The interest of this clinical case lies in the rare occurrence of a keloid scar of the external auditory canal, leading to its total obturation. Scarring processes in the external auditory canal are an important problem that significantly affects the hearing of patients. Acquired fibrous atresia is represented by a conglomerate of soft tissues in the bony part of the external auditory canal, soldered to the outer surface of the eardrum. Keloid is an excessive growth of scar tissue in the dermis and underlying tissues with hyalinosis of bundles of collagen fibers. We described the surgical treatment of a keloid scar that appeared 11 months after the canaloplasty for congenital atresia of the external auditory canal. Previously, the patient underwent two local injections with an interval of 1 month into the anterior and posterior sections of the keloid scar of the prolonged glucocorticosteroid Triamcinolone in order to reduce the size of the pathological tissue. Many practitioners take the view in their work that there is an inextricable connection between pathological changes of the middle nasal concha and risk of developing chronic sinusitis. This in turn determines its resection options, including in the case of bullous transformation. This literature review reveals that, despite the significant prevalence of bullous hypertrophy in patients with chronic sinusitis, none of the analyzed studies could reliably prove a direct relationship of changes in the middle nasal concha and development of chronic sinusitis. Partial resection of the middle nasal concha is a uniform solution if intraoperative correction is needed. Hearing impairment in premature infants is presented in transient or intermittent forms of conductive hearing loss, mixed hearing loss, as well as sensorineural hearing loss (SNHL) and auditory neuropathy spectrum disorders (ANSD). Auditory function characteristics include unstable hearing thresholds, the possibility of improvement up to complete normalization, less commonly delayed hearing loss, and the transformation of ANSD into SNHL and vice versa. The article presents a possible algorithm for clinical and audiological monitoring of children born prematurely from birth to 10 years old. It involves a two-stage newborn hearing screening within the first 1-2 months after birth and verification of the diagnosis by the time infants reach 3 months corrected age. If hearing loss are detected, children are examined up to the age of 2 years 3-4 times a year; children with initially verified normal hearing are examined 2-3 times in the first year of life, and then annually. Separately, there is an algorithm for diagnostic and habilitation intervention for children with ANSD. Attention is also drawn to the need to involve otolaryngologists for timely identification of conductive hearing loss and special education teachers, speech therapists for early diagnosis of central auditory processing disorders. The pathogenetic mechanisms of RM remain insufficiently studied, and existing methods for assessing the state of the nasal mucosa require improvement. The development of an experimental model of RM is necessary for testing new therapeutic approaches.</p><p><strong>Objective: </strong>To develop an experimental model of RM in rabbits, evaluate morphological changes in the nasal mucosa, and test the photoplethysmography method for non-invasive diagnosis of microcirculatory disorders.</p><p><strong>Material and methods: </strong>The study was conducted on 18 Soviet Chinchilla rabbits. The RM model was induced by intranasal administration of 0.1% xylometazoline solution (200 μl/nostril, 2 times/day, 14 days). The control group received saline. Histological analysis of the mucosa was performed on days 8 and 15. The state of microcirculation was assessed by photoplethysmography using a cold test. Data registration was carried out <i>in vivo</i> with subsequent digital signal processing.</p><p><strong>Results: </strong>Progressive histological changes were revealed: epithelial dystrophy, a decrease in the number of goblet cells, and vascular alterations. The photoplethysmography demonstrated a significant decrease in the ratio of photoplethysmogram amplitudes in the RM group (median 0.76 vs. 1.48 in the control; <i>p</i><0.01), reflecting impaired microcirculation.</p><p><strong>Conclusion: </strong>An experimental model of RM in rabbits was developed, confirmed by histological and photoplethysmographic data. {"title":"[The effect of the presence of a submucosal cleft palate on the middle ear and speech].","authors":"I G Andreeva, D I Marapov, P V Tokarev, N A Andreev","doi":"10.17116/otorino2025900416","DOIUrl":"10.17116/otorino2025900416","url":null,"abstract":"<p><strong>Introduction: </strong>Hidden or submucosal cleft palate (SCP) is a rare form of isolated cleft that is characterized by rhinolalia in the presence of an apparently intact palate.</p><p><strong>Objective: </strong>To analyze the age of detection of SCP in children and evaluate the impact of its presence on the middle ear and speech development of patients.</p><p><strong>Material and methods: </strong>17 patients with SCP were examined and treated in the departments of otorhinolaryngology and maxillofacial surgery of the Children's Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan. The duration of observation of patients ranged from 1 to 4 years.</p><p><strong>Results: </strong>The median (Me) age of diagnosis of SCP was 7.18±3.83 years (95% CI 5.21-9.14), of which in 10 patients (58.8%) SCP was first suspected by an otorhinolaryngologist. When analyzing the age of detection of SCP depending on the detection of otitis media with effusion (OME) and epitympanitis, we established statistically significant differences (<i>p</i>=0.007; <i>p</i>=0.043, respectively). When assessing the dependence of the probability of OME on the age of detection of SCP using ROC analysis, an ROC curve was obtained that characterizes the dependence of the probability of OME on the age of detection of SCP. The resulting model was statistically significant for OME (<i>p</i>=0.011). When analyzing the comparison of the age of detection of SCP depending on the presence of speech delay, statistically significant differences were established (<i>p</i>=0.029).</p><p><strong>Conclusions: </strong>The analyzed data from patients with SCP indicate serious changes in the middle ear: from OME to the presence of cholesteatoma. The presence of SCP affects not only the clarity of spoken speech, but also contributes to its delay. If there are signs of general intoxication or ineffectiveness of conservative treatment, it is recommended to remove the palatine tonsils as a source of chronic infection. However, due to the abundant blood supply to the palatine tonsils, surgery is associated with a high risk of intraoperative bleeding. The volume of intraoperative blood loss depends on the vascularization of the tonsils, the surgeon's experience and knowledge of the topographic and anatomical features of the operated area. A standard preoperative examination does not involve determining the anatomical boundaries of the palatine tonsils and their blood supply - parameters that affect the intensity of bleeding, and due to the different depth of the tonsils in the tonsillar niches, it is not possible to determine their true dimensions during pharyngoscopy. Modern research highlights the concept of «deep tonsils», when most of the tonsils are located behind the anterior palatine arch, which complicates their removal and increases the duration of the operation. In this study we research the effect of the depth of the tonsils and their vascularization on the volume of intraoperative blood loss. Special attention should be paid to consideration of issues related to the diagnosis of malignant neoplasms of the skin of the head and neck area.
