Miyoshi","doi":"10.1540/JSMR1965.3.45","DOIUrl":"https://doi.org/10.1540/JSMR1965.3.45","url":null,"abstract":"The electromyogram of the esophagus has been studied by means of intramuscularly implanted electrodes to investigate pathophysiological changes seen in experimentally produced achalasia-like condition in dogs. These studies have been attempted to elucidate the functional mechanism of the esophagogastric junction.Electrodes are implanted into the esophageal muscle at 4 different points; the upper, middle, lower parts of the esophagus and the esophagogastric junction respectively and the electrodes have been kept chronically. In dogs, the electromyogram is taken during spontaneous swallowing without anesthesia or restraint while the studies are done.After the electrodes have been connected with the multi-channel amplifier, the electromyogram can be easily recorded until 3 weeks postoperatively.One day after the operation, dogs can eat and the electromyographical bursts move down to the lower part of the esophagus, judging from the electromyographical patterns at the 4 different points. The bursts are high in the maximum amplitude and long in the duration at the lower part of the esophagus as compared with the one from the upper part. Sugawara","doi":"10.1540/JSMR1965.13.161","DOIUrl":"https://doi.org/10.1540/JSMR1965.13.161","url":null,"abstract":"Eighteen patients with esophageal achalasia were investigated by an esophageal manometry and a mecholyl test in contrast of normals, patients with cancer of the esophagogastric junction and diffuse spasm.Compared with normals, esophageal achalasia showed 1) increased pressure and aperistalsis in the esophagus, 2) increased pressure and impaired relaxation in the lower esophageal sphincter.None of these findings, however, was specific to esophageal achalasia in comparison with the cancer and diffuse spasm. The most specific feature for esophageal achalasia was a positive mecholyl response.Two models of esophageal achalasia were experimentally produced in dogs and a manometric study was performed.A selective destruction of the myenteric plexus in the lower esophagus resulted in the same manometric changes that observed in human esophageal achalasia including a definite response to mecholyl.On the other hand, bilateral thoracic vagotomy produced a temporarily atonic dilatation of the esophagus which differed fundamentally from humans. Experi-ment were undertaken during 3 months on 3 groups of animals vagotomized, splanchnicotomized and transected both extrinsic nerves. The intragastric pressure was measured by balloon-strain gauge method. The intragastricpressurewa elevated in a way of filling with 200 ml of Ringer's solution within 1 minute every 2 minutes till a total of 800 ml.Following results were obtained1) The intragastric pressure at the initial period of gastric fullness and the average intragastric pressure during 2 minutes after filling of Ringer's solution in 3 groups were maintained in a higher level compared to that of the control dog at every time after operation and every stage of gastric fullness. Watanabe","doi":"10.1540/JSMR1965.1.211","DOIUrl":"https://doi.org/10.1540/JSMR1965.1.211","url":null,"abstract":"An electromyographic study of the influence of six refined amines (histamine, acetylcholine, tyramine, putrescine, cadaverine and isoamylamine) on the movement of the gastrointestinal tract was carried out in 60 rabbits to evaluate the biological toxicity of these amines in cancer. Of these six amines, four amines except acetylcholine and tyramine were proved paper-chromatographically in cancerous tissue of the stomach by the author's colleagues, Tsushima et al. The following conclusions were made out of the results obtained: Histamine and acetylcholine slightly accelerate the peristaltic movement of the gastrointestinal tract, while the remaining amines have no effect under intraluminal administration in the amount of 0.3mg/kg. Each of the refined amines markedly accelerate peristalsis by dripping them in 0.5% solution on the serosal sites locally and also by administrating them 0.3mg/kg into the regional arteries. Intravenous administration of these amines shows no effect on the peristaltic movement in the group of rabbits with normal liver function, but accelerates markedly in the group of rabbits in which hepatic dysfuction is induced by carbon tetrachloride. The monopolar tungsten wire (diameter 25 microns) was used for the stimulating electrode. The stimulated areas were determined by the histolo-gical examination.1. The retching was followed by the change of the systemic blood pressure (rise or fall) and the increase or decrease of the movements of the stomach and distal colon, but the correlation between the retching and the other autonomic reactions was not determined.2. The retching was induced in the vagotomized dog and in the spinal dog which was transected between the cervical and the thoracic region.3. The retching was induced both in the anesthetized dog and non-anesthetized dog.4. Fourty-five retching points were obtained in the medulla oblongata. They were found in the Nucl. dorsolateralis n. vagi, Nucl. dorsomedialis n. vagi, Nucl. tractus solitarii medialis, -inferior, -lateralis, Tractus solitarius, Formatia reticularis, Fibrae arcuatae internae and Nucl. intercalatus.5. Only three retching points were obtained in the pons. The spike potentials of the duodenum and choledocho-duodenal junction, the pressure in the common bile duct and the discharge of bile into the duodenum were recorded simultaneously in vivo.In the duodenum and the choledocho-duodenal junction, the rate of the rhythmic burst of spike potentials was 12-16 cycles/min.Similar results were obtained in both regions by the administrations of acetylcholine, prostigmine, bethanechol chloride, pilocarpine, atropine, adrenaline, morphine, cholecystokinin, secretin and amyl-nitrite. Thus, the action of the choledocho-duodenal junction is not independent of that of the duodenum.The pressure in the common bile duct rises simultaneously with the appearance of the spike potentials of the choledocho-duodenal junction.The discharge of bile into the duodenum was prevented by the muscle contraction of the choledocho-duodenal junction, when the bursts of spike potentials appeared. Bile was discharged into the duodenum by relaxation of the choledochoduodenal junction. When the pressure in the common bile duct was kept constant at about 80mm H2O, the discharge of bile into the duodenum was increased by the administration of C. C. Sadanaga","doi":"10.1540/JSMR1965.1.105","DOIUrl":"https://doi.org/10.1540/JSMR1965.1.105","url":null,"abstract":"The physiology of motility of the esophago-cardiac junction, pathophysiological changes of this area due to various esophageal diseases and their clinical applications were studied by our new method of obtaining electric internal pressure curve and several interesting findings were obtained. These examinations were carried out on 91 clicnial cases (including several controls).(1) Order of food passage through the normal esophago-cardiac junction is usually controlled and carried out mainly by pressure of the esophagocardiac junction at rest, positive pressure of the esophagus, and negative and positive waves of the gastric cardia which arise simultaneously.(2) Failure in presence of this negative pressure at the gastric cardia, is noted in all 17 cases of achalasia patients. By various values of the internal pressure at rest, this disease could be divided into two groups; the flaccid type (type A) and accelerated type (type B).(3) As to the surgical indications, cardioplasty type of surgery for group A, and esophago-gastrostomy type for group B, are better suited and effective.(4) Very specific inconsistent internal pressure was obtained in 8 out of 9 clinical patients with carcinoma of the gastric cardia. The contraction of the gallbladder was estimated with Magnus method. The potency of the relaxation of the sphincter of Oddi was estimated by the relaxation of the common bile-duct and by the number of drops through the sphincter before and after the administration of drug.
