Its gen\u0000(CYP19A1) is expressed in preadipocytes\u0000(stromal vascular fraction, SVF)\u0000of adipose tissue. Estrogens are found\u0000to be protective for metabolism homeostasis,\u0000and cardiovascular system.\u0000Disturbed dietary and endogenous\u0000fatty acids (FAs) turnover is responsible\u0000for development of metabolic\u0000syndrome and it complications.\u0000Aim of the work was to investigate\u0000the effect of physiological concentrations\u0000of acids: arachidonic (AA), oleic\u0000(OA), palmitynoic (PA) and eikozapentaenoic\u0000(EPA) on CYP19A1 expression\u0000in differentiating human SVF, able to\u0000form adipocytes as well as endothelial\u0000cells.</p><p><strong>Material and methods: </strong>Human (n=38 healthy woman) SVF cells were\u0000isolated from subcutaneous adipose\u0000tissue harvested intrasurgery. SVF\u0000cells were incubated in proadipogenic\u0000or angiogenic media to obtain\u0000adipocytes (Adipo-SVF) or endothelial\u0000(Angio-SVF) cells (confirmed by\u0000microarray). Changes in the CYP19A1\u0000expression induced by 24hs incubation\u0000in the presence of FAs (10 – 30\u0000μM )were monitored by the Real time\u0000PCR (qRT -PCR).</p><p><strong>Results: </strong>The aromatase gene expression\u0000correlated positively with\u0000BMI of patients, but only in group\u0000of obese or overweight women. The\u0000negative correlation was found in\u0000the group of young, slim women. The\u0000highest expression of aromatase was\u0000found in the fresh, not differentiated\u0000SVF. In differentiating to endothelial\u0000cells (Angio - SVF) OA inhibited\u0000(p=0.008), when n-3 polyunsaturated\u0000AA activated (p=0.003) the CYP19A1\u0000gene expression. In differentiating to\u0000preadipocytes (Adipo-SVF) AA significantly\u0000(p=0.031) inhibited CYP19A1\u0000expression.</p><p><strong>Conclusion: </strong>The changes in the\u0000aromatase gene expression in differentiating\u0000SVF has been confirmed. {"title":"[Could the cytokines concentration be a marker of IBD activity and be useful in evaluation of IBD differentiation?]","authors":"Irena Cieċko-Michalska, Iga Wierzbicka-Tutka, Malgorzata Szczepanek, Danuta Fedak, Tomasz Mach","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>In inflammatory bowel\u0000disease (IBD) the imbalance between\u0000cytokines pro- and antinflammatory is\u0000observed.\u0000The aim of this study was the assessment\u0000of interleukin-10 (IL-10),\u0000interleukin-6 (IL-6) and tumor necrosis\u0000factor-α (TNF-α) concentration usefulness\u0000in the evaluation of the activity\u0000of ulcerative colitis (UC) and Cohn’s\u0000disease (CD).</p><p><strong>Methods: </strong>35 patients diagnosed\u0000with UC and 39 with CD were examined.\u0000The control group (CG) consisted\u0000of 35 healthy volunteers. Diagnosis of\u0000the disease was confirmed by videocolonoscopy\u0000and histopathological\u0000evaluation of intestinal biopsies.\u0000Disease activity of UC was assessed\u0000according to the Mayo Scoring System\u0000and by the Crohn Disease Activiti Index\u0000(CDAI) in CD patients. Among patients\u0000with UC 18 (51%) had severe, 14 (40%)\u0000moderate and 3 (9%) mild disease.\u0000Among patients with CD 7 (18%) was\u0000diagnosed with high, 27 (69%) moderate,\u0000and 5 (13%) with low activity of\u0000the disease. WBC, PLT, serum concentration\u0000of TNF-α, IL-6 i IL-10 were\u0000determined.</p><p><strong>Results: </strong>The average concentration\u0000of TNF-α in UC patients was: 14.3\u0000(IQR=12.6), in CD: 12.6 (IQR=11.9), in\u0000the CG: 3.1 (IQR=1.7). The average\u0000concentration of IL-6 in UC was: 19.6\u0000(IQR=21), in CD: 10.8 (IQR=7.6), in CG\u0000: 3.2 (IQR=1.6). The average concentration\u0000of IL-10 in UC was: 14.4 (IQR=5.9),\u0000in CD: 10.4 (IQR=9.3), in the CG: 3.3\u0000(IQR=2.5). In the IBD TNF-α, IL-6 and\u0000IL-10 concentration was significantly\u0000higher than in CG. However, IL-10 was\u0000significantly higher in UC than CD. In\u0000patients with UC statistically significant\u0000positive correlation between the\u0000concentration of TNF-α, IL-6 and IL-10\u0000and disease activity was noticed. There\u0000were no correlation between TNF-α,\u0000IL-6 and IL-10 concentration and CD\u0000activity.