{"title":"75-g oral glucose tolerance testing does not affect fetal cardiac doppler parameters in low-risk pregnant women","authors":"Aydin Ocal","doi":"10.5455/medscience.2023.08.143","DOIUrl":"https://doi.org/10.5455/medscience.2023.08.143","url":null,"abstract":"The study aimed to examine whether acute hyperglycemia, caused by the 75-g oral glucose tolerance test (OGTT) can produce fetal cardiac adverse effects that are detectable with fetal cardiac Doppler ultrasound in healthy low-risk pregnancies. Healthy low-risk pregnant women who underwent 75-g OGTT during the gestational period of 24 to 28 weeks were considered eligible and during the test, they were examined with fetal ultrasound at the fasting and 1- and 2-h timepoints. The study findings included the parameters of umbilical artery pulsatility index, uterine artery, fetal heart rate, middle cerebral artery (MCA) peak systolic velocity (MCA-PSV), MCA pulsatility index, and fetal cardiac Doppler parameters including myocardial performance index, left isovolumetric contraction and relaxation times, K-index, filling time, mitral E- and A-wave velocities, and E/A ratio. The data of 53 women without gestational diabetes mellitus were used for statistical analyses. There was no statistical significance among the three-time points of OGTT regarding all the Doppler parameters (p>0.05). The median MCA-PSV measured at the fasting of OGTT was significantly higher than the values measured at the first and second hour of OGTT [28 (22-42) vs. 26 (18-39) and 26 (18-38), respectively; p<0.05]. This increase provided no relationship with other study parameters (p>0.05). Overall, 75-g OGTT does not affect fetal cardiac Doppler parameters in healthy pregnant women except for abnormal findings of MCA-PSV. This study aims to investigate the effects of butyric acid, caprylic acid, and oleic acid alone or in combination on nesfatin-1, omentin, adropin, leptin, and ghrelin secretion. In the study, 49 male Wistar rats were used. The rats were randomly divided into 7 groups as control (CONT), butyric acid (BA), caprylic acid (CA), oleic acid (OA), butyric acid+caprylic acid (BA+CA), butyric acid+oleic acid (BA+OA) and caprylic acid+oleic acid (CA+OA) groups. The rats were orally administered fatty acids for twenty-one days. At the end of the study, the levels of nesfatin-1, omentin, adropin, leptin, and ghrelin were determined by the ELISA method. When the groups were compared in terms of nesfatin-1, omentin, adropin, leptin, and ghrelin levels, the butyric acid group had higher nesfatin-1, omentin, adropin, leptin, and ghrelin levels than the other groups (p<0.05). No statistically significant difference was found in the other groups (p>0.05). The precise measurements were obtained using the Microscribe-G2X device, renowned for its accuracy of up to one-thousandth of a millimeter. This study aimed to assess the pan-immune-inflammation value (PIV), encompassing lymphocyte, monocyte, neutrophil and platelet levels along with the PILE score (a composite profile obtained from PIV, Eastern Cooperative Oncology Group Performance Score (ECOG PS) and serum lactate dehydrogenase (LDH)) in patients with diverse tumour types undergoing treatment with immune checkpoint inhibitors (ICIs). A retrospective study was carried out on a cohort of 105 patients with assessed pre-treatment PIV and PILE scores. Only stage III–IV patients were recruited. PIV was determined using absolute values from peripheral blood cell counts (neutrophil x platelet xmonocyte/lymphocytes) integrated at the time of diagnosis. The PILE scoring system further integrated LDH levels (<245 vs. ≥245) and ECOG-PS (0-1 vs. ≥ 2) along with the incorporated PIV values. Elevated PIV levels were significantly associated with a high response rate (RR) compared to those with lower PIV levels (HR:2.63, 95% CI:1.06–6.54; p=0.037). In this study, a PILE score of 3 emerged as a potential surrogate marker for progression-free survival (PFS) (HR=3.393, 95% CI: [0.822–13.996]; p=0,091). Further, an ECOG PS ≥2 was related to an elevated risk of both progression (HR=1.862, 95% CI: [1.030–3.364]; p=0.040) and overall survival (OS) (HR=2.399, 95% CI: [1.286–4.474]; p=0.006), alongside a reduced RR (HR=3.352, 95% CI: [1.397–8.043]; p=0.007). The impact of monocyte levels on PFS was statistically significant (HR=2.291, 95% CI: [0.999–5.251]; p=0.050). The