{"title":"[Investigation of vestibular function in patients with vestibular neuritis].","authors":"N L Kunelskaya, E V Baybakova, E A Manaenkova, Z O Zaoeva, M A Chugunova, E S Yanyushkina, Ya Yu Nikitkina","doi":"10.17116/otorino2024890214","DOIUrl":"10.17116/otorino2024890214","url":null,"abstract":"<p><p>Vestibular neuritis occupies the third place in terms of prevalence in the structure of peripheral vestibulopathies, therefore, the choice of optimal diagnostic and differential diagnostic tactics at different stages of the disease is an urgent task.</p><p><strong>Objective: </strong>To optimize the diagnostic algorithm for vestibular neuritis based on an assessment of the sensitivity of clinical methods for studying vestibular function in the recovery period of the disease.</p><p><strong>Material and methods: </strong>A comprehensive assessment of the sensitivity of clinical methods for the study of vestibular function in the acute (up to 14 days: at the time of initial treatment, on the 7th and 14th day) and subacute (up to 3 months: on the 28th and 90th day) periods of the disease in 52 patients with upper vestibular neuritis was carried out.</p><p><strong>Results: </strong>The timing of the processes of restoration of vestibular function after a transferred vestibular neuritis is individual: after 14 days, restoration of vestibular function was recorded in 52% (<i>n</i>=27) patients, after 1 month - in 62% (<i>n</i>=32), after 3 months - in 71% (<i>n</i>=37) patients with upper vestibular neuritis. Statocoordination, statokinetic, oculomotor tests under visual control have the highest sensitivity in the acute period of vestibular neuritis, within up to 7 days from the onset of symptoms. In the subacute period of vestibular neuritis, the study of spontaneous nystagmus and nystagmus in the head shaking test retains high sensitivity only when using special tools (Frenzel goggles or videonystagmography). Based on the results of the analysis and generalization of literature sources on the treatment and prevention of ear sulfur plugs, presented in specialized publications in the Russian scientific eLibrary and scientific databases MedLine, Web of Science, Scopus, the need to optimize treatment methods and preventive measures has been identified. In this regard, it is recommended to widely use the preparation A-Cerumen Plus as one of the most progressive means. A significant therapeutic and economic aspect is the rational prescription of systemic antibacterial therapy after conditionally clean operations. This issue is especially acute today, in conditions of the rapid formation of multiresistance in pathogens, the growth of allergic sensitization to medical substance and the comorbidity of patients. The need to have clear criteria for prescribing local and/or systemic antibacterial therapy after surgical interventions in the nasal cavity and paranasal sinuses in order to prevent infectious complications is beyond doubt.</p><p><strong>Objective: </strong>To assess the effectiveness and safety of the use of topical antibacterial medication in patients after rhinosurgery.</p><p><strong>Material and methods: </strong>60 patients (38 were men and 22 women) aged 18 to 55 years were observed in the Clinic of Ear, Nose and Throat Diseases of the Sechenov First Moscow State Medical University, who underwent surgery under general anesthesia for non-purulent pathology of the nasal cavity and paranasal sinuses. In the postoperative period, all patients were randomly divided into two groups: the main group, which received Isofra (framecitin sulfate) intranasally, 1 dose 4 times a day for 5 days, and the control group, in which the antibiotic ceftriaxone was administered intramuscularly 1.0 g 2 times a day at the same time. In order to compare the indicators in the two groups on the 1st, 3rd and 5th days after surgery, the severity of subjective and objective symptoms was assessed using visual analogue scales (VAS). All observed patients were discharged from the hospital on the 5th day after surgery; on the 10th day, on an outpatient basis, material was collected from the mucous membrane of the nasal cavity for microbiological and cytological examination and the rate of mucociliary transport (MCT) was assessed using a saccharin test. For 3 months after discharge, all patients were called by a doctor monthly to identify episodes of acute respiratory viral infections, including the development of bacterial complications.