Objective: To determine the MRI based initial diagnosis of acute spinal injuries and determine its pattern with reference to its neurological outcome. Methodology: Study Design: Prospective cohort study, involving radiographic and clinical review. This study was performed on 69 patients of spinal trauma, January to December, 2018, at Department of Radiology, Gujranwala Medical College and DHQ teaching Hospital, Gujranwala. Male and female of any age were included. After initial neurological examination these patients undergone for Magnetic Resonance Imaging of spine, which were analyzed and correlated with American Spinal Injury Association (ASIA) impairment scale (AIS), then subsequently at sub-acute interval to assess neurological outcome. Data analysis was done on SPSS version 17. Results: The most common MRI findings were spinal cord edema 24 cases (34.7%). Among the other findings were, cord contusion 17 cases (24.6%), spinal cord deformity due to extrinsic compression 14 cases (20.2%), focus of hemorrhage within the cord 9 cases (13%), epidural hematoma 3cases (4.3%) and few cases appeared normal cord 2 cases (2.3%). Most of the MRI findings correlated with clinical presentation of the patient according to American Spinal Injury Association (ASIA) impairment scale. Rashid","doi":"10.47883/jszmc.v11i01.54","DOIUrl":"https://doi.org/10.47883/jszmc.v11i01.54","url":null,"abstract":"Background: Saline infused Sono hysterosalpingography (SSG) is the new and safe modality as compared to conventional hysterosalpingogrphy (HSG) for diagnosis of tubal blockage. \u0000Objective: To determine the diagnostic accuracy of saline infused Sono hysterosalpingohgraphy, for detection of fallopian tube blockage, in cases with infertility taking conventional hysterosalpingography as gold standard. \u0000Methodology: This was a cross sectional study conducted in Department of Radiology, Sheikh Zayed Hospital, Rahim Yar Khan, conducted from May to November, 2017. A total of 150 cases, with age range of 20-40 years, irrespective of type and duration of infertility, were included. Findings of tubal blockage with saline infused Sono hysterosalpingography (SSG), were compared to conventional hysterosalpingogrphy (HSG). Sensitivity, Specificity, PPV, NPV and Diagnostic accuracy of saline infused Sono-hysterosalpingography, for detection of fallopian tube blockage, in cases with infertility were calculated, taking conventional hysterosalpingography as gold standard. \u0000Results: A total of 150 cases were examined, with mean age 32±5 years and mean duration of infertility was 3.51±1.86 years. Overall, 99 (66%) cases had primary infertility. Tubal obstruction was seen in 92 (61.33%) cases on HSG and 90 (60%) on SSG. The sensitivity of SSG for tubal obstruction was 77.14%, PPV 78.89% and diagnostic accuracy 75.33% with p= 0.001. The specificity and PPV was 100% in cases with age 30-40 years with p= 0.001. The diagnostic accuracy in multiparous women was 81.82. Fatima, Mashal Fatima, Irum Hayat, Wajahat Hussain, Javaria Sadaf","doi":"10.47883/jszmc.v11i03.128","DOIUrl":"https://doi.org/10.47883/jszmc.v11i03.128","url":null,"abstract":"Background: Knowledge regarding the factors impeding the using antenatal care would help in effective maternal and child health care delivery.\u0000\u0000Objective: To enlist the barriers resulting in the non-utilization of antenatal care in public sector hospitals in Bahawalpur. \u0000\u0000Methodology: This was cross-sectional study conducted in the catchment area of public sector healthcare facilities of Bahawalpur, from 20th February to 20th September 2019. The study population comprised of 152 females of childbearing age selected by systematic multistage random sampling technique. A pre-designed and pre-structured questionnaire was the tool for data collection from the study population. Data were analyzed using SPSS version 20.\u0000\u0000Results: Non-utilization of antenatal care services was found in 44% of women. Zahoor, Muhammad Asif, Shahtaj Khan","doi":"10.47883/jszmc.v12i1.95","DOIUrl":"https://doi.org/10.47883/jszmc.v12i1.95","url":null,"abstract":"Background: Diabetes mellitus is a highly prevalent non-communicable disease with a further implication of the development of life-threatening diseases like cardiovascular disease and stroke. \u0000\u0000Objectives: To determine the glycosylated hemoglobin levels utility in assessing the point of care testing in the control of diabetes mellitus in the population of Nowshera in a hospital-based study.\u0000\u0000Methods: This was a cross-sectional study, conducted in the department of Pathology in collaboration with the Department of General Medicine, Qazi Hussain Ahmed Medical Complex, Nowshera, from February 2019 to January 2020. Descriptive statistic was used for describing the variables. A Chi-square test was used for the association of Hba1c in gender groups. SPSS version 25 was used for data analysis.\u0000\u0000Results:\u0000\u0000Out of the total 119 patients, 44 (37%) were males and 75(63%) were females. The mean age with standard deviation was 53±9.5. The Mean Hba1c with standard deviation was 10.53± 2.38. Sixty-eight (57.1%) of the patients were in the age range 46-60 years. Eighty-eight (73.9%) were poorly controlled diabetics with Hba1c of >9%. Using the chi-square test it was observed that gender has no significant impact on the severity of the disease (p= 0.38). The relative risk of female gender and for age >50years was (RR=1.11).\u0000\u0000Conclusion: We observed that the majority of the patients had poorly controlled diabetes with Hba1c >9% which is an alarming situation. Iqbal","doi":"10.47883/jszmc.v12i1.167","DOIUrl":"https://doi.org/10.47883/jszmc.v12i1.167","url":null,"abstract":"Background: Diabetic patients presenting with Fournier gangrene have the highest mortality rate of all at-risk populations. \u0000\u0000Objective: To compare the efficacy of adjuvant topical oxygen therapy methods and conventional methods in the management of diabetic patients having Fournier's gangrene\u0000\u0000Methodology: This was a comparative cross-sectional study, in Surgical unit 3, Allied Hospital, Faisalabad, from 1st January to 30th June 2020. A total of 120 cases were included by systematic random sampling in the study, who were admitted either through OPD or the Emergency ward. In Group A, Topical Oxygen Therapy was given along with conventional methods. In Group B only conventional methods of wound care were applied. In group A, 100% oxygen was given in a bag with a routinely available oxygen cylinder exposing the whole affected part of the body or limb. Oxygen in the chamber was given for a maximum of one and a half hours twice daily in one sitting for 7 to 10 days. In the interval between cycles, the wound was covered with a soaked antiseptic dressing. After clinical improvement, the patient was discharged and called for follow-up in outdoor on weekly intervals initially and then fortnightly for up to 6 months. \u0000\u0000Results: Mean age was calculated as 36.45±10 and 35.38±9 years in Group-A and B respectively.
