Mansoor, Muhammad Arshad Mahmood, Muhammad Saeed","doi":"10.36351/pjo.v39i3.1506","DOIUrl":"https://doi.org/10.36351/pjo.v39i3.1506","url":null,"abstract":"Purpose:  To determine mean best corrected visual acuity after intravitreal bevacizumab in patients with diabetic macular edema.\u0000Study Design:  Quasi experimental study.\u0000Place and Duration of Study:  Jinnah Hospital, Lahore and Chaudary Muhammad Akram Hospital Lahore from March 2018 to May 2021.\u0000Methods:  A total of 200 patients of either gender with diabetic macular edema and age between 40 and 60 years were included in this study. Patients with dense cataract, corneal opacity, uncontrolled diabetes mellitus and those who were treated with pan retinal photocoagulation were excluded. Best corrected Visual Acuity (BCVA) was evaluated with Snellen chart and converted to log MAR. A standard protocol of 0.05 mL of Bevacizumab was prepared from injection Avastin, 100 mg/4 mL vial. Bevacizumab injection was given to all patients, using topical anesthesia, under aseptic measures in the operation theater. A total of 03 injections were given at 1-month interval. After three months, BCVA was recorded. Results were presented in number and percentages.\u0000Results:  Mean age of the participants was 50.42 ± 6.48 years. Male to female ratio was 1:1.6, with 78 (39.0%) men and 122 (61.0%) women. Mean duration of diabetes mellitus was 5.12 ± 2.23 years. Batool, Sadaf Javed","doi":"10.36351/pjo.v39i3.1509","DOIUrl":"https://doi.org/10.36351/pjo.v39i3.1509","url":null,"abstract":"Purpose:  To find out level of awareness of parents regarding eye diseases, eye care needs and eye care practices among children of district Swat.\u0000Study Design:  Descriptive Cross-sectional study.\u0000Place and Duration of Study:  Hospitals of Tehsil Babuzai, District Swat, Pakistan from 15th March to 15th August 2022.\u0000Methods:  This cross-sectional study was carried out on 200 parents of Tehsil Babuzai, District Swat, Pakistan. Data was collected through an interview-based questionnaire which was divided into four main parts; socio-demographic data, knowledge about eye care, knowledge about eye diseases and eye care practices. Chi-square test was used to find out association among categorical variables. Data was analyzed using SPSS version 26.\u0000Results:  Knowledge about eye care was good in 112 parents (56.0%) and poor in 88 parents (44.0%). Knowledge was good regarding amblyopia in 82 (41.0%) parents, for childhood cataract in 63 (31.5%), and for congenital glaucoma in 56 (28.0%) parents. Attitude regarding children wearing spectacles and allowing ophthalmic surgery was positive in 179 (89.5%) and 165 (82.5%) parents, respectively. Most common eye diseases identified were refractive error (50.9%), followed by amblyopia (23.6%). Participants with higher education level and those having child with existing eye problems had significantly higher knowledge and practices scores (p < 0.05).\u0000Conclusion:  A considerable heterogeneity was observed between different social groups regarding children’s ocular problems and practices. Demographic data was obtained, and patients underwent complete ocular exam including DDLS and CDR, Slit-lamp biomicroscopy, gonioscopy and Intraocular pressure (IOP). Central corneal thickness, visual fields and OCT ONH was done. The patient was said to have glaucoma if his CCT adjusted IOP was greater than 21 mmHg, visual field defects and OCT changes. It was compared with CDR and DDLS, which was taken as suggestive of glaucoma with scores greater than 5.\u0000Results:  Mean age of patients was 54.82 ± 9.29 years, 108 (34.95%) were males. Those with history of systemic diseases, insufficient pupil dilation, sensitivity to drugs and uncooperative patients were excluded. Participants were randomly allocated to two groups; group which was administered ketamine with dexmedetomidine (Ketodex) and group which was given ketamine with propofol (Ketofel). Effect on blood pressure, pulse and sedation was recorded.\u0000Results:  Mean age was 65 ± 9.9 years. Sixty-five were females. Ketodex increased the risk of hemodynamic disorders compared to Ketofel (P = 0.04). The risk of nausea and vomiting was higher in the Ketodex group although not significant. The effect of drug course on heart rate, blood pressure and sedation was significant in both groups (P < 0.05). Neither at baseline, nor in the 1st and 5th minutes of surgery, was the mean arterial pressure (MAP) was significantly different between the two groups. From 10th minute of surgery until the end of surgery and from the beginning of recovery until the 30th minute of recovery, the MAP and pulse rate were higher in Ketofol group. September 2022 and December 2022.