{"title":"Insanity as a consequence of an epileptic seizure and precautionary measures.","authors":"Przemysław Cynkier","doi":"10.12740/PP/OnlineFirst/154806","DOIUrl":"10.12740/PP/OnlineFirst/154806","url":null,"abstract":"<p><p>The analysis covers the case of a driver suffering from epilepsy for many years, who during an epileptic seizure, in a state of profound disturbance of consciousness, caused a road accident. Such situations usually result in the perpetrator being considered insane in relation to the allegation. The task of expert psychiatrists and psychologists is then to assess the likelihood of the perpetrator of the prohibited act repeating it, and to indicate to the court the optimal therapeutic precautionary or penal measure. These legal solutions also apply to perpetrators who were considered insane due to disturbances of consciousness occurring in the course of various somatic diseases, and not permanent mental disorders. Currently, there are no grounds for appointing expert neurologists, diabetologists, cardiologists, pulmonologists, and other specialists who would assess the legitimacy of taking precautionary measures, which may raise judicial doubts. Moreover, applying in such cases only the measures indicated in Article 93a § 1 of the Penal Code does not find any psychiatric and psychological justification. Consideration should be given to extending the catalog of protective measures to include the therapy of various somatic diseases in order to minimize the risk of developing deep mental disorders in the future. It represents a development of the second-generation antipsychotics and is an important addition to the pharmacological treatment options for schizophrenia. The purpose of this article is to present, illustrated by the case of brexpiprazole, how advances in the pharmacological properties of new antipsychotics translate into improved results in the treatment of schizophrenia, not only in terms of symptom reduction, but also in terms of functional improvement. The ratio of activation to blocking of the D2/D3 receptor is lower for brexpiprazole than for aripiprazole and cariprazine, which may translate into a lower risk of akathisia. Brexpiprazole has also stronger antihistaminic activity, which is likely to be associated with a stronger sedative effect, a lower risk of akathisia, excessive agitation and insomnia. Brexpiprazole meets the traditional requirements for an antipsychotic drug's efficacy, i.e., compared to placebo, it brings a greater reduction in schizophrenia symptoms in short-term studies and prevents schizophrenia relapses in long-term follow-up. The highest antipsychotic efficacy was found with the highest registered dose (4 mg/day). In addition to reducing positive symptoms, brexpiprazole treatment also leads to a reduction in negative and depressive symptoms, as well as anxiety. It has also a positive effect on patients' social and personal functioning and quality of life. This action of the drug is in line with the expectations of patients and their families regarding effective treatment. Despite the undeniable benefits associated with improved physical performance, as a result of the decrease of the underlying disease symptoms, it carries the risk of complications in the area of the patient's somatic and psychological status. Long-term circulatory failure can contribute to a weakening of the adaptative mechanism and consequently lead to a variety of emotional disruptions. Patients must face the fear of imminent physical, family, and social changes that LVAD requires. They may experience sleep disorders, mood disorders, anxiety disorders, and in the early postoperative period also disorders of consciousness with a pattern of delirium. For this reason, it is advisable to provide multidisciplinary medical care for the patient at all stages of treatment, including regular monitoring of general health and mental health. Second, we sought to determine the sociodemographic and loss-related correlates of PGD symptom severity according to ICD-11 and DSM-5-TR.</p><p><strong>Methods: </strong>People who had lost a spouse (N = 144) in the past six months were examined using the Polish versions of the Prolonged Grief Disorder-13 scale (PG-13) and Inventory of Complicated Grief (ICG). Selected PG-13 and ICG items were included in the analyses to cover the PGD criteria according to ICD-11 and DSM-5-TR.</p><p><strong>Results: </strong>Confirmatory factor analyses supported the one-dimensional structure of both sets of symptoms of the disorder. Briefer time since loss and loss due to an accident were associated with PGD symptom severity according to both ICD-11 and DSM-5-TR.</p><p><strong>Conclusions: </strong>PGD is a one-dimensional and internally consistent psychopathological syndrome. Research shows that the incidence of anxiety and stress disorders is increasing. For years, the social climate around medical staff in Poland has also been deteriorating. The media write more often about errors and omissions, and less about the daily work of medics. This leads to a decline in public confidence, as well as hate and acts of aggression. The occurrence of the phenomenon of violence against medical staff may be a factor in the development of many diseases associated with high levels of stress and should be of interest to occupational medicine services. The aim of the work is to describe the phenomenon of violence against health care workers and its impact on the working conditions and health of workers. The literature from 2010-2022 was reviewed in PubMed and Web of Science databases by entering the following entries: \"violence\", \"aggression\", \"healthcare\", \"stress\", \"nurses\", \"doctors\", \"workplace\". Eighty-three works on the occurrence of the phenomenon of violence against health care workers were qualified for the study. The results of the analysis indicate a shortage of studies on Polish medical entities. The phenomenon of violence against health care workers is a serious problem of public health in the world. The most common forms of workplace violence were verbal violence, physical assault, bullying, sexual harassment and racial harassment. Most often, the violence was committed by patients and their relatives, colleagues and superiors. Aggression towards medical staff is not a new phenomenon, and Poland as a country is not alone in dealing with this problem. Aggression and violence are most often observed in hospital departments, especially psychiatric