Weaver","doi":"10.21742/ijaner.2020.5.1.02","DOIUrl":"https://doi.org/10.21742/ijaner.2020.5.1.02","url":null,"abstract":"The aim of this integrative review is to sensitize primary care nurse practitioners and physicians to the traumatic care experiences of youth psychiatry patients and provide recommendations for patient care using a trauma-informed approach. Two electronic, combined CINAHL and PsychInfo searches and a manual search of the references of included articles for the years 2000-2019 were completed. Using Whittemore and Knafl’s integrative review method, data from 22 articles were synthesized. Traumatic care, although not widely recognized by primary care providers, was shown to result as a negative effect of admission policies, procedures, and safety measures enforced in youth psychiatry units. Traumatic care themes included coercive care, seclusion, loss of control, safety, and posttraumatic stress disorder after discharge. Trauma-informed care training was found to improve patient-provider relationships and reduce the adverse health outcomes associated with trauma exposure. {"title":"Comparing Results of Alternate Format Test Questions to Standard Multiple Choice Test Questions in First, Second and Third Year Baccalaureate Nursing Students","authors":"E. Before 2015, Canadian nursing graduates were required to pass the Canadian Registered Nurse Exam (CRNE) to obtain licensure to practice. This exam consisted of mainly multiple-choice questions. In recent years, Canada (except for its province of Quebec) has adopted the NCLEX-RN exam as the licensure assessment exam for nursing graduates. The NCLEX-RN exam is a computer adaptive test that utilizes not only multiple choice questions, but also question items in a variety of other formats such as multiple responses, fill-in-the-blank calculation, ordered response, and hot spot. To explore if changes in question item formats influenced an individual's ability to answer a test question correctly, we carried out a descriptive, comparative study with first, second, and third-year Baccalaureate nursing students. We compared the students’ performance on NCLEX-RN style alternate format test questions with their performance on standard multiple choice questions in an anatomy and physiology course. We further compared their performance on both types of questions and their final grade in the course. A convenience sample of students enrolled in one Atlantic Canadian Nursing Program was included. Descriptive statistics resulted in significant differences between the various types of questions used. The study follows a descriptive, cross-sectional design and the participants were 158 nursing students. Data was collected using self-report questionnaires and practice evaluation. There was a significantly positive correlation between self-confidence and competence. Also, critical thinking disposition correlated with self-confidence. {"title":"Culturally Competent Care of LGBT Patients: The NP Experience","authors":"D. Research that examines NP practice as it relates to the culturally competent care of lesbian, gay, bisexual, and transgender (LGBT) patients has been lacking in the literature. To address this gap, an exploratory qualitative descriptive design was used to explore NP experiences with LGBT patients. Data were collected via semi-structured interviews with 22 NPs. Inductive analysis was used to identify, analyze, and report themes within the data. Key findings were that LGBT patients were generally not seen as a distinct cultural group with specific health needs and that NP participants were unclear about the definition or principles of cultural competence as it relates to LGBT persons. Most NPs were not using a model of cultural competence to guide their practice. Instead, they utilized strategies that primarily revolved around the development and maintenance of the therapeutic nurse - patient relationship. The participants were 10 nurses working in the insurance review department of 3 senior general hospitals in Seoul and Gyeonggi-do. Data were collected through focus group interviews. As a result, the core category representing the job experiences of the health insurance review nurses was ‘standing alone as nurse specialist without safeguards’. Four categories and 13 subcategories were derived around the core category. This study provides a comprehensive and holistic understanding of the job experience of health insurance review nurses working at medical institutions in Korean society. Therefore, this will provide new insights into the job of insurance review nurses and serve as the basis to complement the system for securing status. Using the SPSS 21.0 Program for the data analysis, the t-tests and one-way ANOVA were conducted. The performance of therapeutic communication knowledge application and technique showed significant differences in grade, economic condition, and clinical training period. The mean performance of therapeutic communication knowledge application and technique was 1.86±0.15 while the problem recognition was the highest and the provision of information was the lowest. In the results of analyzing the factors having effects on the performance of therapeutic communication knowledge application and technique, the grade explained 19.4%. To achieve this, faculty at Dalhousie University School of Nursing used a unique and creative approach to develop a new nursing curriculum. Extensive preplanning, utilization of small working groups, working through consensus building and utilizing a project plan engaged faculty in all facets of the curriculum development. Draft plans were developed which were reviewed and revised by all faculty through multiple creative planning events. This process allowed consensus around key decisions such as philosophical underpinning of the curriculum, core themes and new educational approaches. The results showed that the of dementia attitude knowledge, dementia attitude stability, and total score of dementia attitude were not statistically significant. However, the average score showed a slight improvement. And there was no statistically significant difference in the geriatric nursing self-efficacy, but it showed a slight improvement in the average score. Based on the results of the research, the cognitive rehabilitation program education is an effective and beneficial program for the care of the elderly with dementia. However, it is difficult to generalize because the number of participants is small. The candidates of study subjects were 16, 277 females from 7th National Health and Nutrition Survey (2016-2017) and raw data of 6,266 females were included in final analysis after. For the analysis, complex sample chi-square test, General Linear Model (GLM), and complex sample logistic model were performed. Results: The sleep time showed statistically significant difference in adolescent and middle-aged group, however, this significance survived only in middle-aged group after applying covariates, indicating that the longer sleep time is associated with the higher risk of diabetes mellitus.
