Fifty patients meeting the MDD (DSM V) diagnostic criteria from teaching hospital were recruited in the study. They were divided in two 2 groups. Control group were administered with Escitalopram 10 mg twice a day. Ayurveda group were intervened with tablet Brahmi vati 500 mg thrice a day and Liquid Aswagandarista 10 ml thrice a day. Interventions were for 60 days. Assessments were done on every 15th day. Assessments criteria included Hamilton Depression Rating scale (HDRS), Hamilton Anxiety Rating scale (HARS), UKU Side effect scale (UKU), Brief Psychotic Rating Scale (BPRS), WHO quality of life -BREF (WHOQOL-BREF), Pittsburgh Sleep Quality Index (PSQI) and Clinical Global Impression scales (CGI) were assessed at all the time points. Blood parameters like Haemoglobin, Serum creatinine and Liver function tests were evaluated at pre and post study.</div></div><div><h3>Results</h3><div>Between group comparison showed significant improvements in WHOQOL-Bref (p &lt; 0.001), UKU (p = 0.04) favouring Ayurveda group and PSQI (p = 0.02) improvements in control group. Improvements in other parameters were comparable. Within group assessment showed significant (p &lt; 0.001) improvement in HDRS, HARS, BPRS, CGI-S, CGI-GI in both the groups. Tubaki ,&nbsp;Rajat Thakur","doi":"10.1016/j.jaim.2024.101033","DOIUrl":"10.1016/j.jaim.2024.101033","url":null,"abstract":"<div><h3>Background</h3><div>Ayurvedic intervention (<em>Brahmi Vati</em> with <em>Saraswatarista</em>) is explored for their possible role in management of Generalized Anxiety Disorder (GAD), a common psychiatric disorder.</div></div><div><h3>Objective</h3><div>The objective of the study was to evaluate the efficacy of <em>Brahmi Vati</em> and <em>Saraswatarista</em> in GAD.</div></div><div><h3>Methods</h3><div>Study is a randomized controlled clinical trial. Patients (n = 50) of GAD (Diagnostic and Statistical Manual of Mental Disorders (DSM-5 criteria), 18–60 years of age, either sex participated in the study. Participants were randomly divided into two groups. Group A, received escitalopram 10 mg/day for first 10 days followed by 20 mg/day for next 50 days. Group B, received Ayurvedic intervention (<em>Brahmi Vati</em> 500 mg thrice a day (TID) and <em>Saraswatarista</em> 10 ml TID) for 60 days. Assessments were with clinical parameters like Hamilton Anxiety Rating Scale (HARS), GAD 7 scale (GAD 7), Beck Depression Inventory scale (BDI), Epworth sleepiness scale (ESS), Pittsburgh Sleep Quality Index (PSQI), WHO Quality of Life- BREF (WHOQOL-BREF), Clinical Global Improvement scale (CGI) and UKU-Side effect scale (UKU). These clinical assessments were measured on every 15th day during the intervention. Haemoglobin, liver function test (LFT), serum creatinine, serum urea were assessed before and after the study.</div></div><div><h3>Results</h3><div>Study results indicate that both the groups were comparable in HARS, GAD7, BDI, WHOQOL-Bref and CGI-Severity. Group B was better in PSQI (standard mean difference = 0.87, 95% CI: 0.28, 1.43), ESS (standard mean difference = 1.42, 95% CI: 0.78, 2.02), CGI [global improvement (standard mean difference = 0.82, 95% CI: 0.23,1.28) and efficacy index (standard mean difference = 0.97, 95% CI: 0.37,1.54)] and had better adverse events profile (standard mean difference = 0.79, 95% CI: 0.21, 1.36). This, in turn, facilitates the development of a strategy to address the decline in the quality of life parameters commonly associated with cancer treatment. Ayurveda personalized approach to disease management typically involves providing customized diets, lifestyle adjustments, medications, and detoxification therapies that target the entire body.</p><p>We present a case involving a patient with numbness and pain in the right hand after undergoing chemotherapy who was referred to us by an oncologist for symptom management. This case report demonstrates the Ayurveda approach for establishing the etiology, pathogenesis, pathophysiology, and treatment of Chemotherapy-Induced Peripheral Neuropathy (CIPN) resulting from taxane-based chemotherapy. Killedar ,&nbsp;Mandeep Singh","doi":"10.1016/j.jaim.2024.101000","DOIUrl":"10.1016/j.jaim.2024.101000","url":null,"abstract":"<div><div>Cystic echinococcosis (CE), which is present in all parts of the world, is caused by the bacterium <em>Echinococcus granulosus.