{"title":"Effect of Ranibizumab Injections on Visual Acuity and Central Macula Thickness","authors":"Mustafa Abdu, Yazan Gammoh, Abd Elaziz Mohamed Elmadina, Mohaned Hassab-Elrasoul","doi":"10.36351/pjo.v39i2.1544","DOIUrl":"https://doi.org/10.36351/pjo.v39i2.1544","url":null,"abstract":"Purpose:  To see the effect of Ranibizumab at 1-monthly and 2-monthly intervals on visual acuity and central macula thickness among patients with diabetic macular edema, neovascular age-related macular degeneration and diabetic retinopathy.\u0000Study Design:  Retrospective review of clinical record.\u0000Place and Duration of Study:  The present study was conducted at four eye centers (Al-Shami Eye Center, Sudan Eye Center, Makkah Eye Complex and Nor-Aloyon Eye Hospital) located at Khartoum state of Sudan from January 2021 and June 2021.\u0000Methods:  A total of 109 records of patients with diabetic retinopathy, age related macular degeneration and macular edema were included. They were categorized into 4 groups; group A (wet age-related macular edema (WAMD, n = 16)), group B (macular edema (ME, n = 32)), group C (proliferative diabetic retinopathy (PDR, n = 31)), and group D (non-proliferative diabetic retinopathy and macular edema (NPDR + ME, n = 30)). All participants underwent full ophthalmic examination before the injection, at one month and two months after the injection. Means, standard deviations and frequencies were calculated. ANOVA was used to find any significant difference between the study groups and the impact of treatment.\u0000Results:  Mean age of patients was 57.73 ± 10.44 years. There was significant improvement in terms of mean visual acuity from baseline to third follow up in all study groups, (F (1, 105) = 14.94, P < 0.001), with no significant differences in this improvement between the study groups, F (1.83, 5.48) =14.6, P = 0.19.\u0000Conclusion:  Use of Lucentis has demonstrated a statistically significant reduction in CMT and improvement in BCVA. Abdullah, Laiba Asif","doi":"10.36351/pjo.v39i2.1504","DOIUrl":"https://doi.org/10.36351/pjo.v39i2.1504","url":null,"abstract":"Purpose:  To compare postoperative refractive outcomes predicted by ultrasonic and optical biometry after phacoemulsification.\u0000Study Design:  Descriptive observational.\u0000Place and Duration of Study:  WAPDA Teaching Hospital and Acuity Eye Center in Lahore Pakistan from January 2022 to May 2022.\u0000Methods:  The study included 59 eyes with cataracts and required phacoemulsification and implantation of a foldable intraocular lens. They were divided into two groups; (29 in the ultrasonic biometry group and 30 in the optical biometry group). The first group was subjected to ultrasound biometry, whereas the second was subjected to optical biometry. We compared ocular refractions following cataract surgery in two groups. The Mann-Whitney U test was used to compare the mean absolute refractive error (MAE). The operating surgeon was the same in both the groups.\u0000Results:  All the participants were between 40 and 70 years of age. The preoperative mean target refraction in the ultrasonic group was 0.05 ± 0.13D (range: -0.01 to + 0.17D) and 0.12 ± 0.33D in the optical group (range:-0.01 to +0.49D). Thus, there was a non-significant difference between both groups regarding target refraction(P = 0.58, U = 398.5). The MAE measured for the first group was 0.14 ± 0.46D and for the second group was 0.60 ± 0.53D. Scopes, Zunaira Mubarik, Fiza Azhar, Amna Mehmud","doi":"10.36351/pjo.v39i2.1545","DOIUrl":"https://doi.org/10.36351/pjo.v39i2.1545","url":null,"abstract":"Purpose:  To compare the reliability of Ophthalmology Surgical Competency Assessment Rubric (ICO-OSCAR: phaco) between assessment of whole of the operation and its parts.\u0000Study Design:  Mixed method study design.\u0000Place and Duration of Study:  Mughal Eye Hospital, Lahore from January 2020 to January 2021.\u0000Methods:  Three raters assessed six anonymized phaco video operations with the help of ICO OSCAR phaco first as whole operations and then as parts of the operation. Inter rater reliability of the assessments of parts of the operation were compared with inter rater reliability of whole of the operation. Intra class coefficient (ICC) was used in SPSS – version 20. Raters also filled a survey form to detect how satisfied they were with the rubric. Raters’ feedback was taken to describe shortcomings/faults found during the use of ICO OSCAR phaco form.\u0000Results:  In the quantitative part, the ICC for the combined parts of the operation (0.910) was better than the value of ICC for the whole operation (0.904). In the questionnaire part, raters were satisfied with ICO OSCAR phaco rubric as a useful tool for learning and assessment of phacoemulsification surgical skills. In the qualitative part, many deficiencies were observed by the assessors in the rubric during videos analysis with the rubric ICO OSCAR phaco. This rubric is more accurate for assessment of operations in the operation theater.