Çocukluk obezitesi için obezojenik aile uygulamaları, çocuk obezitesi ve depresif belirtiler arasındaki ilişkiyi incelemeyi amaçladık.\u0000\u0000Gereç ve Yöntemler: Bu araştırmayı, Ekim 2021 - Temmuz 2021 tarihleri arasında çocuk kliniğimize başvuran 7-15 yaş aralığında olan 96 çocuk ve onların ailesi üzerinde gerçekleştirdik. Çalışmaya katılmayı kabul eden ebeveynler ve çocuklar, sosyodemografik verilerini, çocukların kilo ve boy bilgilerini, “Çocuklar İçin Depresyon Envanteri (CDI)” ve “Aile Beslenmesi ve Fiziksel Aktivite Tarama Aracı (FNPA)” içeren bir online anketi doldurdular.\u0000\u0000Bulgular: Bu çalışma, çocukluk çağı obezitesi için daha yüksek riskli aile uygulamalarının, çocuklarda artan depresif belirtilerle ilişkili olduğunu bulmuştur (r=-0.279, p=0.006). Obezojenik aile ortamı, çocuklarda daha yüksek depresif puanlarla ilişkiliydi. Obezojenik aile ortamına sahip çocukların akıllı telefonlarda daha fazla zaman geçirdikleri de doğrulandı (p<0.001).\u0000\u0000Sonuç: FNPA, obezojenik aile faktörlerini belirlemede kolay uygulanabilir bir araçtır. COVID-19 Pandemisinin Türkiye’de Tipik Gelişen Çocukların Uzun Dönem Davranış Sorunları Üzerindeki Etkisi

Özmert","doi":"10.12956/tchd.1232315","DOIUrl":"https://doi.org/10.12956/tchd.1232315","url":null,"abstract":"Objective: Pandemics such as COVID 19, cause severe stress for both parents and children due to social constraints, changes in daily routines, and economic challenges, and can lead to long-term developmental and behavioral problems in children. We evaluated the behavioral problems of typically developing healthy children at the end of the second year of the pandemic, as well as the effect of family sociodemographic characteristics.\u0000\u0000Materials and Methods: This case-control study included 28 children between the ages of 18 to 60 months who admitted to Hacettepe University İhsan Doğramacı Children’s Hospital between January and February 2022 and 23 children from the pre-pandemic period. All of the children scored normally on the Ages and Stages Questionnaire (ASQ) in all developmental domains. The Child Behavior Check List (CBCL) was completed by parents. \u0000\u0000Results: In terms of family sociodemographic characteristics, no significant difference existed between the two groups. Anxiety-depression scores were significantly higher in the post-pandemic group once CBCL scores were assessed (p=0.047). In the final stepwise multiple regression model, maternal education level was significantly associated with lower anxiety-depression scores (p=0.030). It was revealed that a maternal education level above high school reduced the anxiety-depression scores by 2.53 points in the post-pandemic group.\u0000\u0000Conclusions: Families, pediatricians, and all other healthcare providers need to closely monitor preschoolers, who are among the groups most vulnerable to the negative effects of the pandemic. The number of pregnant women infected by new type of coronoviruses have been on the rise. COVID-19 infection in pregnancy can be experienced with different symptoms as like asymptomatic infection, mild disease (no evidence of pneumonia or hypoxia), moderate disease (viral pneumonia), severe disease (severe pneumonia) or critical illness (acute respiratory distress syndrome, sepsis, septic shock, or complications such pulmonary embolism or acute coronary syndrome). Fortunately, most pregnant women infected with SARS-CoV-2 are asymptomatic, and most symptomatic women experience only mild or moderate cold/flu-like symptoms. The effect of these viruses on the fetus, virus transmission from mother to baby and the protective role of antibodies are not clear yet. Although, the majority of newborns were asymptomatic; tachypnea, most likely secondary to transient tachypnea observed as the most common symptom. On the other hand, late complications of the Coronovirus disease-19 can be presented in some organs and/or systems like heart, brain, lung, gastrointestinal system. Age and gender distribution of patients, complaints at the time of visits, clinical characteristics, diagnoses, diagnostic examinations and treatment methods were recorded.