Gül","doi":"10.5505/TJOD.2013.61587","DOIUrl":"https://doi.org/10.5505/TJOD.2013.61587","url":null,"abstract":"SUMMARY Objective: The aim of the present study is to evaluate etiologic factors in patients with recurrent pregnancy loss (RPL). Material and methods: The records of patients admitted to Obstetrics& Gynecology Clinic of Dicle University Medical Faculty Hospital, from 2008 to 2011 were evaluated. Of the 114 patients who all tests were applied for diagnosis in our hospital were enrolled to study. For etiologic evaluation; Parental chromosome analysis, maternal antiphospholipid antibodies, hysterosalpingography for mullerian anomalies, thrombophilic disorders (factor V leiden mutation, prothrombin gene mutation) and endocrine (diabetes mellitus, thyroid hormone disorder) tests applied to patients. Results: The mean age and mean number of abortus were 29.7±6.6, 3.2±1.3 respectively. Fifty (43.9%) patients had at least one etiologic factor where as 64 (56.1%) were idiopathic. Major chromosomal aberrations were detected in four (%3.5) couples as inversion of the ninth chromosome. The frequency for other pathologies; mullerian anomaly:14 (%12.3), factor V leiden mutation:7 (%6.1), prothrombin gene mutation:6 (%5.3) and antiphospholipid antibodies in 10 (%8.8) patients. Conclusion: The prevalance for major chromosomal aberrations, mullerian anomalies, thrombopylic disorders and other pathologic conditions which evaluated in the study were found similar to reported previously. Polymorphism of 9qh+ (heterochromatic centromeric regions) was seen in 39 (%34.2) parents. Tulunay","doi":"10.5505/TJOD.2013.65668","DOIUrl":"https://doi.org/10.5505/TJOD.2013.65668","url":null,"abstract":"SUMMARY Steroid cell tumors are tumors that account for less than 0.1% of all ovarian malignant tumors and cause increase in secretion of androgen or estrogen. Steroid cell tumors usually occur in the post-menopausal period. This article reports four steroid cell tumor cases who underwent surgery for overian mass during post-menopausal period, who were diagnosed with steroid cell tumor after histopathological examination and displayed different clinical manifestations. No signs of increased hormonal activity in two of these cases were observed. On the other hand, the other two cases applied with complaints of menopausal vaginal bleeding, which led to the consideration of increased estrogenic activity in these two cases. Case 1 The case, who was 77 years of age in her postmenopausal period, applied to our hospital with the complaint of urinary incontinence. The routine laboratory findings of the patient, who had no feature in her history, were within normal limits. No findings of virilization were observed during the physical examination of the patient. During the gynaecological examination, a mass of approximately 6 cm was detected in the right adnexal area. The transvaginal ultrasonography (TVUSG) revealed an endometrial thickness of 9 mm, and an approximately 72mm-sized solid-cystic mass, surrounded with free liquid in the right ovary area. The preoperative Case 2 In the TVUSG performed during the routine follow- up examinations at the menopause clinic of the 52 year-old case, who did not have active complaints, a 66 X 35 mm mass with the possibility of dermoid cyst was detected in the right ovary. There was no feature in the patient's physical examination, her history and family history. The gynaecological examination, during which an approximately 6 cm mass was palpated in the right adnexial area, revealed a CA 125 value of 9.07 IU/ml. With the preliminary diagnosis of pelvic mass, laparoscopic right salpingo-ooforectomy was performed. The intraoperative