{"title":"The risk factors for cardiovascular disease and chronic kidney disease in patients with nonalcoholic fatty liver disease in Saudi Arabia.","authors":"Walaa Mohammedsaeed, Abdullah Al Malki, Salma Alsayed","doi":"10.15537/smj.2025.46.5.20240901","DOIUrl":"10.15537/smj.2025.46.5.20240901","url":null,"abstract":"<p><strong>Objectives: </strong>To identify potential risk factors for chronic kidney disease (CKD) and cardiovascular disease (CVD) in Saudi Arabian patients with non-alcoholic fatty liver disease (NAFLD).</p><p><strong>Methods: </strong>A 6-year prospective cohort study was carried out from January 2018 to January 2023, enrolling 1,500 patients. Data were initially collected between January 2018-2019, and follow-up assessments were carried out annually from 2020-2023. Patients were evaluated using biomarker analyses, and medical records were reviewed to assess the incidence of CVD and CKD. Biomarker levels, including blood creatinine, urine albumin, glomerular filtration rate (GFR), atherogenic index of plasma (AIP), and other key parameters, were monitored throughout the study.</p><p><strong>Results: </strong>Of the 1500 patients, 735 (49%) were diagnosed with NAFLD in 2018, while 765 (51%) did not have the condition. Compared to non-NAFLD patients, those with NAFLD exhibited elevated blood creatinine levels, lower GFR, and higher urine albumin, a key marker of kidney damage that is strongly linked to accelerated CKD progression and increased cardiovascular risk. Furthermore, NAFLD patients had a significantly higher AIP. Over the 4-year follow-up, these individuals showed a notable rise in the incidence of CKD and CVD.</p><p><strong>Conclusion: </strong>Non-alcoholic fatty liver disease is associated with alterations in lipid profiles, elevated high-sensitivity C-reactive protein (hs-CRP), and elevated aspartate aminotransferase (AST), all of which may contribute to early CKD and CVD development. Control samples were concurrently collected from healthy adults (n=59). Samples were screened for anti-dengue antibodies by employing enzyme-linked immunosorbent assay. Hematological parameters, such as hemoglobin (HGB), white blood cells, hematocrit (HCT), platelets count, and red blood cells (RBCs) levels were determined for the dengue positive samples and all the controls.</p><p><strong>Results: </strong>Of the patients examined, 15% were dengue immunoglobulin M positive and 25% were immunoglobulin G positive. Dengue-infected patients showed lower values of the tested hematological parameters than controls, and significantly lower values were observed for RBCs (<i>p</i>=0.0022), HGB (<i>p</i>=0.0002), and HCT (<i>p</i><0.0001). Average HGB (12.47g/dl) and HCT (39.03%) values of dengue patients were abnormally low. The average HCT levels were significantly lower in patients with recent dengue infection (41.41% vs. 45.91%, <i>p</i>=0.0230) and past dengue infection (38.63% vs. 45.91%, <i>p</i><0.0001) compared to controls. This study examined 5 outcomes: the number of FBDOs, FBDO cases, and FBDO cases classified by gender, nationality, and age groups. The analysis used generalized estimating equations and generalized linear mixed models based on a negative binomial distribution.</p><p><strong>Results: </strong>The findings revealed a significant decrease in FBDO counts and cases from 2017-2020, followed by an increase until 2023. Public sources consistently reported higher numbers of FBDOs and cases than home sources. No substantial difference was observed in FBDO cases between men and women. Saudi nationals had higher FBDO case counts than non-Saudi residents. Data were obtained using an online questionnaire assessing asthma severity and stress levels.</p><p><strong>Results: </strong>Cronbach's alpha indicated that the reliability of the stress scale was 0.728 and the reliability of the asthma control tool was 0.819. Of the participants, 52% were women and 48% were men, with 48.7% aged 18-26 years. Asthma severity was classified as low (20.3%) or high (54.6%) and 85.4% of the respondents reported elevated stress levels. A statistically significant correlation was noted between asthma control and sterss (r= -0.22, <i>p</i><0.05), indicating that higher stress levels hinder the management of asthma.</p><p><strong>Conclusion: </strong>There is a strong association between psychological stress and exacerbation of bronchial asthma. Stress impacts immune responses, thereby contributing to the pathogenesis of asthma. Demographic data, clinical data, and histopathological findings were analyzed using counts and percentages.