In adults, and mostly in women, we observe: 1. Köhler’s disease among girls wearing improper shoes. 2. Insufficiency and pain of the front part of feet connected with limited toes flexion, 3. Valgus deformity of the big toes (hallux valgus), 4. “Ankle Joint Pain Syndrome” (AJPS)-sometimes also “Knee Joint Pain Syndrome” (KJPS)-described by us only in USA, India and Czech Republic. In presented article, we describe this special type of foot insufficiency- “instability of ankle or knee, or both joints”-on left leg in drivers and right leg in passengers in countries with right-hand traffic. The illness is known in orthopedics as Legg - Waldenström - Calve - Perthes disease, described as the Perthes disease, or LCP disease. The treatment in all Orthopedics Centers mostly is similar - conservative, by physiotherapy and in cases with deformity of proximal end of femur-like varus deformity, partially dislocation / lateralization of femoral head-is done surgery. In many congresses and symposia-discussions about causes of the Perthes disease are very controversial. In our article we describe the causes of this illness. Late-stage ulcers often lead to chronic osteomyelitis, which carries high rates of morbidity and mortality, as well as significant healthcare costs. Refractory decubitus ulcers are most commonly seen in the pelvis and pose a clinical challenge to provider teams caring for SCI patients. With long-term disease, surgical and antibiotic therapies may fail, which places patients at risk for entering a terminal disease state. A radical treatment for this condition was described in the early 20th century and involves amputation of the lower half of the body. This procedure, the hemicorporectomy, continues to be performed today in a very small, select group of patients. Here, a case report is described of a 39-year-old female T12 paraplegic with refractory decubitus ulcers and terminal pelvic osteomyelitis that underwent hemicorporectomy as definitive treatment. Neal, Brett Lewellyn","doi":"10.21203/RS.3.RS-141489/V1","DOIUrl":"https://doi.org/10.21203/RS.3.RS-141489/V1","url":null,"abstract":"Purpose: The primary purpose of this study was to compare intraoperative and post-operative complication rates for upper extremity surgical cases performed on a standard operating room (OR) table with similar cases done on a typical hospital stretcher. Secondary measures reviewed included surgical time, turnover time, total OR time, blood loss, tourniquet time, and postoperative complications. Methods: Using our institution’s electronic medical record system, we reviewed 100 consecutive upper extremity cases performed on a stretcher as well as 100 consecutive upper extremity cases done on a standard OR table. All cases were performed by the same board certified, fellowship trained orthopaedic surgeon. The cases were performed between February of 2014 and May of 2016 at a level one trauma center and its associated outpatient surgical center. Basic univariate statistical analyses were performed, and the two groups were compared for primary and secondary outcome measures. Results: The data showed no significant increase in intraoperative complication rates when operating on a standard hospital stretcher compared to operating on an OR table. There were a total of 6 postoperative complications in the stretcher group and a total of 11 complications in the OR table group. The most common postoperative complication seen in both cohorts was infection. There was one intraoperative complication in the OR table group and none in the stretcher group. With regard to total operating room time, surgical time, and delta time (overall OR room time minus surgical time which was used to calculate the turnover time), we found that the OR table group had shorter times in each category. The total OR time for the OR table group was a mean time of 105 minutes compared to 146 minutes seen in the stretcher group (p= 0.0002). Similarly, there was a shorter mean surgical time for surgeries done on an OR table (73 minutes) when compared to surgeries done on a stretcher (104 minutes) (p = 0.0026). Finally, the average turnover time (delta time) for the OR table group was 32 minutes while the average turnover time for the stretcher group was 42 minutes (p= 0.0002). The average tourniquet time for the OR table group was 36 minutes as compared to 41 in the stretcher group (p=0.467). Question/Purpose: The purpose of this study was to evaluate caffeine consumption among orthopedic surgery residents. We hypothesized that compared to the general population, residents will consume more caffeine. Patients/Methods: An electronic survey consisting of 13 multiple-choice questions was created to query residents on their caffeine consumption. The survey link was e-mailed to all residency program coordinators in the United States who then forwarded the survey to their residents. A subgroup analysis was performed based on respondents’ post-graduate year (PGY), to identify variations in caffeine use. Results: We received 209 surveys which represents 5.4% of orthopedic residents in the country. Of those 209 residents, 189 (90.4%) indicated regular caffeine usage. Of those who consume caffeine 59.8% indicated that they need caffeine to perform their work while 64.6% indicated that they are dependent on caffeine. Nearly one-fourth (24.3%) of respondents indicated that they consume more than 400 mg of caffeine/day. There were no significant relationships across resident PGY groups. Conclusion: We have demonstrated a large proportion of orthopedic surgery residents consume caffeine. When used in moderation, caffeine can be beneficial by improving efficiency, alertness and memory however, in high doses it may have undesirable side effects. It remains unknown why lupus arthritis lacks bone erosion and destruction. Our recent published paper presents the interesting discovery that joint deposited lupus IgG triggers synovitis but suppresses osteoclastogenesis which is responsible for bone destruction. In this paper, data show that joint deposited lupus IgG induces synovitis through FcgRI on monocytes/macrophages and blocks RANKL-induced osteoclastogenesis through competing for FcgRI binding with RANKL. This study was designed to compare the outcome, morbidity and complications of treatment with pre-peritoneal pelvic pack with external fixator against pelvic pack alone in traumatic pelvic fracture and unstable hemodynamic patients.MethodsIn a retrospective case control study, patients with pelvic fracture and unstable hemodynamic state who referred to emergency and surgery department from August 2018 to August 2019 were enrolled by census manner. In the control group, 25 patients treated just by a pre-peritoneal pack (PPP group), while in the case group, pre-peritoneal pack and external fixator was done as the procedure to control bleeding (PPP Plus fixator group) in 22 individuals. Two groups were compared for presence of thromboembolism, hospital stay, infection, BUN, creatinine, rate of blood transfusion and mortality. ResultsMean age of participants were 34.48 ± 13.79 in pelvic pack group, while it was 32.36±16.29 in PPP plus external fixator ones. The mean Injury Severity Score (ISS) was 23.12±9.85 in Pelvic Pack group, and 19.22±8.09 in PPP plus external fixator group. Acute kidney injury was reported in 6 (24%) individuals of PPP group, while its rate in PPP plus fixator group was 40.9%. Mortality rate was reported 32% and 4.5% in control and case groups, respectively. There was a significant difference between groups (p=0.017, LR=6.42). VTE was reported 12% and 4.5% in PPP group and PPP plus fixator ones, respectively.ConclusionIt was concluded that using external fixator with PPP can be a useful method for treatment of pelvic fracture that is unstable in hemodynamic situation. The expansion of Medicare telehealth allowing all beneficiaries to receive telehealth in any location, including their homes, was the first action taken by the CMS. Soon afterwards, Senator Ted Cruz introduced in Congress a new bill called the “Equal Access to Care Act,” which will open doors to interstate telemedicine. The CMS reported that prior to the COVID-19 pandemic, the average number of telehealth visits conducted by medical providers was 13,000.
