{"title":"[A survey of awareness of guidelines and consensus and an analysis of the effectiveness of methodology training among medical staff in otorhinolaryngology head and neck surgery].","authors":"H Liu, Y Y Yao, Z J Wang, Q L Shi, Y S Qin, H H Lai, L Ge, Y L Chen","doi":"10.3760/cma.j.cn1115330-20250419-00238","DOIUrl":"https://doi.org/10.3760/cma.j.cn1115330-20250419-00238","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the awareness of clinical practice guidelines (referred to as \"guidelines\") and experts consensus (referred to as \"consensus\") among otorhinolaryngology head and neck surgery medical staff, and to analyze the effectiveness of the ninth guideline methodology workshop. <b>Methods:</b> Self-designed survey questionnaires were used to conduct two surveys among otorhinolaryngology head and neck surgery medical staff who participated in the ninth guideline methodology workshop held in Chengdu from Jannuary 10 to 11, 2025, both before and after the training, to assess their awareness and knowledge of guideline and consensus development methodology. The effectiveness of the training was evaluated using the paired chi-square test. <b>Results:</b> A total of 77 peoples (95.06%), who participated in the ninth guideline methodology workshop, completed the survey questionnaires both before and after the training. One-third of the respondents reported a relatively good to very good understanding of guideline and consensus development. The overall methodology of the guidelines and consensus they had participated in developing still showed room for improvement. After the methodology training, the score rate for each test question significantly increased compared to before the training (<i>P<</i>0.05), and the distribution trend of the total scores changed markedly (Before the training, 71 people scored less than 60 points, 4 people scored 60 points, and 2 people scored 70 points. After the training, only 1 person scored less than 60 points, 1 person scored 60 points, and 75 people scored between 70 and 100 points, of which 21 people scored 100 points). <b>Conclusion:</b> The awareness of guideline and consensus development methodology among otorhinolaryngology head and neck surgery medical staff still has room for improvement. {"title":"[Comparison of the modified Kadish, AJCC T and Dulguerov T staging systems for olfactory neuroblastoma: analysis of the SEER database].","authors":"H H Lyu, X Wen, J T Lin, R H Fang, R He, M Y Chen, Y H Wen, W P Wen","doi":"10.3760/cma.j.cn115330-20240723-00438","DOIUrl":"https://doi.org/10.3760/cma.j.cn115330-20240723-00438","url":null,"abstract":"<p><p><b>Objective:</b> To compare the Kadish T staging, AJCC T staging, and Dulguerov T staging system in terms of their impact on surgical treatment selection and survival prognosis in patients with olfactory neuroblastoma (ONB). <b>Methods:</b> The cases of pathologically confirmed ONB from the Surveillance, Epidemiology, and End Results (SEER) database between 2000 and 2018 were collected and screened. Tumors were staged according to Kadish staging system, AJCC T staging and Dulguerov T staging guidelines. Kaplan-Meier analysis was used to calculate 5-and 10-year overall survival rates for different stages, and the log-rank test was used to detect statistically significant differences. Multivariate analysis was performed using Logistic regression and Cox regression models to explore factors influencing surgical treatment choices and prognosis in ONB patients. <b>Results:</b> A total of 519 ONB patients with complete data available for analysis were included in the study. Multivariate analysis revealed that tumor staging, age, and marital status were closely associated with surgical treatment selection. The 10-year survival rates for patients in stage A, B, and C were 74.1%, 68.7%, 55.0%, respectively. The multivariate analysis failed to show a significant prognostic gradient between adjacent stages in any of the three staging systems. <b>Conclusions:</b> The selection of surgical treatment for ONB is influenced by clinical characteristics such as tumor stage and age. {"title":"[Investigation on the prevalence and risk factors in adults associated with allergic rhinitis in Yinchuan].","authors":"X Zhang, Y B Gao, J Y Li, Y Zhang, L Zhang","doi":"10.3760/cma.j.cn115330-20240815-00477","DOIUrl":"https://doi.org/10.3760/cma.j.cn115330-20240815-00477","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the epidemiological characteristics of allergic rhinitis (AR) among adults in Yinchuan, focusing on its prevalence and associated risk factors in recent years. <b>Methods:</b> A cross-sectional epidemiological