Using apparatuses assisting rescue operations in the municipal space constitutes the undeniable convenience and is helping with situations of the sudden threat to the health and the life of the aggrieved party. Showing real reasons for not-applying the AED equipment located in the public sphere in the situation of the NZK riot in extra-mural conditions was a purpose of the work. Material consisted of domestic and foreign publications, of world literature telling about preventive programs and the new AED shopping. also opinions were considered and guidelines of circles of experts in the cardiology. The increasing number of studies suggest that endothelial cells play an indirect role in tumor growth and metastasis. Due to the fact that distant metastases are the main cause of death in breast cancer, markers of early neoplastic dissemination, undetectable by available imaging methods, are sought and that could justify the use of more aggressive neoadjuvant and adjuvant therapies, as well as might be a new potential therapeutic goals. Adhesins involved in the regulation of processes of differentiation, proliferation, migration and apoptosis raised interest among oncologists. Among a large group of recognized markers of endothelial dysfunction, mostly adhesins: E-selectin, ICAM-1 and VCAM-1 were studied. Increase in the concentration of these adhesins occurs in patients with distant dissemination of breast cancer cells. At present, laparoscopic cholecystectomy is a type of minimally invasive intervention, mostly performed for the treatment of patients with cholelithiasis. A large number of procedures worldwide taken into consideration, the issue of the effect of various anaesthesia types on the central hemodynamics becomes increasingly important, since its state often determines possible complications of surgical intervention. Aim. Objective is to assess the effect of various combined anaesthesia types on the central hemodynamics in laparoscopic cholecystectomy. Material and methods. Examination included 119 patients (aged between 20 and 82) who had undergone surgery for acute cholecystitis. Laparoscopic cholecystectomy with the use of carboxy peritoneum (CP) was performed in all cases. Depending on the type of anaesthesia, patients were divided into 3 groups: Group 1 (n = 44) – those who were operated on under combined general intravenous anaesthesia (CGIA); Group 2 (n = 44) – the surgery was performed under combined inhalation anaesthesia (CIA) with sevoflurane; Group 3 (n = 31) – laparoscopic cholecystectomy was performed under combined spinal anaesthesia (CSA) with 0.5% bupivacaine. Subject to comparison were the indices of hemodynamic profile and intra-operative analgesia that was judged on the amount of narcotic analgesics used. The research was authorized by the Bio-ethical Commission of Kyiv City Hospital ? 4. Permission number is U22072016. All patients gave prior written permission to participate in this study. Results. Average systolic arterial pressure in the groups with carbon dioxide insufflated into abdominal cavity: Group 1 – 137.5 ± 18.9 mm Hg; Group 2 – 136.6 ± 17.7 mm Hg; Group 3 – 115.0 ± 13.7 mm Hg. In Group 3, systolic arterial pressure values differed statistically from those in groups 1 and 2 (p < 0.001). Average systolic arterial pressure under carboxy peritoneum: Group 1 – 101.7 ± 14.1 mm Hg; Group 2 – 100.0 ± 12.2 mm Hg; Group 3 – 86.7 ± 10.4 mm Hg. Average doses of fentanyl used for anaesthesia in the groups were: Group 1 – 11.7 mcg/kg; Group 2 – 10.9 mcg/kg; Group 3 – 6.28 mcg/kg. Conclusions. Patients with gastroesophageal reflux disease who did not improve with proton pomp inhibitors are at risk of serious diseases and should undergo endoscopy. Aim. The aim of the study was to present endoscopic and histopathologic findings in the esophagus in patients referred by a general practitioners to the Department of Gastrointestinal and General Surgery, Wroclaw Medical University in years 2008-2013 with initial diagnosis of GERD, in which the family doctor did not achieve the expected results of the empirical treatment with proton pump inhibitors. Material and methods. The clinical material included 122 patients (51 women and 71 men) aged 55.8 ± 14.1 referred for endoscopy by their general practitioners because of a lack of improvement after empirical treatment. Retrospective analysis of the endoscopic and histologic finding was performed. Results. The most prevalent diagnosis was hernia hiatus oesophagi (n = 41; 33.6%) and the least Barrett’s esophagus (n = 6; 4.9%). In men the most prevalent diagnosis was adenocarcinoma (n = 22; 31.0%) and in women hiatal hernia (n = 24; 47.1%). Near 60% patients were found to have histopathologic abnormalities. The most common finding was oesophageal cancer (n = 22; 18.0%) and the least dysplasia (n = 7; 5.7%). In men the most prevalent diagnosis was adenocarcinoma (n = 21; 29.6%) and in women esophagitis (n = 8; 11.3%) and Barrett’s esophagus (n = 8; 11.3%). Conclusions. 1. Near one quarter of patients with ineffective GERD symptoms treatment, who were referred for endoscopy by family practitioners, has serious endoscopic and histologic diagnosis. 