{"title":"An Insight into Upper Gastrointestinal Endoscopy at CDSIMER","authors":"Dr Vinayaka N.S.","doi":"10.18535/jmscr/v11i8.12","DOIUrl":"https://doi.org/10.18535/jmscr/v11i8.12","url":null,"abstract":"Introduction: Upper gastrointestinal endoscopy is very commonly done diagnostic and therapeutic outpatient basis tool for patients with upper gastrointestinal symptoms. Upper gastrointestinal endoscopy was carried out as OPD procedure in department of general surgery at CDSIMER. Objectives: To study the various patterns of upper gastrointestinal diseases in our population and their course of management. Methods: A retrospective study of upper gastrointestinal endoscopy in CDSIMER over past 1 year. Demographics, medical history, indications for upper gastrointestinal endoscopy and the various disease patterns were collected. All patients undergoing upper gastrointestinal endoscopy between 18 and 90 years. Paediatric age group(<18 years),elderly patients above 90 and patients with bleeding disorders are excluded. Results: Among 359 symptomatic patients who underwent upper gastrointestinal endoscopy, 328 (91.36%) had abnormal results. The top four results were antral gastritis (44.29 %), pangastritis (20.33%), GERD (13.09%) and oesophageal candidiasis (4.73%). Inflammation of gastric mucosa in antral gastritis was more severe in the H.pylori positive group (23.17%). 220 males and 108 females were affected. Most affected were the age between 38 to 47 years. Conclusion: Upper gastrointestinal endoscopy is a valuable diagnostic and therapeutic tool in patients with dyspepsia and other upper gastrointestinal symptoms. Training with endoscope is mandatory for surgeon and physicians treating these set of patients. Conducted at a tertiary care hospital in Mumbai, the research aimed to identify the impact of alcohol consumption on PTB incidence, severity, and treatment outcomes among adult male patients. A descriptive case series approach was employed to study patients attending the outpatient department (OPD) and inpatient department (IPD) in May and June 2023. The findings revealed a significant association between alcoholism and PTB, with chronic alcoholic patients being more susceptible to TB infection and developing severe complications. {"title":"Association of Anterior and Posterior occlusal planes with skeletal class I, class II and class III malocclusion","authors":"Rosha Shrestha","doi":"10.18535/jmscr/v11i8.07","DOIUrl":"https://doi.org/10.18535/jmscr/v11i8.07","url":null,"abstract":"Introduction: The occlusal plane is a very vital in stomatognathic system and the association between anterior occlusal planes and posterior occlusal plane plays an important role during orthodontic treatments. Methods: Lateral cephalometric radiographs of 270 adult patients of the different classes of malocclusion were used. Several angular measurements were measured and compared among all study groups to evaluate the existence of relationship between AOP and POP with skeletal and vertical patterns. Results: The skeletal patterns showed a significant difference in anterior occlusal plane angle relative to both SN plane and FH plane among all classes of malocclusion (p-value =0.001**) having a steeper inclined plane in Class II and flat inclined plane in Class III. POP in relation to FH plane and SN plane showed lowest angle in skeletal Class III and highest in Skeletal Class I patients. Conclusion: The occlusal planes were found to have an impact on the jaw base where the variation in its angulation of different occlusal planes had affected both sagittal and vertical facial patterns. AOP relative to SN and FH were steeper in Class II and flat in Class III, while the POP relative to FH showed the same value in Classes II and III. The key interaction between oxidative stress and inflammation in disease etiology is supported by mounting research. Reactive oxygen species (ROS) generated by inflammatory cells cause oxidative stress, which has been recognized as the key mediator of the relationship between inflammation and the spread for diseases. Curcumin (DFM), demethoxycurcumin (DMC), and bisdemethoxycurcumin (BDMC) are the three main components of the rhizomes of turmeric. That filters the superoxide radicals, nitric oxide, and hydrogen peroxide while inhibiting lipid peroxidation, a successful Nano medicine using a curcumin will result in treatment of age-related neurodegenerative diseases such as Alzheimer’s and Parkinson’s disease. Although it may occur in women with no identified risk, women with certain risk profiles are at increased risk PPH. Tranexamic acid has been shown to be effective in low risk women. Aim: To compare the effectiveness of Tranexamic acid to placebo in preventing PPH in at-risk women following vaginal delivery. Research Methods: The study was a randomized controlled trial at ABUTH, Zaria, in which 334 women identified as being at risk for PPH, were sequentially randomized into Tranexamic and placebo groups of 167 each. The Tranexamic acid group received intravenous 1g Tranexamic acid made up to 20ml with 0.9% Normal saline, while the placebo group received 20ml 0.9% Normal saline at delivery. Both groups received I.M 10IU oxytocin as part of AMTSL protocol. Cystic hygroma is a congenital malformation of the cervical lymphatic system responsible for an accumulation of lymphatic fluid commonly in the retrocervical region. Cystic hygromas are most often associated with chromosomal abnormalities. When a cystic hygroma is diagnosed in utero, the fetal survival rate is only 2-6%. When hydrops fetalis is present with cystic hygroma, the mortality rate is close to 100%. We present here the case of a 23-year-old primigravida in whom an ultrasound performed at 17 weeks of amenorrhea revealed a posterior cervical cystic hygroma associated with hydrops fetalis, the evolution of which was marked by intrauterine fetal death two weeks later.