survey was conducted among adult residents in Yinchuan from June to October 2022, including three districts, two counties, and one city, to identify and analyze factors influencing the incidence of AR in the population. <b>Results:</b> A total of 3 345 individuals were surveyed, including 1 891 males (56.5%) and 1 454 females (43.5%). The overall AR prevalence was 17.2% (576/3 345). The self-reported prevalence was 16.2% (306/1 891) in males and 18.6% (270/1 454) in females, with no statistically significant difference (<i>χ</i><sup>2</sup>=3.29, <i>P</i>=0.07). The prevalence was significantly lower among the elderly (aged over 60 years) at 11.8% (60/509) compared to the middle-aged (aged 40-59 years) at 17.1% (202/1 183) and young adults (aged 18-39 years) at 19.0% (314/1 653, <i>χ</i><sup>2</sup>=10.36, <i>P</i>=0.023). Han nationality had the highest self-reported prevalence (18.8%, 446/2 372), followed by other ethnic minorities (17.6%, 13/74), and Hui nationality (13.0%, 117/899; <i>χ</i>²=11.21, <i>P</i>=0.004). Regionally, Xingqing District had the highest self-reported prevalence at (19.5%, 188/964), followed by Jinfeng District (18.7%, 151/808), Lingwu City (17.5%, 58/332), Xixia District (14.9%, 78/522), Yongning County (14.3%, 49/342), and Helan County (13.8%, 52/377; <i>χ</i><sup>2</sup>=35.68, <i>P</i>=0.038). In terms of disease severity, mild cases made up 55.4% (319/576), while moderate to severe cases constituted 44.6% (257/576). In terms of symptom characteristics, perennial AR patients accounted for 18.8% (108/576), seasonal AR patients accounted for 81.2% (468/576), and 25.7% (148/576) of self-reported AR patients had other allergic diseases. Regarding diagnosis and treatment, 38.7% (223/576) of patients underwent allergen tests, 65.1% (375/576) received medication, but 62.3% (359/576) reported symptoms persisting for 2-year post-treatment, 32.6% (188/576) reported uncontrolled symptoms, and 5% (29/576) experienced symptoms worsening. <b>Conclusions:</b> The study provides a preliminary understanding of the epidemiological characteristics of AR in Yinchuan. A three-axis rotating chair was used to perform vertical sinusoidal rotation on 52 healthy young adults (26 males and 26 females, aged 18-40 years) in the left anterior-right posterior (LARP) and right anterior-left posterior (RALP) semicircular canal planes. For each plane, nystagmus was induced with six combinations of different angles and velocity front and back rotation angles of ±30°,±60°,±90°, and velocities of 40°/s and 80°/s, the slow phase velocity (SPV) and their symmetry of 3D nystagmus were analyzed. SPSS 20.0 was used to compare the statistical differences in these two parameters across different stimulation protocols. <b>Results:</b> There were no spontaneous nystagmus in the 52 subjects, and all tests were finished. Except the combinations of (±30°-40°/s), three components of nystagmus were induced stably in the rest of the stimulations. The SPVs of vertical components were no statistically insignificant (<i>P</i>>0.05), and some horizontal or torsion components were statistically significant (<i>P</i><0.05). The 95% reference range of the symmetry was≤25% in the vertical and≤30% in the torsional component of the nystagmus except for (±30°-80°/s), the symmetry was 32.2% and 49.2% respectively. The trend changes of the three components were consistent, among which the vertical and torsional components induced by (±60°-80°/s) and (±90°-80°/s) were the best, the SPV value of the vertical components was higher in the latter group than the former apart from the front RALP, while no significant difference was found in the torsional components (<i>P</i>>0.05). <b>Conclusion:</b> The (±90°-80°/s) combination is the optimal method to detect the function of vertical semicircular canal in 3D chair test. When observing torsional component, the combining scheme of (±60°-80°/s) and (±90°-80°/s) is better. {"title":"[Timing of facial nerve decompression for Bell's palsy: systematic review and Meta-analysis].","authors":"Y Wang, T Pan","doi":"10.3760/cma.j.cn115330-20240625-00372","DOIUrl":"10.3760/cma.j.cn115330-20240625-00372","url":null,"abstract":"<p><p><b>Objective:</b> To compare the surgical effects of facial nerve decompression for bell's facial palsy at different times, and to provide reference for evaluating the best operation time of Bell's palsy. <b>Methods:</b> A systemic search of subject-related clinical studies published before December 2023 was conducted. Meta-analysis was performed with RevMan5.3 software for the literatures meeting the inclusion criteria to compare the efficacy of different surgical timing. Surgical