Their use is increasing globally, across the species with desired beneficial effects when used judiciously. On the other hand, their overuse for prolonged period of time in unjustified doses, duration and indication is leading to drug resistance. Consequently, these agents are becoming ineffective rendering the management of infections increasingly difficult or impossible. In a way, drug resistance is largely a man-made problem and it can be tackled with good prescribing practices. Growing trends in the emergence of AMR is considered a major threat to global health and economy. World Health Organisation (WHO) considered it one of the top 10 global health threats. \u0000\u0000O'Neill J. et al published an excellent review on antimicrobial resistance in 2016. In this report, they studied the global trends, track the possible result of drug-resistant infections on health and economy globally if issue unaddressed and gave recommendations to tackle this menace.1 Even at the time of collection of data (2014-16), AMR was directly responsible for death of 700,000 deaths. It was estimated that if the trends went on un-addressed, world may see loss of 10 million lives by 2050, overtaking diabetes, heart disease and cancer as the leading cause of death in human and an approximate cost reaching 100 trillion USD. No country or region is immune but the low and middle-income countries are likely to suffer most.\u0000\u0000 \u0000\u0000Although some experts and researcher had doubts about these estimates, 2,3 WHO and most of other authorities agree that the spread of AMR is a real, seriously growing global threat to be addressed by a well-coordinated global action plan.\u0000\u0000A recently published systematic analysis confirmed rising trends. Data from 204 countries and territories showed an estimated 1.27 million deaths caused directly by AMR worldwide, and 4.95 million deaths were associated with AMR, in 2019, higher than the mortality of HIV/AIDS and malaria which have been estimated to be 860,000 and 640,000 deaths, respectively, in same year.4\u0000\u0000 \u0000\u0000After the start of COVID -19, in 2020 hospitals have to cater a different patient population. Higher number of seriously ill, aged patients with comorbidities, who needed intensive care for a prolonged period. Without a clear guideline to deal with the newly emerged infectious outbreak, multiple therapeutic agents were used. These agents included many antibiotics, antiviral and sometimes antifungal agents used empirically. Many new and old drugs were also used on experimental basis. Additionally, all the seriously ill patients received corticosteroids as recommended in guidelines and many of the patients required invasive devices, equipment or catheters etc. for prolong time. All these are risk factors for development of drug resistance. Objective: To determine the normative data of central corneal thickness and endothelial morphology in our population. Methodology: This is a Cross-sectional study. The study was done at Ophthalmology Department, FRPMC in accordance with the declaration of Helsinki from March 2021 to August 2021 with approval from the Institutional review board. All healthy individuals aged 20 to 70 years presenting with any ocular complaints but no corneal pathology were included. Individuals with any systemic and ocular co-morbidity were excluded. After a complete ocular examination, corneal specular microscopy was done using REXAM SPM-700 corneal specular microscopes. In our study, we analyzed CCT, ECD, MCA, CV and hexagonality. The data was collected and recorded on an Excel spreadsheet and then transformed to SPSS version 22 for statistical analysis.\u0000\u0000Results: 724 eyes from 362 individuals were studied for CCT and endothelial morphology using Rexam SPM-700 microscopes. There were 241(66.6%) males and 121(33.4%) females with mean age 38.33±10.17 years. Among 724 eyes we found CCT (508.43±33.4um), ECD (2565.12±254.15cell/mm2), AVG (393.81±40.72um2), CV (39.54±6.43%), and Hexa (46.18±6.64%). Statistically, there was no significant difference between the right and left eyes of the study population as well as gender-wise. There is a statistically significant inverse correlation of ECD with age (p<0.00, r:-0.19). However, a statistically insignificant inverse correlation is found for CV and Hexa with age (p<0.17, r:-0.05) (p<0.70, r:-0.014) respectively. Conclusion: Our study reports the normative corneal endothelial morphology and thickness in the Pakistani population attending FRPMC Ophthalmology Department. Objective: To determine job satisfaction in doctors amidst COVID-19 pandemic at the University of Lahore Teaching Hospital (ULTH), Lahore, and to identify factors that influence job satisfaction the most. Methodology: Descriptive cross-sectional study was done from April 30 to October 30, 2021, at ULTH, Lahore. A total number of 135 doctors (House officers, Medical officers & Consultants) working for a duration of 3 months in COVID-19 unit at ULTH were included. Pre validated online questionnaire was used. The data collected were analyzed by descriptive statistics using SPPS, 25th version. A 6-point Likert scale for satisfaction was used to assess the job satisfaction level of doctors. For quantitative variables, percentage scores and cumulative percentages were calculated. Results: More than half of the doctors were male and less than 30 years. Majority (67%) were working during COVID-19 pandemic for more than 6 months. About 90% of doctors willingly performed their duty in this pandemic. About half (40%) of doctors were assessed