An aortic dissection, starting from the RCA ostium and extending to both right sinus valsalva and the ascending aorta (AA) occurred due to deep guiding catheter intubation during post-dilatation with non-compliant balloon. Immediately, another coronary stent was implanted in the RCA ostium and plunged into the AA by 2 mm, with a purpose of covering the entry point of the dissection. The patient underwent emergency surgery consisting of AA replacement and RCA by-pass grafting due to insufficient response for stenting. Mitral valve repair or mitral valve replacement is recommended for symptomatic patients. Preservation of the subvalvular apparatus has the merits of postoperative outcomes during mitral valve replacement for mitral regurgitation. We report a case of a remnant mitral subvalvular apparatus detected by echocardiography after chordal preserving mitral valve replacement. It was performed as mitral valve replacement with posterior leaflet chordal preservation in a 44-year-old male patient due to rheumatic, symptomatic severe mitral regurgitation. On the third postoperative day, routine postoperative trans-thoracic echocardiography showed an unknown mobile mysterious formation in the left ventricle. We clarified the mysterious mass with the help of laboratory, blood culture and serial echocardiographic assessment and presented this case. We emphasize the importance of post-operative echocardiographic evaluation and a complete evaluation of the patients who have undergone mitral valve replacement. Recent studies have shown that MPV is increased in myocardial infarction, unstable angina, and stroke. Peripheral artery disease (PAD) manifests with the narrowing or obstruction of the abdominal aorta and arteries distal to the aortic bifurcation due to progressive atherosclerosis. The objective of this study is to investigate the MPV and platelet count in patients with PAD. Patients and Methods: This study included 67 patients aged 40-80 years who underwent elective surgery for PAD in our hospital between January 2018 and June 2019, along with a control group comprising 67 patients without PAD. The MPV and platelet count values were obtained from a preoperatively performed hemogram analysis. Importantly, the groups were statistically compared in our study. Results: As compared to the control group, the patients with PAD had a significantly higher mean age (p= 0.02), prevalence of diabetes mellitus (p 0.05). The mean platelet count and MPV were also significantly higher in patients with PAD as compared to patients without PAD (p< 0.01). Koramaz","doi":"10.5578/khj.69690","DOIUrl":"https://doi.org/10.5578/khj.69690","url":null,"abstract":"Introduction: The Department of Cardiovascular Surgery at the Sisli Hamidiye Etfal Training and Research Hospital was founded in 2014, and the first open heart surgery was performed on November 19, 2014. We aim to evaluate the results of the first 450 open heart surgeries retrospectively. Patients and Methods: Records of the first 450 consecutive open heart surgeries performed between November 2014 and October 2019 were retrospectively evaluated. All patients were treated with acetylsalicylic acid (100 mg/day) up to the preoperative day, and those who had acute coronary syndrome were continued to be treated with clopidogrel (75 mg/day). In all cases, the surgeries were performed through median sternotomy. The standard cardiopulmonary bypass technique was used in all patients who underwent coronary artery bypass grafting (CABG). Antegrade blood cardioplegia was given at 20-minute intervals to achieve a diastolic arrest of cardiac activity. In patients who underwent CABG, proximal anastomoses were performed under a side clamp, and sometimes, with single clamp technique depending on the patient characteristics. Results: The mean age of the patients was 58.7 years. The most common comorbidity seen in the patients was hypertension (62.9%). Other comorbid diseases observed in the patients were diabetes mellitus (43.6%), hyperlipidemia (28.0%), chronic obstructive pulmonary disease (24.9%), peripheral arterial disease (13.1%), and chronic renal dysfunction (4.7%). In-hospital death occurred in 33 of 450 patients with a mortality rate of 7.3%. Patients and Methods: In total, 490 patients who were recommended to undergo the exercise electrocardiographic test were recruited in this study. The participants were divided into two groups based on both Cri (normal and low CRi groups) and rHGSi (high and low rHGSi groups). Results: The treadmill exercise time, peak heart rate, and metabolic equivalents were higher in the high rHGSi group than in the low rHGSi group. In addition, the 1st minute HRRi and Cri were higher in the high rHGSi group than in the low rHGSi group. The treadmill exercise time, basal and peak heart rate, metabolic equivalents, and 1st minute HRRi were significantly lower in the low CRi group than in the normal Cri group. rHGSi was considered an independent predictor of low chronotropic response