aim is to develop a new questionnaire to evaluate the changes that occurred during the pandemic process and to examine the psychometric properties of this questionnaire. For face validity, 10 individuals were asked to interpret the questions. The Pandemic Process Status Questionnaire was tested with the help of a questionnaire created with the help of Google Forms. In order to evaluate the content validity, the content validity index and content validity ratio were used in line with the opinions of 7 experts. A pilot test was carried out. Exploratory factor analysis was performed for construct validity. The World Health Organization Quality of Life-brief version was used for convergence validity. A total of 25 items with an impact score of less than 1.5 in face validity were deleted. According to the content validity index, 15 items and 21 items according to the content validity rate were removed. In the pilot application, 5 items with an item-total correlation below 0.3 were deleted. The results of the exploratory factor analysis provided 6 dimensions: general status, physical status, psychological status, social and economic status, concerns about the pandemic, and eating habits. This structure explained 71.77% of the total variance. The questionnaire showed a weak correlation with the World Health Organization quality of life-Short Form (r=0.26). In addition, to obtain information about the complications seen after these fractures, especially avascular necrosis of the femoral head (AVN), and to investigate the relationship with the treatment process. Our study is a retrospective study including 22 fractures of 22 patients operated for femoral neck fractures between 2012 and 2021. Thirteen of the patients were boys and 9 were girls The mean age at the time of trauma was 12.6 years (5-17 years). The fractures were classified based on the Delbet/Colonna classification. Fracture type and its effect on functional outcomes and the effect of fracture type on complications were investigated. The relationship between the time and type of treatment and AVN was evaluated. The mean follow-up period of the patients included in the study was 36.9 (12-132 months) months. Orthopedic injury in combination with femoral neck fracture was seen in 4 patients. Femoral neck fractures were classified as type 1 in 1 patient, type 2 in 15 patients, type 3 in 4 patients, and type 4 in 2 patients. In our study, 5 of 8 complications were AVN. The clinical results of the patients were classified with Ratliff score and 14 patients had good, 3 patients had fair and 5 patients had poor results. According to this classification system, 5 patients who developed AVN had a poor outcome, but no significant correlation was found between AVN and the timing of surgery and treatment method. All of the cases with AVN were Type 1 and Type 2 fractures. Pediatric femoral neck fractures are inherently difficult fractures for orthopedists who need long-term follow-up. {"title":"Conservative management of anal fissure accompanying constipation in school-age children","authors":"B. The conventional therapy for anal fissures has been warm sitz baths and lthe ocal application of analgesic ointments, stool softeners ,and behavioral therapy as necessary. Simple wound care may often be sufficient for the treatment of anal fissure, but a comprehensive approach is required for accompanying constipation. Between 2015-2022, a total of 263 children suffering from constipation and perianal pain were diagnosed with anal fissure (AF) based on physical examination. Anal fissure and constipation (AFC) questionnaire forms were retrospectively reviewed and results of routine management protocol were assessed which was focused on local wound care and dietary regulation. The ratio of children consuming more than 2-3 cups of packed beverages containing artificial sweeteners, colorants or corn syrup was found 63,89 % (n=167). Daily water consumtion of less than five glasses was 71.1% (n=187). Daily milk consuming was 71,86% (n=189). Stool withholding and soiling was found 57,41% (n=151) and 48,67% (n=128) respectively. In only four cases (1.52%), constipation persisted for more than eight weeks despite meticulous management and improvement was achieved with anal dilatation on the tenth week. {"title":"Complicated and suppurative skin soft tissue infections: our four-year