Apart from Type 2 diabetes mellitus (DM), IR is implicated in many other clinical syndromes because of its varied metabolic and mitogenic actions. IR has been found to play important pivotal role in pathophysiology of diabesity. IR is defined as when a normal or higher insulin level fails to produce expected biological response; one predominantly affecting insulin mediated glucose disposal 20–30% reduction in the number of insulin receptors on the target cells is observed in majority of Type 2 DM patients but 1/3 of the patients may not manifest loss of number of receptors; therefore, defective post-receptor signalling is considered as the main cause of IR. Main sites of IR are liver, adipose tissue and skeletal muscles. We report a case of 21-year-old female whose initial clinical and laboratory investigations were suggestive of inflammatory arthropathy. The case was complicated by weak anti-nuclear antibody positivity of anti-proliferating cell nuclear antigen (+) antibodies, suggestive of systemic lupus erythematosus. However, after clinical examination and investigations, the patient was diagnosed as a case of disseminated TB with Poncet’s disease. The patient was started on anti-tubercular treatment and showed improvement at the end of 1st month of treatment. PSVT can diagnose with electrocardiography, but most specifically, it can be diagnosed by electrophysiology study (EPS) and further treated with radiofrequency ablation (RFA). Management of PSVT with RFA significantly reduces the reoccurrence of PSVT episodes in future. In the following case, presentation series of three cases will be used for creating panorama of existing knowledge of PSVT, along with diagnostic approach through EPS, and finally, the use of RFA as a treatment method will be demonstrated. Common laboratory values may provide key insights into patients with COVID-19, and may predict the morbidity and outcome. The aim of this study was to evaluate the relation of elevated D-dimer levels on mortality of patients admitted to ICU with COVID-19.\u0000\u0000\u0000\u0000All ICU patients with laboratory confirmed COVID-19 were retrospectively enrolled in Sevenstar Hospital, Nagpur from 27 July, 2020, to 30 October, 2020. D-dimer levels on admission, on Day 3 and Day 5, were collected in all ICU patients and death events were collected. The subjects were divided into two groups discharged and expired. Then, the D-dimer levels between two groups were compared to assess the predictive value of D-dimer level and mortality in hospitals.\u0000\u0000\u0000\u0000A total of 101 eligible patients were enrolled in the study. Thirty-one deaths occurred during hospitalisation. Patients who expired had on admission D-dimer levels of 2729 ± 3243 ng/mL while those discharged had D-dimer values of 973 ± 1553 ng/mL (P < 0.007). D-dimer of expired patients on Day 3 was 3206.5 ± 3338.8 and of discharged patients was 828.8 ± 1268.8 (P = 0.001). D-dimer of expired patients on Day 5 was 5184. 5 ± 3386.1 versus discharged patients was 588.7 ± 645.5 (P < 0.0001). The association of OSA with increased cardiovascular morbidity, mortality, and various cardiovascular risk factors is known for a long time. Metabolic syndrome a cluster of metabolic abnormalities, with insulin resistance as the central pathophysiological feature is ‘Syndrome X’ and in association with OSA is ‘Syndrome Z’. The aims of this study were as follows: (1) To study the incidence of OSA in patients with metabolic syndrome, (2) to study and correlate the severity of OSA with metabolic syndrome.\u0000\u0000\u0000\u0000This is a hospital-based observational study conducted in Department of Respiratory Medicine IGGMC, Nagpur.\u0000\u0000\u0000\u0000In the present study of ‘polysomnography in patients of metabolic syndrome’; 50 cases of metabolic syndrome were evaluated. 20 (55.55%) subjects were in the age group of 40–50 years. 22 (44%) subjects had moderate excessive daytime sleepiness, followed by higher normal daytime sleepiness in 14 (28%) based on Epworth sleepiness scale. They occur at the bifurcations of large to medium sized intracranial arteries. Approximately 80% aneurysms occur in the anterior circulation and about 20% have multiple aneurysm sites. Familial aneurysms rupture at a smaller size and younger age than sporadic cases. We present a case of cerebral artery aneurysm in a 49-year-old female patient. It manifests clinically as albuminuria which is the precursor of end-stage renal failure. This is considered to be an inflammatory process. In recent past, it has been reported that the neutrophil-lymphocyte ratio (NLR) may be a favourable indicator of the inflammatory status. It is cost effective and easily accessible marker. The present research was undertaken to study the clinical profile of DN and to see the association between NLR and DN in patients with type 2 diabetes mellitus (DM).\u0000\u0000\u0000\u0000A total of 210 patients were enrolled, of them 105 healthy individuals without diabetes were taken as controls and cases were 105 subjects with type 2 diabetes. Patients were, further, divided based on urine albumin creatinine ratio (UACR) into DM without nephropathy (UACR <30 mg/g) group and DM with nephropathy (UACR >30 mg/g) group. For intragroup comparison, DM with nephropathy group was, further, divided into microalbuminuria (UACR 30–300 mg/g) and macroalbuminuria (UACR >300 mg/g).\u0000\u0000\u0000\u0000NLR was found significantly increased (P < 0.0001) in patients with DN (2.81 ± 0.51) as compared with DM without nephropathy (2.12 ± 0.64) and healthy adults (2.06 ± 0.29). However, NLR value was unable to differentiate between patients with microalbuminuria and macroalbuminuria. A brain scan on admission revealed multiple lacunar cerebellar infarcts, whereas routine blood investigations did not reveal anything substantial contributions to the cause of presenting symptoms, including negative serology for ANA, DS-DNA, HAL-B51, and anticardiolipin antibodies, done to rule in, presence of systemic autoimmune causes for the gamut of clinical features at presentation. Contemplation over past and associated clinical features, such as ocular ailment, recurrent aphthous ulcers, dermatological lesions (erythema nodosum and peudofolliculitis), and past MRI showing lesions in the thalamus and pons (diencephalon predilection) a diagnosis of Behcet’s disease (BD) was considered based on ‘International Study Group Diagnostic Criteria for BD’. More so in the event of no better explanation for the neurological involvement, in a diagnosed case of BD, presenting delirium was considered to be the fallout of Neuro-Behcet’s Disease (NBD). The Patient responded to steroids and was discharged on a combination of tapering doses of steroids with Azathioprine. Emphasising the fact that the central nervous system affection in a case presenting with signs of systemic inflammation, autoimmune vasculitis as a cause of neurological involvement should be considered, as this is critical for deciding onto the course of treatment.
