A 68 year old postmenopausal lady was admitted to our hospital following multiple episodes of hematemesis and malena for past 5 days. Upper GI endoscopy revealed multiple telengiectasia with active spurting, gastric polyp. Patient was treated with 1 unit of PRBC transfusion for anemia, Inj.Tranexamic acid 500 mg IV tid, Inj. Ondanseteron 4 mg IV for hematemesis. Tab. Thalidomide 100 mg hs have been given for treating hereditary hemorrhagic telengiectasia. The treatment is only palliative, with no consensus on the best treatment option. Although the pathogenesis of the anemia in CKD is multifactorial, decreased production of erythropoietin by the diseased kidneys has been proven and the severity of anemia correlates positively with serum creatinine concentrations in affected dogs. Therapeutic whole and Packed Red Blood Cell transfusion has always been a main stay in practice. However, the unavailability of appropriate dog donors, blood typing modalities and occurrence of post transfusion reactions are the common hurdles encountered. Alternatively, Darbepoetin alfa, a hyperglycosylated synthetic recombinant erythropoietin which is commonly used in human patients with CKD induced anemia is used in recent canine practice. Unlike human transfusion medicine, not much of study has been documented in dogs in India. However anecdotally, darbepoetin is perceived to less likely form antibodies compared to erythropoietin. A study comprising of 21 dogs referred to Madras Veterinary College Teaching Hospital (MVCTH) with CKD (stage II, III & IV were 7, 9 & 5 respectively) of multifactorial etiology during the period of January to December 2017 was done. Injectable Darbepoetin was administered @0.5μg/kg subcutaneously once in a week for three weeks along with supplementation of Iron to achieve a medium hematocrit of 30%. Efficacy of Darbepoetin was evaluated by measuring pre and post therapeutic clinico pathological studies. Darbepoetin proved effective in increasing PCV, anemic crisis potentially in 66.7% of dogs. Hypertension, vomiting, melena, inappetance were common findings in affected dogs and coexistence of normocytic, normochromic progressive anemia which responds to Darbepoetin alfa therapy. Though expensive than Erythropoietin, Darbepoetin @0.5μg/kg s/c proved safe and effective in improving packed cell volume in CKD dogs as an alternative to packed RBC or whole blood transfusion. Further immunological, toxicological and clinical studies on Darbepoetin therapy is warranted in a large population in dogs. Sahin","doi":"10.15226/IJHBD/4/1/00132","DOIUrl":"https://doi.org/10.15226/IJHBD/4/1/00132","url":null,"abstract":"Objective: The aim of this study was to investigate risk factors for cytomegalovirus (CMV) infection, effect of viral reactivation on hematopoietic stem cell transplantation (HSCT) and efficacy of preemptive treatment using a lower dose valganciclovir for the first time. Material and Method: The data of 447 patients who underwent HSCT for malignant and non-malignant hematological disorders at a single center from September 2009 to December 2016 were retrospectively evaluated in this study. DNA levels of CMV were routinely tested two days per week for the first 24 months following HSCT, and in case of clinical suspicion after 24 months using the Quantitative Real-Time quantitative polymerase chain reaction (RT-PCR). Results: Ninety (54.2%) allogeneic transplant patients, and forty one (14.6%) autologous transplant patients had CMV reactivation. There was a statistically significant increase in CMV reactivation in the non-myeloablative (NMA) allogeneic transplant group compared to the myeloablative (MA) group at a value of 150-1000 copies/mL CMV-PCR (p= 0.002). Acute/chronic Graft Versus Host Disease (GVHD) were observed in 30 of allogeneic HSCT patients. 29 of these patients (96.6%) had CMV antigenemia. There was a significant association between the development of acute/ chronic GVHD, and CMV viremia (p= 0.001). The patients without CMV antigenemia had a higher incidence of mortality at first 100-day, and 365-day in allogeneic transplant group (p= 0.022, p= 0.024, respectively). The 5-year overall survival (OS) rate was 61% in the viremia group, and 62% in the non-viremia group. There was no statistically significant difference between the two groups in terms of 5-year OS (p= 0.551). In patients receiving a lower dose of 900 mg/day valganciclovir treatment due to viremia, CMV disease did not develop and no adverse effect that required the drug to be discontinued was observed. Conclusion: The results conducted that development of GVHD was associated with CMV viremia. Early and late mortality rate were higher in the patients without CMV antigenemia. However; CMV reactivation had no impact on patients’ survival post allogeneic and autologous HSCT. These estimates do not include carcinoma in situ for any cancer except urinary bladder and do not include basal cell or squamous cell skin cancers. The respiratory effects of cancer therapy are critically important to the overall health of cancer survivors due to higher incidence of pulmonary disease compared with the general population. In fact, one of the most significant achievements witnessed in the field of immunotherapy has been the success of immune checkpoint inhibitors (CPIs) in microsatellite instability-high colorectal and non-colorectal tumors. For this reason, the US FDA has granted accelerated approval to Pembrolizumab and Nivolumab as monotherapies and Nivolumab plus Ipilimumab as combined therapy. The effect of alcohol consumption among youths and adults resulting to escalating crime rate in the society has generated great concern. The impact of alcohol consumption on haematological variables has not been well documented in Jos metropolis. The study sought to establish whether alcohol consumption has any effect on the haematological status of individuals or not. Following informed consent, we determined haematocrit values by the microhaematocrit method, haemoglobin (Hb) concentration by cyanmethaemoglobin technique, total and differential white blood cell counts (WBC), malaria parasite as well as microscopic blood film reporting of 124 alcohol consumers and 53 non-consumers as a control. SPSS version 23 was used to compare mean values between alcohol consumers and the control group (student t-test), while associations of blood film report were determined by chi-square. Results showed a significantly higher total lymphocytes count among alcohol consumers compared to nonalcohol consumers (48.28±12.98 v 41.91±11.59; p=0.002). Also, the blood film reporting revealed a significantly higher percentages of macrocytes (p=0.001), microcytes (p=0.020), hypochromia (p=0.001), polychromasia (p=0.013), rouleaux formation (p=0.004) and atypical lymphocytes (p=0.011) and malaria parasitaemia (p=0.001) among alcohol consumers compared to the control group. Anisocytosis and hypochromia, which are essential characteristic features of anaemia, have been shown to associate with alcohol consumption.
