The proliferation of vascular smooth muscle cells (VSMC) is a key event in the pathogenesis of various vascular diseases, including atherosclerosis and hypertension. Thrombin is involved in the differentiation and abnormal proliferation of VSMC associated with atherosclerosis and hypertension. Thrombin stimulation results in extracellular signal-regulated kinase (ERK1/2) activation through transactivation of the epidermal growth factor receptor (EGFR). Based on our reacent studies in which PD98059 used to inhibit ERK1/2, we have shown previously that ERK1/2 was involved in the regulation by thrombin of VSMC's proliferation. In addition, protein kinase C delta (PKCd) have also been detected in VSMC and shown to be regulated by thrombin. Our study was conducted in order to determine which risk factors lead to the development of retinopathy of prematurity. This retrospective study included 108 newborns with birth weight (BW) < 1500 g and gestation age (GA) < 33 weeks, over the period of two years, who were treated at the Clinic of Pediatric, University Hospital, Clinic Centre Banja Luka. In all preterm children, the impact of risk factors conditioned preterm birth (gestational age and birth weight), parameters of general health status (respiratory distress syndrome, apnea, perinatal asphyxia, frequent use of blood derivatives, sepsis, hyperbilirubinemia) and parameters of the treatment with oxygen therapy. Out of 108 infants who fit the screening criteria, ROP was detected in 64 (59.2%) infants, 21(19.4%) of which had severe ROP requiring surgical intervention. Severe ROP was expressed in only 7.8% (5/64) of infants with GA > 30 weeks and in 12.5% (8/64) of infants with BW > 1250 g, compared to 25% (16/64) of infants with GA < 30 weeks and 20.3% (13/64) of infants with BW < 1250 g. The incidence of severe ROP was statistically significantly more frequent with progressively smaller birth weight BW < 1250 g (p <0.01) and the lower GA (gestational age) < 30 weeks (p <0.01). Using multiple logistic regression analysis for ROP, a long-term oxygen therapy (OR,15:54CI, 1.99-120.79) and a long duration of mechanical ventilation (OR,9.97; CI,3.06-32.51), there were obtained factors with a strong connection to the development of severe ROP. The following factors have a slightly lower correlation to the development of severe ROP: birth weight < 1250 g, gestation age < 30 weeks, respiratory distress syndrome, apnea, frequent use of blood derivatives and early sepsis. Prematurity and low birth weight are significant risk factors for the development of ROP. FMUB also requests and encourages PhD candidates to publish their results in international journals before thesis submission so as to increase their visibility in the scientific community. In addition to specific topics in medicine, PhD education needs to cover topics of general interest to researchers, in particular copyright and originality issues in scientific research. By adopting the concepts and terms of copyright, PhD candidates learn the importance of intellectual property and avoid plagiarism. As writing a PhD thesis in English may increase textual overlap between the thesis and the candidate's own published papers (in English) originating from the thesis, there may be some dilemmas in practice as to whether such textual overlap should be considered self-plagiarism or not. In line with the regulations at the top universities in the field of medicine, FMUB has adopted the conclusion that textual overlap between the thesis and the thesis-based publications of the candidate does not represent self-plagiarism, provided that certain conditions are met. Namely, regardless of the language of the dissertation, the key aspects of academic integrity in writing a PhD thesis should be honesty and transparency, namely, transparently linking the thesis-based publications with the final thesis; honestly and appropriately referencing the statements or any other content taken from the candidate's own previous work (unrelated to thesis) or other people's work; and transparently acknowledging the contribution from coauthors and team members in the thesis. The loss of conceptus is unforgivable when the possibility to sustain such pregnancy would elevate the level of birth rate that we aspire as a society. The aim of this study was to show the role of low molecular weight heparin in prevention of poor pregnancy outcomes. Methods: The study included all women with inherited thrombophilia referred to the Clinic for Gynecology and Obstetrics of the Clinical Centre of Serbia between 2016 and 2018 who were followed-up until delivery. The patients were divided into two groups. Results: The total number of 180 patients were pregnant for the first time, while 178 patients had had previous pregnancies. In 153 out of 178 patients, the previous pregnancy had poor outcome. 