cases","authors":"Celali Kurt","doi":"10.5455/medscience.2022.11.238","DOIUrl":"https://doi.org/10.5455/medscience.2022.11.238","url":null,"abstract":"Skin and soft tissue infections (SSTIs) are common community and hospital-acquired infections and rarely result in severe morbidity and mortality. It is not always possible to obtain cultures and isolate the causative agent in these patients, primarily treated with empirical antibiotics. In this retrospective cross-sectional study, 57 patients with complicated SSTI followed between 2018 and 2022 with purulent and necrotizing characteristics or growth in blood cultures were included. Thirty-eight patients (66.7%) were male, and 39 (68.4) had diabetes. In 43 (75.5%) patients, the infection was in the lower extremity and gluteal region. Gram-positive agents were detected in 35 patients (61.4%), Gram-negative agents were found in 13 patients (22.8%), and Gram-positive and Gram-negative mixed growth was detected in 1 patient (1.8%), and no causative agent could be produced in 8 (14%) patients. Of the 21 Staphylococcus aureus produced, 4 (19%) were methicillin-resistant (MRSA). Growth was detected in blood cultures in three patients (5.3%), and all were streptococcal bacteria. It was observed that 31 patients (54.4%) required hospitalization for treatment, and 29 (50.8%) required surgical intervention such as drainage, aspiration, debridement, or amputation. Three patients (5.3%) died, and four (7%) underwent amputation. There was no significant difference between diabetic and non-diabetic patients regarding factors and mortality–morbidity. In the logistic regression analysis, none of the variables of gender, type of causative agent, presence of diabetes, and site of infection significantly affected severe morbidity and mortality. Since it was detected at a low rate in our cases, it can be said that MRSA and P.aeruginosa do not need to be considered in every patient for the initial empirical treatment, Gram-negative rods can be taken into account, especially in diabetics, and broad-spectrum and combined antibiotics are not needed in the vast majority of patients. It is known that psychiatric conditions such as anxiety and depression are associated with hypertension (HT) and other cardiovascular diseases. HT patients were assessed using the Hospital Anxiety and Depression Scale (HADS). Sociodemographic and laboratory data of the patients were recorded. LVH was seen in 34 of 103 patients, and LVH was not seen in 69 patients. The mean age was 47.8 years in the group with LVH and 49.5 in the group with non-LVH. According to the results of our study, there was no significant difference between the LVH group and the Non-LVH group in terms of age, gender, BMI, smoking, loneliness status, and EF. The most striking result of our study was that the HADS score was significantly higher in the group with LVH than in the group with Non-LVH (p<0.001). The glucose level was significantly higher in the group with LVH than in the group with non-LVH (p<0.001). Patients should be aware of adverse cardiac outcomes according to their anxiety level, and precautions should be taken while performing mental health interventions and treatments. Invasive methods also occupy an important place in the treatment of this symptom. Our study evaluated the follow-up results of patients injected with Platelet-rich plasma and steroids due to sacroiliac joint pain. Patient files between the years 2018-2022 were evaluated retrospectively. Patients who underwent ultrasound-guided sacroiliac joint injection of platelet-rich plasma and steroids were divided into 2 groups. VAS and The Modified Oswestry Disability Questionnaire scores were evaluated at 1, 3, and 6 months. Eighty-seven patients were included in the study. It was understood that patients in both groups benefited after injection (p<0.001). In the 1st month follow-up, it was concluded that the VAS and the Modified Oswestry Disability Questionnaire scores of the patient group who received steroids were significantly better (p<0.001). In the 3rd month, the VAS (p=0.695) and the Modified Oswestry Disability Questionnaire (p=0.215) scores of both groups were similar in both groups. In the 6th month, the platelet-rich plasma group's results were significantly better (p<0.001). Platelet-rich plasma and steroid injections are safe treatment methods for sacroiliac joint pain.