</p><p><strong>Conclusion: </strong>Determination of\u0000TNF-α, IL-6 and IL-10 serum concentration\u0000can be used for noninvasive\u0000evaluation of inflammation activity in\u0000patients with IBD. Clinical outcomes of anticoagulant\u0000therapy with vitamin K antagonists\u0000(VKA) are often unsatisfactory in\u0000women regardless of age largely due to\u0000a higher risk of unstable anticoagulation\u0000compared with men. Non-vitamin\u0000K oral anticoagulants (NOAC) are\u0000increasingly used for prevention of\u0000stroke in patients with nonvalvular AF\u0000appear to be particularly beneficial in\u0000women overcoming adverse events related\u0000to unstable anticoagulation. {"title":"[Implantation of CRT/ICD in women with heart failure – do we have sufficient evidence?]","authors":"Barbara Małecka","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>There are several gender differences\u0000in epidemiology, the course,\u0000prognosis and treatment of heart\u0000failure which is called the epidemic of\u000021st century. In women heart failure\u0000appears at a later age and this can\u0000be a sufficient reason for incidence\u0000of concomitant diseases. Clinical\u0000symptoms of heart failure in females\u0000are caused at a higher percentage by\u0000non-ischaemic cardiomyopathy and\u0000heart failure with preserved ejection\u0000fraction of the left ventricle. The history\u0000of development of current guidelines\u0000for ICD has significantly influenced\u0000different, gender-dependent access of\u0000patients to ICD implantation in prevention\u0000of sudden cardiac death. Women\u0000with heart failure are qualified for ICD\u0000less frequently than men. It seems that\u0000the existing disparities in utilization of\u0000ICD are significantly higher than sexdependent\u0000differences in the risk of\u0000sudden cardiac death. Most probably\u0000the guidelines for ICD will be improved\u0000only after establishment of the indications\u0000for ICD in patients with heart\u0000failure with preserved ejection fraction.\u0000Participation of females in CRT is\u0000significantly smaller despite the lack\u0000of separate indications for this mode\u0000of heart failure treatment. Women,\u0000however, demonstrate a greater improvement\u0000after CRT, especially as\u0000far as survival rate is concerned. A\u0000high rate of non-responders to CRT\u0000impels the search for its reason. Presently\u0000there is a considerable interest\u0000in morphology and duration of initial\u0000QRS. Including the parameter of “true”\u0000LBBB and more effective ways of\u0000describing mechanical dyssynchrony\u0000of the heart, with higher participation\u0000of women in CRT therapy, has the\u0000chance to decreasing the number of\u0000non-responders.\u0000Women are additionally at a greater\u0000risk of complications of endocardial\u0000lead implantation procedure as well as\u0000of late mechanical lead damage. {"title":"[Carpal tunnel syndrome- etiology and treatment].","authors":"Maksymilian Gniadek, Marek Trybus","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The paper presents the etiology, diagnostics,\u0000management, treatment and\u0000prognosis of carpal tunnel syndrome.\u0000The mechanism of the median nerve\u0000compression as well as the predisposing\u0000factors that cause the syndrome\u0000are discussed, as well as the sequence\u0000of specific symptoms and the classification\u0000of the level of severity of\u0000disease. The diagnostic methods are\u0000presented with both the benefits and\u0000consequences of each method. The\u0000treatment of carpal tunnel syndrome is\u0000divided into conservative and surgical\u0000approaches. Conservative treatment\u0000consists of keeping the wrist immobile\u0000using wrist splints, physiotherapy, systemic\u0000and local pharmacotherapy, as\u0000well as local steroid injections into the\u0000carpal tunnel. The results of conservative\u0000treatment are often short-term.\u0000Operative treatment consists of cutting\u0000the flexor retinaculum tendon. This directly\u0000eliminates the cause of CTS, the\u0000increased pressure in the carpal tunnel\u0000of the wrist. The\u0000incidence of poisoning per 10 000 of\u0000Kraków population in 1966 was 25.0\u0000and increased to 36.2 in 1983. The\u0000mortality in 1966 was 0.75%. It was low\u0000also in 1983 – 1.01%, whatever reached\u00007.8% if forensic data were included (the\u0000fatalities on the spot). There were 847\u0000poisoned patient hospitalized in Clinic\u0000of Toxicology UJ CM in 2015 and mortality\u0000in this group was 1.06%. Forensic\u0000analysis shown decreasing number of\u0000deaths on the spot (154 in 1983 and 89\u0000in 2015). The percentage of poisoned\u0000with ethanol was declining (78.9% in\u00001983 and 57.3% in 2015). The frequency\u0000of patients treated in intensive care\u0000unit were increased from 6% in 1966\u0000to 21.7% in 2015. The percentage of\u0000addicted acute intoxicated patients\u0000was lower (8%) in 1983 and achieved\u000021.7% in 