The most commonly used scale is the Expanded Disability Status Scale (EDSS) for obtaining information on neurological impairments. The main disadvantage of this scale is that it focuses a lot on the ability to walk and is not adequately sensitive for detecting the changes in the patient in time. The Functional Independence Measure (FIM) is the most commonly used scale in rehabilitation. It evaluates the ability of patients to perform daily activities, for example, feeding and dressing. In the study, we focused on the comparison of EDSS and FIM to ascertain the applicability of FIM in patients with multiple sclerosis. \u0000Methods: We performed a cross-sectional study of 38 patients with multiple sclerosis by talking to the patients and reviewing the medical documentation where the EDSS evaluation was conducted. Then, we evaluated the patient using the FIM scale. The data were processed using the IBM SPSS Statistics 25 software and displayed graphically using the Microsoft Excel program. \u0000Results: The statistical analysis showed a strong negative correlation between EDSS and FIM (r = -0.731). The motor part of the FIM scale had a strong negative correlation with EDSS (r = -0.732). The cognitive part had a weak negative correlation (r = -0.262). The sex and the duration of the disease did not impact the FIM evaluation significantly. {"title":"Prolonged, recurrent, spontaneous massive gluteal and femoral bleeding in a patient with five metachronous primary malignancies","authors":"A. Čretnik, Pia Šumer, S. Breznik","doi":"10.18690/actabiomed.244","DOIUrl":"https://doi.org/10.18690/actabiomed.244","url":null,"abstract":"Purpose: This study aimed to present the case of prolonged, recurrent, spontaneous massive bleeding in a patient with five metachronous primary malignancies. \u0000Case presentation: A 70-year-old male patient presented with massive hematomas in the gluteal and femoral regions and a 6-year history of prostatic, urethral (penile), right colonic, and right pulmonary carcinoma, with a myeloproliferative disease that recently ended with lymphatic leukemia. Fasciotomies with the evacuation of massive hematomas were performed due to the development of compartment syndrome, which were followed by persistent recurrent bleeding. Despite several surgical interventions with packing, local hemostatic agents and sutures, and selective embolization with immediate clinical discontinuation of bleeding, the digital subtraction angiography showed recurrent bleeding from the peripheral branches of the internal iliac and deep femoral arteries always a few days after the procedures. The coagulation and aggregation parameters and bleeding disorders tests were within normal ranges. The histology showed no malignancy in the affected regions, and no relapse was reported in any regions with previously operated malignant diseases on control computed tomography and positron emission tomography scans. \u0000Conclusion: We could stop the bleeding only with more aggressive, nontarget gradual embolization. {"title":"Child and adolescent mental health services during the COVID-19 epidemic in Slovenia: Comparison with other European countries","authors":"H. GREGORIČ KUMPERŠČAK, Ivan Podlesnik Peršak","doi":"10.18690/actabiomed.240","DOIUrl":"https://doi.org/10.18690/actabiomed.240","url":null,"abstract":"Purpose: To evaluate the impact of the COVID-19 epidemic on child and adolescent mental health services (CHAMS) during the first and second epidemic wave. \u0000Methods: Members of the Slovenian Association for Child and Adolescent Psychiatry (n=66) were invited to participate in the study. The survey involved two semi-structured questionnaires on CHAMS and clinical and organizational issues with respect to the first and second COVID-19 waves. The findings were compared with similar studies conducted in other European countries. \u0000Results: Eighteen members fulfilled the first and seven the second questioner. Thirty-three percent of respondents reported that their CHAMS colleagues were assigned to the COVID-19 wards during the first and 43% in the second wave. The CHAMS were more affected by the public health measures for COVID-19 during the first wave. During the first wave, half of the respondents experienced worsening anxiety disorders, while an increase in all mental disorders was reported during the second wave. \u0000Conclusion: A concerning finding of this study was limited access to CHAMS as a result of public health measures, especially during the first wave. But the provision of services in Slovenia was not as severely limited as elsewhere in Europe. CHAMS in Slovenia were able to provide better employee safety measures in terms of COVID-19 infections compared to other European countries. {"title":"Case report: Endodontic treatment of a permanent maxillary first molar with three roots and six root canals","authors":"M. Novak, I. Štamfelj","doi":"10.18690/actabiomed.242","DOIUrl":"https://doi.org/10.18690/actabiomed.242","url":null,"abstract":"Purpose: This study aimed to report the diagnosis and management of a permanent maxillary first molar (PMFM) with unusually complex