</p><p><strong>Results: </strong>The majority of the patients were infants (n=83; 52.5%), with male predominance (n=97; 61.4%). Biopsies were the most common specimen type (n=137; 86.7%). Cholestatic/bile duct disorders were the most prevalent pathological pattern (n=92; 58.2%), with extrahepatic biliary atresia being the most common subtype (n=32; 34.8%). Inflammatory/infectious disorders accounted for 13.3% (n=21) of cases, with chronic active hepatitis-not otherwise specified (n=10; 47.6%) being the most frequent. Metabolic disorders represented 12.0% of cases, with glycogen storage disorders and non-alcoholic fatty liver disease both at 31.6% (n=6). Data were collected from December 2023 to July 2024 from outpatient clinics, Prince Sultan Military Medical City, Riyadh, Saudi Arabia. A total of 2 tools, C-ACT and TRACK, were employed to measure asthma control.</p><p><strong>Results: </strong>The results showed that 51.2% of children under 5 years of age exhibited uncontrolled asthma. Among children aged 5-12, 34.5% showed uncontrolled. {"title":"Identifying individuals at risk of post-stroke depression: Development and validation of a predictive model.","authors":"Saeed A Alqahtani","doi":"10.15537/smj.2025.46.5.20250080","DOIUrl":"10.15537/smj.2025.46.5.20250080","url":null,"abstract":"<p><strong>Objectives: </strong>To identify the factors associated with post-stroke depression (PSD) and develop a machine learning predictive model using a large dataset, considering sociodemographic, lifestyle, and clinical factors.</p><p><strong>Methods: </strong>Our 2025 study used data from the 2023 Behavioral Risk Factor Surveillance System, released in September 2024. Data processing was carried out using Google Colab and Python. We carried out descriptive statistics, logistic regression, and feature importance analyses (mutual information and adjusted mutual information). A total of 4 machine-learning models were trained and evaluated: random forest, decision tree, gradient boosting, and logistic regression. Model performance was assessed using the accuracy, precision, recall, harmonic mean of precision and recall (F1-score), and area under the curve - receiver operating characteristic (AUC-ROC). The best-performing model was fine-tuned using GridSearchCV with 5-fold cross-validation.</p><p><strong>Results: </strong>Increasing age, male gender, being married, higher income, and physical activity were associated with lower odds of PSD. Obesity, smoking, diabetes, and high cholesterol are associated with increased odds of PSD. Age and gender were the most informative features for predicting the PSD. Random forest demonstrated the best performance for predicting PSD (accuracy=0.73, precision=0.71, recall=0.77, F1-score=0.74, and AUC-ROC=0.81), which was further improved by hyperparameter optimization.</p><p><strong>Conclusion: </strong>Post-stroke depression's complex etiology involves sociodemographic, lifestyle, and clinical factors, notably age and gender. A novel comprehensive 38-item survey questionnaire was utilized. The questionnaire comprised 2 main sections: demographic data and potential influencing factors and communication skills with children. The communication domain assessed the degree to which the pharmacist could communicate with a child in 7 main areas. Comparisons of communication performance were carried out by Chi-square tests, and associations with 24 factors were determined by logistic regression analyses.</p><p><strong>Results: </strong>Approximately 76% of pharmacists demonstrated unsatisfactory communication with children (<i>p</i>=8.47×10<sup>-10</sup>). Approximately 91% of pharmacists depended on speaking to parents rather than directly talking to children, even when present (<i>p</i>=7.791×10<sup>-26</sup>). In addition, although 88.23% of pharmacists expressed high confidence in their communication skills (<i>p</i>=9.899×10<sup>-24</sup>), their actual performance with children in different age groups showed that 63.52% performed poorly, while only 36.46% demonstrated good performance (<i>p</i>=4.2×10<sup>-4</sup>). Factors which might be associated with pharmacists' performance were identified.</p><p><strong>Conclusion: </strong>Pharmacists in Saudi Arabia face significant challenges in communicating with pediatric patients.