2015. Poisoning structure,\u0000particularly pharmaceuticals, significantly\u0000influenced medical procedures\u0000performed in intensive care unit, especially\u0000administration of antidotes\u0000(5% in 1966 and 48.9% in 2015). Hyponatremia\u0000occurs in approximately 15-20% of\u0000hospitalized patients and up to 20% of\u0000patients in critical condition. It can lead\u0000to a wide range of clinical symptoms,\u0000from mild to even life-threatening, and\u0000is associated with increased mortality\u0000and longer duration of hospitalization\u0000in patients affected by many different\u0000syndromes. The diagnosis and treatment\u0000of hyponatremia in patients’ is\u0000still a major problem.\u0000Hyponatremia is a disorder observed\u0000commonly in toxicological\u0000practice. It is used in a variety of clinical\u0000entities in which pathogenic role of\u0000such particles has been proven e.g.\u0000ANCA-associated vasculitides (granulomatosis\u0000with polyangiitis [GPA]\u0000and microscopic polyangiitis [MPA]).\u0000Efficacy of plasmapheresis in ANCA\u0000antibodies removal and its impact\u0000on disease activity has not been adequately\u0000investigated so far. Influence\u0000of antibodies levels on disease activity\u0000also remains unknown.</p><p><strong>Objectives: </strong>Analysis of plasmapheresis\u0000effect on serum ANCA levels\u0000in patients with ANCA-associated\u0000vasculitides.</p><p><strong>Patients and methods: </strong>Seven patients with diagnoses of ANCA-associated\u0000vasculitides were enrolled\u0000in the study between November 2015\u0000and April 2016. All of them underwent\u0000plasmapheresis procedures. Serum\u0000ANCA levels were measured before\u0000and after plasmaphereses using fluoroimmunoenzymatic\u0000assay (FEIA).\u0000Disease activity was assessed using\u0000Birmingham Activity Vasculitis Score;\u0000BVAS ver. 3 before first plasma exchange\u0000procedure.</p><p><strong>Results: </strong>Patients in disease exacerbation\u0000(BVAS 5-10) with positive ANCA\u0000antibodies were enrolled in the study:\u00002 patients with GPA and 5 patients with\u0000MPA. Number of performed plasmapheresis\u0000procedures ranged from 5 to\u00007 (median=7). Streptococcus agalactiae is\u0000a gram-positive beta-hemolytic streptococci\u0000belonging to the serological\u0000group B (GBS). According to recent\u0000data the GBS is found in 10-30% of\u0000healthy women, and the carriage of\u0000pathogen is usually asymptomatic. In\u0000order to minimize the risk of vertical\u0000transmission of the infection to the\u0000newborn during the labor, the Polish\u0000Society of Gynecology (PTG) has introduced\u0000the guidelines of GBS colonization\u0000screening for all pregnant women.</p><p><strong>Objectives: </strong>In this paper, the level\u0000of implementation of the screening\u0000tests in accordance to the recommendations\u0000of the PTG was evaluated. We\u0000also assessed the level of knowledge\u0000and awareness of pregnant women in\u0000detection of GBS colonization during\u0000the pregnancy and the prevention of\u0000infection in newborn.</p><p><strong>Material and method: </strong>The survey\u0000was conducted among the 172 women\u0000hospitalized in hospital maternity\u0000wards in the first days of postpartum.</p><p><strong>Results: </strong>Despite the introduction\u0000of the Polish Society of Gynecology\u0000guidelines, there are still cases of improper\u0000screening methods and medical\u0000interventions in the event of positive\u0000test results. Biological material was\u0000collected in 86% of patients (swabs\u0000from the genital tract, anus or urine\u0000culture). A positive result was found\u0000in 22% of patients. In 54% of patients\u0000swabs were taken at the recommended\u0000period of 35-37 weeks. The recommended\u0000collection of the material\u0000both from vagina and anus was taken\u0000only in 39% of patients. The majority\u0000of participated women described their\u0000knowledge of the risk associated with group B Streptococcal\u0000colonization as good, however only half of them found\u0000acquired information sufficient. {"title":"[Qualitative and quantitative determination of caffeine in saliva by high performance liquid chromatography].","authors":"Marta Napierała, Ewa Florek","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The European Union drew attention\u0000to the increased consumption\u0000and the reports of adverse reactions\u0000to caffeine in recent years.\u0000The EU Commission in Regulation\u0000No. 536/2013 dated 11 June 2013 expressed\u0000concern about the caffeine\u0000content in the products.
