{"title":"A Novel Technique of Suturing Sclerotomy in Pars Plana Vitrectomy","authors":"Abdullah Muaz, Ahmad Zeeshan Jamil, Muhammad Hannan Jamil, Muhammad Rizwan, Kashif Iqbal","doi":"10.36351/pjo.v39i2.1538","DOIUrl":"https://doi.org/10.36351/pjo.v39i2.1538","url":null,"abstract":"Purpose: To describe a new technique of suturing sclerotomy after 23- gauge pars plana vitrectomy using10 – 0 nylon suture with releasable knot.\u0000Study Design: Quasi experimental study.\u0000Place and Duration of Study: Sahiwal Teaching Hospital, Sahiwal, from January 2022 to June 2022.\u0000Methods: 10 – 0 nylon sutures with releasable knots were applied to close the sclerotomy after 23 – gauge pars plana vitrectomy in 100 cases. Complications like hypotony and wound leakage were recorded. Intra ocular pressures on 1st post-operative day and after 1 week were monitored. Inflammation at suture site, pain experienced by the patient and surgeon’s convenience of suture removal under a slit lamp after 1 week were documented.\u0000Results: Patients included 58 women and 42 men. Post-operatively, there was no wound leakage and hypotony in any patient, with mean intra ocular pressure of 13.5 ± 2.0 mmHg on 1st post-operative day and 17.6 ± .0.5 mmHg after 1 week of surgery. In 4% cases inflammation was observed at suture site. The mean pain score, measured according to visual analogue scale, at post-operative day 1 and day 7 was 1.25 ± 1.19 and 1.08 ± 0.99 respectively. No discomfort was reported during removal of releasable sutures in 96% of the patients. Surgeon found it convenient to remove the sutures under slit lamp illumination.\u0000Conclusion: Closure of 23 – gauge sclerotomy using nylon 10 – 0 suture with releasable knot is a useful option. Age ranged between 40 and 60 years. Patients with less than 6 months of surgery were excluded. Patients with posterior segment pathology like glaucoma, uveitis, high myopia, diabetic retinopathy, macular pathology, and hereditary fundus dystrophies were excluded from the study. After detailed history, complete ocular examination was performed. After pupil dilatation, YAG laser capsulotomy was done.IOP was measured with Goldmann Applanation Tonometer after instilling a topical anesthetic agent. Post-YAG, IOP was again measured at 1 hour and on day 1. Data was analyzed using SPSS version 25. Paired T-tests were used to assess the significance of any change in IOP.\u0000Results: Ninety eyes of 90 patients were inducted into the study in which 51 (57%) were male and 39 (43%) were female. The age varied from 40 to 60 years (mean 52.3 ± 10.25 years). There was no statistically significant difference between pre-YAG and post-YAG IOP. Similarly, no difference in IOP was found depending upon the age of the patients. She was under treatment for 4 years for papilledema. Visual acuity was 6/6 in the right eye and 1/60 in the left eye. Slit lamp examination was normal. Fundoscopy showed lumpy appearances of the optic nerve heads with vessels and macula normal, in both eyes. Her B scan ultrasonography and Computerized Tomography scan were carried out that revealed buried optic disc drusen a cause of pseudo-papilledema. {"title":"Evaluation of the effects of electronic cigarettes on the tear film of Smokers using Keratograph and SM Tube","authors":"S. Alshammeri","doi":"10.36351/pjo.v39i2.1599","DOIUrl":"https://doi.org/10.36351/pjo.v39i2.1599","url":null,"abstract":"Purpose: To evaluate the effects of e-cigarettes on tear film by comparing the tear film parameters of smokers and non-smokers using Keratograph and SM Tube.\u0000Study Design: Cross sectional study.\u0000Place and Duration of Study: Qasim University, from Saudi Arabia.\u0000Methods: A total of 60 males of age group 20 – 38 years were divided into three groups (Twenty non-smokers, 20 tobacco smokers and 20 vapor users). After consent, dry eye symptoms questionnaire was given to the participants. Four different test parameters through the Keratograph (5M JENVIS). Tear meniscus height, non-invasive tear break-up time, ocular redness and lipid layer thickness were measured. The new strip menis cometry tube (SM Tube) was used to test tear production. Data were analyzed using Statistical Package for Social Sciences (SPSS) with p value of less than 0.05 considered as significant.\u0000Results: The average tear breakup time was significantly decreased in both smokers and vapors group (10.15 ± 1.92 and 10.25 ± 2.65 respectively) than in nonsmokers (13.35 ± 2.36). There was also statistically significant difference in tear meniscus height, redness and lipid layer thickness between e-smokers and non-smokers(p > 0.05). The difference in these parameters was statistically non-significant between smokers and e-cigarette users.