Tranexamic acid (TXA) has been used with no systematic review conducted to evaluate its efficacy and safety. The PRISMA guidelines were followed to conduct literature screening and selection. Main outcomes were extracted for qualitative analysis and quantitative analysis. Blood loss level and haemoglobin drop was significantly reduced in TXA group as compared to the control group. TXA is also deemed as protective factor for transfusion requirement. TXA use may be considered in every patient undergoing hemiarthroplasty procedure unless absolute contraindication exists.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-08-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136242217","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The coracoid component is not always radiographically recognized; thus, this combined orthopedic injury is not often common. Management options varied from conservative to surgical management with single or dual fixation strategy. This study aimed at outcome evaluation of clavicular hook plate with coracoid screw fixation in athletics and patients with high-demand activities for at least one year follow-up. Material and methods: Included patients underwent fixation through clavicular hook plate and coracoid screw. The hook plate was removed after an average period of 16.1 weeks, rehabilitation program was followed. Patients were subjected at final follow-up visit to clinical assessment via ROMs, constant, and ASES scores. Besides, radiological judgement of acromioclavicular joint reduction as per CCD and CCD ratio. Results: The average ASES Scores were 39.1±14.6, 67.4±11.5, and 86.7±5.7 at 3, 6, and 12 months, respectively with a statistically significant improvement in between (P-value <0.001). The mean active shoulder abduction and forward elevation ROMs were 156.4°±12.8° and 171.4°±6.3°. The average CCD was 9.04±0.9 mm (7.6-10.2 mm), and the mean CCD ratio was 1.07±0.03% (1.01-1.11%). Conclusion: Dual fixation via hook plate and coracoid screw without CCL reconstruction provides a stable fixation construct with considerable functional and radiological outcome in high demand patients with acute traumatic ACJ disruption combined with coracoid fracture. Study Design: Case series study.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130599102","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is commonly misdiagnosed and treated inappropriately. Although there is no cure for the disease, early and correct diagnosis may slow the disease progress, avoid unnecessary intervention and in turn improve the quality of life. We report a 13 years old girl with fibrodysplasia ossificans progressiva who presented with progressive stiffness of the bilateral hips after a trivial fall. Further history has found progressive joint stiffness involving multiple joints. She had not been diagnosed even though multiple encounters with the health facility for similar complaints. This has highlighted the importance of awareness of the disease among healthcare personnel.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131299276","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Preoperative planning is essential to achieve this and is generally conducted using conventional radiographs. The objective of our study was to examine the clinical feasibility of a 3D planning system. Methods: In 500 patients (average age 72.8 (31-87) years), a CT was performed preoperatively for 3D planning. The data were further processed in DICOM format on an external workstation for 3D planning, with the aid of special software (SYMBIOS® 3D-Hip Plan), whereby an exact presentation of the acetabulum and femur was possible in all three planes. The clinical outcome was documented by using the Harris Hip Score. Results: In 470/500 patients (94%), the acetabular cup was successfully implanted as planned. In the area of the stem, in 10/500 patients (2%) a custom made stem was implanted, as adequate treatment due to anatomical reasons could not have been achieved here. Overall, the stem planning could be implemented exactly in 475/500 patients (95%). The neck length of the head was implemented as planned in 465/500 patients (93%). The Harris Hip Score improved from a preoperative average of 47.6 to 70.6. Conclusion: 3D hip planning enables a preoperative simulation of implant positioning, which makes it possible to optimally determine and reconstruct the centre of hip rotation. Potential difficulties that might arise intraoperatively can already be identified preoperatively. Patients who require a custom made stem can be reliably identified.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"17 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127509630","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Well-informed patients are also likely to be more compliant and have better treatment outcomes. Aims and Objectives: Our study looks at using anatomical models as adjuncts in consultations to determine if they would be useful in improving patient understanding and satisfaction. Study Design/Methods: A survey of patients was carried out at a single Orthopaedic foot and ankle clinic in Singapore, to investigate if patients felt that the use of anatomical models aided in their understanding of their medical condition, foot and ankle anatomy, and surgical procedures offered, if any. 2 different anatomical models were used, depending on the context of the consultation. All conditions encountered were included. Institutional Review Board approval was not required for this study. Results: 30 patients were surveyed in total, all of whom agreed that the models were useful in helping them understand their medical condition and foot and ankle anatomy better. Of the 19 patients who were counselled regarding surgery, 18 felt that the models improved their understanding of the proposed procedures. 26 patients would like similar models to be used in other Orthopaedic clinics, although 3 would not, and 1 was not sure. On the whole, 19 patients found the models very useful, and 11 patients found them useful. Conclusion: Anatomical models are a cost-effective method of contributing to patient education compared to other alternatives, and we would advocate the use of models in other Orthopaedic clinics during consultations for counselling and taking informed consent.