It has strong association with obesity 41-61%, metabolic syndrome 30-56%, Type-2 diabetes 18- 28%, dyslipidemia 52-83% and hypertension 15- 46%. These risk factors promote NAFLD as a global heath issue especially in western countries with a prevalence rate of about 20-30%. In recent years, the prevalence rate 15-30% has been found in Asian countries in general population due to strong influence of life style modification and urbanization pattern to the associated risk factors. There is an urgent need to reduce the enormous clinical and economic burden of NAFLD globally.1, 2\u0000\u0000Life style modification in the form diet, weight loss and physical exercises is advocated as the first line of intervention in NAFLD patients.3 However there is no effective FDA drug treatment for NAFLD patients so far. Due to the presence of associated comorbidities in NAFLD patients various drugs such as insulin sensitizers, anti oxidants, antidyslipidemic, hepatoprotective (silymarin) and miscellaneous agents like pentoxifylline, orlistat and incretin based therapies with varying results have been tried up till now.4\u0000\u0000Green Tea: Green Tea is an unfermented product of the leaves and bar of the plant (Camellia sinensis) and is one of the famous drinks all over the world especially in the region of South East Asia. Green Tea contains thousands of bioactive compounds out of which one third is contributed by polyphenols which are mostly flavonoids.5 Catechins are one of the mainflavonoids in green tea has recently attracted attention for its use in various diseases due to its antiaging, anti-cancer, anti-parkinsonism, anti-stroke, anti diabetic, anti-caries and anti-bacterial, anti-diarrheal, anti-fibrotic, anti-inflammatory, anti-oxidative and anti-atherosclerotic properties.6\u0000\u0000The green Tea has a strong potential to reverse all those suspected mechanisms that has been suspected in the pathogenesis of NAFLD in various clinical and experimental studies. Green tea decrease hepatic steatosis by reducing hepatic insulin resistance which is a key mechanism in its pathogenesis.7 Green tea has a strong potential to boost immune system, reduces the formation of reactive oxygen species (ROS), increases the activity of pro-oxidant enzyme such as glutathione peroxidase and superoxide dismutase through it antioxidant properties and finally green tea reduces various inflammatory chemokines and cytokines as inflammation and oxidative stress is the pathognomonic features in NAFLD related complication.8 In addition green tea has beneficial effect on all NAFLD associated medical conditions such as obesity, metabolic syndrome,9 hypertension and dyslipidemia10 in various clinical studies.\u0000\u0000Nigella sativa: Nigella sativa a natural remedy for variety of medical ailment has been used from centuries due to its historical and religious background. Shahid, Rubab Naqvi","doi":"10.47883/JSZMC.V11I04.152","DOIUrl":"https://doi.org/10.47883/JSZMC.V11I04.152","url":null,"abstract":"Background: Postpartum hemorrhage (PPH) remains a leading cause of maternal mortality in developing countries.\u0000\u0000Objective: To determine the maternal outcome by an audit of one year among the cases of primary postpartum hemorrhage at a tertiary care hospital.\u0000\u0000Methodology: This cross-sectional study was conducted in the Department of Obstetrics and Gynecology, DHQ Teaching Rawalpindi from 1st January 2019 to 31st December 2019. A total of 9122 deliveries were conducted at the hospital. We reviewed the charts of all the patients who fulfilled our inclusion criteria and gathered data on a structured, pre-tested proformas prepared for the purpose.\u0000\u0000Results: A total of 9122 maternities were reported during the study period with the frequency of PPH 70 (0.77%). Booking status, 25 (35.7%), high parity 18 (25.7%), and uterine atony in 54 (77%) out of 70 PPH patients, were associated with PPH. Medical management of PPH with uterotonic drugs was successful in 23 (30%) of patients. Uterine packing was done in 27 (38%) patients and bleeding was successfully arrested in 24 (90%) of these 27 patients. Hysterectomy was performed in 9 (12.8%) patients. Maternal death due to PPH was reported in 1 (1.4%) cases.\u0000\u0000Conclusion: PPH is the leading cause of maternal mortality and morbidity, associated with booking status, parity, and uterine atony. Adenotonsillectomy (AT) is taken as the primary treatment option.\u0000\u0000Objective: To evaluate the efficacy of Adenotonsillectomy among children having obstructive sleep apnea syndrome.\u0000\u0000Methodology: This was a cross-sectional study conducted at Bahawal Victoria Hospital Bahawalpur from January 2017 to January 2018. A total of 52 children of both genders, aged 3 to 10 years, having obstructive sleep apnea syndrome (OSAS) and selected for Adenotonsillectomy (AT) were enrolled. Adenotonsillectomy was performed in all the study participants employing general anesthesia. Preoperative and postoperative characteristics as well as respiratory and sleep parameters were compared adopting chi-square test for qualitative variables while t-test was employed for quantitative variables.