index. Based on a bivariate analysis, a statistically significant positive correlation was observed between Cri and rHGSi. Similarly, there was a positive association between rHGSi and 1st minute HRRi. Contemporary studies suggest that concomitant biatrial lesion sets produce long-term sinus conversion rates of up to 90%. This procedure holds the potential to improve life quality and even survival of the patients; however, many surgeons are reluctant to perform the same. More than half of the patients with atrial fibrillation undergoing cardiac surgery for different reasons never receive any kind of intervention for atrial fibrillation, which means missing a once-in-a-lifetime opportunity to treat atrial fibrillation. Rate control with anticoagulation remains the only alternative treatment for these patients. Anticoagulation can be a relatively safe option at the time of surgery but may become dangerous or even a contraindication with the patient’s increasing age and a possible development of new diseases. Sinus rhythm can become lifesaving at that point. The objective of this report is to describe a successful surgical treatment of a 74-year-old woman with severe mitral and tricuspid valve insufficiency along with atrial fibrillation. She had mild leucopenia and anemia at the time of surgery without any hematological diagnosis. She was diagnosed with myelodysplastic syndrome six months after the surgery. In addition to valve repairs, the maintenance of sinus rhythm prompted hematologists to treat her without using any anticoagulation medicines. Patients and Methods: One hundred forty-nine patients underwent proximal aortic root replacement with composite valve-conduit graft between 1995 and 2014. Bentall procedure and its modifications (Cabrol procedure and hemi-inclusion) were evaluated for short- and long-term outcomes. Patients with concomitant procedures, arch repair, or valve-sparing procedures were excluded from the study. Results: In this study, 64 patients have met the inclusion criteria: 20 patients were included in the button Bentall group, 32 patients in the hemi-inclusion Bentall group, and 12 patients in the Cabrol group. Preoperative patient demographics were similar in all groups. The hemi-inclusion technique had better early outcomes, including less intraoperative blood transfusion, short cardiopulmonary bypass, and better in-hospital mortality rates (p 0.05). Fenofibrate that is used for the treatment of hypertriglyceridemia can prevent cardiovascular events in patients with CAD. However, there is little information regarding the vascular effects of fenofibrate on arterial wall stiffness in patients with hypertriglyceridemia and without CAD, diabetes mellitus (DT), and hypertension (HT). The objective of this study is to evaluate the effects of fenofibrate treatment on the arterial stiffness in the patients with pure hypertriglyceridemia. Patients and Methods: We included 37 patients with hypertriglyceridemia without CAD, HT, and DT in this study. We performed pre- and post-treament physical examination of the patients and took their blood samples. Patients were allocated fenofibrate for a duration of 168 ± 14 days for its administration. We assessed arterial stiffness by aortic pulse wave velocity (PWV) using a SphygmoCor device. Importantly, we estimated central arterial pressure waveform parameters by radial artery applanation tonometry and used augmentation index (AIx) as a measure of wave reflections. Results: Fenofibrate treatment resulted in significantly greater reductions in total cholesterol (201.3 ± 61.0 mg/dL vs. 270.0 ± 93.4 mg/dL), triglycerides (261.3 ± 234.3 mg/dL vs. 704.7 ± 338.7 mg/dL), and the C/H levels (5.3 ± 2.6 vs. 7.2 ± 1.9, respectively) as compared with the pretreatment levels (p 0.05). There was a significant reduction in PWV after fenofibrate treatment (11.3 ± 2.9 m/s vs. 9.2 ± 2.2 m/s, p= 0.001). The purpose of this paper is to discuss the aortic arch reconstruction in the coarctation of the aorta and AAH while focusing on the surgical decisions and patient management for the neonatal and infant patients. Patients and Methods: We performed a retrospective review of 15 patients who underwent aortic arch reconstruction. Importantly, we investigated postoperative outcomes with associated complex congenital heart diseases. Results: The median age and mean weight of the patients were 74 days (range: 4-306 days) and 4192 ± 1253 gram, respectively. The surgical correction of aortic arch congenital abnormalities was performed under selective antegrade cerebral perfusion in all the 15 patients. All the reconstructions were performed with pericardial patch, and the coarcted segment of the aorta was completely resected in seven of the patients. One-stage repair was performed in the seven of the patients. The mean follow-up time was 13.7 ± 9.3 months. In one patient, transcatheter balloon angioplasty was performed for re-coarctation six months after operation. There was one early and one late mortality.