\u0000Conclusion: E-cigarettes cause low tear breakup time, less tear volume and other dry eye parameters, which leads to dry eye. Study included participants older than 30 years of age, of either gender, who were either diagnosed as a case of angle closure-Glaucoma or were angle closure-Glaucoma suspect. Sampling was done using convenient sampling technique. The selected patients underwent gonioscopy and the anterior segment OCT and the grades of the angle were determined. Grading was compared between the two techniques. The collected data was analyzed by using SPSS 23 version.\u0000Result: The mean age of patients was 59.16 ± 6.03 years. Out of 102 patients, 34 (33.3%) patients were male and 63 (66.7%) patients were female. The mean intraocular pressure was 17.80 ± 4.25 (mmHg). The accuracy of angle closure detection by Gonioscopy was 35.3% and by anterior segment, OCT was 60.8%. The p value was significant in both the groups. The Anterior segment OCT turned out to be better than Gonioscopy.\u0000Conclusion: Anterior segment OCT is more efficient in determination of angle of anterior chamber than gonioscopy. Patients of 20-60 years age and visiting ophthalmology clinic were included. Patients with history of glaucoma, hypertension, body mass index (BMI) above 30 kg/m2, under steroid treatment (topical, systemic, peri ocular, inhalation), undergone eye surgery including corneal and refractive surgery, smoker (> 100 cigarettes in his/her life and is still smoking), Alcoholic (> 1 time/month), chronic kidney disease (CKD) and myopia (> 6 diopters) were excluded. Association between CCT and IOP was calculated using Pearson's correlation coefficient.\u0000Results: A total 115 patients were included, 69 (60%) females and 46 (40.0%) males. Mean age was 35.78 ± 11.64 years old. Mean CCT was 550.26 ± 31.66 µm and 551.43 ± 34.92 µm for right and left eye respectively. IOP was 16.06 ± 3.71 mmHg and 15.76 ± 3.52 mmHg in right and left eyes respectively. There was a moderate positive relationship of CCT and IOP. There was a good relationship between right and left “IOP (R = 0.830, p = 0.000) and CCT (R = 0.856, p = 0.000).”\u0000Conclusion: CCT had a moderate positive correlation with IOP (right eyes: R = 0.355, p = 0.000; left eyes: R = 0.381, P = 0.000). Tumors arising from eyelids, may or may not be extending to other tissues were included in study. Data included age, gender, type of lesion, site of lesion, Histo-pathologic diagnosis, management and follow up.\u0000Result: A total of 150 patients with lid lesions were managed which included 50% benign and 50% malignant lesions. There were 77 (51.3%) males and 73 (48.7%) females. Age range was 3 to 85 years. The most commonly diagnosed lid lesion was squamous cell carcinoma (n=45, 30%), 60% among malignant lesions. It was followed by epidermal inclusion cyst (n=25, 16.7%), 33.33% of benign lesions. Patients with benign lesion were younger than 30 years while patients with malignant lesions were more than 50 years of age. Majority of the affected population was aged males, farmers and laborers, with history of prolong sunlight exposure. Among 150 lesions, 126 (84%) underwent excision biopsy and 24 (16%) underwent exenteration. Excision Biopsy was performed in all 75 (100%) benign lesions and 51 (68%) malignant lesions. There are few inpatients with shorter duration of hospital stay. We also find that the standard of safe delivery of medical and surgical facilities is also rising and critical patients are not allowed to be handled by the trainee residents. With overwhelming responsibilities, clinical teachers have very little time for the residents and in private training centers, there is scarcity of patient variety even in the outdoor, resulting in lack of exposure to variety of clinical problems. Both interventional and control groups underwent Visual acuity (VA) for distance and near. VF-25 questionnaire was filled to evaluate quality of life (VFQoL). After prescribing prisms and simple glasses without prisms, both groups were called for a follow-up after one week and one month. VF-25 questionnaire was filled again. IBM SPSS was used for statistics. Categorical variables were presented by the frequencies and percentages. Continuous variables were presented by mean and standard deviation.\u0000Results: There were 63.64% males. Mean age of subjects was 67.28 + 8.0. All patients underwent ocular examination. Group I was given single Intravitreal injection (IVI) of 0.1 mL Triamcinolone Acetonide at 4 mg per 0.1 mL concentration. Group II received same dosage of drug via Suprachoroidal injection (SCI). After 24 hours, they were assessed for side effects. After 6 weeks, second dose of same drug was administered. Patients were followed at 3 and 6 months.\u0000Results: No serious side effect was observed in any patient within 24 hours. At 1 month follow-up period, comparison between the two groups for BCVA, CMT and IOP was not significant (p > 0.05). When the two groups were compared at 3 months, both routes were equally effective but IOP remained more stable via SCI route.