\u0000\u0000Results: Out of a total of 52 children, 32 (61.5%) were male. The majority of the children, 27 (51.9%) were between 3-6 years of age. Statistically significant improvement (p-value < 0.05) was noted at the postoperative interval following AT for obstructive AHI score, respiratory disturbance index, obstructive apnea and hypopnea index, SpO2 nadir, TSpO2<90%, and respiratory arousal index. Channa","doi":"10.47883/JSZMC.V11I04.149","DOIUrl":"https://doi.org/10.47883/JSZMC.V11I04.149","url":null,"abstract":"Background: laparoscopic cholecystectomy is commonly used for the treatment of gallstones.\u0000\u0000Objective: To determine the feasibility and safety of difficult laparoscopic cholecystectomies.\u0000\u0000Methodology: This cross sectional study was based on retrospective collection of data from patient records, including 323 patients with difficult laparoscopic cholecystectomies was conducted in Department of Surgery, Sharif Medical City Hospital, and Rasheed Hospital, Lahore from June 2010 to December 2019. Difficult cholecystectomy was defined on intraoperative findings based on Nassar intraoperative scoring system. Feasibility was defined as successful accomplishment of procedure without complication and safety was defined as having no intraoperative or postoperative complications.\u0000\u0000Results: There were 75 (23.21%) male and mean age was 48±8 years. Class I difficulty was observed in 185 (57.3%) patients, class II difficulty in 83 (25.7%) patients, class III difficulty in 44 (13.6%) patients and class IV difficulty in 11 (3.4%) patients. Mean duration of surgery and mean hospital stay were 98.87±11.76 minutes and 1.91±1 days, respectively. Conversion to open cholecystectomy was done in 10 (3.1%). The procedure was feasible in 313 (96.9%) patients. Overall complications were seen in 19 (5.9%) patients. The complications included Common Bile Duct injury in 1 (0.31%) patient, intraoperative bleeding in 1 (0.3%) patients, bile leakage in 2 (0.62%) patients, postoperative jaundice in 3 (0.93%) patients, superficial infections in 10 (3.1%) patients and deep infections in 2 (0.62%) patients. Safety of laparoscopic surgery was seen in 304 (94.1%) patients.\u0000\u0000Conclusion: Laparoscopic cholecystectomy in difficult situations was found to be feasible and safe in majority of patients. The study was conducted based on the data gathered from Cooperative Bank of Oromia Mida Kegn Branch (CBO). A mixed research approach was used to answer the research questions that emerge through the review of existing literature and the experiences of the researcher in respect of the E-Banking system in Ethiopia. The study statistically analyzes data obtained from the survey questionnaire. The study was conducted on Mida Kegn Branch and 154 customers were selected by Simple Random Sampling. The data gathered through primary method was summarized using descriptive statistics through frequency distribution and percentages, mean and standard deviation, Pearson Correlation to come up the analyses of data. Mean score is used to identify the highest and the lowest of the variables. Pearson‘s Correlation analysis is used to test the degree of the relationship between independent variables and the dependent variable. The aim of current study was to determine the frequency of lower segment cesarean section (LSCS) along with indications of LSCS in multiparous women with previous normal deliveries.\u0000\u0000Methods: This was a cross sectional study, done at the Department of Gynaecology & Obstetrics, Nishtar Hospital, Multan, from 20th July 2018 to 20th January 2019. A total of 222 pregnant women with live singleton pregnancy at term with gestational age between 37 to 41weeks and multipara with no previous LSCS were included in the study. The decision to undertake LSCS if indicated was made in every case after complete assessment. Data was collected for frequency of LSCS and its indications in shape of fetal distress, antepartum hemorrhage and malpresentation.\u0000\u0000Results: Mean age was 32.720±1.96 years, mean height 1.551±0.11 meters and mean BMI was 27.855±3.14 Kg/m2. Most women were 31-35 years (90.5%) of age and 2-4 parity (89.2%). Primary caesearen section was seen in 43 patients (19.4%). Indications for primary caesarean section were malpresentation 27.9%, fetal distress 41.9% and antepartum hemorrhage 30.2%.\u0000\u0000Conclusion: The frequency of LSCS in multiparous women with previous normal deliveries was noted to be 19.4%. The awareness collection tool was a structured questionnaire comprising of sociodemographic variables and questions which explored the awareness of osteoporosis due to andropause. Serum calcium levels were also measured in different age groups. Data collected were analyzed by Statistical Package for the Social Sciences (SPSS) version-20.