It generally occurs associated with other heart diseases and was previously not considered a target for intervention. It is currently of particular interest because it was demonstrated that severe TR is associated with increased morbidity and mortality. Given the high in-hospital mortality risk of isolated tricuspid valve surgery, the early success of transcatheter repair or replacement techniques has increased access to relatively low-risk procedures. The bi-caval system is a heterotopic valve replacement therapy consisting of two self-expanding valves implanted sequentially in the superior and inferior vena cava via percutaneous access through the femoral vein. The search for strategies that optimize the costs of these treatments is essential to ensure access to them. Dose banding is a methodology that has been successfully implemented in several countries, demonstrating effectiveness and efficiency. It is based on establishing dose bands rounding cytostatics ±5% and monoclonal antibodies ±10%, optimizing commercial presentations and avoiding waste. In our country there are no experiences with this system. The objective was to evaluate the savings in drug costs using dose banding.</p><p><strong>Material and methods: </strong>Observational study of the costs of oncology drugs in a sample of patients affiliated with the Institute of Social Work of the Armed Forces (IOSFA) diagnosed with breast, colon and prostate cancer who received treatment during June 2022. The cost of each dose was determined and was simulated according to the dose banding system.</p><p><strong>Results: </strong>Twenty-eight patients who received 29 doses were analysed.</p><p><strong>Diagnoses: </strong>colon cancer (n:7; 25.0%), breast cancer (n:18; 64.3%) and prostate cancer (n:3; 10.7%). Mean age 61.7 years (range 34-85 years); 22 patients were female. Drugs tested: bevacizumab, cabazitaxel, cetuximab, docetaxel, trastuzumab, and trastuzumab+emtasine. The total cost was $14 101 596.39 (U$S 112 605.58 -sale value of the U.S. dollar in the Banco Nación Argentina, June 30, 2022), the simulation of the application of dose banding showed a cost of $11 185 371.49 (U$S 89 318.63). HAE has a 15% to 50% historical asphyxia mortality. It is the consequence of C1 inhibitor deficiency. The identification of bradykinin as the principal mediator of the disease has led to the development of new drugs for the treatment of HAE. HAE treatment is agreed in international consensus. A therapeutic guide for the treatment of the disease was published in Medicina (Buenos Aires) in 2012, but new and relevant diagnostic and therapeutic advances have been described subsequently. This highlights the need for effective interventions to mitigate the effects of these events on professionals.</p><p><strong>Materials and methods: </strong>A bibliometric analysis was conducted on the second victim experience to evaluate the use and validity of the SVEST (Second Victim Experience and Support Tool) in recent studies. Key publications were reviewed to analyze the application of SVEST and the effectiveness of support strategies implemented in healthcare organizations.</p><p><strong>Results: </strong>The analysis confirmed that SVEST is a reliable tool for measuring the emotional impact on second victims and for assessing the quality of the support they receive. Studies highlighted the effectiveness of peer support programs and leadership training as key strategies to mitigate the negative consequences of adverse events.</p><p><strong>Discussion: </strong>The findings underscore the importance of establishing structured support systems within healthcare organizations to create a more supportive and resilient environment. {"title":"[A rare case of autoimmune polyglandular syndrome type 2 in an elderly patient].","authors":"Abrar-Ahmad Zulfiqar","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Autoimmune polyglandular syndrome type 2 (APS-2) is a rare and complex clinical entity, whose etiology and evolution are not fully understood. We present the case of a 78-year-old male, followed and treated for Addison's disease for 45 years, in whom macrocytosis with a hemoglobin level of 11.8 g/dL was detected during a routine evaluation. The immunological vitamin B12 deficiency led to the diagnosis of Biermer's disease, while the thyroid, diabetic and liver immunological evaluations were negative. Treatment was started with periodic injections of vitamin B12 and hormone replacement therapy (glucocorticoids and mineralocorticoids) was adjusted, which produced a favorable clinical response with progressive normalization of macrocytosis and hemoglobin levels. Long-term follow-up confirmed clinical stability and effective control of the autoimmune evolution. APS-2 is defined as the coexistence of autoimmune Addison's disease with autoimmune thyroid diseases (such as Graves' disease or Hashimoto's thyroiditis) and/or type 1 diabetes mellitus, in the absence of hypoparathyroidism. Various genetic and environmental factors appear to contribute to its development, although its exact etiology remains unknown.