\u0000Results: Of the 241 patients, 164 (68.0%) were male and 77 (32.0%) were female. The median age was 11.0 (3.0-15.0) years. According to age groups, the highest number of patients was in the 12-17 age group with 117 (48.5%) patients. The most common complaints were foreign body in eye with 30.3% (n=73), eye redness with 10.8% (n=26), sticky eye with 8.3% (n=20) and eyelid swelling with 8.3% (n=20). The most common diagnoses were foreign body (31.1%, n=75), conjunctivitis (26.1%, n=63) and corneal epithelial defect (10.0%, n=24). Considering the distribution of diagnoses by age groups, conjunctivitis was the most common in the 0-5 and 6-11 age groups, foreign body in the 12-17 age group. While medical treatment was given to 59.3% (n=143) of the patients, intervention under local anesthesia was performed for 29.5% (n=71) of the patients. Of the seven patients that were hospitalized, five of them were treated under general anesthesia and two of them received surgical operations.\u0000Conclusion: Pediatric emergency visits with eye complaints in children were most common between 12-17 years of age and the most common cause was foreign bodies in the eye. All hospitalized patients were 0-5 years old and the most common reason for hospitalization was foreign bodies. Aslan, Harun Demirci","doi":"10.12956/tchd.1252646","DOIUrl":"https://doi.org/10.12956/tchd.1252646","url":null,"abstract":"Objective: In our study, it is aimed to examine the effect of surgery on brain perfusion with ASL sequence perfusion MRI taken before surgery and 12 weeks after surgery in cases of non-syndromic craniosynostosis treated in our clinic.\u0000Materials and Methods: The study was conducted between 28.12.2021-16.09.2022 with 10 patients who evaluated preoperatively and treated surgically at the Neurosurgery Clinic of Ankara City Hospital. Perfusion MRI was performed preoperatively and 12 weeks postoperatively. The preoperative and postoperative images obtained were evaluated comparatively at the workstation. \u0000Results: After the exclusion of patients who did not appropriate the study criteria, the results of 9 patients were examined. MRI scans taken preoperatively and 87 days as average postoperatively. After surgery, there is perfusion enhancement in 7 patients and a perfusion impairment in 2 patients. About the average perfusion rates, the results were found to be statistically significant in four of the seven patients in whom an increase was detected (p≤0.05), while the results were found to be statistically significant in one of the patients in whom a decrease was observed. (p≤0.05)\u0000Conclusion: Surgical treatment is effective on brain perfusion in patients with non-syndromic craniosynostosis. Perfusion alteration may be different according to surgical technique, patient age at the time of surgery and type of craniosynostosis. In addition to cosmetic improvement, surgical treatment can also be effective in changing neurological functions. In this study, we discuss our initial experiences with\u0000pediatric robotic surgical cases.\u0000Material and Methods: Robotic-assisted endoscopic surgery between November 2017 and April 2022 was\u0000retrospectively reviewed. The medical records of the patients were reviewed retrospectively for some demographical\u0000features, surgical details, and follow-up time.\u0000Results: Of the 48 children, 27 were male and 21 were female. The youngest of the cases was 10 months old,\u0000while the oldest was 17 years old (mean 7.72±5.6, median 7). The shortest follow-up period after surgery was 5\u0000months, while the longest follow-up was 60 months. A total of 63 surgical procedures were performed on 48 pediatric\u0000patients. 17 pyeloplasty (left 10, right 7), 9 Lich Gregoir operations (right 6, left 3), 4 ureteral reimplantation and tapering\u0000procedures due to left ureterovesical junction obstruction (UJO), 3 Hemi nephrectomy, one appendicovesicostomy, One\u0000right ureteral reimplantation, 11 Nissen fundoplication, 10 gastrostomy and one gastrojejunostomy, two ovarian sparing\u0000surgeries for ovarian cyst were performed. One urachal remnant excision, one adrenal mass excision, and one thoracic\u0000outlet mass excision were performed.\u0000Conclusion: This is the first study that includes a wide range of pediatric robotic surgical procedures in our country.\u0000Robotic-assisted laparoscopic procedures in pediatric surgery can be safely performed for many surgical pathologies. Cripple hypospadias are the patients who have a\u0000history of previous multiple unsuccessful hypospadias surgery and still having anatomical and functional complications.\u0000Insufficient local or support tissue for the repair of defective urethral plate is always a problem.