2. Male gender and age above 52 years are risk factors for esophageal adenocarcinoma. 3. Siemiatkowski","doi":"10.25121/pnm2019.32.1.41","DOIUrl":"https://doi.org/10.25121/pnm2019.32.1.41","url":null,"abstract":"Postoperative pain is a complex and multifactorial symptom that requires a well thought approach using different treatments to achieve the optimal outcome after surgery. Contemporary anaesthesiology, looking for an alternative to analgesia with the use of opioids, more often turns to the protocols of low-opioid and opioid free treatment and pain control. By replacing opioids with non-opioid analgesics, koanalgetics, as well as using local and regional anesthetic techniques, we limit or avoid adverse effects of opioids while maintaining a satisfactory level of analgesia for the patient. Methods of general anesthesia without or with the minimum amount of opioid drugs are of particular importance in bariatric surgery due to a reduction in the incidence of post-operative respiratory depression and excessive sedation. They also allow to achieve and maintain cardiovascular stability in the intraoperative and postoperative period, prevent the occurrence of opioid-induced hyperalgesia (the so-called opioid paradox), and improve the comfort of patients in the post-operative period due to the lower incidence of post-operative nausea and vomiting and constipation. The particular significance of regional and local analgesia techniques, which supplement general anesthesia and reduce the need for analgesics in the perioperative period, is emphasized. It combines functions of a regular phone with those of a portable computer. Nowadays it is spread worldwide and it is starting to serve as an educational tool more and more each day. Aim. Characteristics of smartphone usage as an educational and management tool amongst chosen group of students in Poland. Material and methods. 299 students of Medical University in Lodz (44.1% emergency medical services, 34.8% nursing and 21.1% public health). 71.9% (n = 215) respondents were females. An anonymous Google survey was used. Results. 51.5% (n = 154) students owe smartphones, 4% (n = 12) do not know which type of a telephone they possess. 68.6% (n = 205) have internet access in their phone. In the study group almost every third student used their phone to browse through educational materials of their University and every fifth used educational materials of different Universities. Conclusions. Lately, intraperitoneal injection of local analgesics has been practised as an element of postoperative multimodal anaesthesia in some minimally invasive gynaecologic surgical interventions. Aim. To analyse efficacy of intraperitoneal bupivacaine injection for pain relief after laparoscopic cholecystectomy. Material and methods. Randomized double-blind clinical examination included 28 patients, aged between 32 and 60 (16 women and 12 men) who had undergone laparoscopic cholecystectomy. Patients were randomly divided into 2 groups, depending on whether bupivacaine had been applied for 24 hours in complex early postoperative anaesthesia (0.125% solution intraperitoneally, in a dose of 0.6 ml/kg (0.7 mg/kg of bupivacaine substance), every 6 hours). Patients’ condition was assessed on a visual analogue scale (VAS) 1, 4, 8, 12, and 24 hours after surgery. In addition, average number of fentanyl (mcg/kg) that had been used for a single pain relief was established. Results. Intraperitoneal bupivacaine injection after laparoscopic cholecystectomy had marked analgesic effect which revealed itself in reliable 1.47-1.77 times decrease in pain index on the VAS, and 1.55 times decrease in the analgesic dose of fentanyl. Conclusions. Hryniewicz","doi":"10.25121/pnm2019.32.1.31","DOIUrl":"https://doi.org/10.25121/pnm2019.32.1.31","url":null,"abstract":"The study presents the current problems concerning the principles of physiotherapeutic treatment of shoulder pain syndromes in the course of rheumatic diseases. The principles of the treatment in the most common rheumatic diseases such as RA, OA and SpA are discussed. Particular attention has been paid to the importance of kinesitherapeutic treatment, while indicating the role of treatment with physical methods in the field of thermotherapy, electrotherapy, laser therapy, the use of ultrasounds and magnetic field. Balneological treatment with myorelaxing, analgesic and anti-inflammatory effects is of complementary importance. The summary emphasizes that the goal of rehabilitation in shoulder pain syndromes in the course of rheumatic diseases is not only to achieve analgesic effect and improve the functional status, but, above all, an individual and comprehensive therapy aimed at improving the efficiency of the patients, enabling return to daily activities. The implementation of an effective and complex therapeutic program requires the simultaneous undertaking of comprehensive activities: educational, developing an individual strategy of physiotherapeutic and psychotherapeutic treatment, the use of individually selected orthopedic supplies, as well as offering social assistance.
