Dorsal, volar open, or percutaneous approaches can be used in surgical treatment. We aimed to evaluate the results of internal fixation of scaphoid fractures with the dorsal mini-open technique. Patients with acute proximal and waist fractures who have been treated with cannulated compression screws with a dorsal mini open approach without grafting between 2015 and 2020 were included. Functional outcomes were analyzed with the DASH questionnaire and MAYO wrist performance scores and compared with the contralateral wrists. The mean age of the patients was 35.6 ±10.8 years (range 20-55), seven were proximal, and 13 were waist fractures. The mean time to surgery was 16±7.3 days (range 5-30) and the mean follow-up time was 47.5±20.2 weeks (range 20-84). The mean DASH score at the last follow-up was 5.9±3.3 points (range 2.5-11.7), and the Mayo wrist performance score was 96.8±5.2 points (range 85-100). In one proximal pole fracture, cracking occurred on the cartilage surface at the screw insertion site but consolidated without any additional intervention. The mean time to union was 8.5±1.2 weeks (range 7-11). Waist region fractures consolidated faster than proximal pole fractures (p=0.002). No significant difference was found between the fracture site and mid-term functional results. {"title":"Comparison of success rates of laser surgery and crystallized phenol treatment in pilonidal sinus treatment","authors":"Bulent Halaclar","doi":"10.5455/medscience.2023.08.157","DOIUrl":"https://doi.org/10.5455/medscience.2023.08.157","url":null,"abstract":"The main purpose of this study is to compare the efficacy of crystallized phenol and laser among treatment options in pilonidal sinus (SPS) discomfort in the sacrococcygeal region. A total of 190 patients were included in this retrospective archive research. Moreover, cases with missing or unclear data were not included in the study. Most patients were men [123 (64.3%) vs. 67 (35.3%); p<0.001]. There was no difference between the two groups in terms of age [31.37±8.92 vs. 31.20±8.85; p=0.92]. The majority of those who have applied laser were men [80 (80%) vs. 20 (20%); p<0.001]. It was determined that the majority of those who underwent laser were smokers [38 (67.9%) vs. 18 (32.1%); p<0.007]. The rate of phenol application was found to be slightly higher in females [47 (52.2%) vs. 43 (47.8%)]. In terms of comorbidity, there was no difference between the cases treated with phenol and laser [14 (40%) vs. 21 (60%); p=0.33]. There was no statistically significant difference in terms of the 12th month [84 (46.4%) vs. 97 (53.6%); p=0.312] or 24th month [82 (46.1%) vs. 92 (53.9%); p=0.167] recurrence rates for cases treated with phenol and laser. Moreover, there was no statistically significant difference between the two groups for cases who had reoperation [phenol: 13 (61.9%) vs. laser: 8 (38.1%); p=0.157]. Bleeding after the procedure was detected only in patients who underwent laser [0 (0.0%) vs. 6 (100%); p<0.03]. There was no difference in terms of infection in both groups [3 (50%) vs. 3 (50%); p=1.00]. With regard to the body mass index, no difference was found between the two groups that were treated with phenol or laser. However, regarding the visual analog scale (VAS), it was significantly lower in the phenol group [1.38±1.63 vs. 0.87±1.37; p=0.002]. The applications to the mental health and diseases hospital emergency department between 01.01.2021 and 01.01.2022 were examined in the study. The demographic data and psychiatric diagnoses of the patients were recorded. The data of 3712 people who applied to the emergency department of the hospital were analyzed. A total of 37.98% of these patients were hospitalized. Men constituted the majority both in all applications and in patients who were hospitalized for observation and treatment. The most frequent application to the emergency department of the hospital was the patients brought by law enforcement officers with a rate of 26.64%. In patients who were hospitalized, schizophrenia and other psychotic disorders were in first place with a rate of 30.35%, bipolar disorder and mood disorders were in second place with a rate of 29.29%, and a general psychiatric examination diagnosis was in third place with a rate of 11.91%. To know the demographic characteristics and clinical diagnoses of people who apply to psychiatric emergency departments in search of help is very important for mental health service planning and development. This study was planned to investigate the frequency of iron deficiency anemia in children with poor appetite. Complete blood count and ferritin values of children admitted to the pediatric outpatient clinic due to poor appetite were analyzed retrospectively. Cases with a hemoglobin (Hgb) value below -2 SD and a ferritin level below 12 ng/ml according to the age group were considered as IDA. Four hundred and twenty-eight children aged between 6 months and 18 years were evaluated. 210 (49.06%) and 218 (50.94%) of children were male and female, respectively. Children were examined in four groups according to their age: 1) 6 months-2 years (n=193), 2) 3-6 years (n=142), 3) 7-11 years (n=67) and 4) 12-17 years (n=26). Children were divided into three groups according to their ferritin values: Group 1: ≥12 ng/ml (n=299), Group 2: between 6-12 ng/ml (n=95) and Group 3: ≤6 ng/ml (n=34). The rate of IDA was found to be 28.2% in children with a ferritin value of ≤12 ng/ml. The relationship between age and gender to the frequency of anemia was not statistically significant (p≥0.05). IDA is a common hematological disorder that can be seen at any age, especially in children aged 6-36 months. This retrospective study included women experiencing term pregnancies and undergoing cesarean delivery, categorized as normal weight (n=37), overweight (n=59), or obese (n=48) due to body mass index values of 18.5 to 24.9; 25 to 29.9; and ≥30, respectively, as well as presence of excessive weight gain