In more than 95% of cases, the cause of IHD is coronary blood flow reduction caused by coronary artery atherosclerosis, therefore the term “coronary heart disease” is often used to describe this syndrome. Ischemic heart disease in assessment of the World Health Organization is still the most common causes of death in Poland and in the world. Due to chronic ischemic heart disease in Poland, 2.5% of the population suffers, i.e. about 1 million people, of whom 100,000. She has a heart attack every year. Over the last two decades in Poland, mortality from ischemic heart disease has increased in people under 65 years of age. In spite of wide prevention, there is a problem between the current guidelines and the patient's continuing of recommendations. Good communication between medical staff and patient with ischemic heart disease and increasing the pressure on education is a guarantee of therapeutic success, which will be reflected in the health, social and economic sphere. {"title":"The influence of emergency medical procedures and event circumstances on the acute effectiveness of resuscitation in out-of-hospital sudden cardiac arrest in adults","authors":"K. Sudden cardiac arrest is a serious medical event that may occur unexpectedly. It is a serious medical and social issue. Aim. The aim of the study is an analysis of the relationship between the type of medical rescue actions taken and return of spontaneous circulation (ROSC) in adults. Material and methods. The case-control study was based on the medical documentation of the Rescue Service in Katowice collected in 2017. The results have been presented by means of proportions (sample size and sampling rate), median and IQR. Non-parametric methods (Pearson’s chi-squared test or Mann-Whitney U test) were used to compare the group of ROSC-patients with the group of no-ROSCpatients. The default statistical significance adopted for the purpose of all analyses was 0.05. Results. The analysis covered 1713 out-of-hospital sudden cardiac arrest (SCA) cases. Male patients constituted 63.68% (N = 1091) of the group, whereas female patients constituted only 34.5% (N = 591) of the cases. Women were older than men (69.5 vs. 62.9; p = 0.000). However, defibrillation was more frequently applied in men than in women (p = 0.000). In a majority of cases, SCA occurred in domestic conditions (p = 0.000) during the day (p = 0.000). ROSC was reported in 591 cases (34.51%). The ROSC rate was higher in cases when witnesses provided first aid to patients (35.6 vs. 31.41%; p = 0.08). The rate was also higher in patients with ventricular fibrillation than in patients with asystole (56.27 vs. 24.95%; p = 0.000). Conclusions. ROSC depends on the emergency medical procedures actions performed at the place of incident. Drejza","doi":"10.25121/pnm.2018.31.6.361","DOIUrl":"https://doi.org/10.25121/pnm.2018.31.6.361","url":null,"abstract":"The aim of this publication is to illustrate the realistic potential of digital technologies – mobile applications, telemedicine, automated analysis systems applied in the several key elements in stroke patient management. According to data provided by WHO, it is estimated that strokes have caused 5.78 million deaths in 2016. Review has been conducted searching for digital health technologies used for stroke management in PubMed database, and several references have been snowballed from the search terms. More and more scientific evidence speak for the efficiency of using digital technologies in care of stroke patients. Unfortunately, most of the recommendations linked to digitalization of patient care are part of guidelines provided by American associations, with no European or Polish equivalents. The chosen technologies (and especially those making primary and secondary prevention feasible) can be easily applied by wide groups of patients and healthcare practitioners. Technological progress and the minimization of portable devices has made it possible to perform ultrasound examinations outside the hospital, including in emergency medical teams. The ability to diagnose internal bleeding in patients with injuries is one of the key diagnostic elements. Performing ultrasound based on F.A.S.T., BLUE or FEEL protocols in the conditions of medical rescue teams can significantly shorten the time of diagnostics by directing the patient directly to the emergency room. Thanks to the progress in medical technology it is possible to perform ultrasonography with the use of mobile devices. In an increasing number of medical rescue teams, portable ultrasound devices are available. In order to use these devices effectively, it is necessary to know the protocol of examination and interpretation of ultrasound image. This article describes the F.A.S.T. procedure. The purpose of the procedure is to detect free fluid in the peritoneal cavity and pericardium. When we observe hemodynamic instability with signs of anemia, we may assume that there has been internal hemorrhage. Thanks to this examination, the patient can be quickly diagnosed and transported directly from the ambulance or emergency department to the operating theatre, where he or she will be operated on immediately, without wasting unnecessary time, which is so important for the patient's health and life. Cerebrovascular events are the third most frequent cause of death globally and the most common cause of disability in the general population. There may be other, rarer disorders predisposing to cardiovascular complications, besides traditional risk factors of ischemic stroke, as the data prove, one of them is hyperhomocysteinemia. Aim. Relation between intima-media complex thickness (IMT) and homocysteine level in patients with ischemic stroke. Material and methods. The study group consisted of 32 patients with first-ever ischemic stroke diagnosed. The control group contained of 28 healthy people matched in terms of gender and age. The concentration of homocysteine was measured in the blood serum using the IMX immunoassay ABBOTT analyzer. Evaluation of intima-media thickness of carotid arteries was performed with Sonoline ELEGRA using a 7.5 L 40 line probe at 9 MHz. Results. The average homocysteine concentration in patients with ischemic stroke was 16.55 umol/L. Elevated values of homocysteine were found in 20 patients. In Doppler examination of the carotid arteries, the mean IMT in patients with stroke was 0.88. The correlations between the homocysteine level and the IMT were evaluatedin the control group and in patients with stroke. Conclusions.