\u0000Material and Methods: In the study fourteen male 8-10 months old which is weighting four kg, New Zealand type\u0000rabbit are divided into two equal groups. Juguler vein graft (1st group) and buccal mucosal graft (2nd group) harvested\u0000and transferred to defective area in the urethral plate as a free flap.\u0000Results: In the histopatologic examination, epithelization, fibrosis and inflammation degrees were evaluated between\u0000the groups and in-groups. In the histopatological examination of the 1st group, uroepithelization is complete or nearly\u0000complete, fibrotic activity is (+) and the inflammation was found minimal. According to the feeding pattern, the patients who fed only breast-fed were classified as Group I, and the patients who fed with combination of breast-fed and formula were classified as Group II. Results were retrospectively compared between groups.\u0000Results: A total of 88 children undergone detailed ophthalmological examinations, of which 40 (45.5 %) were males and 48 (54.5 %) were females. Maternal birth age of 27.3% (n=24) of the children included in the study was over 30 years and birth weight of 96.6% (n=85) was over 2500 grams.\u0000The mean ± standard deviation of the spherical, cylindrical and spherical equivalent (SE) values were 1.61±1.46 D; -0.98 ± 0.69 D and 1.12±1.43 D in group I and 1.00±2.57 D; -0.90 ± 0.81 D and 0.55 ± 2.72 D in group II, respectively (p=0.507 for spherical; p=0.299 for cylindrical and p=0.799 for SE). Although myopic refraction was less common in the breastfed group, there was no significant difference in myopic refraction between the groups (p= 0.507). \u0000Conclusion: In conclusion, mean refractive error were different according to feeding pattern, but this study did not show significant differences between groups (p<0.050). Because of the devastating consequences of the pathology aggressive treatment can prevent permanent renal damage. Fifteen-year-old-girl patient without medical follow-up applied to emergency room with fever and vomiting. Computed Tomography (CT) revealed EPN. After diagnostic procedure minimal invasive procedures like double J catheter and nephrostomy tube insertion failed. While taking into account family’s treatment and medical follow-up compliance, also severe dilatation in both ureters made bilateral ureterocutaneostomy (UC) to be considered as a feasible option. UC creation can be thought as a safe and effective alternative surgical procedure. Pediatric neurogenic bladder (NB) population incompatible to treatment and follow-up are candidates for EPN development. Metotreksat (MTX), leflunomid (LFN) gibi hastalık modifiye edici antiromatizmal ilaçlar (DMARD) JİA'da birinci basamak tedavilerdir. MTX en sık reçete edilen ilaçtır ve çalışmalar ağırlıklı olarak MTX etkinliğini ve güvenliğini ele almaktadır. Ancak LFN ile ilgili veriler sınırlıdır. Bu çalışmada, JİA hastalarında LFN tedavisi ile ilgili kliniğimizin deneyimlerini sunmayı amaçladık. \u0000Gereç ve Yöntem: Bu retrospektif çalışmaya hastanemiz çocuk romatoloji polikliniğinde düzenli olarak takip edilen ve LFN tedavisi verilmiş JİA hastaları dahil edildi. Hasta demografik bilgileri, klinik ve laboratuvar özellikleri ile ilgili veriler tıbbi dosyalardan elde edildi.\u0000Bulgular: Çalışmaya ortanca (çeyrekler arası aralık) hastalık başlangıç yaşı 7,3 (3,1-12,0) yıl olan 18 hasta (15 kadın ve 3 erkek) dahil edildi. 8 hastada oligoartiküler JİA, 7 hastada poliartiküler JİA, 2 hastada sistemik JİA ve 1 hastada entezitle ilişkili artrit (ERA) vardı. Tüm hastalara başlangıç tedavisi olarak MTX verildi (ERA tanısı konan bir hasta sulfasalazin ile tedavi edildi hariç). Gastrointestinal sistem (GİS) intoleransı nedeniyle başlangıçta MTX alan tüm hastalarda MTX kesildi ve LFN tedavisi başlandı. Daha önce MTX alırken GİS intoleransı gelişen hastalık aktivitesi düşük olan yedi hastadan altısına LFN tedavisi verildi. Bu hastalarda LFN ile tam remisyon sağlandı. MTX ile remisyonda izlenen dört hastada hastalık aktivasyonu görüldü. Daha önce MTX intoleransı olan bu hastalara LFN tedavisi verildi. Dört hastanın üçünde LFN ile remisyon sağlandı. MTX ile remisyon sağlanamayan orta ve yüksek hastalık aktivitesine sahip altı hastaya biyolojik tedavi başlandı. Yeterli yanıt alınamayan bu hastalarda MTX kesilerek LFN tedavisi başlandı. LFN ve biyolojik ajan kombinasyonu ile sadece bir hastada inaktif hastalık elde edildi.