during pregnancy. During cesarean delivery, amniotic fluid samples were obtained without contamination of blood or meconium. Lamellar bodies in these samples were quantified using a standard hematologic counter. No significant difference was found among the LBC of the normal weight, overweight, and obese groups [42 (32-60), 40 (26-63), and 43 (24-52) x103/µL; p=0.307], although there was a subtle decrease in the LBC of the obese group with excessive weight gain. Although it is noteworthy that the LBC in the amniotic fluid of mothers with obesity and excessive weight gain during pregnancy showed a partial decrease, this decrease did not reach statistical significance, likely due to the relatively low number of cases in the study groups. A retrospective analysis was performed on 233 hormone replacement therapy (HRT) methods of endometrial preparation in frozen-thawed embryo transfer (FET) cycles (HRT-FET). HRT-FET cycles that were conducted between January 2021 and December 2022 at the In vitro fertilization (IVF) Clinic of 19 May University Medical Faculty Training and Research Hospital. The patients' age, infertility duration, basal follicle-stimulating hormone (FSH) level, body mass index (BMI), number and day of embryos transferred, and pregnancy rates were evaluated. Endometrial thickness was measured using transvaginal ultrasound (TV USG) on the day of progesterone administration. The endometrial thickness of patients who achieved clinical pregnancy and those who did not were analyzed. No significant differences in demographic factors were found between the groups in terms of age, infertility duration, basal FSH level, and BMI (p=0.209, p=0.959, p=0.866 and p=0.685, respectively) . The success rates of clinical pregnancy did not strongly correlate with endometrial thickness. CBCs made between 31.03.2018 and 31.03.2022 in the Samsun Maternity and Child Health Training and Research Hospital were examined retrospectively. The study period was subdivided into two sections, one from March 31, 2018, to March 30, 2020 (Group 1, the pre-pandemic period) and the other from March 31, 2020, to March 31, 2022 (Group 2, the pandemic period). Patients' demographic data, the reason for the CBC and where it was issued, the time elapsed before the team arrived on the scene, and the outcomes were then compared between the groups. Seventy-nine CBCs, 34 (43.1%) of which were ‘false,’ were included in the study. Forty-six calls were made in the pre-pandemic period, 26 (56.5%) of which were true calls, while 33 calls were made during the pandemic, of which 19 (57.6%) were true calls. Twenty-seven (60%) of the 45 true CBCs were for boys and 18 (40%) for girls. The children’s mean age was 73.06±66.85 months, and 57.7% of the CBCs were issued during the pre-pandemic period. No significant difference was observed between groups 1 and 2 in terms of code blue team arrival times (114.03±59.14 vs. 138.1±95.96 sec, respectively (P=.305). The total mortality rate at the end of 30 days was 13.3% (P=.679). No difference was observed in CBC results between the pre-pandemic and pandemic periods in this study. Although melanin is produced in many different ways today, it is more practical, and more economical to produce melanin using bacterial strains. The eumelanin pigment, which is the subject of this study, was obtained using Streptomyces parvus BSB49 strain, and its antiproliferative effect at various concentrations against small cell (DMS-114) and non-small cell (H-460) lung cancer was determined. According to our results, the synthesized eumelanin molecule was statistically cytotoxic on non-small cell lung cancer cell line (H-460) at concentrations of 3.125 and 1.56 μg/mL after 24 hours of treatment (IC50=8 μg/mL) and only at a concentration of 3.125 μg/mL after 48 hours of treatment (IC50=8.4 μg/mL). A total of 52 patients who underwent SRP between 2018-2023 were included in the study. Two groups those who operated in the winter (winter group, n=26) and those who performed in the summer (summer group, n=26) were evaluated with the NOSE (Nasal Obstruction Symptom Evaluation) scale to reveal the QoL after surgery. Compared with the summer group, NOSE scores in the winter group showed a statistically significant difference (p<0.05). Septorhinoplasty is a surgical procedure expected to increase the QoL of patients regardless of the season and climate. The low mean NOSE score observed in the winter showed that the postoperative patient comfort was significantly higher compared who underwent surgery in the summer. Therefore, encouraging patients to undergo surgery intensively during the winter months in SRP will be positive for postoperative QoL. In this study, the association between primary tumor characteristics (grade, histologic type, stage, localization) and the FDG-PET/ CT data of the primary tumor at the diagnosis were evaluated. The study aimed to determine the diagnostic role of numerical metabolic values determined in PET/CT in predicting grade, prognosis, and other patient characteristics of sarcoma patients. Patients with soft tissue and bone sarcoma who applied to the oncology department of Gazi University Faculty of Medicine between 2000 and 2014 were evaluated retrospectively. All patients underwent PET imaging with FDG before initiation of neoadjuvant chemotherapy or resection. Sarcoma patients (27 soft tissue, 18 bone sarcoma) were evaluated retrospectively in this study. Tumor size was found to correlate with SUVmax. Histologic grade was found to be associated with tumor 18F-FDG, but this was not statistically significant because of the small patient number. The optimal cut-off SUVmax value in showing the difference between low and high grades was 5.5. Overall survival was shown to be shorter in patients with SUVmax ≥5.5. In this study, we aimed to show the relationship between histologic grade and tumor metabolic activity. PET-CT could give an idea about tumor histologic grade and predict survival.