for stress, anxiety, and depression. About 78% did not get free health care services when infected. About 32 % of them experienced physical abuse and 67.4% verbal abuse by the patients. Meanwhile, 31.1% of doctors were somewhat satisfied with the distribution of workload and about 30 % were satisfied with the availability of PPE. Water supply and hygiene are unsatisfactory in Pakistan's Primary Healthcare Centers. Interventions must be introduced to improve the conditions in water, sanitation, healthcare waste, and hand hygiene. Objective: To assess the outcomes of the WASH-FIT process at randomly selected Rural Health Centres in district Bagh, Azad Kashmir. Methodology: Three Rural Health Centers in District Bagh, Azad Jammu, and Kashmir including; Malot, Rehra, and RHC Birpani, with a sample size of 32, were examined in this quasi-experimental study, from March to May 2020. The WASH practices of the aforementioned RHCs were assessed using WASH-FIT. Shortcomings were identified initially, and later an assessment was done after the intervention had been implemented. The intervention measures included a range of actions including; the use of chlorine tablets, giving water coolers, hand rubs, dustbins with lids, maintenance of the hospital register, use of gloves, masks along with health education sessions. SPSS version 24 was used for the analysis. Results: Without intervention, in these RHCs only 0 to 50% of the WHO criteria for water, sanitation, healthcare waste, and of hand hygiene. Water, sanitation, healthcare waste, and hand hygiene outcome were significantly raised by 25-100% (p value<0.05) following intervention in certain indicators. Hence with the intervention introduced, the performance of all the indicators was improved to up to 100%. One of the most dangerous complications is the formation of left ventricular(LV) thrombus, especially after STEMI, and is considered a major high-risk factor for morbidity and mortality. Although nowadays with primary percutaneous coronary intervention its incidence is reduced but still significant.\u0000\u0000Objective: To determine the frequency of left ventricular thrombus in ST-elevation myocardial infarction (STEMI) patients treated with primary percutaneous coronary intervention (PCI).\u0000\u0000Methodology: This was a cross-sectional study done at the Department of Cardiology, National Institute of Cardiovascular Disease (NICVD), Karachi from October 3, 2021, to April 2, 2022. Patients who fulfilled the inclusion criteria and were able to give informed consent were enrolled. Primary PCI was performed by a consultant cardiologist and post PPCI all the patients were observed, and transthoracic echocardiography (TTE) was performed to assess the left ventricular thrombus. The data was collected on predesigned proforma.\u0000\u0000 Results: Mean ± SD of age was 59.39±14.79 years. In the distribution of gender, 101 (73.7%) were male while 36 (26.3%) were female. Left ventricular thrombus was noted in 24 (17.5%) patients.\u0000\u0000Conclusion: It is to be concluded that ventricular thrombus is prevalent after primary percutaneous coronary intervention for STEMI patients. Place and Duration of Study: Department of Surgery, Chandka Medical College Hospital, Larkana from November 2019 to January 2022. A total of 217 diagnosed cases of breast carcinoma patients who underwent a modified radical mastectomy after initially receiving neoadjuvant chemotherapy were selected for this study. Pre-designed proforma was used to collect information related to different variables.\u0000\u0000Results: The average age of the women was 47.29±8.38 years. The frequency of seroma formation in patients undergoing modified radical mastectomy with neo-adjuvant therapy was found in 41.94% (91/217). The average seroma Volume and number of resected lymph nodes were 56.64±5.64 ml and 7.59±1.08.\u0000\u0000Conclusion: Prediction of seroma formation following Modified Radical Mastectomy with neo-adjuvant therapy remains challenging. {"title":"Admission Hyperglycemia in COVID-19 as Outcome Predictor: A Single Centre Study","authors":"","doi":"10.47883/jszmc.v13i2.204","DOIUrl":"https://doi.org/10.47883/jszmc.v13i2.204","url":null,"abstract":"Background: Hyperglycemia on admission among COVID-19 may affect the patient outcome.\u0000\u0000Objective: To determine the effect of admission hyperglycemia on outcome among COVID-19 patients.\u0000\u0000Methodology: It was a cross-sectional analytical study in which 421 COVID-19 patients were admitted with high blood sugar levels (BSR>180mg/dl) to the High Dependency Unit, Sir Sadiq Abbasi Hospital, Bahawalpur, from April to May 2021 were included. Preexisting diabetes status was confirmed on the basis of history taken from the patients. Patients were divided into two groups on basis of BSR, moderate hyperglycemia (180-299mg/dl) and severe hyperglycemia (300-450mg/dl). The primary outcome was taken as an increase in oxygen demand leading to shifting of the patient to ICU for Non-Invasive Ventilator (NIV) support or a decrease in oxygen demand leading to discharge home.