These deficits are thought to mediate the relationship between etiological factors and the manifestation of the disorder, making them important targets for treatment. Within this context, and based on promising results, computerized cognitive training (CCT) through adaptive programs and/or video games has garnered considerable interest. Therapeutic programs and video games for ADHD have even been commercialized. However, the most rigorous and reliable evidence currently available suggests that CCT, in its current forms, lacks sufficient empirical support to be considered either a standalone or an alternative treatment to first-choice interventions for ADHD. Its effects on core ADHD symptoms are modest and limited, with minimal transfer or generalization to other contexts or functional skills. The complexity of ADHD suggests that highly specific, singular interventions such as CCT may be neither appropriate nor sufficient as standalone treatments. Neuroendocrine tumors represent less than 2% of all invasive cervical cancers. They tend to be highly aggressive, showing rapid growth and distant metastasis at the time of diagnosis. We present a 25-year-old female, with no relevant medical history, who came to our hospital with 1-monthold acne-like lesions on the face, neck, and chest, facial edema, blurred vision, and progressed with delirium, incoherent speech, and suicidal ideation. Hypertension, hypokalemia, hyperglycemia, and metabolic alkalosis were found. The condition was interpreted as a severe, rapid-onset Cushing's syndrome, with significant compromise in general condition. The syndrome and its dependence on ACTH were confirmed by biochemical tests. Computed tomography revealed multiple lesions in the liver and spine suggesting metastatic cancer. After reporting a history of postcoital bleeding, a speculoscopy was performed, and a biopsy was taken of the cervix, as well as the liver lesions. In both cases, histopathological findings revealed the presence of small cell neuroendocrine carcinoma, and chemotherapy with cisplatin and etoposide was initiated. This case represents an example of an uncommon disease that usually implies an important diagnostic and therapeutic challenge. Despite the trend toward decreased mortality from acute stroke in the Northern Hemisphere, reports in our region show variable figures. This study reports in-hospital mortality for the second time from a medical center in Ciudad de Buenos Aires. Factors associated with mortality in these patients were also identified.</p><p><strong>Materials and methods: </strong>A retrospective study was conducted using data from the FLENI Stroke Database between 2010 and 2019, analyzing clinical characteristics, risk factors, complications, and mortality of hospitalized patients. Univariable and multivariable statistical analyses were used to identify risk factors associated with in-hospital mortality.</p><p><strong>Results: </strong>Data from 1645 patients were analysed, of which 1476 (90%) had ischemic stroke, 95 (6%) had hemorrhagic stroke, and 74 (4%) had aneurysmal subarachnoid hemorrhage (aSAH). There were 45 in-hospital deaths (2%): 26 deaths (1%) from ischemic stroke, 11 deaths (11%) from hemorrhagic stroke, and 8 deaths (10%) from aSAH. Factors associated with mortality include advanced age, low hemoglobin levels at admission, higher scores on the NIHSS scale at admission, and a history of diabetes mellitus.</p><p><strong>Discussion: </strong>In-hospital mortality from stroke remains low in our institution, with differences between ischemic and hemorrhagic stroke. Its intraoral location is extremely rare, close to 1%. Its clinical presentation is variable, from an asymptomatic mass to dyspnea and dysphagia. Computed tomography allows adequate tumor definition for planning surgical tactics, with surgery being the only effective treatment for the patient's complete restitution. Its timely diagnosis together with effective surgical treatment avoids sustained mechanical trauma that, if it persists, could lead to the subsequent malignant transformation of the tumor. Here we describe the case of a soft tissue tumor with an unusual topographic location, inside the oral cavity, linked with pain during chewing, being this association truly uncommon. Immediately after the percutaneous coronary intervention, he developed a greater requirement of vasopressors, the addition of inotropes, and an increase in oxygen supply by mechanical respiratory assistance. An echocardiogram was performed, which showed acute ischemic mitral regurgitation due to complete rupture of the posteromedial papillary muscle. Under ventricular assistance with a counterpulsation balloon, a mitral valve replacement was decided, and he left the operating room with extracorporeal membrane for oxygenation (ECMO). During the postoperative period, the patient evolved adequately, achieving the retirement of the ECMO at 72 hours with good hemodynamic tolerance. Respiratory weaning was prolonged. Maternal cortisol secretion increases from the 25th week of pregnancy, estimated to reach twice the value of nonpregnant women in the third trimester. The objectives of this study were to demonstrate the increase in cortisol values in pregnancy and establish reference ranges of plasma cortisol trimester specific to our population of healthy pregnant women.</p><p><strong>Materials and methods: </strong>Cortisol measurement was performed in 186 healthy pregnant women between 2020 and 2023, 62 women were analyzed for each trimester of pregnancy, including age, body mass index (BMI), previous pregnancies, glycemia and ionogram.</p><p><strong>Results: </strong>The median age was 27 (18 - 47) years, previous pregnancies 1 (0-9), BMI 25.7 (16.6 - 46.2) kg/m2. In the first trimester, the median cortisol level was 14.1 (7.53 - 31.0) ug/dL; in the second trimester 20.5, (10.0 - 47.2) ug/dl and in the third trimester 23.2, (11.8 - 33.1) ug/dL. The increase in cortisol throughout the trimesters was statistically significant p < 0.001.</p><p><strong>Discussion: </strong>In this study, cortisol values were identified by specific trimester, which could show an increase throughout pregnancy. Furthermore, it is now known that the immune system activated by high sodium intake plays an essential role in the development of hypertension. Changes observed in components of the immune system in an animal model of menopause, show the key role of estrogens in the regulation of the immune response to a high salt load. Faced with the same challenge, intact rats have adequate renal sodium management, and the immune system is not activated, while hormone-deprived rats retain sodium and show a pro-inflammatory profile. Pharmacological treatment of hypertension reduces blood pressure, but the observed renal damage remains.