pathological examination reported \"malignancy was not observed\". The paraffin block indicated a right ovarian steroid cell tumor; the largest diameter of the capsule was 8 cm and the capsule had a ruptured appearance; there was 1 mitosis /10 HPF, focal light atypia and focal hemorrhage but there was no presence of necrosis. Based on these findings, it was decided that the potential for malignancy was low. Adjuvant treatment during the postoperative period was not considered. The patient did not come to the follow-up examinations after the operation. Figure 2: The postoperative paraffin block section of case 1. Case 3 The 64-year-old case who was in her postmenopausal period applied to our hospital with vaginal bleeding complaints. Material and methods: Totally, n = 70 eligible women with bulky (≥4 cm) FIGO IB2 IIA2 cervical cancer that diagnosed and treated at our clinic between the years of 1/1994 1/2010 were reviewed retrospectively. Data regarding demographic and disease related characteristics were obtained for analysis from patients' files. Different treatment modalities (radical hysterectomy followed by adjuvant chemoradiation (RH + CTRT); primary chemoradiotherapy (CTRT); neoadjuvant chemotherapy followed by radical hysterectomy (NACT + RH) and neoadjuvant chemoradiotherapy followed by radical hysterectomy (NACRT + RH) were compared in terms of disease-free survival (DFS) and overall survival (OS) by Kaplan-Meier test. A p value of less than 0.05 was accepted as statistically significant. Results: Sixty-three patients (90%) had Stage IB2 and seven (10%) had stage IIA2 disease. (RH + CRT), (CTRT), (NACT + RH) and (NACRT + RH) were performed to 32, 23, 10 and 5 patients, respectively. Mean follow-up period was 78.1 ± 51.6 months [range: 10-210]. Thirteen patients (18.6%) developed recurrence and 10 (14.3%) died from disease. Mean disease-free (DFS) and overall survival (OS) were 64.6 ± 46.7 [4-210] and 68.2 ± 47.3 [10-210] months; respectively. No statistically significant difference was found in terms of survival between four treatment methods (p = 0.85 for DFS and p = 0.9 for OS). Gereç ve yöntemler: Bu retrospektif çal›flmada 2005-2012 y›llar› aras›nda ‹stanbul T›p Fakültesi Kad›n Hastal›klar› ve Do¤um Anabilim Dal›'nda plasenta invazyon anomalisi endikasyonuyla sezaryen histerektomi yap›lm›fl olan tüm hastalar›n dosyalar› retrospektif olarak incelendi. Bulgular: Bu süre zarf›nda toplam 85 hastaya plasenta invazyon anomalisi nedeniyle sezaryen histerektomi uyguland›. Olgular›n 81'ini (%95.3) daha önce en az bir defa sezaryen geçirmifl hastalar oluflturmaktayd›. Operasyonlar›n büyük ço¤unlu¤u (%82.4) elektif flartlarda yap›ld›. Onalt› (%18.8) olguda operasyon s›ras›nda kanaman›n çok fliddetli olmas› nedeniyle bilateral hipogastrik arter ligasyonu uyguland›. Hastalar›n hiçbirisinde operasyon s›ras›nda veya sonras›nda mortalite izlenmedi. Onsekiz (%21.2) olguda operasyon s›ras›nda üroloji hekimlerinden konsültasyon istendi. Onyedi olguda tam kat mesane laserasyonu reparasyonu, bir olguda mesane arka duvar›na invaze olan plasenta perkreata nedeniyle parsiyel sistektomi yap›ld›. Olgular›n 72'sine (%84.7) operasyon s›ras›nda, 50'sine (%58.8) operasyon sonras›nda kan ürünü transfüzyonu yap›ld›. Masif transfüzyona ba¤l› olarak 17 (%20) hastada dilüsyonel trombositopeni geliflti. Histopatolojik olarak olgular›n 55'inde (%64.7) plasenta akreata, 1'inde (%1.2) plasenta inkreata ve 25'inde (%29.4) plasenta perkreata tan›s› koyuldu. 2005-2006 y›llar›nda %0.2 (7/3435) olan plasenta invazyon anomalisi insidans› giderek artarak 2011-2012 y›llar›nda %0.8 (37/4344)'e ulaflt›. Sonuç: Plasenta invazyon anomalisi giderek artan s›kl›kla görülmektedir.