root canal morphology. \u0000Case report: A 30-year-old male patient was referred to the endodontic department to continue endodontic treatment of the left PMFM. The referring dentist could not establish the patency of the canals in the mesiobuccal and distobuccal roots. Radiographically, the tooth had three almost straight roots associated with periapical radiolucencies, indicating periapical periodontitis. The examination of the pulp chamber floor, using a dental operating microscope, revealed the presence of six root canal orifices, three in the mesiobuccal root, two in the distobuccal root, and one in the palatal root. No confluence of multiple canals was detected using the electronic apex locator and K-files. The canals were shaped and cleaned with reciprocating nickel-titanium instruments and obturated using gutta-percha cones and a bioceramic sealer. After 2 years, the patient was asymptomatic and the tooth was clinically and radiographically without pathological findings. \u0000Conclusions: Careful examination of the pulp chamber floor using magnification and illumination is important for successfully managing complex canal morphologies. Detailed knowledge of imaging findings is needed to avoid unnecessary invasive diagnostic procedures. \u0000Materials and Methods: One follow-up case and three companion cases are presented. All children were between the ages of 5 and 10 years. \u0000Results: The MRI findings demonstrated ill-defined subcutaneous masses in three cases while in one case the mass was mildly hypointense and heterogeneous on fluid-sensitive sequences. Contrast agent was administered intravenously only in one case, where moderate enhancement was observed. \u0000Conclusion: Detailed clinical description of the lesion location was crucial. This study aimed to examine the incidence of pediatric burns requiring hospitalization, and different treatment modalities and outcomes. \u0000Methods: In this retrospective observational study, 28 children with burns (19 boys and nine girls) were admitted between 2014 and 2018 for treatment. We monitored the incidence of burns and analyzed different treatment modalities and treatment outcomes. Children in this study were treated at the Pediatric Intensive Care Unit and the Department of Plastic and Reconstructive Surgery of the University Medical Centre Maribor. \u0000Results: Scalds were found to be the most common type of burns in children (68%). Children younger than 5 years of age were the most affected (75%). The average burned total body surface area was 9%. Furthermore, 89% of children needed surgical treatment. The hospitalization lasted for an average of 14.6 days, with complete healing occurring in 72% of children within 4 weeks. \u0000Conclusions: This study showed that the most vulnerable age group of children with severe burns who required hospitalization was between 1 and 2 years and that the most common type of burn was scalding. Studies suggest that the anti-inflammatory response is more detrimental to the patient with abdominal sepsis than the initial pro-inflammatory response. We therefore studied the serum levels of pro-inflammatory and anti-inflammatory interleukins in patients with abdominal sepsis treated by abbreviated laparotomy. \u0000Methods: We performed a prospective study of 42 patients treated by abbreviated laparotomy. The patients were divided into a high-risk (more than two procedures before abdominal closure) group (24 patients) and a low-risk (two or fewer procedures) group (18 patients). The differences and correlations between the serum levels of pro-inflammatory and anti-inflammatory cytokines on days one and ten after the initial procedure were assessed. \u0000Results: The mortality was significantly higher in the high-risk group (41.7% vs. 5.6% in the low-risk group; p = 0.012). IL-10 serum levels were significantly higher in the high-risk group (22.5 pg/mL (IQR 5.25)) compared to the low-risk group (12.15 pg/mL (IQR 6.725)) (p = 0.012). Age was significantly correlated with mortality (p = 0.007). The logarithmic value of IL-10 serum levels on day one (HR: 2.5; 95% CI: 1.109–5.638; p = 0.027) and the IL-10 cut-off value (HR: 3.816; 95% CI: 1.047–13.910; p = 0.042) were significantly correlated with worse disease course in multivariate analysis. \u0000Conclusions: Patients who exhibit a greater anti-inflammatory response on day one are at increased risk of a protracted course and higher mortality. Prva številka je izšla 1.12.2008. Revija je bila zasnovana kot znanstvenoraziskovalna in strokovna revija s področja medicine in biotehnike, njen ustanovitelj in izdajatelj je Medicinska fakulteta Univerze v Mariboru. Acta medico–biotechnica je tako prva znanstvena revija v izdaji Medicinske fakultete v Sloveniji. Revija je namenjena uveljavljanju in razširjanju znanstvenih spoznanj s področja medicine in biotehnike s poudarkom na interdisciplinarnem pristopu, njen namen je povezovanje kliničnega in raziskovalnega dela. Revija izhaja dvakrat letno v slovenskem in angleškem jeziku, saj je del mednarodnega evropskega prostora. Odziv raziskovalcev na področju medicine in biotehničnih