Fassio","doi":"10.37421/2795-6172.2021.5.121","DOIUrl":"https://doi.org/10.37421/2795-6172.2021.5.121","url":null,"abstract":"Allergic Rhinitis (AR) is a “type 2“ inflammatory disease, caused by an immunoglobulin E (IgE)-mediated reaction to inhaled allergens and characterized by nasal symptoms. Pharmacologic treatment of AR mainly consists of oral/intranasal anti-histamines, intranasal glucocorticosteroids and Allergen-Specific Immunotherapy (AIT). When used correctly, these therapies are effective in most - but not in all - patients and can improve symptoms and Quality of Life (QOL); however, many patients show poor adherence to treatment, do not follow prescriptions, and coincidently experience side effects that lead to treatment cessation. Here, we briefly review and discuss recent findings from a proof-of-concept study about a synbiotic food supplement (Lactobacillus acidophilus NCFM, Bifidobacterium lactis BL-04 and Fructo-Oligosaccharides), which was evaluated in birch pollen allergic patients with AR, in a highly standardized setting of an Allergen Exposure Chamber (AEC). The study has demonstrated significant symptom improvement after the intake of the synbiotic food supplement. Both, the median TNSS was reduced by 50% (adjusted p-value=0.025) and the median TSS by 80% (adjusted p-value=0.0097), as well as the personal well-being of the patients was increased by 50%. In addition, the synbiotic food supplement revealed an excellent safety and tolerability profile. The present study is an excellent example that demonstrates efficacy for a specific synbiotic product and a superior tolerability and safety profile in the treatment of AR. It can be considered as an adjunct therapy for patients who do not improve with conventional medication for rhinitis, do not tolerate it or do not fit criteria for AIT. The thalassemias are the most common genetic disorder on a worldwide basis. The requirement of frequent blood transfusions in these patients pose a substantial burden on the health care system. Methods: A prospective observational study was conducted across 6 months (July 2018-December 2018) in a tertiary care hospital, Pune. The present study included 30 registered patient and their past 6 months record of blood transfusion (esp frequency), previous hemoglobin levels, height and weight. All these parameters were compared 6 months after supplementation with omega 3 fatty acids and proteins. Results: 16 of 28 patient showed that the average durations between two blood transfusions was increased by minimum 01 day to a maximum of 5 days. The average number of blood bag required was less than required blood bags in the period of pre supplementation. 20 of 28 patients showed a rise in hemoglobin level from a range of 0.5 to 1.2 gm/dl. Conclusion: In a country like India, with the high frequency of hemoglobinopathies, causing increased burden on the society, it is necessary to control the incidence by effective steps. Methods: In this study, articles were searched in specialized databases and 40 related articles were selected based on inclusion and exclusion criteria. Then the effect of physical activity on VEGF and HIF-1 signaling was investigated. Results: Exercise and physical activity by stimulating and activating VEGF and HIF-1 signals may induce generating new arteries and angiogenesis. Conclusions: The present study showed that physical activity increases capillary density by increasing the level of diffusion, increasing the time of exchange between blood and tissue, and decreasing the oxygen diffusion distance. The reason for this is the ability of the specific group of drugs to promote carcinogenesis by blocking certain types of receptors, presented in certain types of tumors. The available in vivo and in vitro data in the world literature on the influence of antihypertensive drugs (Sartans and Thiazide diuretics) on melanogenesis and development of other cancerous forms, although disputed, are numerous. Even more disturbing are the further data in the medical literature on the development of multiple tumors after administration of Sartans alone or in combination with hydrochlorothiazide. According to other literature data, even monotherapy with the already mentioned diuretichydrochlorothiazide, could be seen as a possible key or risk factor for the development of cutaneous melanoma. We present a clinical case of a 67-year-old patient with anamnestic data for systemic antihypertensive therapy with a combined preparation containing Valsartan / hydrochlorothiazide 160 mg / 12.5 mg for the period from the end of 2011 to March, 2020, came on occasion of a prophylactic dermatological examination. The examination found a pigmented lesion in the right torso, clinically and dermatoscopically suspected of thin cutaneous melanoma. According to the anamnesis