\u0000\u0000Results: Among 421 COVID-19 patients; 349 (83%) patients had moderate hyperglycemia and among them, 172 (49.4%) were shifted to NIV, and 177 (50.6%) patients were discharged after improving. Seventy-two patients came out to have severe hyperglycemia out of which 61 (84.3%) were shifted to NIV and 11 (15.3%) patients were discharged home (p-value = 0.01). Today it is one of the biggest health threats to the populations globally, and researchers have consistently raised their voices, which is based on sound evidence.1 As described, overuse of antibiotics is one of the causes of antibiotic resistance, however it can occur naturally occasionally. The underlying factor is that standard treatment guidelines are not mostly followed to treat infections. Additionally, incorrect diagnosis, wrong dose, lack of source control, poor qualities of the drugs, extended hospital stay, increasing prevalence of resistant organisms along with co-infections are other important causes of antibiotic failure.2\u0000\u0000Many studies have reinforced these findings and found that inappropriate prescription practices, frequent over-the-counter sale of antibiotics, widespread use of antibiotics in veterinary medicine, poorly functioning drug regulatory authority, and inefficient patient education system about antibiotic use are other causes of antimicrobial resistance in developing countries.1,2 Considering this grave situation of the prevalence and factors associated with antibiotic resistance, stewardship regarding antibiotic use is an important strategy to overcome the bacterial resistance, irrational use of antibiotics, and prolonged hospital stay.\u0000\u0000Another important factor related to antibiotic resistance is that although the topic of antibiotics is usually taught in all medical colleges during undergraduate education in Pakistan, unfortunately components of antibiotic stewardship are missing in the curriculum of MBBS and allied health sciences. Whereas, in comparison with the developed countries like the United States and the United Kingdom, medical schools emphasize various components of antibiotic stewardship to their medical undergraduates. However even in their medical schools, the content of antibiotic stewardship is usually taught by lectures, and only a few medical schools teach antibiotic stewardship principles and make it part of the interprofessional curriculum. Antibiotic Stewardship practice is a very vast subject indeed and for undergraduate students, it needs to be tailored to the requirements of the community where it is going to be practiced. The core elements of antibiotic stewardship mainly emphasize seven important areas such as leadership, commitment, accountability, drug expertise, action, tracking, reporting, and education. Moreover, it's a multidisciplinary approach to implementing policies and interventions to improve antibiotic use for optimal patients.2\u0000\u0000Pakistan is facing many drug-resistance diseases like typhoid fever, and multidrug resistance Tuberculosis. Using Hounsfield Units (HU), a CT scan of the abdomen and pelvis may be utilized to diagnose and evaluate renal stones (all types).\u0000\u0000Objective: To determine the role of CT in the diagnosis of Calcium oxalate renal calculi on the basis of Hounsfield unit, taking chemical analysis as the gold standard.\u0000\u0000Methodology: Study Design: Cross-sectional study. Settings: Department of Radiology, Allied Hospital, Faisalabad. Study duration: 1st July 2020 to 31st December 2020. Sampling: Non-probability consecutive sampling technique. A sum of 200 patients with urinary stones of both genders (Male and Female) with age 15-55 years were incorporated. Hounsfield units were calculated for each stone using in-built software in the CT scan machine. The presence of CT attenuation density of >1200 HU was taken as calcium oxalate stone and ≤1200 HU was taken as non-calcium oxalate stones. Stones of all patients after surgery were shipped off to the pathology research facility for compound investigation.\u0000\u0000Results: In CT positive cases, 95 were true positive and 08 were false positive. While in CT negative patients, 90 were true negative and 07 were false negative. {"title":"Drug sensitivity Profile of Pseudomonas spp. isolated from a tertiary care hospital against commonly used antibiotics","authors":"","doi":"10.47883/jszmc.v13i2.206","DOIUrl":"https://doi.org/10.47883/jszmc.v13i2.206","url":null,"abstract":"Background: Antimicrobials agents are being abundantly used to control bacterial growth, like Pseudomonas, leading to resistance.\u0000\u0000Objective: To find out the drug sensitivity patterns of the Pseudomonas spp. against various antibiotics in the tertiary care hospital (Ghurki Trust & Teaching Hospital).\u0000\u0000Methodology: This cross-sectional study was designed on the retrospective collection of the records of 1000 sample reports, of the patients admitted in Ghurki Trust Teaching Hospital from the period of January 2018 to December 2019, who presented with multiple infections or developed pseudomonas infections during their stay in the hospital. Tests were carried out in the clinical microbiology Laboratory of Lahore Medical & Dental College for bacterial culture. Specimen included were breast tissues, pus, urine and stool culture, blood culture, wound discharge, ear discharge, abscess from the wound, various infected implants, and dead necrotic tissues from bone. Data were analyzed using SPSS version 26.\u0000\u0000Results: Out of the 1000 samples, a total of 150 (15%) were positive for bacterial growth. Among these 150, 70 were positive for gram-negative. The bacterial profiles of these 150 samples have shown that frequently (42) isolated gram-negative bacteria was Pseudomonas. On further tests, these bacteria were more sensitive to Imipenem and Cilastatin (69%) followed by levofloxacin ( 31%). No sensitivity was found against vancomycin and amoxicillin.\u0000\u0000Conclusion: In this study, the Pseudomonas were isolated and showed sensitivity to Imipenem and Cilastatin followed by levofloxacin.
