Ashraf","doi":"10.47883/JSZMC.V11I03.5","DOIUrl":"https://doi.org/10.47883/JSZMC.V11I03.5","url":null,"abstract":"Background: Fluid replacement is one of the important issues in the management of burn cases.\u0000\u0000Objective: To compare the outcome between Ringer lactate and Hydroxyethyl starches (HESs) in cases with burn injuries.\u0000\u0000Methodology: Study design: Randomized controlled trial. Place and duration of study: Department of Plastic Surgery, Sheikh Zayed Hospital, Rahim Yar Khan, from June to December 2018. Study subjects selected were those who had a burn of any type (thermal, chemical, electric) and at least more than 10% of the surface area of the body. The cases were then divided into two equal groups. The fluid requirement was calculated according to the Parkland formula. After this, the cases in group A were given Ringers lactate solution 500 ml while those in group B were given 500 ml of 6% hydroxyethyl starches (HESs) and further fluid was administered in group A as ringer only and in group B as ringer and HES in a ration of 2:1. The cases were then followed to look for various outcomes in the form of urine output, serum creatinine, and mean hospital stay.\u0000\u0000Results: In this study, there were a total of 60 cases (30 in each group). The mean age in group A and B was 17.33±5.39 vs 19.11±4.79 years (p= 0.33). Mean hospital stay was 14.65±3.31 vs 13.49±2.78 days with p= 0.41. Different studies have associated burn injuries with psychiatric disorders from different parts of the world. Objective: To determine the frequency of depression among the burn patients. Methodology: This cross sectional study was conducted at the Pak Italian Modern Burn Unit, Nishtar Medical University, Multan and the Department of Psychiatry, Nishtar Medical University, Multan. Non probability consecutive sampling technique was used to collect the data from study subjects, which consisted of 150 patients of burn. All patients were assessed by a single psychiatrist on Hospital Anxiety and Depression scale (HADS) scale for screening of depression. Chi square test was applied to determine association between the socio demographic factors of burn patients and depression. Results; Of these 150 study cases, 91 (60.7 %) were male patients while 59 (39.3 %) were female patients. Mean age of our study cases was 33.94 ± 8.88 years. The frequency of depression was noted as 88 (58.7%). Post stratification Pearson chi square test revealed significant associations between age (p=0.001), residential status (p=0.038), occupation (p= 0.000), etiology of burn (p=0.044) and duration of illness (p=0.000) with depression. Conclusion: A high frequency of depression was noted among burn patients in our study. Depression was significantly associated with age, residential status, occupation, duration of illness and etiology of the burn. Niazi","doi":"10.47883/jszmc.v11i2.105","DOIUrl":"https://doi.org/10.47883/jszmc.v11i2.105","url":null,"abstract":"Background: Surgeons are performing millions of operations on diabetic patients daily, and lack of awareness among diabetic patients is leading to complications. \u0000Objective: To determine chronic glycemic control in general surgical patients admitted at a tertiary care hospital. \u0000Methodology: This was a cross-sectional study conducted from June 2018 to January 2019, on fifty-seven consecutive patients, suffering from diabetes and needing surgical intervention in any form were included in this study. Diabetes status in terms of HbA1c, causes of admission to the surgical ward, and intervention done were noted. Data were analyzed by using SPSS 20. \u0000Results: Among these diabetic patients, 46 (80.70%) were male, and 36 (63.16%) were known diabetics. HbA1c level was normal in 9 (15.79%) patients, pre-diabetic in 13 (22.81%) and uncontrolled diabetic in 35 (61.4%) patients. Aetiologically diabetic foot was seen in 36 (63.16%) patients, abdominal catastrophe 12 (21.05%), leg swelling 7 (12.09%), 5 (8.77%) scrotal abscess, carbuncles 4 (7.02%), miscellaneous 5 (8.77%). Incision drainage and closure were done in 10 (17.54%), drainage wound debridement, decompression of compartment syndrome, and constructive procedure in 11 (19.3%), laparotomy 8 (8.77%) and watchful conservation in 4 (7.01%) patients. Two patients were saved from the mortal blow of the diabetic coma. Hypertension and nephropathy were seen in 8 patients each, and 5 patients have Hepatitis C, and 1 patient has ischemic heart disease. \u0000Conclusion: This study showed that many patients did not know their diabetes status, two-third of patients were having uncontrolled diabetes. Transformations, influencing the different levels of β-globin quality articulation causing β-thalassemia, have been found in an overwhelming number. \u0000Objective: To evaluate the biochemical and anti-oxidative status in patients suffering from Thalassemia. \u0000Methodology: This was a comparative study conducted from June 1st to November 30th, 2018. A blood sample of 5 ml of 60 diagnosed Thalassemia patients conveniently selected and 60 healthy individuals were taken from a vein in clotted gel vials from Children Hospital and Jinnah Hospital Lahore. Variables included were, Reduce Glutathione (GSH), Catalase (CAT), Superoxide Dismutase (SOD), Malondialdehyde (MDA), Nitric oxide (NO), Vitamin A, C and E, Na+ and K+, Complete Blood Count (CBC), Hemoglobin (Hb) and serum iron level. \u0000Results: MDA level in Thalassemia patients was elevated (3.02±0.45) vs controls (1.29±0.21) (p= 0.000). The level of GSH was decreased (0.18±0.14) as compared to controls (6.32±0.13). The CAT level was elevated in patients (2.64±0.11) than controls (4.11±1.02). The amount of SOD that was elevated in patients (5.33±0.81) vs controls (3.21±1.07). Vitamin E level in patients was (2.38±0.59) vs controls (4.33±0.95). Red Blood Cells were also decreased in Thalassemia patients (18.61±3.32) as compared to controls (39.8±7.19). Serum Iron level was elevated in patients (131.33±2.56) vs controls 93.21±1.11. (p=0.000). \u0000Conclusion: The expanding level of MDA demonstrates the high rate of lipid peroxidation (oxidative worry) in thalassemic patients. Methodology : It was a cross sectional study, conducted at Emergency Department of Sheikh Zayed Medical College/Hospital, Rahim Yar Khan, from August to October, 2019. A total of 139 subjects who were patients attending emergency department were involved in the study by using random sampling technique. Data was collected by a pre-designed proforma. Variables included were age, sex, occupation, door to treatment time, presenting complaints of patient, designation of health care provider, further investigation advised, given treatment, referral to OPD or ward. \u0000Results: Mean age of patients was 33±19 years. Out of 139 patients, 59.7% were males and from urban area each, 44.6% were illiterate, 75% of the study subjects were treated by a doctor, and 25.9% came with GIT complaints. Group I, was Control group, group II and III were experimental groups, given drugs A (Slim Smart) and B (Ultra Slim Plus) respectively via oral gavage. Each group was further subdivided into subgroups \"a\", drug was given for 4 weeks, and \"b\", the drug was given for 8 weeks. Control group animals were given plain distilled water via oral gavage tube. Body weight and weight of the liver was measured and its gross features were observed. \u0000Results: Mean body weight of animals significantly increased in intervention groups, at 4 weeks, p=0.000 and 8 weeks p=0.000. Mean body weights of animals in experimental group II and III in 4 weeks categories were 33.92±3.23 grams in group II and 28.46±3.53 grams in group III. The mean relative tissue weight index at 4 weeks for group II was 6.66±0.67 and for group III was 5.71±0.41, p=0.004, while at 8 weeks were 5.88±0.64 and 5.91±0.69, p=0.713, respectively. Many studies show their utilization in the treatment of diarrheal disease especially among children of under-five. \u0000Objective: To assess the treatment outcome of diarrhea with and without probiotics among children aged 12-60 months. \u0000Methodology: This comparative cross-sectional study was undertaken from July to December 2017. A pre-designed questionnaire was used to collect the information in terms of antibiotics, rehydrating agents, antipyretic agents, and probiotics. A total of 109 patients were selected randomly and then divided into two groups as \"probiotic\" having 55 children and \"Non-Probiotic\", having 54 children. Guardians of patients were contacted telephonically after 48 hours and 72 hours to ask about the time frame of relief from symptoms, diarrheal episodes, and cure from disease. Data were analyzed by using SPSS 20. \u0000Results: Out of 109 patients, 54 patients were in the non-probiotic group and 55 in the probiotic group. The overall age of patients remained 31±12 months. Saccharomyces boulardii (56.4%) and Lactobacillus GG (43.6%) were probiotics prescribed by pediatric physicians. A significant difference (p=0.000) was observed in terms of treatment outcome showing early relief (12 hrs) 43 (78%) vs 22 (41%) in probiotic vs non-probiotic group and early cure (<48 hrs) from diarrhea in probiotic group 48 (87%) vs 35 (65%) as compared to the non-probiotic group. Shafiq","doi":"10.47883/jszmc.v11i2.34","DOIUrl":"https://doi.org/10.47883/jszmc.v11i2.34","url":null,"abstract":"Background: Foreign bodies are commonly impacted in the esophagus and button battery cells are one of the foreign bodies, which could be found. \u0000Objective: To find out the frequency and complications of button battery cell intake, among the cases of esophageal foreign bodies. \u0000Methodology: A cross-sectional study was conducted at the Department of Otorhinolaryngology of Nishtar Hospital, Multan, from December 2017 to November 2018. After getting anesthesia fitness, rigid endoscopy was performed to remove the foreign body in 250 cases. Nasogastric intubation was done kept in place for two weeks before starting oral sips. Antibiotics, steroids, and H2 receptor antagonists were given in cases of the battery cell. Age, type of foreign body, endoscopic findings, level of impaction, and type of complication were documented. Data was put in SPSS 23 and analyzed. \u0000Results: Out of 250 cases of foreign bodies in the esophagus, there were 50 (20%) cases of button battery intake, with an age range from 8 months to 8 years. The delay between ingestion and presentation ranged from 3 hours to 28 days.
