{"title":"[Our experience in the treatment of congenital nasal median heterotopias in children and an overview of various treatment tactics].","authors":"E V Molodtsova, A S Yunusov, N A Daikhes, D P Polyakov, A S Korobkin, O M Larina","doi":"10.17116/otorino20248902128","DOIUrl":"10.17116/otorino20248902128","url":null,"abstract":"<p><p>Dermoid nasal cysts (congenital nasal median heterotopias) are a rare congenital pathology in children.</p><p><strong>Objective: </strong>Yo consider the clinical picture, methods of radiation diagnosis and to study the surgical results of a dermoid cyst of the nose according to the literature.</p><p><strong>Materials and methods: </strong>A retrospective review of medical histories with the diagnosis of \"Dermoid cyst of the back of the nose and nasal cavity, epidermal cyst of the back of the nose, glioma, encephalocele\" was conducted from 2017 to 2022 in the Pediatric Otorhinolaryngological Department of the National Medical Research Center for Otorhinolaryngology of the Federal Medical-Biological Agency of Russia. The case histories were analyzed by the nature of the lesion, the imaging techniques performed, the course of the operation and the results obtained.</p><p><strong>Material and methods: </strong>A total of 16 medical histories were analyzed, the average age was 4.5 years (range 10 months - 15 years), over the past 5 years with a diagnosis of \"Dermoid cyst of the nasal dorsum and nasal cavity, glioma, epidermal cyst of the nasal dorsum, encephalocele\". All patients underwent magnetic resonance imaging (MRI) in the preoperative period, 14 patients also underwent computed tomography (CT).</p><p><strong>Result: </strong>Of these, 7 had a confirmed dermoid cyst with a fistula, 3 patients without a fistula, 3 patients had glioma, and 1 had encephalocele, 2 patients had an epidermoid cyst. A fistulous opening of the dermoid cyst of the nasal dorsum and nasal cavity was observed in the upper third of the nasal dorsum in 3 children, in the middle third in 2 patients and in the lower third in 2 children. The article presents a scheme for the characteristics of the lesion and the tactics of surgical treatment in comparison with the data of foreign authors. Intraoperatively, intracranial spread occurred in 6 patients. Various surgical approaches for intracranial proliferation and a corresponding literature review are also presented. Catamnestic follow-up ranged from 1 to 5 years (on average, 3.5 years), no relapses or postoperative complications were noted.</p><p><strong>Conclusion: </strong>Nasal median heterotopias are a rare congenital anomaly. Preoperative preparation should include CT and MRI to assess the lesion and exclude intracranial spread. The surgical approach depended directly on the localization of heterotopia and its spread. The study included 20 patients with cicatricial stenosis of the larynx and trachea, who were previously performed plastic reconstructive treatment with scar tissue excision in the lumen of the respiratory tract and restoration of the supporting frame of the larynx and trachea using allografts based on costal allocartilage. The age of the patients ranged from 21 to 54 years, the duration of the disease was from 1 to 5 years. After a standard clinical and laboratory examination, with a mandatory video endoscopic examination of the larynx and trachea, multislice computed tomography of the larynx and trachea, patients underwent plastic closure of the tracheal defect using DM. Dynamic outpatient monitoring was carried out once a week for 1 month, once a month for 3 months, control examination was done 6 months after surgical treatment. The results of the study demonstrated a full-fledged social and labor rehabilitation of all 20 patients after the final stage of surgical treatment using DM, the absence of rejection reaction and migration of allo-implantation material, the preserved lumen of the larynx and trachea with a rigid supporting skeleton and the absence of anterior tracheal wall floatation. {"title":"[Preoperative protective preparation of the nasal mucosa of rhinological patients].","authors":"I E Berest","doi":"10.17116/otorino20248905123","DOIUrl":"https://doi.org/10.17116/otorino20248905123","url":null,"abstract":"<p><p>To reduce the inflammatory reaction that occurs in the nasal mucosa after surgical interventions, it seems promising to develop methods of preventive preparation of the epithelium to the effects of damaging factors in the preoperative period.</p><p><strong>Objective: </strong>To evaluate the effectiveness of the protective effect of sodium deoxyribonuclease solution on the nasal mucosa in the preoperative period in rhinological patients.</p><p><strong>Material and methods: </strong>71 patients were examined after septoplasty with vasotomy of the lower nasal concha for curvature of the nasal septum and vasomotor rhinitis. Patients were divided into two groups: patients of group 1 (<i>n</i>=36) did not undergo protective preoperative preparation; patients of group 2 (<i>n</i>=35) were locally prescribed 0.25% sodium deoxyribonuclease solution as protective therapy. All patients underwent cytological examination of nasal secretions using smear prints at the initial treatment, before surgery, on the 5th, 10th, 21st, 30th, 42nd and 60th days of the postoperative period.