departments, hospital emergency departments and emergency rooms. Patients and their families are most often regarded as the source of rude behavior. Crisis situations, such as the COVID-19 pandemic, have intensified the scale of the phenomenon. Managing a pandemic also requires establishing preventive procedures for aggression and violence. Additional factors hindering the work of medical personnel may lead to The shortage of studies on Polish medical entities indicates the need to conduct work aimed at determining the scale of the phenomenon and its causes, taking into account the division into organizational units as well as groups of patients and their relatives. Accurate determination of the scale of the phenomenon and predisposing factors will allow to take appropriate innovative preventive actions, which will contribute to limiting the negative consequences. In addition, it was decided to check whether there were any relationships between age, gender, duration of treatment, and the intensity of anxiety, divided by the occurrence of individual side effects.</p><p><strong>Methods: </strong>The study involved 91 patients and was conducted at the Hematology Clinic of the University Hospital in Krakow. The following questionnaires were used: created by the author, David Goldberg Questionnaire GHQ-28, and the four-dimensional 4DSQ Questionnaire.</p><p><strong>Results: </strong>The most frequently mentioned side effects of treatment were bone and joint pain, muscle cramps and pain, water retention, and fatigue. The mean duration of the disease was ten years. Our research showed that fatigue, nausea/indigestion, frequent infections, bone and joint pain, abdominal pain, and loss of appetite were the most common side effects of TKI treatment, which resulted in increased anxiety symptoms.</p><p><strong>Conclusions: </strong>The work of doctors, psychologists, and pharmaceutical companies on reducing/alleviating side effects may in the future affect a better quality of life for these patients. Early detection of severe anxiety and taking appropriate steps can prevent the emergence of more significant disorders. {"title":"Exercise addiction symptoms and mental health during the forced exercises deprivation in greatest COVID-19 pandemic restrictions in Poland.","authors":"Julia Wyszomirska, Ewa Martyniak, Monika Bąk-Sosnowska, Katarzyna Piekarska-Bugiel, Artur Chwalba, Marek Krzystanek","doi":"10.12740/PP/OnlineFirst/147190","DOIUrl":"10.12740/PP/OnlineFirst/147190","url":null,"abstract":"<p><strong>Objectives: </strong>The paper describes the relationship between the symptoms of exercise addiction, behavioral strategies in situations of limited workout possibilities and mental health state in exercising individuals.</p><p><strong>Methods: </strong>The study included 391 participants, 286 women (73.1%) and 105 men (26.9%), aged 18 to 68 years. The respondents were surveyed online after 17-19 days of barriers to routine training due to greatest COVID-19 restrictions in Poland. Subjects completed the Exercise Dependence Scale, General Health Questionnaire - 28 (GHQ-28) and questionnaires enabling the collection of demographic and clinical data as well as data related to exercise behaviors.</p><p><strong>Results: </strong>The variables related to exercise addiction and some related to behavioral changes are predictors of mental health, especially in terms of anxiety, insomnia and somatic symptoms. All the introduced variables accounted for 27.4% to 43.7% of the variation in the mental health status of the subjects, depending on GHQ subscales. Breaking the restriction rules by outdoor training protected against symptoms of psychological disorders, especially in relation to somatic symptoms (Beta = -0.23; p< 0.001). Individual assessment of stress induction in a given situation was a predictor of results in all GHQ subscales, which was the strongest for symptoms of anxiety and insomnia (Beta = 0.37; p< 0.001).</p><p><strong>Conclusions: </strong>Individuals with features of exercise addiction are at risk of deterioration of their well-being during forced abstinence. In addition, the subjective level of stress induction in a given situation is an important determinant that conditions psychological well-being, especially the aggravation of depressive symptoms. {"title":"Predictive efficiency of phoenixin, spexin and kisspeptin neuropeptides concentration levels in diagnosis of bipolar disorder in paediatric population.","authors":"Lena Cichoń, Małgorzata Janas-Kozik, Artur Pałasz, Krzysztof Maria Wilczyński, Aleksandra Suszka-Świtek, Anna Żmijowska, Ireneusz Jelonek","doi":"10.12740/PP/OnlineFirst/155178","DOIUrl":"10.12740/PP/OnlineFirst/155178","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of the study was to assess concentrations of the following neuropeptides: phoenixin, spexin and kisspeptin in venous blood serum of children and adolescents suffering from bipolar disorder, and by this their predictive efficiency in this disorder.</p><p><strong>Methods: </strong>The study covered 75 individuals with a mean age of 15.26 years (95% CI: 14.86-15.67), of which the study group comprised of 57 individuals diagnosed with bipolar affective disorder and the control group - 18 individuals with no psychiatric diagnosis and no pharmacological treatment. Phoenixin, spexin and kisspeptin levels were determined in the peripheral venous blood serum. Neuropeptide concentrations were measured with the enzyme-linked immunosorbent assay (ELISA).</p><p><strong>Results: </strong>The mean phoenixin concentration in the studied group equalled 1.57 ng/ml (95% CI: 1.35-1.79), while in the control group - 2.69 ng/ml (95% CI: 2.38-3; U Mann-Whitney test p-value < 0.05). For spexin, these results were 639.65 pg/ml (95% CI: 558.86-720.44) in the studied group, and 354.28 pg/ml (95% CI: 310.33-398.22; U Mann-Whitney test p-value < 0.05) in the control group. The observed differences were statistically significant. The mean concentration of kisspeptin levels in the studied group was 126.02 pg/ml (95% CI: 39.82-212.23; median: 59.85), while in the control group - 54.83 pg/ml (95% CI: 39.23-70.43; median: 51.3; U Mann-Whitney test p-value = 0.29), and the observed difference was not statistically significant.</p><p><strong>Conclusions: </strong>The occurrence of bipolar disorder symptoms is statistically significantly linked with a decreased phoenixin concentration and to a small degree - with an increased spexin concentration in blood serum of patients.
