In the present report, the results of bilateral internal iliac artery ligation in cases with postpartum hemorrhage were presented, in addition, the impact of the procedure on ovarian reserve were discussed. Design and patients: Patients who gave birth between August 2010 and August 2011 in our center, treated surgically due to a diagnosis of postpartum hemorrhage were retrospectively examined. Setting: A tertiary referral center Main outcome measures: The obstetric and demographic characteristics, applied surgical procedures, the need for intensive care unit, the amount of transfusion, length of hospital stay and associated morbidities and mortalities were evaluated. Postpartum concentrations of follicle stimulation hormone (FSH) and estradiol with ovarian volume calculated by ultrasonography to evaluate the impact of procedure on ovarian reserve were also examined anc compared normal controls. Results: The prevalence of postpartum hemorrhage requiring a surgical treatment was 12.5% during the period of study. Thirteen of those underwent the ligation of bilateral internal iliac artery. The mean age of 32 (24 44), mean gravida 3,2 (1 5), mean parity 2 (0 4), mean birth week 38 (35 41) and mean birth weight were 3373 g (2500 4200). The effectiveness of bilateral internal iliac artery ligation was 84,7%. FSH and estradiol concentrations and the mean volume of the ovaries were similar between the study (n:10) and control (n:56) groups (P= 0.650, P= 0.245 and P=0.281, respectively). Conclusion: The ligation of bilateral iliac artery, a fertility preserving method, possess high efficacy for the management of postpartum hemorrhage. Although the accuracy of fetal well-being assesmernt of the method still remains controversial. The basic goal is to predict fetal hipoxia and severe asphyixia and intervene before fetal death happens. On the other hand, prenatal outcome does not improve as desired due to discordance between NST findings and severity of fetal hipoxia. Aim: To determine the power of decision-making role of NST, via retrospective datas of cesarean deliveries indicated with fetal distress according to NST findings. Materials and methods: In Istanbul Medical Faculty Hospital, the datas of 590 cesarean deliveries indicated with fetal distres according to NST findings, between the dates of 2007-2009 were detected retrospectively. Cases were divided into two groups as preterm and term labors. The postpartum term datas selected were; birth weight, APGAR scores of 1st and 5th minutes, cord blood pH, pH, pO2, pCO2, HCO3, base excess (BE). As measuring the value of NST to predict fetal hipoxia, positive predictive value was calculated. As for APGAR scoring; positive and negative predictive values, spesificity, sensitivity were calculated. Result: NST was not found as effective as it was obtained. Only 30% of the cases were found hipoxic. Additionally between those fetuses with hipoxia, 38% had 1st minute APGAR score of 7 (cut-off value) and under. Although in cases with findings of uteroplacental insufficiency NST should give much valuable information, regarding to increase of unnecessary surgical procedures. Design: Prospective study Setting: Gaziantep Üniversity, Medical School, Department of Obstetrics and Gynecology Patients: This study was performed with 57 preeclamptic pregnant women and 50 pregnant women having no medical problems who has admitted to the Department of Obstetrics and Gynecology of Gaziantep University Medical Faculty between January 2008 and June 2008 Main outcome measures: Serum total SA levels were measured by Denny's colorimetric method and analysis of SIAE gene variation was performed by polymerase chain reaction -single stranded conformational polymorphism (PCR-SSCP). Statistical analyses were achieved using the SPSS 13.0 software. P<0.05 was considered to be statistically significant. Results: In this study, we observed higher serum total SA levels in PE (p<0.00001) and increased PE risk 10.4 times by SIAE gene variation. There was a weak-intermediate correlation between serum SA levels and diastolic blood pressure, uric acid levels (p=0.0001, r=0.34; p=0.0001, r=0.37 respectively). There was a strong correlation between serum SA levels and proteinuria (p=0.001, r=0.5). There was a significant association between SIAE gene variation and proteinuria (393±238 vs 107±185 mgL, p= 0.021) , diastolic blood pressure (102.7±21.9 vs 85.7±20.6 mmHg, p=0.01). Methods: Demographic characteristics and risk factors of 300 pregnant women at delivery at Etlik Zubeyde Hanim Obstetrics and Gynecology Research and Training Hospital were recorded. Rectovaginal swab samples obtained from pregnant women were analysed for GBS using two different methods (CAMP and latex agglutination). Antibiotic sensitivity of the isolated bacteria was determined using disk diffusion method. Statistical evaluations were performed using chi-square tests. Results: Rectovaginal colonization with GBS was detected in 3% of cases. No associations observed between GBS colonization and age, occupation, underlying disease, previously used contraception methods, education and income level, and number of previous pregnancies and deliveries. GBS colonization was not associated with preterm birth act or early or prolonged membrane rupture. Antimicrobial resistance rate in isolated GBS was 22% for clindamycin and erythromycin, and 89% for tetracycline. Conclusions: This is the first report about the role of paraoxonase 1, which is a HDL-associated antioxidant enzyme, in etiopathogenesis of intrauterine growth restriction in English literature. Design: The presentation of the case. Setting: University hospital. The patient: The sixteen-years-old girl, who had lost her right adnexa because of torsion, suffered successive torsion in the left adnexa after preventive surgery. Intervention: Through the laparoscopic method, the utero-ovarian ligament was shortened and ovariopexy was applied to the back of the uterus. The ovarian reserve between the attacks was monitored. Main Outcome Measure: The prevention of recurring torsions through the surgical method and thereby the preservation of the ovarian reserve. Result: The ovary must be protected under all circumstances during the treatment of torsion. More than one method of adnexa fixation may prove necessary to prevent further attacks. Conclusion: The case we present here is the unique in which the two sides were successively affected, despite the surgical intervention to prevent any recurrence, and also the unique in which the ovarian reserve between the attacks was monitored. Apparently, the ovarian reserve is affected badly by recurrent torsion attacks. Design: The files of the patients followed and treated in our clinic between 1996 and 2010 with a diagnosis of gestational trophoblastic disease were examined. Setting: Yuzuncu YIl University, Faculty of Medicine, Department of Obstetrics and Gynecology, Van. Patients: The 147 patients with satisfactory information in their files within 173 patients treated and followed in our clinic between 1996 and 2010 with a diagnosis of gestational trophoblastic disease. Interventions: No intervention to the patients. Main outcome measures: Demographic and obstetric parameters, blood group,obstetric history in the previous pregnancy, contraceptive method, gestational week and complaints on admission, hystological type, stage, treatment and complications of gestational trophoblastic disease and presence of associated clinical problems. Results: In our clinic, 18.324 deliveries occured and 173 cases had a diagnosis of gestational trophoblastic disease between 1996 and 2010. Out of 147 patients, 72 (49%), 61 (41.5%), 3 (2%), 7 (4.8%) and 1 (0.7%) had diagnoses of complete mole, partial mole, invasive mole, choriocarcinoma and placental site trophoblastic tumor, respectively. In 3 patients (2%), subtype could not be determined. Mean age was 31.45 ± 10.29 years and the most common complaint was vaginal bleeding (77.6%). As the primary therapy, suction curettage with oxytocin infusion or hysterectomy was appplied to 140 and 7 patients, respectively. A single agent chemotherapy was performed to 26 patients while a multiagent chemotherapy was given to 6 patients. All patients were followed up by serial serum ßhCG measurements. Conclusions: The incidence of gestational trophoblastic disease in our clinic was calculated as 8.1 per 1000 deliveries and socio-economic and educational status of majority of the patients were low.
