Outcome measures were Box and Block Test (BBT) and Nine Hole Peg Test (NHPT) scored before, after training, and at follow-up of 4 and 9 months. After 18 sessions, grasp function (BBT) improved in 3 patients, pinch grip (NHPT) improved in 2 patients. One patient was lost to follow-up. At 4 and 9 months, BBT improved further in 1 patient, deteriorated in 1 patient, while NHPT deteriorated in both patients. Conclusion: Grasp function improved at long term follow-up in 1 chronic stroke patient who consistently used the affected dominanthand. No improvement was seen in 1 patient affected at the non-dominant hand. 摘要

目的:研究对侧控制肌电调制电刺激联合肉毒毒素A (BONT-A)运动点阻滞和手功能训练对脑卒中患者的远期疗效。方法:3例卒中患者,发病≥6个月,腕、指伸肌麻痹(MRC 2/5),屈肌痉挛>3 (MAS),训练前给予BONT-A注射。训练包括在患侧进行40分钟的电刺激,与未患侧的肌电图信号成比例,同时进行手功能训练,每周3-5天。结果测量为训练前、训练后、随访4个月和9个月的盒块测试(BBT)和九孔钉测试(NHPT)评分。18个疗程后,3例患者抓握功能(BBT)改善,2例患者捏握功能(NHPT)改善。1例患者失访。在4个月和9个月时,1例患者的BBT进一步改善,1例患者恶化,而2例患者的NHPT恶化。结论:长期随访1例慢性脑卒中患者,持续使用受影响的主掌器,其掌握功能得到改善。非优势手1例未见改善。在长期随访中,握力的改善消失。
