Mahboob, Nudrat Zeba","doi":"10.51253/pafmj.v6i6.2610","DOIUrl":"https://doi.org/10.51253/pafmj.v6i6.2610","url":null,"abstract":"Objective: To share our experience of handling mass casualties at remote locations with an aim to help formulate a policy regarding future training of doctors. \u0000Study Design: Case series. \u0000Place and Duration of Study: Combined Military Hospital Thal Pakistan, from Jan 2016 to Sep 2018, including three months in Forward Treatment Center in operational areas. \u0000Methodology: The injuries due to war trauma were included in our study. Resource management and changes made to accommodate the influx of mass trauma that required damage control surgery were described. \u0000Results: A total of 16 casualties from two mass casualty incidents at two medical centers were included in our study. There was no difference in triage class (p=0.96). Splinter injury limbs were most common injury. One patient required damage control surgery done at Combined Miliary Hospital with most requiring hemostasis prior to transport (8 at Combined Military Hospital versus 3 at Forward Treatment center), p=0.346. \u0000Conclusion: Most of mass casualties at our hospital were minor injuries requiring immediate first aid. Early post operative feeding is a risk factor for development of PCF. Delayed oral feeding (DOF) and inserting a nasogastric tube has been considered a safe practice among head and neck surgeons, and there is no general agreement on the timing of initiation of the oral intake. This study compared the effect of EOF and DOF on PCF formation. \u0000Study Design: Prospective case-controlled trial. \u0000Place and Duration of Study: Combined Military Hospital Rawalpindi, from Apr 2019 to Jul 2020. \u0000Methodology: Non-probability convenience sampling was done for both groups. The patients were not matched and were assigned to either EOF or DOF alternately. EOF was defined as feeding at seventh post operative day and DOF criterion was feeding on fourteenth post operative day. The primary outcome was development of PCF within the 30th post operative day. A total of 20 patients with prior radiotherapy for laryngeal cancer who presented with recurrence of carcinoma were included in the study. Same technique closure of neopharynx was done in all cases of laryngectomy. \u0000Results: Overall PCF frequency was 20% (4/20). In EOF group, 30% (3/10) of patients developed PCF whereas 10% (1/10) of patients in DOF developed PCF. The questionnaire was distributed to the participants of both institutions; data was collected & entered into SPSS version 22. \u0000Results: Female predominance (64.4%) was observed in post-graduates of Children’s Hospital, Lahore in comparison to institutions from Balochistan while there were 61% males. Majority (90%) post-graduate residents of Balochistan recognized time management, deficient laboratory and other facilities followed by lack of mentoring by faculty 59 (83%) and lack of motivation 60 (84%) as the most important factors that hinder their scientific activities. Additionally, 91 (86.6%) postgraduates of Children’s Hospital, Lahore suggested lack of proper research training and 87 (83.3%) suggested lack of previous exposure as the main hindering factors in their research work. \u0000Conclusion: Post-graduate residents of diverse economic, educational and cultural backgrounds present different perceptions about research hurdles. After taking relevant history and physical examination, venous blood sample of each patient was taken and sent to institutional laboratory for analysis of serum total cholesterol (TC) level, serum triglyceride (TG) level, low density lipoprotein (LDL), high density lipoprotein (HDL) were analyzed. Body mass index among all the study participants was also calculated. \u0000Results: Out of a total of 320 cases, there were 152 (47.5%) male and 168 (52.5%) female. Mean age was 41.7 ± 8.1 years. Most of the cases, 97 (30.3%) were between 41-50 years of group. Dyslipidemia was noted in 144 (45%) cases. Increasing age and increasing Body mass index were found to have statistical significance with the presence of dyslipidemia (p-value <0.05). \u0000Conclusion: Increasing age and body mass index have significant association with dyslipidemia in patients with chronic hepatitis C virus infection. Questionnaire was made following seven steps as mentioned in AMEE (Association for Medical Education Europe) guide 87. Data was analyzed by SPSS version 21. Reliability was be checked by Cronbach@. Descriptive statistics of responses including frequencies and percentages were calculated. Exploratory factor analysis of questionnaire was made. \u0000Results: Skin lightening cream users had age range from 16-56 years, majority 190 (76%) were females. Out of 158 (63.2%) were married, 180 (72%) well educated and 146 (58.4%) had Fitzpatrick skin type 3-5 predominate. Cronbach alpha of our questionnaire was 0.802. The 5-point likert scale was used to record users’ perceptions in the domains i.e., motivational factors, awareness about product and intentions. Majority, 89 (35.6%) used the product on social media recommendation. Out of 201 (80.4%) developed side effects with telangiectasia and skin thinning being the commonest ones. Out of 88 (35.2%) were using other