</em> One of the most serious parasitic infectious diseases affecting both humans and animals, CE causes hydatid cysts that can lodge in various host organs, including the liver, lungs, heart, and brain, which can be fatal. Treatment options for cystic echinococcosis are active anthelmintics, surgery, and percutaneous aspiration. Numerous scolicidal chemical agents are in practice, but due to increased drug resistance and adverse reactions associated with drugs, usage is limited. Finding alternative therapies with the least or no side effects to manage the condition is the need of the hour. Here, we report a case of an asymptomatic hydatid lung cyst that was managed using Ayurvedic treatment principles. A 57-year-old businessman, asymptomatic, came with a diagnosis of Hydatid lung disease confirmed by CT scan. Surgery was advised, but the patient approached for Ayurveda treatment. The case was treated with oral medicines like <em>Arogyavardhini gutika, Goarka, Kanchanara guggulu, Vidangasava, Kapalabhati pranayama</em> for a period of 8 months. Complete resolution of the cyst was seen, which was evident by a CT scan. Case-control, cohort, cross-sectional or retrospective observational studies were also included for the systematic review. Participants included children aged 0–18 years receiving palliative care for cerebral palsy and undergoing Ayurvedic Medicine practices.</div><div>Descriptive analysis was conducted, including data such as year; author; design; sample size; intervention and comparison; outcomes and conclusion.</div><div>Two interventional studies compared Ayurvedic practices with each other or with physiotherapy in children with cerebral palsy. The meta-analysis demonstrated an improvement in spasticity for children using Ayurvedic medicine. However, there were limitations in terms of heterogeneity in interventions, control groups, and assessed outcomes.</div><div>Integrative practices, including Ayurvedic medicine have the potential to improve quality of life, manage disease symptoms and provide emotional support. However, more robust evidence is needed to support their widespread use. Ayurveda, a natural system of medicines helps to enhance health benefits when used as a complementary therapy in combination with conventional treatment through various herbal, herbo-mineral or herbo-mineral-metallic formulations. One of such formulations, <em>Raupya Suvarna Sutshekhara (RSS),</em> has <em>Raupya bhasma</em> and <em>Suvarna bhasma</em> (incinerated Silver and Gold respectively) ingredients and they are described as <em>Rasayana</em> (immunomodulatory) and possess <em>Medhya</em> (enhances the brain functions and intelligence) activity.</div></div><div><h3>Objective</h3><div>The study documents effects of <em>Raupya Suvarna Sutshekhara</em> in alleviating symptoms of malignant brain tumors.</div></div><div><h3>Materials and methods</h3><div>This retrospective cohort study was done for malignant brain tumor patients (n = 110) registered at our centre who were treated with <em>Raupya Suvarna Sutshekhara</em> 125 mg–250 mg (a herbo-mineral metallic formulation) as a treatment of choice of neural tonic along with supporting Ayurvedic medicines. Treatment response evaluation period ranged from 3 months to 5 years (median period 29 weeks). Clinical symptoms, weight, Karnofsky Performance Score (KPS), Quality of Life (QoL) score, and survival data with respect to treatment with RSS were analyzed.</div></div><div><h3>Results</h3><div>RSS showed a very significant (<em>p</em> &lt; 0.001) effect on symptoms related to memory loss, headache, imbalance, loss of appetite, and generalized weakness while significant (<em>p</em> &lt; 0.05) decrease in urinary incontinence, seizures, difficulty in thinking/articulating, and weakness in one part/side of the body. However, a not-quite significant change was seen in symptom confusion/disorientation. No significant improvement was seen in vision changes, facial numbness or tingling, swallowing difficulties, and tingling in extremities. Body weight and KPS also showed improvement. Migraine patients experience headaches of varying degrees, which are related with a higher level of disability and are triggered by psychological and physiological stressors. Migraine is estimated to affect 16.6% of the world's population, with women being three times more likely to experience it than men. Despite considerable advancements in modern and traditional medicine, a complete cure remains uncertain. In <em>Ayurvedic</em> treatises, migraine headache is referred to as <em>Ardhavabedhaka</em> under the classification of <em>Shiroroga</em> (diseases related to the head region).</div><div>35-year-old Indian male police inspector, suffering from recurrent right hemi-cranial headaches once in three days for the last three months, presented symptoms of <em>Tridhoshaja Ardhvabhedaka</em> (Migraine) associated with <em>Amlapitta</em> (hyperacidity).</div><div>After a thorough assessment of the patient, the treatment was meticulously planned based on the patient's <em>Dosha pradhanyatha</em> and <em>Vyadhi avastha</em> <em>(stage of disease)</em>. For the proper <em>Samprapti vighatana, Nidana parivarjana</em> (abstinance from the eitiological factors), <em>Deepana, Paachana, Siravyadha</em>, <em>Nasya</em> and <em>Dosha Shaman</em><em>a</em> <em>Chikitsa</em> principles were adopted. Further, the severity of the migraine was assessed by MIDAS and NPR score, which subsequently decreased from 19 to 4, and the NPR scale decreased from 8 to 2 till completion of the therapeutic intervention.</div><div>This case report unequivocally highlights the pivotal role of the Ayurveda treatment regimen in effectively managing migraine.
