Some patients supported with CRRT suffer from cardiac arrhythmias, which are often treated with amiodarone. While amiodarone is a very effective antiarrhythmic, it has a relatively narrow therapeutic window and a long half-life, making it challenging to dose safely. This is especially true in patients supported with CRRT, where drug pharmacokinetics are likely altered. This ex vivo study measured the extent of amiodarone extraction by the CRRT circuit. Amiodarone was administered to a closed-loop CRRT circuit. Drug was dosed to achieve therapeutic concentrations. Circuits were primed with a human blood-plasma mixture and maintained at physiologic temperature and pH. Serial blood samples were collected over time and drug concentrations were quantified. Amiodarone was heavily extracted by the ex vivo CRRT circuit with only 23% amiodarone remaining in the plasma at 6 h. The relative concentration was significantly greater in the controls than in the CRRT circuits within 2 h (n = 3; p = 0.0059). Human induced pluripotent stem cell-derived cardiomyocyte (hiPS-CM) sheet transplantation ameliorated left ventricular dysfunction in preclinical studies, indicating its efficacy in RV failure in congenital heart disease. This study aimed to evaluate whether hiPS-CMs could improve RV function in rats with pressure-overloaded RV failure. F344/NJcl-rnu/rnu rats underwent pulmonary artery banding (PAB) via left thoracotomy. Four weeks after PAB, hiPS-CM patch transplantation to the RV was performed in the hiPS-CM group (n = 33), and a sham operation was performed in the sham group (n = 18). We evaluated cardiac catheterization, positron emission tomography data, and pathological results 8 weeks following PAB. RV end-diastolic pressure, the time constant of isovolumic relaxation, and end-diastolic pressure-volume relation were significantly ameliorated in the hiPS-CM group compared with in the sham group. Picrosirius red staining revealed that anti-fibrotic effects were significantly higher in the hiPS-CM group than in the sham group. Von Willebrand factor staining revealed significantly higher myocardial capillary vascular density in the hiPS-CM group than in the sham group. hiPS-CMs were detected in the epicardium 4 weeks after hiPS-CM sheet transplantation. The angiogenic gene expression in the myocardium was significantly higher in the hiPS-CM group than in the sham group. Eleven months earlier, the connector between the modular and pump cables had gotten wet when he was taking a shower. The cable connector was dried immediately, and no alarm had been observed during follow-up. Subsequently, the modular cable and system controller were replaced, with corrosion found on the modular cable connector. The \"Communication Fault\" alarm recurred, and complete damage to the communication cables was discovered. The pump was replaced, and the removed pump cable connector showed corrosion as well. Livers removed from rats were divided into Krebs-Henseleit (control), 50% blood (hemoglobin: 7 g/dL), and whole blood (hemoglobin: 14 g/dL) groups, then perfused (total perfusate volume: 25 ml) with a small oxygenation filter at 37 °C for 120 min. Blood or perfusate was collected over time, and blood gas and blood cell were measured. In addition, bile volume and portal venous pressure measurements were taken. In all groups, the partial pressure of oxygen was controlled to approximately 400 mmHg. Flow rates were maintained at approximately about 20-30 ml/min according to liver size. Portal venous pressure was normal in the 50% blood and whole blood groups, while lower than the reference value in the Krebs-Henseleit group. Twice as much bile was produced in the 50% blood and whole blood groups relative with the Krebs-Henseleit group. We observed no differences in hemoglobin and red blood cell levels. Lactate levels were normal in the 50% blood and whole blood groups, but were elevated in the Krebs-Henseleit group. Our compact perfusion system using oxygenation filters was able to maintain rat liver function by perfusing a small amount of extracorporeal blood. Acute myocardial infarction remains the predominant etiology of cardiogenic shock, followed by heart failure. Recent findings from the DanGer Shock trial indicate that the percutaneous micro-axial flow pump support, in conjunction with standard care, significantly reduced 6-month mortality in patients with acute myocardial infarction-related cardiogenic shock compared to those receiving standard care alone. However, real-world registry data reveal that the 30-day mortality among patients with acute myocardial infarction-related cardiogenic shock, who received concomitant veno-arterial extracorporeal membrane oxygenation support along with micro-axial flow pump, remain suboptimal. The persistent challenge in the field is how to incorporate, escalate, and de-escalate these temporary MCS to further improve clinical outcomes in such clinical scenarios. Despite significant advancements in simulating various components of the digestive system, the rectum has been largely neglected due to societal stigma. This study seeks to address this gap by developing soft circular muscle actuators (CMAs) and rectum models to replicate the defecation process. Using soft materials, both the rectum and the actuators were fabricated to enable seamless integration and attachment. We designed, fabricated, and tested three types of CMAs and compared them to the simulated results. A pneumatic system was employed to control the actuators, and simulated stool was synthesized using sodium alginate and calcium chloride. Experimental