outcome was defined as the effective by house-Brackmann grade Ⅰ-Ⅲ, and satisfactory recovery was defined by grade Ⅰ-Ⅱ. <b>Results:</b> A total of 10 literatures were included, including 498 cases. The overall quality of literatures was high, and no obvious publication bias was suggested. The effective rate of facial nerve decompression in general was 86.1%-100.0%, and the satisfactory recovery rate was 50.0%-80.5%. The effective rate of operation within 3 months (91.4%) was higher than that after 3 months (81.4%, <i>OR</i>=3.66, 95%<i>CI</i>: 1.53-8.73, <i>P</i>=0.003); The effective rate of operation within 6 months (90.3%) was higher than that after 6 months (69.0%, <i>OR</i>=4.34, 95%<i>CI</i>: 1.74-10.86, <i>P</i>=0.002). The satisfactory recovery rate of operation within 2 months (64.9%) was higher than that of operation group (37.9%, <i>OR</i>=2.54, 95%<i>CI</i>: 1.26-5.11, <i>P</i>=0.009). The satisfactory recovery rate of the operation group within 3 months (58.9%) was higher than that of the operation group after 3 months (15.4%, <i>OR</i>=7.14, 95%<i>CI</i>: 2.40-21.20, <i>P</i><0.001). <b>Conclusions:</b> For Bell's palsy with complete facial paralysis, facial nerve decompression is an effective treatment option. The effective rate of operation within 3-6 months after onset is higher than that after 3-6 months. The satisfactory recovery rate of operation within 2-3 months after onset is higher than that after 2-3 months. {"title":"[Evaluation of timing and short-term clinical efficacy of sinus stent implantation in chronic rhinosinusitis with nasal polyps].","authors":"H Q Zhou, L Tan, P Q Liu, Y Xu","doi":"10.3760/cma.j.cn115330-20250425-00252","DOIUrl":"10.3760/cma.j.cn115330-20250425-00252","url":null,"abstract":"<p><p><b>Objective:</b> To assess the optimal timing and short-term clinical efficacy of sinus drug-eluting stent placement in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). <b>Methods:</b> The minimum sample size was calculated using G-power 3.1.9.7 software. From March 2021 and May 2023, a total of 114 eligible patients with CRSwNP were recruited in this study at the Department of Otolaryngology Head and Neck Surgery, Renmin Hospital of Wuhan University. The patients were randomly assigned to three groups: the control group, the intraoperative stent group, and the postoperative stent group. In the intraoperative stent group, drug-eluting stents were implanted during endoscopic sinus surgery (ESS), while patients in the postoperative stent group received drug-eluting stent 2 weeks after ESS, following routine debridement of the surgical cavity. Bilateral ethmoid sinus stenting was performed for both stent groups, while the control group only underwent ESS with standard postoperative cavity debridement. All subjects were followed up at 2, 4, 8, and 12 weeks postoperatively. Nasal symptom Visual Analog Scale (VAS) scores and endoscopic evaluations of the ethmoid cavity-assessing obstruction, crusting, polypoid mucosal changes, epithelialization of ethmoid cavity, need for intervention, and complications such as middle turbinate lateralization and adhesions-were collected to evaluate the treatment efficacy of three groups. Statistical analyses were performed using GraphPad Prism 9. Analysis of variance (ANOVA) was applied to analyze continuous variables among the three groups, and chi-square tests were used for categorical variables. <b>Results:</b> Among 114 CRSwNP patients, 21 lost follow-up patients and 7 postoperative oral corticosteroid intervention patients were excluded. Finally, 86 patients were included in the analysis, including 45 males and 41 females, aged 18-65 years. The cohort comprised 29 in the control group, 29 in the intraoperative stent group, and 28 in the postoperative stent group. Successful bilateral ethmoid sinus stent implantation was achieved in both stent groups. At 4 weeks postoperatively, compared with the control group, both stent groups showed significant improvements in nasal congestion and rhinorrhea scores (<i>P</i><0.05). At 8 weeks, the postoperative group continued to demonstrate superior outcomes in these two symptoms (both <i>P</i><0.05), while the intraoperative group only showed significant improvement in nasal congestion (<i>P</i><0.05). No significant differences were observed in facial pressure, olfactory loss, or nasal dryness scores among the three groups (all <i>P</i>>0.05). Endoscopic evaluation revealed that both stent groups had significant improvements in ethmoid sinus obstruction scores at 4 weeks compared with the control group, with the postoperative group maintaining this advantage at 8 weeks (<i>P</i><0.05).
