The use of US in the evaluation of abdominal injury during multiple casualty incidents (MCIs) has been described, but never evaluated. The purpose of this study was to determine the accuracy of US for evaluation of the unique injury patterns associated with MICs. Patients and methods: We conducted a retrospective study of patients admitted to a Level 1 trauma center during MCIs resulting from terrorist attacks in the Tel‐Aviv area. Results: During the 4‐year study period there were 43 patients who had an US examination as part of their initial assessment. The overall accuracy of the US examination was 77%, with a sensitivity of 40%, and a specificity of 88%. The positive predictive value (PPV) was 50%, and the negative predictive value (NPV) was 83%. An evacuation maneuver was started the following day, and a total of 23 flights carrying over 2000 tourists and victims were arranged between December 27th 2004 and January 2nd 2005. The Medical Director of the Helsinki an Uusimaa hospital district was in charge of the co‐ordination of the medical sector, as well as the managing of all hospitals in the district and information regarding medical data. A medical center was set up at the Helsinki‐Vantaa international airport in order to provide immediate medical services to the arriving victims and to preclude an overflow of patients and uninjured passengers to the hospitals in the district. In all, 239 patients were evaluated, sent to hospitals, and treated by this center during the week. Accidental release of chlorine gas with exposure of large numbers of people is a constant threat to urban areas. The toxicity of chlorine is related almost entirely to effects in the respiratory system. The treatment of victims exposed to chlorine gas is mainly symptomatic including supplemental oxygen and antitussive medication. Administration of nebulized β2‐agonists is supported by recent experimental studies, and early treatment with inhalational or systemic corticosteroids is supported by case reports and experimental data. A set of 11 different performance indicators previously proposed was used as a template when examining ambulance staff in medical command and control. The results were graded 0 to 2 where 0 = not acceptable, 1 = partially correct and 2 = correct. A total of 46 examinations comprising more than 200 students was included. Performing a correct second report (score 1.15), the ability to establish general guidelines for medical response (score 1.20), setting level of medical ambition (score 1.24) and informing media (score 1.33) had significantly lower scores than the other performance indicators. {"title":"The 2001 World Trade Center disaster - summary and evaluation of experiences","authors":"J. Pryor","doi":"10.1080/15031430310000847","DOIUrl":"https://doi.org/10.1080/15031430310000847","url":null,"abstract":"Objectives To collect and analyse data from deaths and injuries, and from evaluation of the responses by medical services and by fire-, rescue-, and police services one year after the terror attack on World Trade Center. Methodology Epidemiologic data were collected from all involved agencies and analysed. The authors' personal experience from working at the scene during the event and several other personal testimonies were also included in this analysis. Results Totally 2.762 death certificates were issued by the state of New York for victims of the terror attack. 1.361 (49.9%) of these were issued for victims whose remains could not be identified. All but nine of these died at the day of the attack. 77% of the victims were male, medium age 39 years. Of the dead were 342 fire fighters and paramedics and 60 police officers. A total of 1.103 patients were treated during the first 48 days in five key-hospitals receiving the majority of the injured. 29% of these were rescue workers. 66% of the injured were male, average 39 years. The most common injuries were respiratory impairment (49%) and ocular affection (26%), many severe. The most common trauma was lacerations (14%) and sprains (14%). Of those administered to hospital, 19% had trauma and 19% burns. Head injuries were registered in 6% and crush injuries in 4%. With regard to response from involved agencies, communication failure was the most common and difficulties in command operations and scene control were also prevalent. Conclusions The difficulties encountered were very similar to those commonly seen in major accidents or disasters, although on a great scale.