12 patients with AC therapy had FMU in previous pregnancies, 49 patients had missed abortion, 54 patients had second trimester miscarriage, 4 patients had both FMU and missed abortion, 9 patients had missed abortion and second trimester miscarriage while one patient had FMU, missed abortion and second trimester miscarriage. In previous pregnancies, 92 babies were born out of 313 pregnancies while in the current pregnancies treated with therapy there were 173 babies from 151 pregnancies. However, this is not always the case in clinical practice, hospitalization can be prolonged, and the question is what factors affect it. Methods: A multivariant logistic model of LOS predictors analyzed from the clinical data of 446 patients who underwent appendectomy at the Clinic for Emergency Surgery, Belgrade. The patients were divided into Short LOS group (SLOS hospital stay ≤3 days, 157 patients) and Long LOS group (LLOS hospital stay >3 days, 289 patients). Results: Significant differences were found between SLOS and LLOS groups in age (p<0.001), comorbidities (p=0.001), preoperative WBC (p = 0.004); preoperative CRP (p < 0.001); peritonitis (p < 0.001), using ≥ 2 antibiotics (p < 0.001), complicated appendicitis (p < 0.001), surgical time (P < 0.001). No significant difference was found concerning gender, postoperative WBC, CRP, and complications (p>0.05). Patients who underwent laparoscopic appendectomy (LAP) had a statistically significantly shorter hospitalization time compared to those who underwent Mini-Incision Open Appendectomy (MIOA) (p < 0.001). The multivariate logistic model found three statistically significant predictors of longer hospitalization: CRP preoperatively (B=0.006, p=0.047), OR=1.006, the type of surgery (B=1.199; p<0.001), OR=3.3 complicated appendicitis (B=0.762; p=0.003), OR=2.142. Conclusion: Surgical approach has statistically the most significant impact on LOS. The objectives of this research were: analysis of the clinical specifics of people with HD and definition of chronology in the occurrence of milestone disease progression. 56 patients with HD were interviewed by telephone about their age, family history, age at onset of the disease, first symptoms, current symptoms and their beginning. Also, the number of trinucleotide (CAG) repeats was determined by PCR method. In group of 50 alive patients the average age was 54.5 years, age at the beginning of disease 46.7 years, disease duration 7.8 years and latency to diagnosis 2 years, approximately. Family history was positive for 22 patients. Most frequently first symptoms were involuntary movements as isolated symptom (24 patients). In a progression of the disease swallowing difficulty and choking were first problem in everyday life for patients and they occurred 4.5 years after the first symptoms. Talking difficulty, falls, dependence on the assistance of another person during bathing and eating and walking difficulty happened after 4.6, 5.2, 6.2, 7.5 and 7.5 years, respectively. The average number of trinucleotide repeats was 43.8, in range from 40 to 51 repeats. Opavski","doi":"10.5937/medist1602049g","DOIUrl":"https://doi.org/10.5937/medist1602049g","url":null,"abstract":"The aim of this study was to: determine susceptibility patterns of invasive Streptococcus pneumoniae to antimicrobial agents, evaluate macrolide resistance phenotypes and genotypes and identify capsular serotypes among invasive isolates of S. pneumoniae circulating in Serbia. The total of 85 invasive pneumococcal strains, collected during 2012 and 2013, were sent from regional laboratories to the National Reference Laboratory. Susceptibility testing was performed by disk diffusion and E test, while phenotypes and genotypes of macrolide resistant strains were determined by double-test and PCR, respectively. Serotyping was performed by Quelling reaction. The overall penicillin and erythromycin non-susceptibility rates were 14.12% and 28.23%, respectively. Resistance rates were significantly higher in children than in adults and (p<0,01). Co-resistance to penicillin and erythromycin was detected in 7.06% strains. Resistance rates to tetracycline and chloramphenicol were 29.41% and 15.29%, respectively. The rate of multi resistance was 27.06%. cMLS phenotype was detected in 62.5% of all macrolide resistant isolates, while 37.5% expressed M phenotype. All 15 isolates with cMLS phenotype harbored ermB gene and all M isolates harbored mefA. The most common resistant serotypes were 3, 9A and 23F. This study revealed that penicillin and macrolide resistance among invasive pneumococcal isolates is high. It is a condition with great medical, social and economic burden. Although its concept has been recognized for over a century, disease