\u0000\u0000Results: Overall 51% of men were aware of the term andropause while the percentage of the participants who believed that andropause may be the underlying cause of osteoporosis in advancing age was only 26.6%. Serum calcium showed a negative correlation with advancing age (Pearson correlation coefficient, r-value: -0.58).\u0000\u0000Conclusion: Though more than half of the men were aware of the knowledge of andropause they lack the awareness that osteoporosis might occur due to androgen deficiency. {"title":"Diagnostic Accuracy of High-Resolution Computed Tomography in Assessing Activity of Pulmonary Tuberculosis","authors":"Mian Waheed Ahmad, Nawaz Rashid, Sadaf Arooj, Sidra Shahzadi","doi":"10.47883/JSZMC.V11I04.99","DOIUrl":"https://doi.org/10.47883/JSZMC.V11I04.99","url":null,"abstract":"Background: High-resolution computed tomography (HRCT) has been used in the diagnosis of Pulmonary TB and rapid TB diagnosis is essential and critical for TB control.\u0000\u0000Objective: To evaluate the role of High-Resolution Computed Tomography in diagnosing and assessing the activity of pulmonary tuberculosis in patients.\u0000\u0000Methodology: Study Design: Cross-sectional study involving radiographic (HRCT) and clinical analysis of patients with suspected tuberculosis, from October 2018 to March 2019. This study was done on a group of 100 patients with suspected tuberculosis having symptoms of fever, cough with sputum, and hemoptysis. After informed consent first detailed clinical history was taken from patients. Criteria standardized for inclusion was; all age groups and both genders. Patients with a history of previous chest surgery and carcinoma were excluded. Then patients undergoing HRCT chest were analyzed and correlated with clinical findings. Features of primary TB include consolidation, lymphadenopathy, pleural effusion, and miliary nodules whereas post-primary TB include apical consolidation, nodules, and cavitation.\u0000\u0000Results: Overall 80 (80%) out of 100 subjects were diagnosed with Pulmonary TB on High-Resolution Computed Tomography (HRCT). The most common HRCT findings were tree in bud appearance (77%), fibrotic changes (72%), consolidation (68%), cavitation (40%), bronchiectatic changes (18%), ground-glass haze (18%), and calcified granulomas( 10%).\u0000\u0000Conclusion: Although chest radiography is the foremost imaging technique in the diagnosis and evaluation of pulmonary tuberculosis in our setup, HRCT can be important in early diagnosis and management with greater sensitivity. Mothers must do proper weaning practices in order to prevent malnutrition and infection in children.\u0000\u0000Objective: To determine the weaning practice among mothers with infants aged 6 months to one year of age, and causes of delayed weaning.\u0000\u0000Methodology: A cross-sectional descriptive study was carried out among the residents of the slum area of Jahangirabad, Multan, from 31st March to 25th May 2018. A close-ended questionnaire was used for collecting data from 85 mothers using a non-probability convenient sampling technique. Frequency and percentages were calculated for qualitative variables like education, occupation, socio-economic status, the start of weaning time. Data were analyzed using SPSS version 20.\u0000\u0000Results: Mothers who started weaning of their children at age of 6 months and above were 91%. Mothers who felt their milk was enough for the baby were only 37%. Mothers using marketed food for weaning were, 48 %. Illiterate mothers were, 45%. Those who were doing weaning less than 6 times per day were 98%. Families belonging to low socioeconomic status were, 61%. On applying the chi-square test there was no significant association found between the start of weaning time and mother's education (p=0.3) or occupation (p=0.3).\u0000\u0000Conclusion: A large proportion of children's population has started weaning above 6 months of age. Duration of study: May to July 2019. A total of 100 questionnaires were filled, 50 from Lahore and 50 from Bahawalpur. Information on demographics, family size, and mother in laws responses to her wish related to family size was noted. The influence of mother-in-law on family size was assessed by analyzing the family size of the daughter in law with regards to the mother in law wishes to have more children. \u0000\u0000Results: Overall, 57% of mother in law wanted more children, 66% of sons favored mothers, and 62% of mothers hold household budget. This study showed that in Lahore, the majority, 29 (58%) families had 0-2 children, whereas, in Bahawalpur, 14 (28%) % had 0-2 children. (P-value =0.00) In Bahawalpur, 23(46%) of mother-in-law wanted their daughter-in-law to have more children versus 34(68%) in Lahore, (p value=0.02). In Bahawalpur, 33(66%) and in LHR 34(68%) of mother-in-law said that the bearing of more children will badly impact the health of their daughter-in-law. (p value=0.4).\u0000\u0000Conclusion: Mother-in-law's wish to have more grandchildren has a significantly inverse influence on the family size of her daughter in law. The majority of the mother in-laws knew that more children could result in poor child care.
