Malnutrition is demonstrated to be accompanying to several disease, yet there is a lack of evidence on the impact of malnutrition on cardiac functions in patients with diabetic patients with diabetic retinopathy. Herewith our primary goal was to evaluate the relation between any degree malnutrition with left atrial strain in relevant patient group.\u0000 Patients and Methods: Patients with diabetes mellitus with retinopathy, who were referred to the outpatient cardiology clinic were assessed. Those who had sinus rhythm, had no findings of significant valvular heart disease and coronary artery disease underwent to 2-dimensional speckle tracking echocardiography and assessed for malnutrition in their prior examinations were included to the study. Hundred and seventy patients met the inclusion criteria. Malnutrition scores such as CONUT score, NRI, and PNI were used to evaluate nutrition condition of the patients.\u0000 Results: Thirty percent of all diabetic patients were demonstrated to have malnutrition according to the CONUT score. In fact, the postextubation period in this group of patients can be complex and the use of continuous positive airway pressure or non-invasive ventilation may be necessary if conventional oxygen therapy is not sufficient. The aim of our study was to compare efficiency and outcomes of high-flow nasal cannula or conventional oxygen therapy post-extubation after pediatric cardiac surgery .\u0000 Patients and Methods: A single centre retrospective study was conducted between January and december 2020 in our 12 beds pediatric cardiac intensive care unit. Patients were divided into two groups. In one group 45 patients (Group I), those who received high-flow nasal cannula after extubation, and in the other group 45 patients (Group II), those who received oxygen therapy with a mask after extubation. The aim of the study was to evaluate the relative efficacy of high flow nasal cannula and conventional oxygen therapy on PaCO2 ,PaO2 and PaO2/FiO2.\u0000 Results: PaO2 values at 1, 6, 12, 24 and 48 h post-extubation were significantly higher in high-flow nasal cannula group (p<0,05). PaCO2 values were significantly lower in the high-flow nasal cannula group at 1, 6, 12, 24 and 48 h post-extubation (p=0,01). PaO2/FiO2 values in high-flow nasal cannula group at all-time points post-extubation were significantly higher than in conventional oxygen therapy group (p=0,01).\u0000 Conclusion: High-flow nasal cannula is useful in decreasing PaCO2 and improving PaO2 in children following extubation after cardiac surgery. In addition, the simplicity of and tolerability to high-flow nasal cannula is also important. Data were collected from the patient information form and HLB Scale-II. The patients were visited by the researcher one day before their discharge and verbal information was given about the HLB. Discharge information calendar that is designed as a desktop calendar and includes 14 main headings on HLB was used to inform the patients in the study group. HLB of the patients were evaluated at the day before discharge, 2nd and 6th weeks following discharge.\u0000 Results: The mean scale scores obtained from the 2nd and 6th weeks of the study group were significantly higher compared with the control group (p<0.001).\u0000 Conclusion: The information provided using discharge information calendar to the patients after CABG surgery are effective in gaining HLB for the patients. These patients might be needed advanced treatment methods to overcome vasospasm. Ertürk","doi":"10.51645/KHJ.2021.M107","DOIUrl":"https://doi.org/10.51645/KHJ.2021.M107","url":null,"abstract":"Objectives: Redo mitral valve replacement (redo-MVR) represents a clinical challenge due to a higher rates of peri-operative morbidity and mortality.\u0000 Patients and Methods: This retrospective study enrolled a total of 103 patients who underwent isolated redo-MVR due to prosthetic valve dysfunction. Patients who had an isolated bypass, low echocardiographic quality, history of repeated re-replacements (more than twice), paravalvular leak repair without preoperative and intraoperative transesophageal echocardiography examination, isolated congenital surgery or isolated open-heart surgical intervention (of any type) without a valve procedure at their first or later operations were excluded. The primary endpoint of the study was in-hospital death. Secondary endpoint included individual morbidity.\u0000 Results: A total of 103 patients (mean age: 50.7 13.4 years; male: 58) who underwent isolated redo-MVR were enrolled in this study. The most common complaint of the patients at admission was obstruction or heart failure-related symptoms (80.6%) and the primary indication for redo-MVR was prosthetic valve thrombosis in 58 patients (56.3%). In-hospital mortality was 12.6% (13 patients). The post-operative complications included major bleeding (n=11) post-operative infection (sepsis, mediastinitis, pneumonia, wound infection, n=15), low cardiac output syndrome (n=10), acute kidney injury (n=17), pericardial effusion with tamponade (n=10), pleural effusion requiring hospitalization and drainage (n=18), ischemic stroke (n=4), fatal ventricular arrhythmia (n=1), peripheral embolism (n=1), moderate to severe paravalvular leak (n=5). There was not any catastrophic heart laceration.\u0000 Conclusion: In-hospital mortality and complications of the isolated redo-MVR in our center are acceptable. {"title":"Brugada electrocardiographic pattern unmasked by COVID-19 induced fever","authors":"Cemalettin Yılmaz, G. On admission, the patient was conscious with a 37.8°C fever, an 108 bpm heart rate and 135/75 mmHg blood pressure and normal respiratory findings (SaO2 95%). The electrocardiogram (ECG) showed a ST-segment elevation in the right precordial leads with no reciprocal changes (Figure 1). An echocardiogram demonstrated a mildly depressed global ejection fraction. Due to the persistence of substernal chest pain and ST-segment elevation, emergent coronary angiography (CAG) was performed, showing normal coronary arteries. After the diagnostic CAG, the patient was transferred to a dedicated COVID-19 intensive care unit. The patient had a 39.1°C fever. ECG was repeated and it revealed coved ST-segment elevation in lead V1 and V2 with a rise of the J-point by 0.25 mV indicative for Brugada type 1 ECG (Figure 2). He denied syncope, dizziness, or palpitations and there was no history of arrhythmic diseases in his family. Laboratory data were unremarkable. The high-sensitivity troponin level was within the normal range along with normal electrolytes. The N-terminal probrain natriuretic peptide (NT-proBNP) level was normal. The C-reactive protein (CRP) level was 40.2 mg/L (normal range: 0-5 mg/L). In order to investigate the cause of fever, the patient was tested for COVID-19 and chest computerized tomography (CT) scan was performed. Chest CT scan showed bilateral pulmonary parenchymal ground glass opacities of the lower lobes consistent with COVID-19 infection (Figure 3). Twenty-four hours after nasoand oropharyngeal swabs, the patient tested positive for COVID-19 by polymerase chain reaction (PCR). He received favipravir, hydroxychloroquine and antipyretic therapy. No anti-arrhythmic treatment was initiated. With defervescence, the ECG demonstrated complete resolution of the initial Brugada-like ECG pattern (Figure 4). He was discharged from the hospital after the 7-day hospital stay when second PCR revealed a negative result. The patient had no prior history of ventricular arrhythmias or syncope and no family history of sudden cardiac death. Since the risk of sudden death in patients with asymptomatic Brugada pattern is low, defibrillator implantation was not done.
