (BI) score on the first day of rehabilitation treatment and the last day in the hospital. Factors that were thought to associate with BI changes were analyzed using SPSS 23, including door-to-rehabilitation time, length of hospital stay, hemiparetic side, aphasia, and spasticity. Results: A total of 208 medical records, 121 data were included. Subjects were 52.1% male with a mean age was 57.36 (SD=11.03). There were 33.1% of subjects who had at least one comorbidity. Most of the subjects experienced paresis unilateral (90.3%). Stroke-related complications were spasticity (19.8%), dysarthria (19%), aphasia (14.9%) and others. The median time of door-to-rehabilitation treatment and length of stay was 2 days and 9 days. The increase of the BI score was ranging between 5 to 70. Among the factors analyzed with BI score improvement, only spasticity that was statistically significant (p=0.002). One of the most frequent is a disease named hernia nucleus pulposus (HNP) that triggering the low back pain sensation. There are various factors that lead to the occurrence of the disease. One of those is sitting in prolonged duration (≥5hours)/day. This study aims to determine whether prolonged sitting duration influences the incidence of hernia nucleus pulposus among the factory workers. The method of this study is analytical-cross sectional and used non-random consecutive sampling. The study was in Tangerang city, on March 2019. There were 75 factory workers, each subject underwent the Laseque and Bragard test to assess for suspicion of hernia nucleus pulposus. 48 respondents were categorized as those who sit in prolonged duration and 27 respondents were categorized as those who sit in normal duration. Nine people were suspected of having the hernia nucleus pulposus, seven of which admitted having prolonged sitting duration, while the remaining two sit in a normal period (p= 0.48) by Fischer test. The prevalence ratio was 2.11 (confidence interval 95%). Besides the development in medical science, the health care system also faces changes in conceptual and administration models which rise challenges to PM&R better services in the future. several indicators can be used to assessed PM&R service improvement, these indicators include ease of access for PM&R care, the ratio between population and PM&R specialist, also the availability of PM&R facilities following hospital standardization. Proper strategies and steps of anticipation must be prepared to tackle this rising challenge. Medical sciences development was growing continually and fast, including in Physical Medicine and Rehabilitation (PM&R). Currently, and also in the future, findings in biomedicine and technology of assistive devices for disabled people are one of the best inventions in PM&R. this advancement results in the improvement of care and the disabled's quality of life, however, it can also raise challenges for a physiatrist to develop their skills and explore the latest science following the advancement of science and technology to provide the best care for patients. In general, several indicators can be used to assessed PM&R service improvement, these indicators include ease of access for PM&R care, the ratio between population and PM&R specialist, also the availability of PM&R facilities following hospital standardization. Proper strategies and steps of anticipation must be prepared to tackle this rising challenge Currently, the health care system faces challenges that require conceptual changes and organization administration model to ease access to health care service, maintain a high quality of care, and maintain financial viability simultaneously. As a part of the health care system, PM&R service faces equivocal challenges so improvements are required to provide better service in the future. Demographic change in society was one of the changes occur in our health care system. Increased in the geriatric population were seen in many developing countries. This results in two consequences: (1) high prevalence of functional impairment caused by a normal degenerative process which results in health deterioration, and (2) pressure in society and health care system to provide care for this age group. The direction of organizational development and medical rehabilitation service will be affected by these consequences. Currently, a shift in disease patterns was occurring. An acute disease which previously common was now predominated by chronic disease. Many hospitals cares still set their focus on acute disease therapy, while the majority of the case is shifting towards the need for long term care. This discrepancy between patients’ needs and the care given by health care providers will rises challenges for both sides (Faria, 2010).