</p><p><strong>Results: </strong>The use of sodium deoxyribonuclease solution in the preoperative period led to statistically significant cytoprotective, anti-inflammatory, reparative and immunostimulating effects compared with the control group. With careful collection of anamnesis, it was found that the patient suffers from gout for a long time, has concomitant diseases that aggravate its course, and there is no compliance to treatment. Based on these data, the presence of gouty tophi, which are deposits of sodium monaurate in tissues in the form of nodes, is suspected. Auricles are not a common place of localization, due to which tophi can be mistaken for other neoplasms. Tophi are amenable to conservative treatment with urate-lowering therapy and non-pharmacological methods. However, our patient is shown their surgical removal based on complaints and low adherence to treatment. The final diagnosis was confirmed by histological examination. Children with this pathology are more likely to have behavioral problems, impaired auditory, linguistic and cognitive development, and especially difficulties with learning at school.</p><p><strong>Objective: </strong>To analyze the literature data on the epidemiology of central auditory disorders in school-age children.</p><p><strong>Material and methods: </strong>As a result of an initial search in databases (PubMed, ClinicalTrials, SCOPUS, ResearchGate, eLibrary, CyberLeninka), 328 articles were found by keywords, after an initial and further detailed analysis, a description of 9 studies is included in the review.</p><p><strong>Results: </strong>The prevalence of CSD in school-age children varies from 0.2% to 43.3% (median 8.6±14.6%), depending on the research methods used and the criteria for diagnosis. In this regard, high-speed shunting of blood from the arterial part of the vascular system to the venous through fistulas of various calibers occurs. This disease is characterized by a variety of clinical manifestations. The auricle is the second most common site of extracranial AVM. Despite a large number of studies concerning etiological factors, diagnostic criteria and clinical aspects, differential diagnosis of auricular AVM in some cases is still difficult. Based on international clinical recommendations, olfactory tests are the gold standard for the diagnosis of olfactory disorders. There are many different psychophysical tests: UPSIT (USA, Pennsylvania), Sniffin' Sticks test (Germany), BAST-24 (Spain), etc. Currently, there is an acute shortage of olfactory tests available for clinical practice In Russia. This problem is related to the fact that there are no olfactory tests registered as medical devices on the territory of the Russian Federation. Also, a significant limitation is the unrecognizability of odors by the population of our country, which include foreign analogues (licorice, anise, turpentine, etc.).</p><p><strong>Objective: </strong>To develop and validate the national olfactory test on healthy volunteers.</p><p><strong>Material and methods: </strong>The development and validation of the olfactory test included several stages. First, the development of an olfactory test was carried out, the selection of aromas to assess the threshold and identification ability of olfaction. 25 dilutions of <i>n</i>-butanol were used for the assessment of the threshold olfactory ability. For the stage of assessing the identification ability of the sense of smell, in our previous study, an assessment of the recognition of odor names in the territory of the Russian Federation was carried out. A total of 3.000 people from 8 federal districts of the Russian Federation were interviewed. During the development of the test, 20 names of flavors with the highest rating were used. By the 8<sup>th</sup>, the selection of monocomponent substances was carried out. Commercially available certified food and perfume flavorings have been used for fragrances whose equivalent in the test cannot be a monocomponent substance. A group of 25 healthy volunteers selected a flavor or a monocomponent for each of the 20 positions. To carry out the identification stage of testing, a booklet was developed with answer options for each fragrance, including 80 images associated with the smell. A methodology for conducting diagnostics has been created. Next, the validation of the developed olfactory test was carried out on 150 healthy volunteers. The study included an assessment of the threshold and identification ability of the sense of smell using the developed test and conducting a comparative analysis with a set of flavors and descriptors corresponding to the Sniffin' Sticks test.</p><p><strong>Results: </strong>The developed test includes: 2 panels - panel 1 to assess the threshold ability of smell, panel 2 to assess the identification ability of smell, a booklet with 80 images and captions to them. The norms of threshold and identification olfactory abilities were also determined in the developed test.