\u0000Conclusion:  Parts of the operations can be used instead of the whole operations for the assessment of surgical skills of phacoemulsification with the help of ICO OSCAR phaco rubric. Complete ocular examination was done with special attention to visual acuity, refractive errors, and frequency of associated syndromes in 103 individuals, from July 2022 to December 2022. Age ranged between 5 and 25 years. There were 50.5% females and 49.5% males. Majority of the subjects (62%) had visual acuities of 6/6 – 6/12 in both eyes, followed by 27% of subjects with visual acuities of 6/18 – 6/60, 7.7% had visual acuities of 6/60-3/60, and 2.9% had less than 3/60. Down syndrome was seen in 34%, mental retardation in 55.3% and ADHD in 10.7%. Visual acuity and intraocular pressure was measured. Anterior segment examination was performed on slit-lamp bio microscope and posterior segment examination was done with the help of Volk 90D lens after pupillary dilatation with Tropicamide 1% eye drops. All findings were saved on excel spreadsheets and analyzed through Statistical Package for Social Sciences (SPSS) version 25.\u0000Results:  Out of total 500 patients, 208 (41.6%) were male and 292 (58.4%) were female. Ages ranged from 35 to 77 years with mean age of 55.7 years. Type II diabetes was present in 467 (93.4%) patients and only 147 (29.4%) patients had good glycemic control. Most common anterior segment finding was cataract in 321 (64.2%) patients followed by dry eye and posterior blepharitis in 300 (60.0%), delayed pupillary response in 103 (20.6%) patients. All eyes underwent phacoemulsification with IOL implantation. They were divided into two groups; group A included twenty eyes of eleven patients who were implanted AcrySof IQ PanOptix trifocal IOL Model TFNT00. Group B included twenty eyes of ten patients who were implanted monofocal AcrySof IOLs as a control group. A questionnaire was given to every patient after explaining to himher the questions in Arabic, and clarifying the aim of evaluation.\u0000Results:  Mean age was 56.6 ± 6.9 years in group A and 62.8 ± 7.1 years in group B, range 50 – 70 (P = 0.861). We found statistical significant difference between both groups with group A showing better post operative uncorrected distance, intermediate, near, and best corrected near visual acuity (P values were 0.001, 0.556, 0.001, 0.177, 0.001, 0.001 respectively). An abnormal buildup of glycoprotein in numerous organs is known as lipoid proteinosis (LP), an autosomal recessive disorder. Hoarseness of speech, skin lesions, scars, papules that bead up around the eyelids, are all potential symptoms. Epilepsy and neuropsychiatric diseases can result from calcifications of brain tissue. In this paper, two cases of 13 and 15-yearold Iraqi family siblings (born to consanguineous parents) are reported. They were presented to Ibn Al-Haithem Hospital with hoarseness and characteristic symptomatic moniliform eyelid lesions. Biopsy confirmed the diagnosis. Though ocular involvement in LP is rare, ophthalmologists may encounter diverse ocular complications. With the exception of possible acute respiratory or neurological sequelae, the disease runs a slowly progressive but otherwise benign course. There was no restriction of ocular motility, but we found right inferior oblique over action. She had a head tilt of approximately 30 degrees towards the left side. The Patient underwent \"Antero-medial transposition of the inferior oblique\" to correct strabismus. After surgery, she was orthotropic in the primary position with an average head-chin posture. The referring pediatrician made diagnosis of delayed milestones. A detailed ocular examination was done to rule out presence of any ocular abnormality. Data were collected, and statistical analysis was done using Excel (Microsoft 2015, version 15.15) and Stata (version 13).\u0000Results:  A total of 406 children were included in the study. Forty one percent were in the age bracket of 1 – 2 years. There were 63% boys. Among all, 77% had compromised vision, of which 60% had abnormal ocular findings, while cortical visual impairment (CVI) was seen in 17%. The most common ocular issue was refractive error (30.29%), of which hyperopia was seen in 70.7% and myopia in 29.3%. Strabismus was seen in 15.5% of children and exotropia being more prevalent. Visual delay was detected in 19.8%. Out of these 42% had hyperopia, whereas 21% had esotropia.\u0000Conclusion:  The significant association of ocular abnormalities with development delay is a cause of great concern as this can lead to an increase in overall morbidity.