\u0000Method:  We included 126 untreated eyes of 120 patients. The indications of IVI were macular edema, and age-related macular degeneration.IOP and refractive status were determined. Bevacizumab, Ranibizumabor Triamcinolone Acetonidewas injected. Presence of vitreous reflux was noted at the injection site immediately after injection. IOP was measured at 10, 20, 30, and 45-minutes after injection. Descriptive statistics were calculated as mean and standard deviation for quantitative variables and frequency and percentage for qualitative variables. Repeated measure ANOVA was applied and p-value of <0.05 was considered significant.\u0000Results:  Transient IOP rise was observed in 95.24% cases whereas 4.76% patients showed sustained rise beyond 30minutes, which returned to normal after use of topical beta-blocker.Hypermetropic eyesinjected withTriamcinolone injection showed significant sustained rise of IOP (p<0.05). Vitreous reflux was seen in 47 (37.3%) eyes that showed significantlylowermean IOP of 20.89 ± 5.159 mm Hg (p<0.05) when compared to eyes with no vitreous reflux (IOP=30.75± 7.384 mm Hg).\u0000Conclusion:  IVI results in transientrise of IOP which is lesser with vitreous reflux. The eyes were divided into two groups on the basis of presence and absence of diabetic retinopathy. Data included age, gender, visual acuity, intraocular pressure, type, and duration of diabetes mellitus. Fundus examination was carried out using a 90 Diopters lens. Diabetic retinopathy was graded according to the early treatment diabetic retinopathy scale (ETDRS). Primary open angle glaucoma was diagnosed on the basis of optic nerve head changes, defective visual field and raised intraocular pressure. Data was analyzed by IBM SPSS Statistics for Windows, Version 20.0.\u0000Results:  Of 103 subjects 52 subjects were male and 51 were female. Of the 206 eyes of 103 subjects, 100 eyes had no signs of diabetic retinopathy while 106 eyes had signs of diabetic retinopathy. Fifty six eyes had primary open angle glaucoma. Chi-square and a multivariate regression model were used for analysis. A total of 333 admitted patients with laboratory-confirmed SARS-CoV-2 infection were included. Patients were enquired and examined for any ocular symptoms that developed along with COVID-19. The results were stratified by sex, age, comorbidities, High Resolution CT (HRCT) and condition of conjunctiva. Data was analyzed using SPSS 28.0.1.1. Chi square test was performed to measure the p value of the reported eye symptoms. Out of the 333, 81 subjects fulfilled the inclusion criteria. Forty-six patients (56%) were females and 52 patients out of 81 (64.2%) reported redness of conjunctiva. A positive relationship between the COVID-19 and the frequency of eye symptoms was observed with p value of 0.001. Careful gonioscopic examination of the anterior chamber angle will reveal angle recession with widened ciliary body.\u0000We describe three cases of blunt trauma to the eye resulting in raised intraocular pressure (IOP)with and without damage to the optic disc. Two of these patients with high IOP were controlled medically while third patient required surgical intervention. The risk of high IOP after trauma can occur earlier or in the later part of the life. Data regarding demographics and eye examination including visual acuity, refractive error, and angle of deviation, type of strabismus (intermittent, constant, or alternating)were collected. Data analysis was done by SPSS 23, and statistical significance was set at P< 0.05.\u0000Results:  The commonest strabismus was esotropia 522(71.5%). There were 415(56.8%) females. Most patients had alternating strabismus 436(59.7%), followed by constant strabismus 206(28.2%). Vision differed by types of strabismus (P =0.032) with worse vision found in esotropia(mean, 0.69±0.34)compared to exotropia (mean, 0.78±0.33). Refractive error varied by types of strabismus. Hyperopia was common with esotropia (mean, +3.9±3.8 D) and myopia was associated with exotropia (-3.4±2.8 D). The main cause of horizontal strabismus was congenital and presence of refractive error. Hyperopia was a major cause of esotropia for children aged 6 – 10 years which was highly significant (P <0.003) and myopia was the primary reason for exotropia at ˃15 years (P =0.485).\u0000Conclusion:  The most common type of strabismus among Sudanese was esotropia. Tear film break-up time was performed in all the patients to confirm dry eye using fluorescein strip. Screen time per day was graded as mild (< 2 hours), moderate (2 – 6 hours) and marked (> 6 hours). Chi square test was performed for data analysis on SPSS version 23 and P≤0.05 was considered statistically significant.\u0000Results:  Mean age of the patients was 44.34 ± 16.43. There were 66.1% females and 33.9% males. The commonest symptom was itching (35.5%). Tear film break up time test was positive in 37 (59.7%). Out of 18 patients with mild screen time, 10 (55.5 %) subjects had dry eye. Out of the 23 subjects with moderate screen time, 17 (73.9%) had dry eye while out of 21 subjects with severe screen time, 10 (47.6%) had dry eye. The results of our study were not statistically significant with p value 0.189.\u0000Conclusion:  There is no association between increased screen time and the dry eye.