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"44 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128202509","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Aims and Objectives: The purpose of this study was to evaluate the general and musculoskeletal health literacy of the parents or guardians (caregivers) of pediatric patients presenting to a general pediatric orthopedic clinic and identify risk factors for limited health literacy. Study Design: This cross-sectional study obtained a convenience sample of 200 English speaking caregivers of children presenting with musculoskeletal complaints. Caregivers who were taking their children for either first-time or follow-up visits were included in the study. Participants were excluded if they did not meet the aforementioned criteria, had any cognitive impairment, were unable to read English, or were unable to sign their own consent. Setting: The general pediatric orthopedic clinic of a large children’s hospital. Materials and Methods: Caregivers completed a demographic survey, the Literacy in Musculoskeletal Problems questionnaire, and the Newest Vital Sign to measure musculoskeletal and general health literacy, respectively. Statistics: The data was summarized using frequencies and proportions for categorical variables as well as mean and standard deviation for quantitative variables. In the univariate analysis the group comparisons were conducted using the chi-squared test for categorical variables. Multivariable logistic regression was used to model the odds of adequate musculoskeletal health literacy. Results: Limited musculoskeletal health literacy was seen in 46.7% of participants. Inadequate general health literacy was observed in 18.0% of participants. Non-Hispanics (p = 0.0210), those who worked in a health care (p = 0.0055), and those with a level of education of some college or greater (p = <0.0012) were more likely to have adequate musculoskeletal health literacy on multivariable logistic regression. Only at least some college experience correlated with adequate general health literacy (p = 0.003) upon multivariable logistic regression. Conclusion: Almost half of all caregivers who bring their children to a pediatric orthopedic clinic have limited musculoskeletal health literacy and may lack the necessary comprehension required for making informed decisions about their child’s care. Limited musculoskeletal health literacy is more prevalent than limited general health literacy.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"20 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127849013","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Our aim was to evaluate the functional and radiological outcome of patients with Stage 2 and 3 Kienbock’s disease who underwent vascularised bone graft from 4, 5 extensor compartement artery to lunate. Methods: We retrospectively reviewed patients, who underwent vascularised bone grafts (VBG) using 4 and 5 th extensor compartment artery for Kienbock’s disease, between 2013 and 2019. We excluded those secondary to trauma, negative ulnar variances and advanced stage of disease. We used the Visual Analogue Score (VAS) for pain, qDASH for the functional outcome and radiological assessment at final follow up. Results: Ten patients underwent VBG for Kienbock’s disease stage 2 and 3a. They were all operated by a single specialist hand surgeon, using the same surgical technique where a bone graft was harvested from the distal dorsal radius with vascular pedicle from the 4 th and 5 th extensor compartment artery. The average age at surgery was 40.6 years. There were 7 males and 3 females. The mean follow-up is 30 months. At the final follow up the mean VAS is 2.6 out of 10 against 9.5 pre-operatively and mean qDASH is 25.4/100 against 70.7/100 pre-operatively. Radiographs at the final follow up showed no progression of disease. All patients reported improvement in range of movement following surgery. Conclusions: Our study has demonstrated that vascularised bone graft is a viable treatment option for Kienbock’s disease stage 2 and 3 on a carefully selected group of patients with improvement in pain and function.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"33 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115747631","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The present study was done to compare the effectiveness of the suprascapular nerve block (SSNB) under ultrasound guidance and hydrodistension in the management of AC. Materials and Methods: This prospective randomized control study was conducted in 60 patients visiting the Department of Orthopaedic Surgery, BARC Hospital, Mumbai, with AC not improving with physiotherapy. Patients were divided into Group A (n = 30) who received SSNB under ultrasound guidance in addition to physiotherapy and Group B (n = 30) who underwent hydrodistension of shoulder in addition to physiotherapy. Values for the ROM, Quick DASH score, and visual analog scale (VAS) score were obtained for each patient at the baseline and at 4, 8, and 12 weeks. Results: Female preponderance was observed in both the groups and overall 22% werediabetics. The difference in improvement in flexion, abduction, external rotation, and internal rotation from baseline to 12 weeks, 4 to 8 weeks, 4 to 12 weeks, and 8 to 12 weeks was higher in hydrodistension group as compared to SSNB group (p < 0.05). The decrease in the VAS and Quick DASH scores from baseline to 12 weeks was higher in hydrodistension group as compared to SSNB group (p < 0.05). Conclusion: Both SSNB and hydrodistension are useful in the management of AC. However, hydrodistension displayed better outcome as compared to SSNB in improving the functional outcome of patients.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"215 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132417619","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this systematic review was to identify and critically analyse studies examining the functional outcomes and complications of operative versus non-operative treatments for these common injuries. Materials and Methods: A literature search was performed, using MEDLINE, EMBASE, PubMed, Cochrane Central databases and Google Scholar for relevant articles published between January 2005 and December 2015. Results: 253 studies were identified, of which 23 met the rigorous inclusion and exclusion criteria. No significant differences in functional outcomes between the treatment groups were reported by any of the studies. There was a 21.4% complication rate with operative treatment and a 7.2% complication rate with non-operative treatment. Conclusion: This systematic review demonstrates that there is an increasing tendency towards operative management over non-operative treatment during the last decade, especially in older children and adolescents.","PeriodicalId":252930,"journal":{"name":"Hong Kong Journal of Orthopaedic Research","volume":"40 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123937878","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