ved je bil za ta prostor izreden. {"title":"Nagovor dekana Medicinske fakultete Univerze v Mariboru","authors":"Ivan Krajnc","doi":"10.18690/actabiomed.1","DOIUrl":"https://doi.org/10.18690/actabiomed.1","url":null,"abstract":"Mnogi, ki prebirate te vrstice, veste, da je Medicinska fakulteta Univerze v Mariboru (MF UM) mlada fakulteta, ki po letih ni niti prvošolček, pa vendar je v vaših rokah že prva številka znanstvene revije, katere izid nam je uspel že v letu, v katerem je bila revija zasnova. Tako kot fakulteta, ki je bila ustanovljena z Odlokom o preoblikovanju Univerze v Mariboru, sprejetim v Državnem zboru Republike Slovenije 2. oktobra leta 2003, in na katero se je prva generacija študentov vpisala v letu 2004/05, tako je tudi revija dosegla svoj prvi cilj. Načrtovanih ciljev pa je še veliko in verjamem, da bodo doseženi. \u0000Septembra 2004 smo na Ljubljanski ulici 5 svečano odprli zgradbo, v kateri se nahajata Inštitut za anatomijo, histologijo in embriologijo ter Inštitut za fiziologijo. Dobro leto po tem je minister za visoko šolstvo, znanost in tehnologijo namenu predal obnovljene prostore Laboratorijskega centra MF UM na Magdalenskem trgu. V letu 2007 smo registrirali novi raziskovalni skupini in številne raziskovalce. Vse to je bilo v prid razvoju znanstvenoraziskovalnega dela na mladi fakulteti. V želji povezati čim več posameznikov, ki opravljajo raziskovalno in razvojno dejavnost na Univerzi v Mariboru, smo organizirali kar tri sestanke raziskovalcev, in sicer dva v letu 2007 in enega letos, na katerih smo beležili skupno več kot 150 udeležencev. Srečanja bomo organizirali tudi v prihodnje, saj bomo podprli pobudo strokovnega direktorja UKC, da srečanja postanejo tradicionalna. Prepričani smo, da je predstavitev raziskovalnega dela povezala posamezne strokovnjake, kar se nedvomno kaže tudi v tem, da bodo naši raziskovalci v novem programskem obdobju aktivni v več raziskovalnih programih oziroma skupinah, kot so bili v preteklosti. \u0000Ob vsem tem smo spoznali, da imamo v tem prostoru velik potencial za nastanek in razvoj znanstvene in strokovne revije, in se odločili še močneje povezovati medicinsko in biotehnično znanost. {"title":"Ob izidu 1. številke revije Acta medico-biotechnica","authors":"Dušica Pahor","doi":"10.18690/actabiomed.2","DOIUrl":"https://doi.org/10.18690/actabiomed.2","url":null,"abstract":"Pred nami je prva številka znanstvene revije Acta medico-biotehnica, ki jo izdaja Medicinska fakulteta Univerze v Mariboru, ki je obenem tudi njen ustanovitelj. Veseli in hkrati ponosni smo, da nam je uspelo v zavidljivo kratkem času prehoditi pot od začetnih idej in želja pa vse do uresničitve tega tako visoko zastavljenega cilja. Številni so verjeli, da se bo ta zahteven projekt uresničil, verjeli so v nas in v to, da nam bo uspelo kljub skromnim izdajateljskim izkušnjam. To nam je dajalo moč. Seveda pa so obstajali tudi skeptiki, ki so menili, da se nam ne bo posrečilo. \u0000Vztrajnost, trdo delo, nabiranje izkušenj, pomoč in predlogi bolj izkušenih so morali dati rezultate. Tako se je v desetih mesecih rodila nova revija, in sicer revija, za katero upamo, da bo s svojo kvaliteto dokazala upravičenost nastanka in v naslednjih letih prispevala k razvoju in doprinosu znanosti s področja biomedicine. \u0000In kako je nastajala in se rodila naša revija? Naj omenimo le nekaj dogodkov. \u0000Aktivnosti so se začele na pobudo dekana Medicinske fakultete, red. prof. dr. Ivana Krajnca, dr. med., in s sklepom Senata Medicinske fakultete 28. januarja 2008. Za koordinatorja vseh aktivnosti je bila imenovana izr. prof. dr. Dušica Pahor, dr. med. V marcu 2008 so senatorji sprejeli sklep o potrditvi Akta o ureditvi ustanoviteljskih razmerij, s katerim je bilo določeno delovanje revije. \u0000Na prvi seji Izdajateljskega sveta 4. aprila 2008, ki ji je predsedoval red. prof. dr. Ivan Krajnc, dr. med., dekan Medicinske fakultete Univerze v Mariboru, je bil sprejet predlog o programski zasnovi revije, za glavno urednico pa je bila imenovana izr. prof. dr. Dušica Pahor, dr. med. 22. maja 2008 je Ministrstvo za kulturo Republike Slovenije vpisalo revijo v razvid medijev pod številko 1339 (odločba 61510-143/2008/4). \u0000Revija Acta medico-biotechnica bo objavljala znanstvene članke s področij medicine in biotehnike, ki bodo dolgoročno vplivali na temeljne in praktične vidike teh področij. Obravnavala bo področja medicinskih ter področje biotehničnih znanosti s poudarkom na interdisciplinarnem pristopu. \u0000Namen revije je uveljavljanje in širjenje znanstvenih spoznanj s področja medicine in biotehnike. \u0000Pri izbiri člankov se bodo upoštevali uveljavljeni kriteriji tovrstnih revij v svetu, kar pomeni, da bodo vsi prispevki recenzirani s strani najmanj dveh priznanih tujih ali domačih strokovnjakov z obravnavanega področja. Revija bo objavljala le visoko kvalitetne znanstvene prispevke in tako prispevala k uveljavitvi revije in njenemu mednarodnemu priznanju. \u0000Revija izhaja pod okriljem Univerze v Mariboru, Medicinske fakultete. Vodijo jo izdajateljski svet, večinsko sestavljen iz predstavnikov ustanovitelja, ter uredniški odbor z glavnim urednikom.