mechanisms, diagnostic criteria and treatment modalities remain unclear and generate confusion and debate. Dementia and cerebrovascular disease share risk factors and neuropathology, and may contribute to VaD. Several mechanisms have been proposed, including vascular risk factors (hypertension, diabetes, hyperlipidemia) and behavioral factors (physical inactivity, obesity) as major substrate for both cerebrovascular disease and dementia. Also, macro and micro-embolic events and chronic brain hypoper fusion contribute to vascular dementia . Having in mind that disease mechanisms for vascular dementia and Alzheimer's disease are overlapping, and that clinical manifestations of cognitive impairment are often very similar, setting the diagnosis of vascular dementia is not an easy task. In clinical research, various diagnostic criteria are proposed. They are based on two major requirements: clinical diagnosis of dementia and its vascular origin. According to its multi causal nature, vascular dementia stands as a difficult condition to treat. Scoliosis is a problem which exists in all countries around the world, especially in the developing ones. In RM, scoliosis among school children at the age of 7-14 is represented by 7.5 %. Objective: To certify the representation of radiologically diagnosed scoliosis among school children in RM, the specific types of scoliosis in relation to age, location, degree of scoliosis according to the angle of deviation of the spine, as well as to define the risk factors for the occurrence. Study Design: Casecontrol study from casual type conducted on children at the age of 7-14. Methods: In order to fulfil the purposes of the research, two groups were formed: examined and control group. Children were divided into three groups according to age: the age of 7-9, 10-12 and 13-14. A questionnaire was also prepared on the basis of recommendations given by SRS. The questionnaire consisted of 15 rules and was intended for parents. Statistical processing was done in Statistics for Windows 8.0. Results: According to gender distribution, scoliosis is more likely to be found in females, 64.49% in comparison to male children. Distribution according to age illustrates that scoliosis is most likely to appear among age groups of 10-12, 58.7%. Juvenile and adolescent scoliosis is much more likely to appear in female children. The connection between the socioeconomical status and scoliosis was also confirmed. Namely, 52.9% of children from average socioeconomical status according to FAS-scale have 2.18 times significantly greater risk of developing scoliosis. Ratio of scoliosis according to the place of living (urban and rural area) is 52.9% : 47.1%. Sitting in front of the computer, physical inactivity, as well as high BMI is a great risk factor for development of scoliosis. The questionnaire also confirmed low level of education among parents regarding this problem, 15.9%. Symmetrical sports prevent scoliosis from developing. Within the examined group of children, the division is made according to the level of scoliosis: mild, medium and severe scoliosis. Most often developed is the double scoliosis 66.87% in comparison to single thoracic 12.32% and lumbar 21.02% deviation. The angle of deviation (Cobb) moves in intervals of 9-78 degrees, with average value of 15.76+8.5. The aim of our study was to present the frequency, demographics and treatment options of the patients hospitalized and treated at the tertiary healthcare Department of Vitreoretinal Surgery and Eye Trauma in a two-year period. Material and Methods: The retrospective study was conducted at the Clinic for Eye Diseases, University Clinical Center of Serbia in Belgrade, Serbia in the period between November 30, 2020 and December 4, 2022. |Data on demographics, seasonality and treatment methods were collected from \"Heliant\" healthcare software and subsequently interpreted. Results: During the study period, a total of 1295 patients were hospitalized. Cataract was the most common reason for hospitalization (n=355, 27.4%), followed by ocular trauma (n=303, 23.4%) and rhegmatogenous retinal detachment (n=287, 22.2%). The majority of patients with eye trauma (n=125, 41.2%, p<0.001) were between 19 and 44 years old. Male predominance was found in ocular trauma (n=261, 86.1%, p<0.001), and also in patients hospitalized for rhegmatogenous retinal detachment surgery (n=166, 57.8%, p<0.001) and surgery due to complications of proliferative diabetic retinopathy (n=41, 71.9%, p<0.001). Patients aged between 45 and 64 years were at the highest risk (n=136, 47.4%, p<0.001) and vitrectomy was performed more often as a treatment of rhegmatogenous retinal detachment (n=193, 67.2%, p<0.001). Rhegmatogenous retinal detachment most commonly occurred in autumn (n=103, 35.9%, p<0.001).