results indicated that the third type of actuator exhibited superior performance in pressure generation, enabling the area contraction to reach a maximum value of 1. This review aims to evaluate the current status, enabling technologies, ethical considerations, and future prospects of donation after circulatory death (DCD) heart transplantation in the Asia-Pacific region. A comprehensive narrative review was conducted using published literature and country-specific reports to assess global and regional trends in DCD heart transplantation. Particular focus was given to enabling technologies (e.g., thoracoabdominal normothermic regional perfusion [taNRP] and ex situ machine perfusion), legal and ethical frameworks, and implementation barriers across countries in the Asia-Pacific. While countries such as the UK, US, and Australia have achieved comparable survival outcomes between DCD and donation after brain death (DBD) heart transplantation, most Asia-Pacific countries remain in early stages. Australia leads the region with over 70 successful DCD heart transplants using the Organ Care System. Japan lacks legal clarity and clinical protocols for withdrawal of life-sustaining therapy, restricting DCD to kidney transplants. China employs a hybrid DBCD model but faces logistical and ethical constraints. In South Korea, India, and others, DCD heart programs are not yet established. High cost and limited availability of enabling technologies, alongside ethical controversy surrounding taNRP, are key barriers. Broader adoption of DCD heart transplantation in Asia-Pacific countries requires legal reform, ethical consensus, cost-effective perfusion strategies, and public engagement. The relationship between smoking and perioperative outcomes in LVAD implantation remains inconclusive, as evidence has been limited to single-center studies. This study aimed to examine the association between smoking and in-hospital outcomes of LVAD implantation in a large-scale population-based analysis. Patients who underwent LVAD implantation were selected from National Inpatient Sample from Q4 2015-2021. Multivariable logistic regression was used to compare in-hospital outcomes between smokers and non-smokers, where demographics, socioeconomic status, primary payer status, hospital characteristics, comorbidities, and transfer/admission status were adjusted. There were 1346 (26.5%) smokers and 3737 (73.5%) non-smokers who underwent LVAD implantation. Smokers presented with a higher burden of comorbidities. After multivariable adjustment, smokers had lower in-hospital mortality (aOR 0.68, 95 CI 0.52-0.889, p < 0.01), MACE (aOR 0.74, 95 CI 0.554-0.987, p = 0.04), neurological complications (aOR 0.555, 95 CI 0.367-0.839, p = 0.01), stroke (aOR 0.508, 95 CI 0.311-0.832, p = 0.01), pericardial complications (aOR 0.705, 95 CI 0.545-0.913, p = 0.01), renal complications (aOR 0.691, 95 CI 0.595-0.801, p < 0.01), venous thromboembolism (aOR 0.523, 95 C = 0.295-0.929, p = 0.03), hemorrhage/hematoma (aOR 0.746, 95 CI 0.641-0.869, p < 0.01), and superficial wound complication (aOR 0.458, 95 CI 0.286-0.733, p < 0.01). Moreover, smokers had a shorter time from admission to operation (p = 0.02), shorter length of stay (p < 0.01), lower transfer out rate (p < 0.01), and lower hospital charge (p < 0.01). This study uncovered a \"smoker's paradox\" in LVAD implantation. These findings added to the long-standing observation of a \"smoker's paradox\" in cardiac surgery. Advances in both insulin pumps and continuous glucose monitoring have enabled the transition from sensor augmented pump therapy, where insulin delivery is manually adjusted, to hybrid closed-loop insulin pump therapy, which automatically adjusts basal insulin infusion. Furthermore, fully automated insulin delivery systems that adjust insulin based on variations due to meals and exercise are now possible. These systems have been primarily applied to patients with type 1 diabetes but are now expanding to all insulin-dependent patients. Wearable artificial pancreas systems measure glucose levels in subcutaneous tissue fluid, while bedside artificial pancreas systems measure glucose levels in venous blood, making them suitable for managing the highly variable blood glucose levels of perioperative and critically ill patients. The 2D in vitro intestinal model of Caco-2 cells cultured on a semi-permeable membrane is widely used in drug development and the evaluation of absorption functions. However, these intestinal models lack the structural characteristics of the small intestine in vivo, and the cell behavior is not properly controlled. Previous studies have reported that the microstructure of the villi and crypts on a small intestine-mimicking scaffold promotes Caco-2 differentiation; however, the effect of the nanostructure of the small intestine-mimicking scaffold on Caco-2 differentiation remains unclear. This study aimed to elucidate the effects of nanostructures on the small intestine mimetic scaffold in Caco-2 differentiation. We fabricated a decellularized small intestine in which the basement membrane nanostructure was altered through a subtractive process. Caco-2 cells were cultured on decellularized small intestine for 21 days, and the differentiation of Caco-2 cells was assessed. The microvillus density of Caco-2 cultured on decellularized small intestine that retained the unique nanostructure of small intestinal basement membrane was significantly higher than that of Caco-2 cultured on decellularized small intestine that did not retain the unique nanostructure of small intestinal basement membrane.
