O foco infecioso encontra-se retido numa estrutura anatómica específica onde não se consegue por vezes uma resolução permanente, o que significa que a onicomicose se torna frequentemente uma condição crónica que retoma o estado ativo no intervalo de vários anos. A prioridade nos recentes desenvolvimentos da farmacoterapia da onicomicose foi atribuída à monoterapia tópica com o intuito da sua utilização em doentes que não toleram a medicação oral ou preferem um fármaco tópico e, com a vantagem adicional, de combiná-los com fármacos orais e/ou dispositivos médicos para expandir as possibilidades de tratamento atuais. Esta revisão engloba as opções terapêuticas da onicomicose no status quo. Gonorrhea is the second most notified sexually transmitted infection in Europe, and its incidence has been increasing in the last years. Although traditionally considered a treatable infection, antimicrobial resistance of Neisseria gonorrhoeaeincludes at present also macrolides, tetracyclines, sulfonamides and trimethoprim combinations, quinolones, and even cephalosporins. These high levels of gonococcal resistance to antimicrobials resulting in untreatable infections in the future may become one of the greatest challenges to the prevention and control of sexually transmitted infections, which may be a significant major public health issue. Therefore, the development of novel antimicrobials and/or new dual antimicrobial therapy regimens is urgently needed. Occupational Allergic Contact Dermatitis is a very common occupational disease and epoxy resins are among its main causes. The aim of this study was to characterize patients with positive patch test reactions to epoxy resin detected in the Coimbra Hospital and University Center between 2012 and 2018 and compare with the results of patients patch tested between 1999 and 2008 at the same Institution. \u0000Method and Materials.  Within aretrospective analysis of the files of patients with positive patch test reactions (1+ or more intense) to epoxy resin of bisfenol A tested at 1% pet. within the Baseline Series between 2012 and 2018, we characterized demographic and clinical data of reactive patients, evaluated the relevance of the reaction, sources of exposure to epoxy resin with particular attention to occupational exposures, other positive reactions and the impact of the results of the tests in the work conditions of these patients. Lastly, we compared these results with a previous study performed in the same Hospital between 1999 and 2008. \u0000Results. Among 2363 patch tested patients during the study period we found 23 patients (0.97%), 17 males and 6 females, who developed contact allergy to epoxy resins. In 22 cases we identified a relevant occupational exposure: 9 from construction industry; 9 workers from wind-mill turbines factory for Eolic energy; 2 from fiberglass factories; 1 from a chemical factory and 1 from a Wastewater Treatment Plant. In 1 patient (a teacher) no relevance was found. Of these 22 workers, 9 (39.1%) had both hand and airborne lesions, while 8 (34.8%) had lesions exclusively on the hands and 5 (21.7%) had predominately airborne dermatitis. Four of the 23 (17.4%) reacted exclusively to the epoxy resin, and 11 of 18 (61.1%) also reacted to 0.25% hexanediol diglycidyl ether. Avoidance resulted in a significant improvement of symptoms. Compared to the previous period (1999-2008), there are no significative changes apart from a slight increase in the frequency of the allergic reactions to epoxy resins and its main source of exposure. \u0000Conclusions. The prevalence of allergic contact dermatitis to epoxy resin has slightly increased in this Portuguese Center mostly due to workers from wind-mill turbines factory for Eolic energy, a recent industry implanted in this region. Specific CLE - defined by the presence of an interface dermatitis on histopathological evaluation - is divided into several sub-types, namely acute CLE (ACLE), subacute CLE (SCLE) and chronic CLE (CCLE). CCLE includes discoid lupus erythematosus (DLE), as well as other rarer forms such as LE profundus (LEP). Nonspecific skin findings, such as livedo reticularis or purpura are more frequently seen in patients with systemic disease. Diagnosis requires classification of the subtype, through a combination of physical examination, laboratory studies, histology and sometimes direct immunofluorescence, at the same time ensuring to exclude systemic disease. Regarding the treatment of CLE, antimalarials and topical steroids continue to be the standard of care; however, immunosuppressants, thalidomide analogs and monoclonal antibodies are possible systemic therapies for recalcitrant disease. Patient education on proper sun protection and avoidance of triggers is crucial. A permanência na rua, associada a cuidados de higiene e alimentação desadequados, a elevada prevalência de comportamentos aditivos e de patologia psiquiátrica torna esta população mais suscetível a doenças dermatológicas. \u0000O objectivo do trabalho foi fazer uma observação dermatológica, sempre que possível completa, dos sem-abrigo acompanhados pelo Projeto de Intervenção com os Sem-Abrigo do Concelho de Coimbra. Em caso de ser identificada patologia dermatológica, aquisição da terapêutica instituída ou orientação para consulta de Dermatologia. Em todos os casos sensibilização e educação para a importância dos cuidados com a pele. \u0000Material e Métodos: O estudo teve lugar entre 24 de fevereiro de 2018 e 19 de janeiro de 2019 avaliando utentes, voluntariamente inscritos, a residir em centros de acolhimento temporários de Coimbra (CAIS, Farol, Casa Abrigo Padre Américo) e apoiados pela equipa de rua Reduz, pelo Centro Municipal de Inserção Social e pelas associações Sol Nascente e VHIDA +. Além da observação dermatológica, eram recolhidos dados demográficos, peso, altura e os antecedentes pessoais (ex.: doença mental, comportamentos aditivos, infeção VIH). Alguns participantes preencheram ainda o questionário Dermatology Life Quality Index (DLQI). \u0000Resultados: As 111 pessoas avaliadas tinham uma média de idades de 47,0 anos, sendo 83,8% do sexo masculino, maioritariamente solteiros (60,7%) ou divorciados (29,9%), de nacionalidade portuguesa (86,5%) e com baixa escolaridade (63,9% com habilitações até ao 6º ano). As doenças de pele mais observadas foram eczemas, tinea pedis, onicomicose, dermatite seborreica e calosidades. Dos 54 sem-abrigo observados que preencheram o DLQI, a maioria (85,1%) referia pouco ou nenhum efeito sobre a sua qualidade de vida. \u0000Conclusão: A maioria das situações que encontrámos eram ligeiras e facilmente tratáveis, o que associamos aos bons cuidados de alimentação e higiene existentes nos centros de acolhimento temporários, bem como à articulação existente entre estas unidades e os cuidados de saúde primários. É uma variante agressiva e generalizada da sífilis secundária com pródromos de febre, cefaleia, mialgia e artralgia, seguida de erupções papulopustulosas e necróticas. Devido à sua raridade e apresentação clínica pleomórfica, esse quadro é geralmente subdiagnosticado, o que pode levar, por vezes, ao desfecho fatal. A etiologia é desconhecida mas podem estar implicados fatores hormonais, já que é mais prevalente em mulheres (particularmente no período pós menopausa) e ambientais (já que só existem casos descritos nas últimas décadas). A sua incidência crescente e a irreversibilidade da alopecia tornam crucial saber detetar sinais de atividade que possam indiciar progressão, justificando, em tais casos, abordagem terapêutica mais agressiva. Since the lips are paramount for symmetry and overall aesthetics, even minor deformities can lead to disastrous changes. One of these could result from improper intra-operatory antero-posterior alignment between the anterior (cutaneous) and posterior (mucosal) vertex of the wedge, as the latter is not easily determined with precision by the naked eye. The authors present their surgical pearl to accurately define the vertex of the wedge in the posterior surface of the lip. A controlled, antero-posterior transmural penetration at a 90º angle is performed with a hypodermic needle. The entry point is the vertex of the cutaneous wedge. The sharp tip of the needle will now be the reference to the vertex of the mucosal wedge.