skin whitening practices besides these creams. \u0000Conclusion: False perceptions about fairness and lack of user’s awareness govern the skin lightening practices in our society resulting in increased usage and side effects experienced by users. Demographic sheet, 36- Item Short Form Health Survey (SF-36) and Quick Disabilities of the Arm, Shoulder and Hand questionnaire (Quick-DASH) were used to collect data followed by analysis using SPSS-22. \u0000Results: Mean score on Quick Disabilities of the Arm, Shoulder and Hand questionnaire was 49.2+25.4. Usman","doi":"10.51253/pafmj.v6i6.5685","DOIUrl":"https://doi.org/10.51253/pafmj.v6i6.5685","url":null,"abstract":"Objective: To assess the cross-reactivity of COVID-19 IgG assay with known immune-mediated and infectious disorders and evaluate for any false-positive reactions to determine the specificity of the serological assay. \u0000Study Design: Cross-sectional analytical study. \u0000Place and Duration of Study: Department of Chemical Pathology, Chughtai Institute of Pathology, Lahore Pakistan, from Sep to Oct 2020. \u0000Methodology: A total of 116 samples were included in the study of both males and females. Diagnosed cases of typhoid fever, viral hepatitis, systemic lupus erythematosus (SLE), syphilis, multiple connective tissue disorders (MCTD), varicella-zoster infection, rabies, toxoplasmosis, epstein-barr virus (EBV) infection, rubella, rheumatoid arthritis, AIDS, cytomegalovirus (CMV) infection and dengue fever were included in the study. Three samples of multiparous women aged more than 40 years were also included in the study. IgG antibody levels were measured against SARS-CoV-2 with a cut-off index of 1.4. \u0000Results: Out of 116, only 3 (2%) samples were reactive for IgG against SARS-CoV-2. The categories showing cross-reactivity were typhoid, hepatitis C, and CMV. All specimens showing cross-reactivity were of females. Assay under consideration showed a specificity of 97.4%. \u0000Conclusion: Cross-reactivity was seen in pre-pandemic cases of infectious diseases with COVID-19 IgG antibody assay. Saleem, Usman Arif","doi":"10.51253/pafmj.v6i6.7198","DOIUrl":"https://doi.org/10.51253/pafmj.v6i6.7198","url":null,"abstract":"Objective: To determine the frequency of anterior knee pain after intramedullary interlocking nailing in tibial shaft fractures and to compare the intensity of anterior knee pain between positive and negative anterior cortex nail groups.Study Design: Quasi-experimental study. \u0000Place and Duration of Study: Department of Orthopaedic Surgery, Combined Military Hospital Rawalpindi, from Oct 2018 to Apr 2019. \u0000Methodology: 100 cases of tibial shaft fracture were divided into two groups according to anterior cortex nail distance. Patients 18-40 years of age of both genders, closed fractures, Gustilo Andersen I open tibial diaphyseal fractures and nail tip more than 5mm from tibial tuberosity were included. Patients with osteoarthritis, pathological fractures, renal disease, open fracture GA-II & GA-III and knee instability were excluded from the study. Group-I patients were given topical anesthesia with 2% Lignocaine while group-II patients, in addition to topical Lignocaine, had 2% Lignocaine injected into bilateral internal laryngeal nerves for total laryngeal anesthesia. Assessment of efficacy of anesthesia was evaluated by Reasoner scale. \u0000Results: In group-I, 26 (74.28%) patients showed mild or moderate cough and gagging during stage-1. Fifteen (42.85%) patients showed moderate cough and gagging that interfered with the procedure during stage-2 and 19 (54.28%) patients showed mild cough or gagging that did not interfere with the procedure in stage-3. In group-II, 17 (48.57%) patients exhibited mild cough or gagging during stage-1. Sixteen (45.71%) exhibited mild cough or gagging that did not interfere with the procedure during stage-2 with all the patients showing either no cough or mild cough and gagging that did not interfere with the procedure during stage-3. Iqbal","doi":"10.51253/pafmj.v1i1.7929","DOIUrl":"https://doi.org/10.51253/pafmj.v1i1.7929","url":null,"abstract":"Objective: To carry out the comparison of the effectiveness of Semont manoeuvre and Vestibular Sedative in the management of benign paroxysmal positional vertigo (BPPV). \u0000Study Design: Quasi experimental study. \u0000Place and Duration of Study: Department of ENT, Combined Military Hospital Peshawar Pakistan, from Jan to Jul 2020. \u0000Methodology: The study was conducted on 60 patients who were diagnosed Benign Paroxysmal Positional Vertigo by Dix Hallpike test. The patients were divided into 2 groups of 30 each. Group A was treated with vestibular sedative and group B was treated with Semont manoeuvre. The examination of all patients was done on day 3, day 7 and day 14 and the results were documented. \u0000Results: The study was conducted on 60 patients who were diagnosed benign paroxysmal positional vertigo by Dix Hallpike test the study revealed that the 30 cases which were treated by Semont manoeuvre, 25 cases were completely symptom free after 14 days. Meanwhile out of 30 cases which were managed by oral vestibular sedative, only 4 cases showed complete recovery after 14 days. Chi square test was used to compare the results.
