For such systems, numerous bioresponsive materials are utilized to construct functional modules for the desired devices. The typical closed-loop drug delivery systems recently reported in this review. The stimuli-responsive polymers serve to provide a snapshot of the utility and complexity of polymers that can sense, process, and respond to stimuli in modulating the release of a drug. Stimuli-responsive drug delivery vehicles come in the form of polymersomes, liposomes, micelles and dendrimers. Sleep medications are associated with adverse events risk in older adults. The objective of this study is to quantify the relationship between different levels of sleep medication use with falls and fall-related worries in United States adults aged 65 years and older using a nationally representative sample.\u0000\u0000Methods\u0000\u0000Using the 2011 National Health and Aging Trends Study, survey-weighted multiple logistic regression was used to determine the association between participants who reported sleep medication use and the outcomes: falls in the last month, falls in the last year, multiple falls in the last year, fall-related worries, and limitation of activities due to fall-related worries.\u0000\u0000Results\u0000\u0000In adjusted analyses, older adults who used sleep medications every night compared to non-users of sleep medications were at increased odds of falls in the last year with an odds ratio (OR) of 1.51 (95% confidence limit [CL] 1.27, 1.78) and of multiple falls with an OR = 1.67 (95% CL 1.35, 2.06). For those who used sleep medications less than every night compared to non-users, there were no statistically significant increased odds of fall outcomes. Older adults who used sleep medication most nights (5–6 nights per week) and every night compared to non-users had increased odds of having fall-related worries with an OR = 1.61 (95% CL 1.06, 2.45) and an OR = 1.32 (95% CL 1.11, 1.58), respectively.\u0000\u0000Conclusion\u0000\u0000Older adults who use sleep medication every night are at greater odds of experiencing falls and having fall-related worries. There are two forms of hypertension, primary hypertension and secondary hypertension. Primary or essential hypertension accounts for 90–95% of patients with hypertension, with its etiology unknown, while secondary hypertension accounts for 5–10% of the population, due to chronic kidney disease, endocrine disorders, or usage of ibuprofen or venlafaxine.(1)\u0000\u0000If left untreated, hypertension can lead to heart attack, stroke, heart failure, vision problems, kidney damage, and a variety of other problems, depending on which vessels it affects. As seen in Figure 1, either having low or high systolic or diastolic blood pressure can be detrimental to one's health. While focused on primary care providers, the guidelines specifically acknowledge the need for interprofessional, integrative care involving collaboration amongst pain management clinicians, behavioral health specialists, and pharmacists to optimize opioid use.(1) However, aside from looking for “red flags” in filling prescriptions, specific guidance to direct widespread pharmacist involvement is lacking. There is also currently limited data and information on how pharmacists can best aid in this public health issue. Their education and training has lacked standardization; however, this is beginning to change as the result of stricter state licensing requirements and newer laws that better define and expand the technician's responsibilities. These newer and expanded technician responsibilities and the education required will only further enhance their value to the pharmacy organization. Clancy, Shari Morgan, Mary-Margaret Hillstrand","doi":"10.37901/jcphp18-00023","DOIUrl":"https://doi.org/10.37901/jcphp18-00023","url":null,"abstract":"Purpose\u0000Describe a case of Listeria monocytogenes meningitis following initiation of alemtuzumab therapy in a patient diagnosed with relapsing–remitting multiple sclerosis (RRMS).\u0000\u0000Summary\u0000A 44-year-old female presented with headache and fever seven days after completion of an initial course of alemtuzumab for the treatment of RRMS. Blood cultures were positive for gram-positive bacilli. A lumbar puncture revealed cell counts, consistent with bacterial meningitis and magnetic resonance imaging (MRI) revealed abnormal signal enhancement of the left cingulate gyrus. Vancomycin, ceftriaxone, and ampicillin were initiated empirically. Both blood and cerebrospinal fluid cultures resulted positive for Listeria monocytogenes. Antibiotics were narrowed to ampicillin for a treatment duration of 21 days, after which symptoms resolved and she was transitioned home. Alemtuzumab is a CD52-directed cytolytic monoclonal antibody that has previously been implicated in association with cases of Listeria monocytogenes meningitis in other countries. Based on its mechanism of action, previous association, and positive temporal relationship, we hypothesize that alemtuzumab played a substantial role in the development of L. monocytogenes meningitis in this patient.\u0000\u0000Conclusion\u0000To our knowledge, this is the first reported case of alemtuzumab associated Listeria meningitis in the United States. This case highlights the severe lymphopenia associated with alemtuzumab therapy and risk of subsequent opportunistic infection. Heightened vigilance in counseling and monitoring of dietary restrictions is critical both prior to and during alemtuzumab therapy. Two multivariate logistic regression models identified factors associated with each outcome, adjusting for income, educational attainment, receipt of sex education, and birth country. Pharmacist intervention categories included code participation, medication recommendations, home medication reconciliation activities, provision of drug information to medical staff, expediting medication delivery, and patient encounters. Resulting intervention data were summarized, and probability of harm and cost avoidance data were applied. (3) Prednisone, prednisolone, and methylprednisolone are intermediate-acting corticosteroids with a peak effect four to six hours after administration. (1)(2) After just one dose, postprandial blood glucose may be affected without much effect on fasting blood glucose. (1) However, after repeated doses, these intermediate-acting corticosteroids can cause persistent hyperglycemia. (1) Dexamethasone, a long-acting corticosteroid, is associated with hyperglycemia that may persist for longer than 24 hours. (1) High doses of corticosteroids, defined as more than 30 mg of prednisone equivalent per day, lead to more adverse effects, such as hyperglycemia. (5) Likewise, many adverse effects, including glucose intolerance, are related to the cumulative corticosteroid dose and longer duration of corticosteroid use. A prospective, observational study of 35 patients without pre- existing diabetes, taking high-dose corticosteroids for 6 to 12 weeks, found 40.6% of patients to have elevated fasting or postprandial blood glucose at eight weeks (fasting blood glucose >126 mg/dL). (6) Both higher cumulative corticosteroid doses and continuous corticosteroid administration were associated with a higher incidence of impaired fasting glucose. A case-control study found a slight association between oral glucocorticoid prescriptions and diagnoses of diabetes. (7) The majority of the patients in this study received low daily doses of glucocorticoids. (7) It is presently unknown whether short-term administration of high-dose corticosteroids is associated with the subsequent development of type 2 diabetes, defined as hemoglobin A1c (HbA1c) ≥ 6.5% (48 mmol/mol).