</p><p><strong>Results: </strong>Statistically significant differences were recorded on the 5th day after surgery in terms of the severity of nasal obstruction (<i>p</i><0.05) - in the main group, the degree of difficulty in nasal breathing was lower than in the control group. There were no significant differences in the results of microbiological and cytological studies, or MCT indicators; the results of the main and control groups were comparable. {"title":"[Tinnitus features and masking in patients with normal hearing and with high frequency hearing loss].","authors":"Yu V Levina, A L Guseva, Z O Zaoeva, O A Melnikov, S A Makarov, Ya Yu Nikitkina","doi":"10.17116/otorino2024890514","DOIUrl":"10.17116/otorino2024890514","url":null,"abstract":"<p><strong>Objective: </strong>To access tinnitus features and effectiveness of its masking in relation to sensorineural hearing loss (SHL), gender, age, affected side and tinnitus pitch.</p><p><strong>Material and methods: </strong>53 patients (32 (60.4%) females and 21 (39.6%) males) aged 20 to 46 years with unilateral chronic tinnitus without any external or middle ear pathology were enrolled in the study. All patients underwent pure tone audiometry, evaluating SHL, tinnitus pitch and tinnitus masking. Pure-tone thresholds were measured at all standard frequencies from 0.25 to 8 kHz, and at 10 kHz, 12.5 kHz, 14 kHz, 16 kHz, and 20 kHz.</p><p><strong>Results: </strong>Normal hearing at the studied frequencies was observed in 24 patients, and 29 patients were found to have SHL. Patients with high-frequency SHL identified tinnitus pitch more often identified tinnitus pitch compared to those with normal hearing (75.9% vs. 45%, respectively; <i>p</i>=0.02). Tinnitus pitch in patients with normal hearing was 8 (6-13) kHz, whereas in all patients with SHL tinnitus pitch was always located in the range of maximum hearing loss and reached 10 (8-12) kHz. The mean tinnitus pitch, which was successfully masked, was 8 Hz, and tinnitus pitches with mean value above 12 kHz were not associated with effective masking (<i>p</i>=0.04). No statistically significant differences were found between the effectiveness of tinnitus masking and the affected side, gender, presence of SHL. The higher tinnitus pitch (above 12 kHz) was more common in younger patients and was associated with less successful masking.</p><p><strong>Conclusion: </strong>Our study included patients of predominantly young age (up to 48 years old), in whom, probably, the factors influencing the tinnitus characteristic differ and have a correlation with anxiety-depressive spectrum disorders prevailing at a younger age. The number of referrals from patients with CP reaches 70% of the total number of referrals to an otorhinolaryngologist. The development of this disease is facilitated by the neuro-reflex factor and a violation of trophic and metabolic processes. It should be noted that of all forms of CP, the greatest impact on the quality of life is noted precisely in atrophic pharyngitis (AP), due to the presence of pronounced subjective sensations from the oropharynx. Many of the modern methods of treatment do not provide a lasting effect due only to the application, superficial local effect on the altered mucous membrane of the posterior pharyngeal wall, without taking into account the changes caused by a violation of trophic processes in the tissue. A promising direction in the treatment of atrophic pharyngitis is the use of a combined technique - ozone therapy and low-intensity laser therapy. The article presents the results of the application of fine-drip irrigation of the mucous membrane of the posterior pharyngeal wall with Ozonide oil in combination with low-intensity laser radiation on the projection of vessels supplying blood to the middle parts of the oropharynx.</p><p><strong>Objective: </strong>Improving the effectiveness of treatment of patients with atrophic pharyngitis through the use of ozone therapy and low-intensity laser therapy.</p><p><strong>Material and methods: </strong>A single-center experimental controlled randomized open-label study of 90 patients with AP aged 18 and over was conducted. All patients were randomly divided into three groups depending on the treatment performed: group I - traditional treatment method (rinsing the oropharynx with antiseptic solutions, the use of tablets for resorption), group II - treatment with ozone therapy (fine drip irrigation of the mucous membrane of the posterior pharyngeal wall with Ozonide oil), group III - treatment with ozone therapy and laser therapy. During the examination of patients, anamnesis collection, examination of ENT organs, cytological and microbiological examination of the mucous membrane of the posterior pharyngeal wall, contact endoscopy of the mucous membrane of the posterior pharyngeal wall were performed. 5-point visual analogue scales (VAS) were used to assess complaints and pharyngoscopic signs.</p><p><strong>Results: </strong>Our results showed a statistically significant improvement in the quality of life of patients with AP (<i>p</i>=0.012), an improvement in the pharyngoscopic picture (<i>p</i>=0.003). The results obtained by us indicate an improvement in microcirculation under the influence of ozone therapy and low-intensity laser radiation.