and available documentation, it is clear that within this combined antihypertensive medication, the patient has developed two additional neoplasms in the recent past: choroidal melanoma and colorectal carcinoma successfully treated surgically. To what extent, the shared data (on the occurrence of multiple tumors after combination treatment with antihypertensives such as sartans and thiazide diuretics) would lead to the necessary and desirable rethinking of therapeutic strategies for the treatment of hypertension and clarification of the mechanisms at molecular level, at this moment remains unclear. Objectives: the aim of this study was to investigate the effect of N treatment with and without resistance training (RT) on superoxide dismutase (SOD) concentration and tissue pathology of kidney tissue in rats. Methods: In this experimental study, 20 male Wistar rats were randomly divided into four groups of five rats including 1) control (C), 2) sham (normal saline) (Sh), 3) N, and 4) N + RT. Groups 3 and 4 received 10 mg/kg N peritoneally, and the N + RT group performed 1 m ladder climbing for eight weeks and three sessions per week. SOD levels of kidney tissue were measured by ELISA and radioimmunoassay. Hematoxylin-eosin (H&E) staining was used to evaluate oxidative stress levels in kidney tissue. One-way ANOVA with Bonferroni’s post- hoc tests were used for analysis of research findings in SPSS version 22 (P ≤ 0.05). Results: SOD levels in the C group were higher than the Sh (P = 0.001), N (P = 0.001), and N + RT (P = 0.001) groups. SOD levels were lower in the Sh group than in the N (P = 0.049) and N + RT (P = 0.001) groups. However, there was no significant difference in SOD levels in the N + RT group and N group (P = 0.28). Also, oxidative stress levels were normal in tissue studies in all groups. As a result, all healthcare workers have been profoundly affected. Objectives: The goal of our study is to identify the level of knowledge and the effect of COVID-19 on dermatology residents. Methods: A cross-sectional analysis in which 77 dermatology residents from three Gulf Cooperation Council (GCC) countries and Canada completed an online questionnaire-based survey. The questionnaire consisted of four sections: one general information about the resident and three on knowledge, safety measures and impact of COVID-19, with a total of 26 questions. The questionnaire was scored out of 10 with those above the mean considered as having satisfactory knowledge. Results: The mean (SD) knowledge score was 6.25 (1.6). There was a statistically significant difference noted between the GCC countries and Canada in terms of the knowledge score (p-value=0.035). Only 14% of dermatology residents felt competent in managing COVID-19 patients. Seventy percent felt that the pandemic has negatively affected their dermatology training. Conclusion: Dermatology residents demonstrated a difference in knowledge score in relation to the geographic location of the program. Almost 46% of residents illustrated a satisfactory knowledge score about COVID-19. Only a small percentage of residents are confident in treating COVID-19 patients. We describe the first case of multiple cutaneous melanomas in the world literature, which developed simultaneously after the introduction of systemic antihypertensive therapy with Valsartan. When teachers need to assist asthmatic students, they need to have a sufficient level of knowledge to be effective until medical help arrives. This study used a sample of convenience to examine the level of teacher asthma knowledge for educators in Middle Tennessee and the effects of four demographic factors on their understanding of asthma using a sample of convenience. Methods: Teachers completed a 13-question online survey (n= 218) which evaluated their knowledge of asthma and the effects of demographic factors such as gender, school level taught, teachers’ own asthma status, and educational attainment on knowledge of asthma. Results: Teachers scored positively on the overall assessment of asthma knowledge with a mean score of correctness was 78% (Likert 3.90). This is above the asthma knowledge efficacy threshold set at 70% (Likert 3.50) by an expert panel. In the demographic analysis, only teachers with a history of asthma had demonstrated a higher level of asthma knowledge (M= 4.09) as compared to teachers without asthma (M= 3.87). Other demographic factors such as gender, educational attainment, or school grade taught had no effect on asthma knowledge. Conclusions: Teachers and educators held a level of asthma knowledge which was above the knowledge level threshold (Likert > 3.5). Teachers with a history of asthma held a higher overall knowledge of asthma than non-asthmatic teachers. No other demographic factors yielded a statistically significant effect.
