Le port de lentilles de contact est le principal facteur de risque. <em>Staphylococcus</em> et <em>Pseudomonas</em> sont les agents pathogènes les plus fréquemment isolés dans les pays industrialisés. Les KB requièrent une prise en charge standardisée afin d’éviter tout retard diagnostique et thérapeutique susceptible d’assombrir le pronostic visuel. Sémiologie cornéenne, rapidité d’évolution des lésions, présence de facteurs de risque permettent au clinicien d’évoquer un diagnostic microbiologique mais seul le grattage cornéen permet, en isolant la (les) bactérie(s) en cause, de porter un diagnostic de certitude. La nature du traitement antibiotique dépend de la sévérité des lésions, du facteur de risque en cause et de la (des) bactérie(s) identifiée(s). Les corticoïdes ont montré leur efficacité en tant que traitement adjuvant et peuvent être utilisés dans certaines circonstances bien définies. Un traitement chirurgical est parfois nécessaire.</div></div><div><div>Bacterial keratitis (BK) is an infection of the cornea caused by one or more bacteria. Contact lens wear is the main risk factor. <em>Staphylococcus</em> and <em>Pseudomonas</em> are the most frequently isolated pathogens in developed countries. BK requires a standardized work-up to avoid diagnostic and therapeutic delays that may negatively affect visual prognosis. Corneal signs, the speed at which lesions progress and the presence of risk factors allow the clinician to presume an empirical microbiological diagnosis, but corneal scraping, which allows the isolation and identification of the bacteria involved in the infection, is the only way to confirm the diagnosis. The type of antibiotic treatment depends on the severity of the lesions, the risk factors involved, and the bacteria identified. Corticosteroids have been shown to be effective as adjuvant therapy and may be used under certain well-defined circumstances. Elles ont en commun leur potentiel de transmission et sont très largement dominées en fréquence par les conjonctivites et kératoconjonctivites liées aux adénovirus (ADV). Ces dernières constituent les infections de la surface oculaire les plus fréquentes et peuvent être à l’origine de fibrose conjonctivale et d’opacités cornéennes. Les autres conjonctivites virales sont le plus souvent satellites d’une infection systémique et ont, en général, une évolution spontanément favorable. Les conjonctivites chlamydiennes sont quant à elles dominées par le trachome, qui sévit encore dans 42 pays et reste la première cause de cécité infectieuse dans le monde. Dans les pays industrialisés, les Chlamydia sont responsables de conjonctivites néonatales et de conjonctivites parfois chroniques, pouvant être associées à des maladies sexuellement transmissibles. Cette revue synthétique fournit aux cliniciens l’essentiel des données microbiologiques, épidémiologiques, cliniques et thérapeutiques de ces deux grands groupes de conjonctivites infectieuses.</div></div><div><div>Viral conjunctivitis is the most common type of conjunctivitis. It is contagious and is predominantly seen as adenovirus-related conjunctivitis and keratoconjunctivitis (ADV). These infections are the most frequent ocular surface infections and can lead to conjunctival scarring and corneal opacities. Other types of viral conjunctivitis are often secondary to systemic infection and typically resolve spontaneously. Chlamydial conjunctivitis, on the other hand, is dominated by trachoma, which remains endemic in 42 countries and is still the leading cause of infectious blindness worldwide. In industrialized countries, chlamydia can also cause neonatal conjunctivitis and sometimes chronic conjunctivitis, occasionally associated with sexually transmitted diseases. Elle reste cependant une maladie rare dans ses formes sévères. Bien que les recommandations relatives au dépistage de la maladie soient bien établies en France, la prise en charge en cas de ROP nécessitant un traitement est moins codifiée, d’autant plus que l’arsenal thérapeutique inclut l’utilisation des médicaments ayant récemment obtenu une autorisation de mise sur le marché (AMM). Cette prise en charge complexe requiert le concours multidisciplinaire d’ophtalmo-pédiatres, de chirurgiens vitréo-rétiniens, des néonatologistes, d’anesthésistes pédiatriques, d’infirmières et d’orthoptistes, au sein d’une structure adaptée au nouveau-né prématuré. Il existe un réel besoin d’unifier les pratiques nationales autour de cette pathologie, avec une demande forte des différents acteurs de cette prise en charge. L’objectif de ce Protocole national de diagnostic et de soins (PNDS) est de présenter aux professionnels concernés la prise en charge diagnostique et thérapeutique optimale actuelle et le parcours de soins des prématurés nécessitant un traitement pour une ROP. Il a pour but d’optimiser et d’harmoniser la prise en charge de cette maladie sur l’ensemble du territoire. Les principales indications au traitement, les différentes modalités de traitement incluant la photocoagulation laser, les injections d’anti-VEGF et la chirurgie vitréo-rétinienne, sont revues afin d’établir des recommandations de bonne pratique pour leur réalisation et pour le suivi post-traitement.</div></div><div><div>Retinopathy of prematurity (ROP, ORPHA: 90050) is the main cause of visual impairment in preterm infants and the leading preventable cause of childhood blindness in high- and middle-income countries. However, severe stages of the disease remain rare. While screening recommendations for the disease are well-established in France, management of ROP requiring treatment is less standardized, especially since new therapeutic options have been approval on this indication. The management of preterm infants requiring treatment for ROP is complex and involves a multidisciplinary team, including pediatric ophthalmologists, vitreoretinal surgeons, neonatologists, pediatric anesthetists, nurses, and orthoptists, within an adapted structure for premature infants care. There is a genuine need to unify national practices, with a strong demand from physicians involved in ROP care along the country. The objective of this National Diagnostic and Care Protocol (PNDS) is to provide guidelines for diagnostic and management for ROP, and to optimize and harmonize the management of this disease across the country. Data was analysed from a surgical outcome database. Primary outcomes included postoperative change in the mean intraocular pressure (IOP) and medications. Success was defined at two IOP cut-off points:<!--> <!-->≤<!--> <!-->21<!--> <!-->mmHg and<!--> <!-->≤<!--> <!-->15<!--> <!-->mmHg with (qualified) or without (complete) drops. Failure was repeat glaucoma surgery or loss of light perception (LP).</div></div><div><h3>Results</h3><div>Fifty-seven eyes with a mean follow-up of 32<!--> <!-->months (range 12<!--> <!-->months to 36<!--> <!-->months) were included. Mean IOP changed from 19.7<!--> <!-->±<!--> <!-->4.2<!--> <!-->mmHg (range 12–28) to 14.0, 14.9 and 14.6 at 12, 24 and 36<!--> <!-->months. Drops reduced from 2.2<!--> <!-->±<!--> <!-->0.7 to 0.92<!--> <!-->±<!--> <!-->1.24 at the last follow-up. Complete success for an IOP<!--> <!-->≤<!--> <!-->21<!--> <!-->mmHg was 49%, 39%, 37% and qualified success was 96%, 92% and 95% at 12, 24 and 36<!--> <!-->months. An IOP<!--> <!-->≤<!--> <!-->15<!--> <!-->mmHg (qualified) was achieved in 74%, 58%, 54% at 6, 12, 24 and 36<!--> <!-->months. Three patients (5%) required further glaucoma surgery, one patient underwent selective laser trabeculoplasty and one patient requiring peri-ocular steroid treatment due to persistent cystoid macular oedema.
